Entre las causas de dolor torácico, la costilla deslizante presenta baja prevalencia, antecedentes traumáticos y manejo controvertido. Este síndrome merece ser incluido en el diagnóstico diferencial de causas de dolor torácico en niños. Al no asociarse a traumatismos previos y la deformidad de cartílagos, nos induce a pensar en una alteración en el desarrollo costal, al margen de la etiología traumática típica en adultos. Se presenta una serie de pacientes pediátricos intervenidos por costilla deslizante en un centro de referencia entre 2001 y 2022. Se incluyeron nueve pacientes, con un rango de edades de 11 a 16 años. Solo dos casos describen traumatismo previo. Todos presentan un inicio súbito de dolor toracoabdominal intenso. Los pacientes fueron intervenidos mediante resección abierta de cartílagos costales afectos, con resolución del dolor.

BACKGROUND: Late hemothorax is a rare complication of blunt chest trauma. The longest reported time interval between the traumatic event and the development of hemothorax is 44 days. CASE PRESENTATION: An elderly patient with right-sided rib fractures from chest trauma, managed initially with closed thoracostomy, presented with a delayed hemothorax that occurred 60 days after initial management, necessitating conservative and then surgical intervention due to the patient's frail condition and associated complications. CONCLUSIONS: This case emphasizes the clinical challenge and significance of delayed hemothorax in chest trauma, highlighting the need for vigilance and potential surgical correction in complex presentations, especially in the elderly.

Slipping rib syndrome should be included in the differential diagnosis of causes of chest pain in children. When not associated with previous trauma and cartilage deformity, it is necessary to consider an alteration in rib development, regardless of the typical traumatic etiology in adults. Here we describe a series of pediatric patients with slipping rib seen at a referral hospital between 2001 and 2022. Nine patients aged 11 to 16 years were included. Only 2 had a history of trauma. All patients described a sudden onset of severe thoracic abdominal pain. The patients underwent open resection of the affected costal cartilages, with resolution of pain.

Abstract Background: The present study explored the role of continuous erector spinae plane (ESP) block for analgesia as well as its impact on pulmonary functions in patients with multiple rib fractures. Methods: Ten patients with multiple rib fractures were enrolled after getting informed and written consent. Ultrasound-guided ESP block was performed at the level midway between the fractured ribs followed by the insertion of the catheter. Pre- and post-block VAS score, hemodynamics, respiratory rate (RR), peripheral oxygen saturation (SpO2), inspiratory capacity (IC), blood gases (PaO2 and PCO2), and complications were compared. Results: Pain scores at rest as well as on movement showed a significant reduction from 5.9 and 7.5 pre block to 1.6 and 2.5 respectively at 96 hours (p < 0.0001). Similarly, RR, SpO2, IC, and PaO2 were significantly better after the block placement (p < 0.001). Conclusion: Continuous ESP block provide adequate analgesia with better respiratory functions in patients with multiple rib fractures.

PURPOSE: Conflicting evidence exists on the choice of surgical or non-surgical treatment of flail chest injuries. We aimed to perform a meta-analysis comparing outcomes in patients presenting flail chest undergoing surgical or non-surgical treatment. METHODS: Embase, PubMed, and Cochrane databases were searched for randomized controlled trials (RCTs) comparing surgery to no surgery in patients with acute unstable chest wall injuries. We computed weighted mean differences (WMDs) for continuous outcomes and risk ratios (RRs) for binary endpoints, with 95% confidence intervals (CIs). Random effects meta-analyses were performed. Heterogeneity was assessed using I2 statistics. RESULTS: Six RCTs (544 patients) were included, and surgical treatment was used in 269 (49.4%). Compared to no surgery, surgery reduced mechanical ventilation days (WMD - 4.34, 95% CI - 6.98, - 1.69; p < 0.01; I2 = 87%; GRADE: very low; PI - 13.51, 4.84); length of intensive care unit stay (WMD - 4.62, 95% CI - 7.19, - 2.05; p < 0.01; I2 = 78%; GRADE: low; PI - 12.86, 3.61) and the incidence of pneumonia (RR 0.50, 95% CI 0.31, 0.81; p = 0.005; I2 = 54%; GRADE: moderate; PI 0.13, 1.91). No difference in mortality (RR 0.56, 95% CI 0.19, 1.65; p = 0.27; I2 = 23%; GRADE: moderate; PI 0.04, 7.25), length of hospital stay (WMD - 5.39, 95% CI - 11.38, - 0.60; p = 0.08; I2 = 89%; GRADE: very low; PI - 11.38, 0.60), or need for tracheostomy (RR 0.59, 95% CI 0.34, 1.03; p = 0.06; I2 = 54%; GRADE: moderate; PI 0.11, 3.24) was found. CONCLUSIONS: Our results suggest that surgical treatment is advantageous compared to non-surgical treatment for patients with flail chest secondary to rib fractures.

