Numerous innovative decentralised urban water solutions have been described over many years, yet their application in practice is still not common at all. While many proposed solutions may have some techno-economic advantages over current systems, the real reasons for the slow uptake have more to do with system-wide inertia and technology 'lock-in' where existing solutions are preferred for simplicity and familiarity. A key factor is also the inadequate assessments in project decision making processes that should consider all relevant social, environmental and economic benefits and values. This paper highlights some key barriers and how to address them in a more holistic way. It also identifies opportunities where more integrated, hybrid solutions could offer significant benefits over current technologies. It calls on all key partners in this sector to foster broad and strong collaborations, and on water service providers to be empowered to take an inclusive leadership role in creating such innovative solutions that help address our growing challenges driven by rapid urbanisation and climate change.

RESUMEN

The World Health Organization Commission on Social Determinants of Health (SDH) places great emphasis on the role of multi-sector collaboration in addressing SDH. Despite this emphasis on this need, there is surprisingly little evidence for this to advance health equity goals. One way to encourage more successful multi-sector collaborations is anchoring SDH discourse around 'sustainability', subordinating within it the ethical and empirical importance of 'levelling up'. Sustainability, in contrast to health equity, has recently proved to be an effective collaboration magnet. The recent adoption of the Sustainable Development Goals (SDGs) provides an opportunity for novel ways of ideationally re-framing SDH discussions through the notion of sustainability. The 2030 Agenda for the SDGs calls for greater policy coherence across sectors to advance on the goals and targets. The expectation is that diverse sectors are more likely and willing to collaborate with each other around the SDGs, the core idea of which is 'sustainability'.

Chronic malnutrition, including stunting, is an important example of a global challenge that spans multiple sectors, specifically health, agriculture, and the environment. The objective of this paper is to review current knowledge on the causes and consequences of chronic malnutrition and their relationship with multiple sectors. Understanding the causes includes approaching chronic malnutrition from the basic, underlying, and immediate levels. The causes reach from macro-level environmental influences to specific micronutrient intake. In order to effectively address stunting, it is important to understand the timing of stunting and the ability of individuals to catch up in terms of linear growth, cognitive ability, and immune function. The consequences of chronic malnutrition are transgenerational and they have an impact at the individual, community, and national level in the short- and long-term. There are still many gaps in knowledge regarding both the causes and consequences of chronic malnutrition, particularly when it comes to the interaction with agriculture and the environment, and understanding these gaps is important to addressing the burden of chronic malnutrition through evidence-based interventions.
