RESUMEN

BACKGROUND: This study aimed to introduce a potential alternative percutaneous treatment for AO types C1, C2, and C3 distal radius fractures using dual-external fixator (a no-bridging cemented-pin frame and a conventional wrist-bridging external fixator). MATERIALS AND METHODS: From January 2018 to January 2021, 52 patients (52 distal radius fractures) were treated with dual-external fixator. For comparison, 61 patients (61 distal radius fractures) were treated with a plate and screw system. Wrist function was assessed using the Mayo Wrist Score. Patient satisfaction was assessed using the Short Assessment of Patient Satisfaction. A P < 0.05 was considered statistically significant. RESULTS: Fracture healing was achieved in all patients. At the final follow-up of 29 months (range, 24-34 months) vs 36 months (range, 26-39 months) (P > 0.05), the patients treated with dual-external fixator and a plate and screw system achieved mean ulnar deviations of 31° vs 29° (P < 0.05), mean Mayo Wrist Scores of 91.12 ± 5.98 vs 88.12 ± 7.54 (P < 0.05), and mean patient satisfaction scores of 23.42 ± 2.47 vs 23.04 ± 2.32 (P > 0.05). CONCLUSIONS: AO types C1, C2, and C3 distal radius fractures can be treated successfully using dual-external fixator. The technique is a potential alternative in addition to the conventional treatments. LEVEL OF EVIDENCE: Level IIa.

Three-dimensional carbon nanosheets (3D-CNS) were synthesized by salt template spray-drying method in order to solve the agglomeration of 2D nanocarbon by a traditional mixing method. MgB2 bulks doped with 3D-CNS with molar ratio composition of MgB2-x(3D-CNS)x (x = 0, 0.1 and 0.2) have been prepared by in situ sintering process. The microstructure, critical current density and flux pinning of the sintered samples have been investigated. Differing from the structure in previous studies, the 3D-CNS doping is more efficient for the refinement of the MgB2 grains due to the 3D network structures. The results clearly show that more active C releasing from 3D-CNS at high temperature can provide effective flux pinning centers by the substitution of C for B in MgB2 lattice. Furthermore, the lattice distortion and increased grain boundaries should be responsible for the enhancement of critical current density (Jc) at high magnetic fields as well as the increased irreversible magnetic field (Hirr). However, the positive action in Jc at low field has been extremely offset by the concentration of impurities at MgB2 grain boundaries such as released extra C without substitution and MgO, which is considered to further deteriorate the grain connectivity.

BACKGROUND: This retrospective study included an alternative treatment for types A2, A3, and B1 distal radius fractures using percutaneous fixation with a cemented K-wire frame. METHODS: From January 2017 to January 2020, 78 patients with distal radius fractures were treated with percutaneous internal fixation using a cemented K-wire frame. There were 47 male patients and 31 female patients. The fractures were classified into types A2 (n = 10), A3 (n = 46), and B1 (n = 22). X-rays were taken immediately after surgery and after the bone had healed. Wrist function was assessed using the Mayo Wrist Score (90-100, excellent; 80-90, good; 60-80, satisfactory; < 60, poor). Patient satisfaction was assessed using the 10-cm visual analog scale. RESULTS: Neither fixation failure nor K-wire migration was found (P > 0.05). Osteomyelitis was not observed in this series. All patients achieved bone healing after a mean of 4.5 weeks (range, 4 to 8 weeks). Follow-up lasted a mean of 27 months (range, 24 to 33 months). The mean score of wrist function was 97 (range, 91 to 100). Among them, 66 results were excellent and 12 results were good. The mean patient satisfaction was 10 cm (range, 8 to 10 cm). CONCLUSIONS: Percutaneous fixation with cemented K-wire frame is a safe and preferred choice for the treatment of types A2, A3, and B1 distal radius fractures. The frame provides support to prevent wire migration. The fixation technique is a minimally invasive procedure that is easy to perform. LEVEL OF EVIDENCE: Therapeutic study, Level IVa.

Animal posture, limb gait, and body weight-bearing in dogs can be evaluated by limb gait score. In this study, nine adult male dogs of local breed were used to induce a complete transverse femoral mid-shaft fracture fixed internally by intramedullary pining under general anesthesia with the aseptic technique and followed for 60 days postoperative. The data were analyzed, including five degrees that indicate the correlation among animals' posture, limbs, and body weight tolerance per day. The results demonstrated that in grade 5, the animals completely hold the affected limb during standing position 1st-3rd day postoperative. From the 4th-10th day postoperative, in grade 4, the animals supported the body weight on the affected limb in the standing position and hold it during waking. In grade 3, from 11th-21th day postoperative, the operated animals supported body weight on the affected limb in slow waking and hold it in speed waking and running. From the 22nd-28th day postoperative, in grade 2, the animals used the limb in speed walking, from the 29th-35th day postoperative. In grade1, the animals could use the limb in running from the 36th-2nd day postoperative. In grade 0, the animals used the limb normally in walking, running, and jumping until the end of experimental periods (60th day postoperative). The results indicated that limb gait scores in dogs can be used for the evaluation of the relationships among animals' posture, limb gait, and body-weight bearing per day during bone regeneration processing of femur fracture fixed by intramedullary pining.