Meat is an important part of the food pyramid in Mexico, to such an extent that it is included in the basic food basket. In recent years, there has been great interest in the application of so-called emerging technologies, such as high-intensity ultrasound (HIU), to modify the characteristics of meat and meat products. The advantages of the HIU in meat such as pH, increased water-holding capacity, and antimicrobial activity are well documented and conclusive. However, in terms of meat tenderization, the results are confusing and contradictory, mainly when they focus on three HIU parameters: acoustic intensity, frequency, and application time. This study explores via a texturometer the effect of HIU-generated acoustic cavitation and ultrasonoporation in beef (m. Longissimus dorsi). Loin-steak was ultrasonicated with the following parameters: time tHIU = 30 min/each side; frequency fHIU = 37 kHz; acoustic intensity IHIU = ~6, 7, 16, 28, and 90 W/cm2. The results showed that acoustic cavitation has a chaotic effect on the loin-steak surface and thickness of the rib-eye due to Bjerknes force, generating shear stress waves, and acoustic radiation transmittance via the internal structure of the meat and the modification of the myofibrils, in addition to the collateral effect in which the collagen and pH generated ultrasonoporation. This means that HIU can be beneficial for the tenderization of meat.

PURPOSE: Malignant chest wall tumors are rare in pediatrics. They require multimodal oncological treatment and local surgical control. Resections are extensive; therefore, thoracoplasty should be planned to protect intrathoracic organs, prevent herniation, future deformities, preserve ventilatory dynamics, and enable radiotherapy. METHODS: We present a case series of children with malignant chest wall tumors and our surgical experience with thoracoplasty using absorbable rib substitutes (BioBridge®), after local surgical control. BioBridge® is a copolymer made of a polylactide acid blend (70% L-lactic acid y 30% DL-lactide). RESULTS: In 2 years, we had three patients with malignant chest wall tumors. Resection margins were negative, without recurrence at follow-up. We achieved good cosmetic and functional results, and no postoperative complications. CONCLUSION: Alternative reconstruction techniques such as absorbable rib substitutes provide protection, guarantee a flexible chest wall, and do not interfere with adjuvant radiotherapy. Currently, there are no management protocols in thoracoplasty. This option represents an excellent alternative for patients with chest wall tumors. Knowledge of different approaches and reconstructive principles is essential to offer children the best onco-surgical option.

SUMMARY OBJECTIVE: This study aimed to evaluate the effectiveness of cryotherapy in elderly patients with rib fractures due to blunt thoracic trauma. METHODS: In this prospective randomized controlled study, geriatric patients were assigned to groups to receive either coolant spray (n=51) or placebo spray (n=50). The visual analog scale scores of all patients were recorded before starting spray application (V0), as well as at 10th (V1), 20th (V2), 30th (V3), 60th (V4), 120th (V5), and 360th (V6) minute. The mean decreases in the visual analog scale scores were calculated. RESULTS: The differences between V0 and V1, V0 and V2, V0 and V3, and V0 and V4 mean visual analog scale scores measured in the coolant spray group were found to be significantly higher (p<0.001). In V1, V2, V3, and V4 measurements, the incidence of "clinical effectiveness" in the coolant spray group was significantly higher than in the placebo group (p=0.001). CONCLUSIONS: Coolant spray therapy can be used as a component of multimodal therapy to provide adequate analgesia due to rib fractures in geriatric patients.

