RESUMEN

Aims/Background Artificial intelligence technology has attained rapid development in recent years. The integration of artificial intelligence applications into pressure reduction mattresses, giving rise to artificial intelligence-powered pressure reduction mattresses, is expected to provide personalised intelligent pressure reduction solutions, through automatic user's data-based adjustment of the patient's local pressure condition to prevent pressure injury. The purpose of this study was to investigate the effectiveness of artificial intelligence-powered smart decompression in the prevention of postoperative medium- and high-risk pressure injury in middle-aged and elderly patients. Methods A total of 400 middle-aged and elderly patients admitted to our hospital from June 2021 to December 2023 were selected as study subjects. Patients were categorised into observation and control groups according to the medical record system. General demographic data of the patients were collected. The propensity score matching method was used to balance the baseline data of the two groups of patients. The incidence, severity, complications and sleep quality in the matched study subjects were also compared. Results After matching, there were 96 patients in the two groups, and the differences in baseline data between the two groups were not statistically significant. Pressure injury and the total incidence of complications in the observation group were significantly lower than those in the control group (p < 0.05). Before treatment, there was no difference in the scores of all aspects of the Richards Campbell Sleep Questionnaire between the two groups (p > 0.05). After treatment, the scores of all aspects of Richards Campbell Sleep Questionnaire in the observation group were significantly lower than those in the control group (p < 0.05). Conclusion The artificial intelligence-powered smart decompression mattress can significantly prevent moderate- and high-risk pressure injury, effectively reducing the incidence of pressure injury and complications in postoperative long-term bedridden patients, alleviating the severity of pressure injury, relieving the pressure on various parts, and improving the sleep quality of patients.

OBJECTIVE: Osteoporosis (OS) is a systemic bone disease characterized by low bone mass and bone microstructure damage. This study. METHODS: According to the T value, 88 elderly fracture patients were grouped as the control group (without OS, 43 cases) and observation group (with T value <-2.5, which could be diagnosed as OS, 45 cases). The content of boney containing protein (BGP), total type 1 collagen amino terminal extender peptide (TPINP), ß-Crosslaps (ß-CTX), parathyroid hormone (PTH) and insulin-like growth factors-1 (IGF-1) was compared. Multivariate logistic regression was adopted to analyze the correlation between biochemical indexes and the occurrence of senile OS fracture and the related risk factors. The diagnostic value in the elderly was analyzed by receiver operating characteristic (ROC) curve. RESULTS: The levels of BGP, TPINP, ß-CTX, PTH and IGF-1 were elevated, and the level of IGF-1 was decreased in the observation group compared with the control group (P < 0.05). The elevated content of BGP, TPINP, ß-CTX and PTH, and the decreased expression of IGF-1 were influencing factors for OS fractures in the elderly (P < 0.05). The sensitivity and specificity to predict the occurrence of OS fractures in the elderly were 91.70% and 90.50%, respectively. The AUC of combined detection was 0.976 (95% CI: 0.952-1.000), which was memorably higher than single indicator detection (P < 0.05). Among 45 patients, 32 cases had good prognosis and 13 had poor prognosis. In comparison with the good prognosis group, the content of BGP, TPINP, ß-CTX and PTH were sensibly higher, the level of IGF-1 was prominently lower, and the proportion of fracture history was much higher in poor prognosis group (P < 0.05). Fracture history, BGP, TPINP, ß-CTX, PTH and IGF-1 were independent risk factors for poor prognosis of elderly OS fractures (P < 0.05). CONCLUSION: Bone metabolism factors were associated with poor prognosis of OS in the elderly. The combined detection had higher diagnostic value in calculating the risk of OS fracture in the elderly than single indicator detection.

