RESUMEN

This paper examines the dosimetric uncertainty arising from the use of thermoplastic masks in the treatment of head and neck cancer through radiotherapy. This study was conducted through Monte Carlo simulations using the Monte Carlo N-Particle eXtended (MCNPX code), and the theoretical results are compared with radiochromic films. Using material characterization techniques, the compounds of the thermoplastic mask were identified, confirming that most of the material corresponds to the polymer C10H16O4. The theoretical results show increases ranging from 42% to 57.4% in the surface absorbed dose for 6 and 15 MV photon beams, respectively, compared to the absorbed dose without the mask. The experimental data corroborate these findings, showing dose increases ranging from 18.4% to 52.1% compared to the expected surface absorbed dose without the mask. These results highlight the need to consider the bolus effect induced by thermoplastic masks during the precise and safe planning and application of radiotherapy treatment in order to ensure its therapeutic efficacy and minimize the associated risks to patients.

This work presents an exhaustive visualization of the airflow expulsed by a person while breathing, talking, exhaling, and blowing inside a closed room wearing a disposable face mask like those used in hospitals for patient protection and those who care for them. An optical schlieren experimental arrangement was used to obtain some of the relevant physical characteristics of the airflow, such as its refractive index gradient, the distribution of temperature, and the associated velocity field for all the tests developed. We tested three face masks, one of the surgical types and the others of the N95 series with denominations KN95 and 3MN95 (Aura TC-84A-8590). The results show appreciable differences between the masks evaluated; the surgical mask was the one that allowed the most abrupt output airflow through it in the field of view of the experimental setup. However, were also found some differences in the performance of the KN95 and 3MN95 masks. The KN95 face mask had the best performance since it expulsed to its surroundings the lowest airflow with different physical properties to the input airflow. The results obtained are relevant since it was possible to estimate the expulsed airflow velocity as a function of the distance for every face mask tested, which allows for understanding its filtering capacity by restricting the flow of potential pathogens from the mouth or nose of one person to another. Undoubtedly, the airflow behavior determination around a face mask can help to reduce the risk of spreading infectious airborne particles.

BACKGROUND: Nurses are essential members of the healthcare workforce and were among the first-line carers for patients in community and hospital settings during the COVID-19 pandemic. As a result, they were at a heightened risk of infection, resulting in several reported deaths among nursing staff. Several preventive measures were adopted to contain the spread of the COVID-19 virus. This study aims to explore the knowledge, attitudes, and practices (KAP) of nurses regarding hand hygiene, mask wearing, and social distancing measures in healthcare settings in Barbados during the COVID-19 pandemic. METHOD: An online survey of nurses working in public hospitals and polyclinics (public primary care clinics) in Barbados from March 2021 to December 2021 was conducted. A nonsystematic convenience sampling method was employed to recruit nurses who were readily available and willing to participate. A questionnaire captured the sociodemographic information and knowledge and practices related to hand hygiene, the use of face masks, and social distancing. Each correct response received one mark. Overall knowledge scores were categorized as poor (<60%), average (60-80%), or good (>80-100%). RESULTS: Of the 192 participants, the majority were female (82.8%) and had >5 years of experience (82%). The findings revealed that 45.8% had poor knowledge of hand hygiene, and that the knowledge of 43.8% of respondents was average. Multivariable logistic regression showed that, after adjustment for age and gender, registered nurses had 2.1 times increased odds (95% confidence interval 1.0, 4.2) of having good knowledge compared to other nursing categories. Regarding mask wearing, 53.6% of nurses had average knowledge, and 27.1% had good knowledge. Multivariable logistic regression showed that, after adjustment for age and gender, registered nurses had 3.3 times increased odds (95% confidence interval 1.5, 7.4) of having good knowledge compared to nursing assistants. A total of 68.6% of respondents followed the correct steps of handwashing every time, and 98.3% wore a mask in public places. More than half of the nurses (51.2%) kept a safe distance from others to avoid spreading SARS-CoV-2; one-third were in a crowded place(s) in the past three months, and 55.8% usually followed guidelines for social isolation as recommended by the WHO. CONCLUSIONS: The study identified knowledge deficiencies related to hand hygiene and wearing masks among nurses. It is imperative to provide additional training on infection control measures.