A new miniature species of Characidium is described from the midde rio Guaporé, rio Madeira basin, Rondônia, Brazil. The new species can be readily distinguished from all congeners by the sexual dichromatism, with females having narrow dark bars on body absent in males, and by the presence, in both sexes, of a black midventral dashed line extending from area between contralateral pectoral fins to at least the anal-fin origin. It can also be diagnosed from congeners by having strongly tricuspid teeth, with well developed and similar sized cusps in the premaxilla and outer series of dentary, and short lateral line with 6­8 perforated scales. Morphological novelties to Characidium observed in the new species include a rounded process on the first pleural rib near the vertebra, three or four haemal spines of the first caudal vertebrae distinctly elongate, neural and haemal spines of the antepenultimate vertebra not reaching origins of dorsal and ventral procurrent rays nor the origin of the caudal-fin rays.(AU)


Uma nova espécie miniatura de Characidium é descrita do médio rio Guaporé, bacia do rio Madeira, Rondônia, Brasil. A nova espécie pode ser facilmente distinguida de todas as congêneres pelo dicromatismo sexual, com barras escuras estreitas no corpo das fêmeas ausentes nos machos, e por ter uma linha preta mediana ventral tracejada em ambos os sexos, que se estende da área entre as bases das nadadeiras peitorais até pelo menos a origem da nadadeira anal. A espécie também pode ser diagnosticada das congêneres por ter dentes fortemente tricúspides, com cúspides bem desenvolvidas e de tamanho semelhante no pré-maxilar e série externa do dentário, e linha lateral curta com 6­8 escamas perfuradas. Novidades morfológicas para Characidium observadas na nova espécie incluem presença de um processo arredondado na primeira costela pleural próximo à vértebra, três ou quatro espinhos hemais das primeiras vértebras caudais distintamente alongados, espinhos neurais e hemais da antepenúltima vértebra não atingindo as origens dos raios procorrentes dorsal e ventral, nem a origem dos raios da nadadeira caudal.(AU)

ANTECEDENTES: la nariz mestiza constituye un reto en la adecuada trasformación estética y funcional de la rinoplastia, más aún cuando esta se realiza en una segunda intervención y los tejidos nasales ya se encuentran alterados, no pudiendo hacer una adecuada reconstrucción de las estructuras anatómicas sin el uso de injertos cartilaginosos de otras áreas del cuerpo. OBJETIVO: el propósito de este trabajo es de evaluar los resultados obtenidos de una reconstrucción de la punta nasal, realizada en pacientes que ya fueron sometidos a una rinoplastia sea esta secundaria o cuaternaria. METODOLOGIA: estudio retrospectivo, de casos de pacientes que se sometieron a una nueva intervención para mejorar aspectos estéticos y funcionales nasales, que ya fueron operados en más de 1 oportunidad. HALLAZGOS: se tiene un universo de 10 pacientes en los que se realizó una rinoplastia secundaria o cuaternaria donde se utilizó injerto costal para conformar el esqueleto cartilaginoso y el uso de un injerto de extensión septal, todo esto para mejorar la estética y funcionalidad nasal.

Vinte e quatro cordeiros cruzados, machos inteiros em terminação, foram utilizados para avaliar a influência da substituição do feno de Tifton 85 por silagem de rama de mandioca sobre o desempenho animal e as características de carcaças. Os animais foram distribuídos nos tratamentos que consistiram em quatro níveis de substituição do feno por silagem de rama de mandioca (0, 33, 67 e 100%) em dietas que continham 60% de concentrado e 40% de volumoso. A silagem de rama de mandioca apresentou menor teor de fibra em detergente neutro (56,81%) que o feno (79,71%), com teor de lignina mais elevado (14,37 e 5,24%, respectivamente), levando a menores valores de degradabilidade efetiva (5%/h) e degradabilidade potencial, além de apresentar maior não degradável. O ganho de peso corporal (302 g/dia), a ingestão de matéria seca (93,79 g/kg 0,75) e a conversão alimentar (4,63) não apresentaram diferenças entre tratamentos. As características de carcaças foram afetadas através da diminuição da área de olho de lombo e espessura de gordura de cobertura com o aumento da substituição do feno de Tifton 85 pela silagem de rama de mandioca. Conclui-se que a silagem de rama de mandioca tem valor nutritivo inferior, com maior quantidade de fibra lignificada do que o feno, prejudicando a terminação das carcaças dos cordeiros, mas sem afetar o desempenho dos animais.