ObjectiveTo identify factors related to the occurrence of choking and coughing while eating in the disabled elderly and conduct importance matrix analysis to provide a reference basis for controlling choking on food in the disabled elderly. MethodsA convenience sampling method was used to select 80 disabled elderly individuals in a hospital between October 2019 and April 2022 as the study population. The occurrence of choking and coughing while eating was recorded, and a questionnaire was administered to collect general information. Additionally, assessments were conducted using the intelligent mental status examination scale (MMSE), oral health checklist (BOHSE), eating assessment tool⁃10 (EAT-10), and chewing function evaluations. Univariate and multifactorial analyses were conducted to analyze the influencing factors of choking and coughing while eating in the elderly with disabilities. ResultsThe incidence of choking and coughing while eating was 52.50% (42/80) among the 80 disabled elderly. The degree of disability (OR=2.895, 95%CI: 1.352‒6.201), age (OR=4.040, 95%CI: 1.121‒14.562), BOHSE score (OR=2.473, 95%CI: 1.002‒6.102), EAT-10 score (OR=5.345, 95%CI: 2.112‒13.527), and chewing function score (OR=3.453, 95%CI: 1.247‒9.562) were identified as risk factors for choking and coughing while eating in the disabled elderly. The MMSE score (OR=0.343, 95%CI: 0.135‒0.869) was identified as a protective factor. The importance matrix analysis indicated that EAT-10 score, MMSE score, and chewing function had high importance with slightly lower difficulty in improvement, and were thus listed as items in the priority improvement area. Age and degree of disability had high importance and high difficulty in improvement, and therefore belonged to the suboptimal improvement area. BOHS score was slightly lower both in importance and difficulty of improvement, entering the alternative improvement area. ConclusionAge, degree of disability, BOHSE score, EAT-10 score, and chewing function score are risk factors for the occurrence of choking and coughing while eating, while the MMSE score is a protective factor. The importance matrix analysis can provide a basis for targeted intervention in clinical practice.

Background: We systematically quantified the currently inconclusive association between Mediterranean diet patterns and the risk of lung cancer. Methods: We searched the PubMed, Cochrane, Database of Abstracts of Reviews of Effects (DARE) and Web of Science electronic databases to identify relevant articles published before October 2021. We used the Newcastle-Ottawa scale to assess the quality of the published research and a random-effects model to estimate the aggregate hazard ratios and 95% CIs. As a result of significant heterogeneity, we performed subgroup analysis, meta-regression analysis, and sensitivity analysis. Where data were available, we also performed a dose-response analysis. Results: Nine articles were included in the meta-analysis. The meta-analysis showed that there was a significant negative correlation between Mediterranean diet patterns and the risk of lung cancer in the general population with a hazard ratio of 0.82, a 95% CI of 0.74-0.92, and a high heterogeneity (I 2 = 59.9%, P < 0.05). As a result of the significant heterogeneity, we conducted subgroup analysis, meta-regression analysis, and sensitivity analysis and found that the study design was the source of the heterogeneity. Subgroup analysis and sensitivity analysis showed that the final results did not change very much, the sensitivity was low and the results were relatively stable. The dose-response relationship showed that, based on the lowest Mediterranean diet score (0 points), for every three-point increase, the risk of lung cancer was reduced by 9%. Conclusion: The evidence in this meta-analysis shows that there is a significant negative correlation between Mediterranean diet patterns and the risk of lung cancer, suggesting that Mediterranean diets are a protective factor in lung cancer.

Knowledge is limited about the lifetime risk of chronic pelvic pain syndrome (CPPS) occurrence in Chinese middle-aged males and the prognosis of CPPS patients with and without treatment. Noninstitutionalized Chinese males aged 40 to 81 years were enrolled in this study from a total of 76 local communities across 30 provinces of China. Information about the occurrence of CPPS, symptom relief, and previous treatment was collected. Based on completed answers to specific questions about self-reported and physician-diagnosed CPPS, the lifetime risk of CPPS occurrence was 25.3% (1,091 out of 4,315) in Chinese males aged 40 to 81 years. Over 77% of patients with CPPS had received treatment. The symptom relief rate was not significantly different between the treatment (57.3%, 142/248) and nontreatment (50.1%, 422/843) groups. Regardless of whether patients had received medical treatment, engagement in sedentary work and regular alcohol consumption had a significant negative influence, while marriage had a positive influence, on the prognosis of CPPS. A good prognosis could be achieved without treatment for some cases of CPPS, while others required a timely symptom-orientated treatment using adequate medications combined with lifestyle adjustment and follow-up.