OBJECTIVE: This study explored the barriers and facilitators of mask-wearing behaviors during the pandemic in Taiwan, the United States, the Netherlands, and Haiti. METHODS: Face-to-face interviews were conducted in Taiwan and online interviews were conducted with participants in the United States, the Netherlands, and Haiti. RESULTS: In general, the habit of wearing a mask before coronavirus disease 2019 (COVID-19) was reported by Taiwanese participants. Additionally, Taiwanese participants perceived that wearing a mask was a social responsibility during the pandemic, suggesting that the collectivistic context might influence mask-wearing behavior. Unlike the Taiwanese population, some people in the United States and the Netherlands were reluctant to wear masks due to perceived restrictions on their freedom. Participants from Haiti mentioned that people who wore masks encountered violence, bullying, and discrimination. The results of this study suggest that political leadership and mask mandates have a strong impact on people's mask-wearing behavior. CONCLUSIONS: These findings have valuable implications for the design of diverse behavioral interventions to enhance mask-wearing as part of infectious disease preparedness. Additionally, the findings from these countries offer valuable insights for the development of effective public health interventions to enhance society's resilience during the current pandemic and future infectious disease outbreaks.

INTRODUCTION: To guarantee treatment reproducibility and stability, immobilization devices are essential. Additionally, surface-guided radiation therapy (SGRT) serves as an accurate complement to frameless stereotactic radiosurgery (SRS) and stereotactic radiotherapy (SRT) by aiding patient positioning and real-time monitoring, especially when non-coplanar fields are in use. At our institute, we have developed a surface-guided SRS (SG-SRS) workflow that incorporates our innovative open-face mask (OM) and mouth bite (MB) to guarantee a precise and accurate dose delivery. METHODS: This study included 40 patients, and all patients were divided into closed mask (CM) and open-face mask (OM) groups according to different positioning flow. Cone beam computed tomography (CBCT) scans were performed, and the registration results were recorded before and after the treatment. Then Bland-Altman method was used to analyze the consistency of AlignRT-guided positioning errors and CBCT scanning results in the OM group. The error changes between 31 fractions in one patient were recorded to evaluate the feasibility of monitoring during treatment. RESULTS: The median of translation error between stages of the AlignRT positioning process was (0.03-0.07) cm, and the median of rotation error was (0.20-0.40)°, which were significantly better than those of the Fraxion positioning process (0.09-0.11) cm and (0.60-0.75)°. The mean bias values between the AlignRT guided positioning errors and CBCT were 0.01 cm, - 0.07 cm, 0.03 cm, - 0.30°, - 0.08° and 0.00°. The 31 inter-fractional errors of a single patient monitored by SGRT were within 0.10 cm and 0.50°. CONCLUSIONS: The application of the SGRT with an innovative open-face mask and mouth bite device could achieve precision positioning accuracy and stability, and the accuracy of the AlignRT system exhibits excellent constancy with the CBCT gold standard. The non-coplanar radiation field monitoring can provide reliable support for motion management in fractional treatment.