Twenty-four crossbred lambs, complete males in finishing, were used to evaluate the replacement of Tifton 85 hay by cassava foliage silage on animal performance and carcass characteristics. The animals were distributed in treatments that consisted of 4 levels of replacement of hay by cassava foliage silage (0, 33, 67 and 100%) in diets containing 60% concentrate and 40% roughage. Cassava foliage silage had lower neutral detergent fiber content (56.81%) than hay (79.71%), with higher lignin content (14.37 and 5.24%, respectively), leading at lower values of effective degradability (5%/h) and potential degradability, with greater non-degradable residue for cassava foliage silage. Body weight gain (302 g/day), dry matter intake (93.79 g/kg0.75) and feed conversion (4.63) did not differ between treatments. The carcass characteristics were affected, with a decrease in rib eye area and cover fat thickness with an increase in the replacement of Tifton 85 hay by cassava foliage silage. Cassava silage has lower nutritional value, with a higher amount of lignified fiber than hay, damaging the finishing of the lamb carcasses, but without affecting the performance of the animals.

Se presenta el caso clínico de un paciente de 47 años de edad, quien acudió al Servicio de Ortopedia y Traumatología del Hospital Docente Clinicoquirúrgico Joaquín Albarrán de La Habana por presentar dolor en el cuello irradiado al miembro superior izquierdo. En la exploración física se destacó atrofia de los músculos romboides de la escápula izquierda y deformidad alar, acentuada durante la maniobra. Se realizó radiografía y electromiografía; en la primera, se observó costilla cervical supernumeraria izquierda y, en la segunda, lesión del nervio dorsal escapular, por lo cual se diagnosticó síndrome del nervio dorsal escapular secundario a síndrome del escaleno anterior. Se indicó tratamiento fisioterapéutico y el paciente evolucionó satisfactoriamente.


The case report of a 47 years patient is presented, who went to the Orthopedics and Traumatology Service of Joaquín Albarrán Teaching Clinical Surgical Hospital in Havana due to a pain in the neck irradiated to the left superior member. In the physical exploration atrophy of the left scapula romboid muscles and alar deformity were notable, accented during the maneuver. Radiography and electromyography were carried out; in the first one, left supernumerary cervical rib was observed and, in the second one, dorsalscapular nerve leison, reason why the secondary dorsal scapular nerve syndrome to anterior scalene syndrome was diagnosed. Physiotherapeutic treatment was indicated and the patient had a favorable clinical course.

INTRODUCTION: Tuberculosis (TB) is an infectious, transmissible and immune disease caused by the Mycobacterium tuberculosis-complex (MTBC). Although osteoarticular tuberculosis (OATB) has been widely described, the ribcage variety remains a rare form. CASE REPORT: A thirteen-month-old male and a twenty-month-old female, both with pain and increased volume of anterolateral left rib cage were described. Physical examination revealed the presence of a soft consistent mass at the level of the 9th and 5th costal arches in the male and female patients respectively. Upon clinical evaluation, tuberculosis was suspected, which was confirmed by X-ray and histopathological studies. After confirmation, the management, based on anti-tuberculosis therapy was started as follows: nine months of anti-tuberculosis therapy for the male patient and fourteen months for the female. The outcomes were favorable for both patients. However, further interventions, consisting of abscess drainage in the male patient and excisional biopsy in the female patient were necessary. With these therapeutic interventions, to date, the patients are without any evidence of active TB.

