RESUMEN

STUDY OBJECTIVES: Common symptoms for patients with narcolepsy can have a significant impact on social health. As one peak for symptom onset is adolescence, these symptoms impact social relationships during a critical developmental period. Much of the existing literature in this domain has relied on broad questionnaires, with less insight into the nuances of patients' potential social struggles. METHODS: Adolescents (aged 12-17 years) with narcolepsy and their parents individually completed a semistructured interview (n = 14 dyads). Interview transcripts were analyzed using a multistage thematic analysis. RESULTS: An overarching theme was the difficulty adolescents experienced trying to balance narcolepsy symptom management with engaging in social activities in a meaningful way. Narcolepsy affected social relationships in 3 primary domains: mood, physical activities, and driving. Adolescents reported that they were frustrated with feeling as though narcolepsy sometimes defined their social lives. Adolescents and parents expressed a desire for medical providers to better understand their evolving priorities, to validate their social limitations, and to provide more information around the social implications of narcolepsy and its treatment. CONCLUSIONS: Narcolepsy has a significant impact on social relationships in adolescents, one that is not adequately managed in current clinical care models. A routine, structured assessment of social health is a vital first step for providers treating adolescents with narcolepsy. Medical centers and patient organizations can play an important role in facilitating social opportunities for this underserved population. BACKGROUND: Across a range of studies, health scientists have found that being in a romantic relationship can have positive and negative influences on one's health. A couple's health outcomes are often influenced by relationship quality-or how they perceive the positive or negative character of their relationship. These findings have important implications for how scientists and interventionists may leverage romantic relationships facilitating good health among couples. However, in general, couples research has not included Black same-sex male couples in large enough numbers to make previous studies' findings relevant to them. This represents a gap in the scientific literature and, more importantly, a missed opportunity to understand how romantic relationships influence health for a group that must navigate distinct, multilevel health and social inequities. OBJECTIVE: This study aims to (1) decode and understand the ways in which Black same-sex male couples express their romantic relationships in virtual contexts via symbolic indicators, (2) determine how Black same-sex male couples describe the quality of their romantic relationships, and (3) explore how Black same-sex male couples make meaning of their relationship quality and its impact on their relational and individual health. METHODS: We will use joint dyadic interviews embedded within a symbolic netnography research design to accomplish our aims. We will use grounded theory to analyze our qualitative data. We will then triangulate our findings to determine how well they answer our research questions. RESULTS: This study received ethical approval on October 8, 2020 and we began data collection in November 2020. Results are expected to be available no later than December 31, 2022. CONCLUSIONS: This study will apply novel symbolic netnographic qualitative methods to further our understanding of Black same-sex male couples' romantic relationships and how they contribute to their health. The findings will be used to develop programs to improve Black same-sex male couples' health in community and virtual settings. The Welch Emotional Connection Screen (WECS) is a novel instrument that is a brief, practicable, evidenced-based observational screening tool for assessing relationship health between parent and child. The WECS requires observing 2-3 min of face-to-face interactions between parent and child, without toys, prompts, paradigms or technology. Here, we describe a translational project from the coding lab to the primary care provider via a residency training program conducted with 50 residents during a 30-day developmental and behavioral pediatrics medical resident education rotation. The aims of this study were to evaluate the efficacy of WECS pediatric resident training: 1) to improve residents' accuracy in recognizing the dyadic behaviors of emotional connection (EC) via WECS training; and 2) to improve residents' attitudes, self-efficacy, and perceived professional norms (ASPPN) pertaining to Early Relational Health in Pediatrics. Results indicate that using a rapid prototyping approach to training, residents improved in their identification of dyads showing low to midrange levels of emotional connection. As well, resident attitudes about the importance of relationship health in pediatrics and their self-efficacy in identifying emotional connection improved significantly after this brief resident training.

O presente estudo teve como objetivo conhecer e analisar, na perspectiva de médicos e pacientes, a relação de cuidado durante a internação hospitalar. A pesquisa se configurou como quanti-qualitativa, exploratória-descritiva, sendo realizada na Clínica Médica de um Hospital Universitário, em Cuiabá ­ MT. Foram entrevistados 15 pacientes, de ambos os sexos, de 40 a 59 anos, com diagnósticos cardiovasculares, que estiveram hospitalizados durante o segundo semestre de 2013; e nove médicos residentes, atuantes na clínica médica do referido hospital. A coleta de dados se deu por questionário e entrevista semi--estruturada. O processamento dos dados ocorreu pelo software ALCESTE, e a análise foi fundamentada por resultados de pesquisas com propósitos semelhantes, a partir de revisão da literatura. Concluiu-se que médicos e pacientes têm boa relação, mas elencam aspectos que podem melhorar. Têm expectativas diferentes quanto ao que seria uma relação ideal no cuidado ­ médicos priorizam aspectos técnicos, enquanto pacientes equivalem esses aos aspectos afetivos, concordando que a melhora estaria relacionada ao tempo e disponibilidade do médico. Como forma de superação do modelo biomédico, ainda dominante no hospital investigado, destaca-se a medicina centrada no paciente, no âmbito das ciências médicas, e a Abordagem Centrada na Pessoa, proposta psicológica desenvolvida por Carl Rogers (1902-1987).