The appearance of skin strongly influences people´s self-esteem and well-being. Among the characteristics that affect the most, we can cite acne and wrinkles. Therefore, minimizing those characteristics by using cosmetics has important value for the consumer. Nowadays, there is a demand for natural ingredients, mainly concerning cosmetic products. The use of clays in beauty care is old, with therapeutic uses since Prehistory. There is an essential advantage in using clays for cosmetic purposes, as it is a low-cost, environmentally friendly, natural, and abundant ingredient, which is chemically inert with a low level of toxicity (when in adequate conditions), easy to apply and remove, dries, and hardens fast. In this research work, we aimed at developing and investigating the effect of formulations containing red, green, and black clays as active components in oiliness reduction, and firmness and elasticity improvement in vivo, as well as characterizing clays mineralogically. All clays presented different compositions. Black clay was the one with the simplest mineralogic content, lowest density, and smallest particle size. It was the richest in Si and Al. Green clay presented expandable smectite and the highest density. Red clay presented the largest particle average size and was the richest in iron content. By thermal analysis, clays presented two characteristic events: the water loss (dehydration) and the dihydroxylation of kaolinite, turning into meta kaolinite. In Sebumetry, clays showed a significant reduction of skin oiliness on the forehead in both concentrations after 2h of contact compared to control, baseline, and placebo. However, after 4h (2h after removal), no significant difference was observed with the control. In the Cutometry, clays did not present significant efficacy in skin firmness and elasticity improvement when compared to control and placebo sites, thus, further studies should be performed comparing the efficacy of different vehicles with dispersed clays. Finally, clay mask formulations were developed with black, red, and green clay dispersed in an oil-free gel vehicle, which proved to be adequate for oily skin


A aparência da pele influencia fortemente autoestima e o bem-estar das pessoas. Dentre as características que mais afetam podemos citar acne e rugas. Portanto, minimizar estas características com o uso de cosméticos tem um valor importante para o consumidor. Atualmente existe uma procura por ingredientes naturais, principalmente no que diz respeito a produtos cosméticos. O uso das argilas nos cuidados de beleza é antigo, com usos terapêuticos desde a Pré-História. Há uma vantagem essencial na utilização de argilas para fins cosméticos, pois é um ingrediente de baixo custo, ecologicamente correto, natural, e abundante, que é quimicamente inerte e tem baixo nível de toxicidade (quando usado em condições adequadas), fácil de aplicar e remover, seca, e endurece rapidamente. Neste trabalho de pesquisa, objetivamos desenvolver e investigar o efeito de formulações contendo argilas vermelha, verde e preta como componentes ativos na redução da oleosidade e na melhoria da firmeza e elasticidade in vivo, bem como caracterizar mineralogicamente as argilas. Todas as argilas apresentaram diferentes composições. A argila preta foi a de conteúdo mineralógico mais simples, mais baixa densidade, e menor tamanho de partícula. Foi a mais rica em Si e Al. A argila verde apresentou esmectita expansível e mais alta densidade. A argila vermelha apresentou o maior tamanho médio de partícula e foi a mais rica em ferro. Segundo a análise térmica, as argilas apresentaram dois eventos característicos: a perda de água (desidratação) e a desidroxilação da caulinita, transformando-se em meta-caulinita. Na Sebumetria, as argilas demonstraram significativa redução na oleosidade da pele na testa em ambas as concentrações após 2h de contato comparada ao controle, basal e placebo. Porém, após 4h (2h após remoção), nenhuma diferença significativa foi observada com o controle. Na Cutometria, as argilas não apresentaram Eficácia significativa na melhora da firmeza e elasticidade quando comparadas aos sítios controle e placebo, portanto, mais estudos devem ser realizados comparando a eficácia de diferentes veículos com argilas dispersas. Finalmente, as formulações de máscaras de argilas foram desenvolvidas com argilas preta, vermelha e verde dispersas em um veículo em gel livre de oleosidade, o qual provou ser adequado para pele oleosa

(1) Background: Mask-associated dry eye (MADE) has been associated with increased dry eye symptoms, apparently due to reduced tear break-up time (TBUT). This study aimed to determine the short-term impact of surgical face mask (FM) on tear film stability by measuring non-invasive tear break-up time (NIBUT). (2) Methods: Twenty-six healthy participants had NIBUT evaluated without FM, with surgical FM and with a surgical FM secured to the skin with adhesive tape (TFM). NIBUT-first was measured with Keratograph 5M (K5M, Oculus, Wetzlar, Germany). Each participant had NIBUT measured in four sessions on four consecutive days. Session 1: without FM vs. with FM. Session 2: with FM vs. without FM. Session 3: without FM vs. with TFM. Session 4: with TFM vs. without FM (3). The time between each measured setting was 2 min. Results: The mean ± SD NIBUT without FM was 8.9 ± 3.7, with FM 10.2 ± 4.1, and with TFM 8.4 ± 3.8 s. No significant differences were observed in NIBUT in any of the evaluated settings: without FM vs. with FM (p = 0.247), without FM vs. with TFM (p = 0.915), and with FM vs. with TFM (p = 0.11). (4) Conclusions: This study did not find a significant short-term effect of FM on NIBUT. Other variables or longer periods of exposure might trigger the symptoms and ocular surface alterations in MADE.