BACKGROUND: True neurogenic thoracic outlet syndrome (TNTOS) is rare, and evaluation of surgical treatment is limited to a few studies in the literature. The purpose of this study is to present the results from a surgical series of 21 patients with TNTOS. METHODS: Retrospective analysis on 21 patients diagnosed with TNTOS who underwent surgery. Demographic data and neurological status were characterized, and patients were classified in accordance with a pre-established scale for assessing the severity of hand impairment before and after surgery. Neuropathic pain was assessed using a visual analogue scale (VAS) and functional disability was quantified using the QuickDASH questionnaire. The results from before and after surgery were compared using the Wilcoxon test, and the significance level was taken to be 5%. RESULTS: There was a significant difference in VAS values from before to after the operation (Wilcoxon test: p = 0.0001; r = 0.86). Most patients (90%) improved after surgery, and in 85% of these patients, the VAS improvement was greater than 50%. Improvement in hand function occurred in seven patients (33.3%), and in most of these cases (28.6%), this improvement was classified as mild. Most patients (93.3%) showed moderate to very severe functional disability at the end of the follow-up. CONCLUSION: After surgery, only one-third of the cases showed improvement in motor function and most patients had significant functional disability. However, the improvement regarding pain was significant. Surgery to control this symptom should be recommended, even in cases of late presentation and severe motor impairment.

Chest compression is a parameter of injury criteria assessment for human beings. Additionally, it is used to find the external compression response as a result of vehicle crashes, falls, or sporting impacts. This behavioral feature is described by many deterministic models related to specific experimental tests, hindering distinct scenarios. The present study evaluates the energy absorbed as a function of rib compression. The proposed model was obtained from three different computed tomography (CT) studies. The anthropometric values are interpolated to obtain a parametric curve for a human rib's average size. The computed results are compared against an STL-DICOM® file used to obtain a virtual reconstruction of one rib. A numerical model of the behavior of the thorax displacement expressed injury in the human rib model's stiffness. The proposed model is used to determine the correlation of the input payload versus the numerical stiffness value. The outcome is confirmed by numerical analyses applied to a virtual human rib reconstruction.

RESUMO O manejo deficiente das secreções é uma das complicações mais frequentes em pacientes em ventilação mecânica invasiva. A depuração das secreções por meio da fisioterapia respiratória é um aspecto crítico do tratamento desses pacientes. A compressão torácica manual é uma das técnicas de fisioterapia respiratória mais praticadas em pacientes ventilados, mas seu impacto nos desfechos clínicos permanece controverso devido a questões metodológicas e ao pouco conhecimento sobre sua ação. Nesta revisão, apresenta-se uma análise detalhada dos princípios físicos envolvidos na execução da técnica de compressão torácica. Também investigam-se os efeitos fisiológicos observados em estudos experimentais e clínicos, que mostram que o uso de compressão torácica curta e vigorosa, baseada no aumento de fluxos expiratórios (diferença de fluxo aéreo inspiratório-expiratório > 33L/minuto), pode melhorar o movimento do muco em direção à glote. Por outro lado, o uso de compressão torácica suave e gradual ao longo de toda a fase expiratória não afeta os fluxos expiratórios, resultando em efeitos ineficazes ou indesejados em alguns casos. Mais estudos fisiológicos são necessários para entender os princípios da técnica de compressão torácica em pacientes ventilados. No entanto, de acordo com as evidências, a compressão torácica tem mais benefícios potenciais do que riscos, o que incentiva sua implementação.


ABSTRACT Defective management of secretions is one of the most frequent complications in invasive mechanically ventilated patients. Clearance of secretions through chest physiotherapy is a critical aspect of the treatment of these patients. Manual rib cage compression is one of the most practiced chest physiotherapy techniques in ventilated patients; however, its impact on clinical outcomes remains controversial due to methodological issues and poor understanding of its action. In this review, we present a detailed analysis of the physical principles involved in rib cage compression technique performance, as well as the physiological effects observed in experimental and clinical studies, which show that the use of brief and vigorous rib cage compression, based on increased expiratory flows (expiratory-inspiratory airflow difference of > 33L/minute), can improve mucus movement toward the glottis. On the other hand, the use of soft and gradual rib cage compression throughout the whole expiratory phase does not impact the expiratory flows, resulting in ineffective or undesired effects in some cases. More physiological studies are needed to understand the principles of the rib cage compression technique in ventilated humans. However, according to the evidence, rib cage compression has more potential benefits than risks, so its implementation should be promoted.