The present study aims to identify and analyze, from the perspective of physicians and patients, the care relationship during hospitalization. The survey was configured as quantitative-qualitative, exploratory, descriptive, being held at the Medical Clinic of a University Hospital in Cuiabá - MT. 15 patients were interviewed, of both sexes, aged 40 to 59 years with cardiovascular diagnoses who were hospitalized during the second half of 2013; residents and nine physicians, working in the medical clinic of the hospital. The collection of data was a questionnaire and a semi-structured interview. The processing of data was carried ALCESTE software, and analysis was supported by research findings with similar purposes, from literature review. It was concluded that physicians and patients have a good relationship, but this papers presents ways this relationship could be improved. There are different expectations as to what would be an ideal relationship in care - physicians prioritize technical aspects, and patients feel that affection is as important as medical expertise and agree that the improvement is related is to time and availability of the doctor. As a way of overcoming the biomedical model, still predominated in the hospital in question, there is the patient-centered medicine within the medical sciences, and the Person Centered Approach, psychological approach developed by Carl Rogers (1902-1987).

AIMS: Systemic lupus erythematosus (SLE) mostly affects young women, adversely affecting their quality of life (QOL). Caregivers may experience caregiver burden (CGB), and it may lower the quality of their relationship. Herein we studied caregiving and CGB and their effects on QOL and relationships in SLE. METHODS: We recruited 10 dyads from the Lupus Clinic. Data collected included demographics, CGB (CGB Scale, screen for CGB), QOL (SF-36) and the quality of the dyadic relationship (Dyadic Adjustment Scale (DAS)). We calculated correlation coefficients for associations between (i) CGB and (ii) dyadic QOL or DAS. RESULTS: The mean (± SD) age of SLE patients was 35.2 (± 9) years and of caregivers was 37.3 (± 9.64) years. The mean (± SD, min-max) total CGB score was 9.1 (± 5.8, 0-19). The caregiver's QOL correlated strongly with some of the domains of the patient's QOL. The SLE-related CGB was associated with the caregiver's own QOL and their SLE partner's QOL. The dyadic DAS was linked to the patient's QOL. CONCLUSIONS: Because (i) CGB in SLE is associated with the caregiver's own QOL and with their SLE partner's QOL, and because (ii) the dyadic DAS score is linked primarily to the patient's QOL, then to optimize patient health outcomes and to decrease CGB, focus should be not only on the patient but should include the dyadic unit.Significant findings: To optimize patient outcomes of SLE patients, focus should be on the dyadic unit. CGB in SLE is associated with the caregiver's own QOL and with the SLE partner's QOL, making it crucial to study this relationship in more detail.

Com a compreensão do cuidado ao paciente não restrita à atividade médica,mas envolvendo também outros profissionais de saúde como dentista, enfermeiro,psicólogo, nutricionista, assistente social e fisioterapeuta em unidades de atenção àsaúde, a proposta do artigo é realizar uma reflexão sobre como o contexto social epolítico de uma época pode influenciar a forma do relacionamento do profissional desaúde com seus pacientes. Isto porque o Zeitgeist, o espírito da época com seu pensamentofilosófico e cultural estruturam as concepções de saúde e doença. Através de uma revisãohistórica, chega-se a modelos de relacionamento entre profissionais e paciente. Aconstrução de uma antiga prática de cuidado à saúde até a atualidade é sujeita à influênciahistórica, e como recorte estabelece-se aqui um segundo objetivo do artigo: realizarbreve apresentação sobre a forma de olhar o cuidado na saúde na Psicologia Analítica deC. G. Jung. Este autor considerava os aspectos sociais e culturais, atuais e ancestrais,na busca da compreensão do sujeito nas características biológicas, psicológicas, sociaise também transdisciplinares. A perspectiva junguiana ressalta a atenção do profissionalde saúde com os fenômenos da transferência e contratransferência, sendo que a análiseeducativa pode auxiliar esse processo. A atenção integral ao sujeito favorece o vínculoterapêutico, que auxilia os cuidados de sua saúde(AU)


With the understanding that patient care is not restricted to the medical activity,but also envolving other the medical health professionals such as dentist, nurse,psychologist, nutritionist, social worker and therapist in units of health care, the articleproposes a reflection on how the social and political context of an era can influence theway of relationship of health professionals with their patients. This is because of theZeitgeist, the understanding of those times, with its philosophical and cultural thoughts,who structured the concepts of health and disease. Through a theoretical historicalreview we study models of the relationship between the health professional and thepatient. The construction of a former practice of health care until the present date is subject to a historical influence, and as the second objective of the article: a briefpresentation on how to look at health care in the Analytical Psychology of C. G. Jung.This author considered the social and cultural aspects, bothly the actual and formerones, in search of understanding the individual in the biological, psychological, socialand transdisciplinary aspects. The Jungian perspective emphasizes the attention ofhealth professionals with the phenomena of transference and countertransference, andthe analysis of education can assist in this process.