Introduction: In Peru, a high proportion of fully immunized people against COVID-19 has been achieved, which has mitigated the severity of the disease. Thus, under the recommendation of experts, the Peruvian government issued Supreme Decree 118-2022-PCM, which indicated that the use of masks is optional in open and ventilated enclosed spaces as of October 1, 2022. Objective: The aim of the present study was to find out the reasons why people continued to wear masks even though it was not mandatory in outdoor environments. Methods: A total of 335 people participated, with an average age of 26.11 years (73.1% women). A data collection form was prepared where sociodemographic and health information was requested; in addition, the question "Why do you continue to wear mask despite it being optional in open and closed ventilated spaces?" was included. The data obtained from the last open question of the instrument was analyzed by means of a discursive textual analysis (DTA) and an exploratory analysis through IRAMUTEQ. Results: Results indicate that people continue to wear masks, despite the lifting of the restrictions, to avoid contagion of COVID-19 and other respiratory diseases. In addition, the prevention of possible infection and transmission of the virus also focused on the family and vulnerable people. Discussion: It is concluded that the practice of wearing masks may be influenced by individual perceptions of risk and interpretations of responsibility and solidarity towards others.


Introducción: En el Perú, se logró una alta proporción de personas completamente inmunizados contra COVID-19 que ha mitigado la severidad de la enfermedad. Es así que, bajo la recomendación de expertos el gobierno peruano emitió el Decreto Supremo 118-2022-PCM, donde se indicó que el uso de la mascarilla es opcional en espacios abiertos y espacios cerrados ventilados a partir del 1 de octubre del 2022. Objetivo: El presente estudio fue conocer las razones por las cuales las personas continuaron usando mascarilla a pesar de que no era obligatorio en ambientes exteriores. Metodología: Participaron un total de 335 personas con un promedio de edad de 26.11 años (73.1% mujeres). Se elaboró una ficha de recolección de datos donde se solicitó información sociodemográfica y de salud; además, se incluyó la pregunta "¿por qué sigue utilizando la mascarilla a pesar de que es opcional en espacios abiertos y cerrados ventilados?" Los datos obtenidos de la última pregunta abierta del instrumento se analizaron mediante un análisis textual discursivo (ATD) y un análisis exploratorio a través de IRAMUTEQ. Resultados: Los resultados indicaron que, las personas seguían utilizando mascarillas, a pesar de estar levantadas las restricciones, para evitar contagios de la COVID-19 y de otras enfermedades respiratorias. Además, esta prevención de una posible infección y transmisión del virus es enfocada también a la propia familia y personas vulnerables. Discusión: Se concluye que, la práctica de usar mascarillas puede estar influenciada por las percepciones individuales del riesgo y las interpretaciones de responsabilidad y solidaridad frente a los demás.

The chemical formula of the title compound, 2C17H17N4 +·2C7H5O5 -·C17H16N4·2.94C4H8O2, was established by X-ray diffraction of a single-crystal obtained by reacting 1,3-bis-(benzimidazol-2-yl)propane (L) and gallic acid (HGal) in ethyl acetate. The mol-ecular structure can be described as a salt (HL)+(Gal)- co-crystallized with a mol-ecule L, with a stoichiometric relation of 2:1. Moreover, large voids in the crystal are filled with ethyl acetate, the amount of which was estimated by using a solvent mask during structure refinement, affording the chemical formula (HL +·Gal-)2·L·(C4H8O2)2.94. The arrangement of components in the crystal is driven by O-H⋯O, N-H⋯O and O-H⋯N hydrogen bonds rather than by π-π or C-H⋯π inter-actions. In the crystal, mol-ecules and ions shape the boundary of cylindrical tunnels parallel to [100] via R (rings) and D (discrete) supra-molecular motifs. These voids, which account for about 28% of the unit-cell volume, contain disordered solvent mol-ecules.