OBJECTIVE: To investigate the effects of Erector Spinae Plane Block (ESPB) and Retrolaminar Block (RLB) on intra- and postoperative analgesia in patients with Multiple Rib Fractures (MRFs). METHODS: A total of 80 MRFs patients were randomly divided into the ESPB (Group E) and RLB (Group R) groups. After general anesthesia, ESPB and RLB were performed under ultrasound guidance, respectively, together with 20 mL of 0.5% ropivacaine and Patient-Controlled Intravenous Analgesia (PCIA). RESULTS: Thirty-four cases in Group E and 33,cases in Group R showed unclear paravertebral spaces. The intraoperative dosage of remifentanil (mean ± SD) (392.8 ± 118.7 vs. 501.7 ± 190.0 µg) and postoperative morphine PCIA dosage, (7.35 ± 1.55 vs. 14.73 ± 2.18 mg) in Group R were significantly less than those in Group E; the Visual Analog Scale (VAS) scores in Group R at 2 (2.7 ± 1.2 vs. 3.4 ± 1.4), 4 (2.2 ± 1.1 vs. 2.8 ± 0.9), 12 (2.5 ± 0.9 vs. 3.0 ± 0.8), and 24 hours (2.6 ± 1.0 vs. 3.1 ± 0.9) after surgery were significantly lower than those in Group E. Finally, the normal respiratory diaphragm activity (2.17 ± 0.22 vs. 2.05 ± 0.19), pH (median [IQR] (7.38 [7.31-7.45] vs. 7.36 [7.30-7.42]), and partial pressure of carbon dioxide (PaCO2) (44 [35-49] vs. 42.5 [30-46]) after the operation in Group R were significantly better than those in Group E (p < 0.05). CONCLUSIONS: RLB was a more effective analgesic method than ESPB in the treatment of MRF.

BACKGROUND: The cervical rib can be a rare finding in asymptomatic subjects. When it produces compression, it affects variably the vasculo-nervous structures of the brachial plexus; the clinical history is progressive. The imaging studies indicate the site of osteo-neuro-vascular conflict and electrophysiological studies indicate the degree of neural involvement. CASE REPORT: We present a patient with an eight-month history of paresis and right dysesthesias, with an image of bilateral cervical ribs with different anatomical characteristics. This case discusses how a variant of the anatomy produces pathology and when it is not related to compression.


ANTECEDENTES: La costilla cervical puede ser un raro hallazgo en sujetos asintomáticos. Cuando produce compresión, afecta variablemente a las estructuras vasculonerviosas del plexo braquial. El cuadro clínico es progresivo. Los estudios de imagen indican el sitio de conflicto osteoneurovascular, y los electrofisiológicos, el grado de afección neural. CASO CLÍNICO: Presentamos un paciente con historia de 8 meses de paresia y disestesias derechas, con imagen de costillas cervicales bilaterales con diferentes características anatómicas. Con este caso se discute cómo una variante de la anatomía produce patología y cuando no está relacionada a compresión.

BACKGROUND: The present study explored the role of continuous erector spinae plane (ESP) block for analgesia as well as its impact on pulmonary functions in patients with multiple rib fractures. METHODS: Ten patients with multiple rib fractures were enrolled after getting informed and written consent. Ultrasound-guided ESP block was performed at the level midway between the fractured ribs followed by the insertion of the catheter. Pre- and post-block VAS score, hemodynamics, respiratory rate (RR), peripheral oxygen saturation (SpO2), inspiratory capacity (IC), blood gases (PaO2 and PCO2), and complications were compared. RESULTS: Pain scores at rest as well as on movement showed a significant reduction from 5.9 and 7.5 pre block to 1.6 and 2.5 respectively at 96 hours (p < 0.0001). Similarly, RR, SpO2, IC, and PaO2 were significantly better after the block placement (p < 0.001). CONCLUSION: Continuous ESP block provide adequate analgesia with better respiratory functions in patients with multiple rib fractures.
