RESUMEN

The authors report a case of endobronchial migration of a POD packing coil following embolization of a pulmonary artery pseudoaneurysm in a patient with cavitary tuberculosis and its successful management by bronchoscopy-assisted removal of the coil. Coil migration may be secondary to continued cough and persistence of a bronchial to pulmonary artery fistula from tuberculous infection and inflammation.

RESUMEN

CT-guided lung biopsy is a widely used procedure for tissue identification. The complications are divided into minor and major with the latter being described as low rate. Hemothorax is reported at a rate of 0.092% and predominantly results from the injury of intercostals or internal mammary arteries. We present the case of 81-year old woman with a right upper lobe mass referred for a CT-guided biopsy. Four hours after the procedure, rapid deterioration of patient's status was observed. A massive hemothorax was reported due to the transection of an intratumoral pulmonary branch. The following management involved successful emergent embolization of the injured branch of the pulmonary artery using a combination of coils and gel foam. One of the theories possibly explaining this extremely rare complication involves the possibility of underlying pulmonary hypertension.

RESUMEN

The pinch-off syndrome is defined as the embolization of the central catheter inserted via the subclavian approach due to the mechanical compression between the clavicle, the first rib, the subclavius muscle, and the costoclavicular ligament. Embolization to the pulmonary artery is an extremely rare condition. In this article, we present a rare case with ovarian cancer who had multiple metastases both locally and lungs, the port catheter was fractured into three parts, the proximal part was removed, the middle part was left between the subclavian vein and the skin, and the long distal part was embolized to the pulmonary artery.

RESUMEN

N-Butyl cyanoacrylate embolization of peripheral pulmonary artery aneurysms has rarely been performed. This report presents a young woman with a history of patent ductus arteriosus and massive hemoptysis secondary to pulmonary artery aneurysm, successfully managed by embolotherapy using n-Butyl cyanoacrylate. This is the first report of n-Butyl cyanoacrylate embolization treatment of a bleeding pulmonary artery aneurysm in the setting of patent ductus arteriosus. The pertinent literature has been reviewed and the clinical background and the procedure have been explained.

RESUMEN

An emergency thoracic endovascular aortic repair (TEVAR) with zone 2 landing without revascularization of the left subclavian artery was performed due to the impending rupture of a distal arch aneurysm in an old patient presenting hemoptysis. Two months later, the patient had recurrent massive hemoptyses and continued after additional zone 0 TEVAR. The lung parenchyma was considered to be the bleeding source and transcatheter pulmonary artery embolization was performed, and the episodes of massive hemoptysis appeared to have ceased. However, the patient died of sudden recurrent massive hemoptysis 40 days later. Inflammation and/or infection of the lung parenchyma adjunct to the aortic aneurysm could be cause of fatal hemoptysis, and aggressive therapy such as lung resection should be considered in such patients.