The world has been greatly affected by the COVID-19 pandemic, causing people to remain isolated and decreasing the interaction between people. Accordingly, various measures have been taken to continue with a new normal way of life, which is why there is a need to implement the use of technologies and systems to decrease the spread of the virus. This research proposes a real-time system to identify the region of the face using preprocessing techniques and then classify the people who are using the mask, through a new convolutional neural network (CNN) model. The approach considers three different classes, assigning a different color to identify the corresponding class: green for persons using the mask correctly, yellow when used incorrectly, and red when people do not have a mask. This study validates that CNN models can be very effective in carrying out these types of tasks, identifying faces, and classifying them according to the class. The real-time system is developed using a Raspberry Pi 4, which can be used for the monitoring and alarm of humans who do not use the mask. This study mainly benefits society by decreasing the spread of the virus between people. The proposed model achieves 99.69% accuracy with the MaskedFace-Net dataset, which is very good when compared to other works in the current literature.

PURPOSE: Investigate whether a cloth facemask could affect physiological and perceptual responses to exercise at distinct exercise intensities in untrained individuals. METHODS: Healthy participants (n = 35; 17 men, age 30 [4] y, and 18 women, age 28 [5] y) underwent a progressive square wave test at 4 intensities: (1) 80% of ventilatory anaerobic threshold; (2) ventilatory anaerobic threshold; (3) respiratory compensation point; and (4) exercise peak (Peak) to exhaustion, 5-minute stages, with or without a triple-layered cloth facemask (Mask or No-Mask). Several physiological and perceptual measures were analyzed. RESULTS: Mask reduced inspiratory capacity at all exercise intensities (P < .0001). Mask reduced respiratory frequency (P = .001) at Peak (-8.3 breaths·min-1; 95% confidence interval [CI], -5.8 to -10.8), respiratory compensation point (-6.9 breaths·min-1; 95% CI, -4.6 to -9.2), and ventilatory anaerobic threshold (-6.5 breaths·min-1; 95% CI, -4.1 to -8.8), but not at Baseline or 80% of ventilatory anaerobic threshold. Mask reduced tidal volume (P < .0001) only at respiratory compensation point (-0.5 L; 95% CI, -0.3 to -0.6) and Peak (-0.8 L; 95% CI, -0.6 to -0.9). Shallow breathing index was increased with Mask only at Peak (11.3; 95% CI, 7.5 to 15.1). Mask did not change HR, lactate, ratings of perceived exertion, blood pressure, or oxygen saturation. CONCLUSIONS: A cloth facemask reduced time to exhaustion but had no major impact on cardiorespiratory parameters and had a slight but clinically meaningless impact on respiratory variables at higher intensities. Moderate to heavy activity is safe and tolerable for healthy individuals while wearing a cloth facemask. CLINICALTRIALS: gov: NCT04887714.

OBJECTIVE: To evaluate the possible noninferiority of surfactant administration via laryngeal mask airway (LMA) vs endotracheal tube (ETT) in avoiding the requirement for mechanical ventilation in preterm neonates with respiratory distress syndrome (RDS). STUDY DESIGN: This was a randomized controlled trial including infants born at 27 to 36 weeks of gestation, >800 g, diagnosed with RDS and receiving fraction of inspired oxygen 0.30-0.60 via noninvasive respiratory support. Infants were randomized to surfactant via LMA (with atropine premedication) or ETT (InSuRE approach with atropine and remifentanil premedication). Primary outcome was failure of surfactant treatment to prevent the need for mechanical ventilation. RESULTS: Patients were randomized, 51 to LMA and 42 to the ETT group. Both groups had similar baseline characteristics, with birth weights ranging from 810 to 3560 g. Failure rate was 29% in the ETT group and 20% in the LMA group (P = .311). This difference was due to early failures (within 1 hour), with 12.5% in the ETT group and 2% in the LMA group (P = .044). Surfactant therapy via LMA was non-inferior to administration via ETT; failure risk difference -9.0% (CI -∞ to 5.7%). Efficacy in decreasing fraction of inspired oxygen, number of surfactant doses administered, time to wean off all respiratory support, rates of adverse events, and outcomes including pneumothorax and BPD diagnosis did not differ between groups. CONCLUSIONS: Surfactant therapy via LMA was noninferior to administration via ETT and it decreased early failures, possibly by avoiding adverse effects of premedication, laryngoscopy, and intubation. These characteristics make LMA a desirable conduit for surfactant administration. TRIAL REGISTRATION: ClinicalTrials.gov, NCT02164734.

Introduction: Personal Protective Equipment (PPE) is part of the work routine of health professionals, especially during pandemics. During the Covid-19 pandemic, the use of PPE became constant for long working hours, resulting in adverse effects on the health of professionals, especially headache. Objective: In this review, we explore the scientific literature on headache associated with prolonged use of PPE during the coronavirus pandemic. Method: This is a narrative literature review conducted through the PubMed and Web of Science databases according to the following MeSH descriptors: "Face shield", "Headache" and "Covid-19". Articles that analyzed the presence of headache and other adverse events in health professionals in prolonged use of PPE were included. Results: The included studies point to headache as the most prevalent adverse event, which may be a new headache or the worsening of a previous headache. Other effects were also found, such as pressure marks on the skin, hyperemia in contact areas; suffocation; reduced concentration and excessive sweating. Conclusion: The use of PPE for long periods can cause headaches due to external pressure, in addition to other unwanted events.These effects reveal the importance of studies to make PPE more efficient, ensuring protection for the individual without causing discomfort.


Introdução: Os Equipamentos de Proteção Individual (EPI) fazem parte da rotina de trabalho dos profissionais de saúde, principalmente durante as pandemias. Durante a pandemia da Covid-19, o uso de EPI tornou-se constante durante longas jornadas de trabalho, resultando em efeitos adversos à saúde dos profissionais, principalmente cefaleia. Objetivo: Nesta revisão, exploramos a literatura científica sobre cefaleia associada ao uso prolongado de EPI durante a pandemia do coronavírus. Método: Trata-se de uma revisão narrativa da literatura realizada por meio das bases de dados PubMed e Web of Science segundo os seguintes descritores MeSH: "Face Shield", "Headache" e "Covid-19". Foram incluídos artigos que analisaram a presença de cefaleia e outros eventos adversos em profissionais de saúde em uso prolongado de EPI. Resultados: Os estudos incluídos apontam a cefaleia como o evento adverso mais prevalente, podendo ser uma nova cefaleia ou o agravamento de uma cefaleia anterior. Também foram encontrados outros efeitos, como marcas de pressão na pele, hiperemia nas áreas de contato; asfixia; concentração reduzida e transpiração excessiva. Conclusão: O uso de EPI por longos períodos pode causar dores de cabeça por pressão externa, além de outros eventos indesejados. Esses efeitos revelam a importância de estudos para tornar os EPI mais eficientes, garantindo proteção ao indivíduo sem causar desconforto.

RESUMEN

Glaucoma is an eye disease that gradually deteriorates vision. Much research focuses on extracting information from the optic disc and optic cup, the structure used for measuring the cup-to-disc ratio. These structures are commonly segmented with deeplearning techniques, primarily using Encoder-Decoder models, which are hard to train and time-consuming. Object detection models using convolutional neural networks can extract features from fundus retinal images with good precision. However, the superiority of one model over another for a specific task is still being determined. The main goal of our approach is to compare object detection model performance to automate segment cups and discs on fundus images. This study brings the novelty of seeing the behavior of different object detection models in the detection and segmentation of the disc and the optical cup (Mask R-CNN, MS R-CNN, CARAFE, Cascade Mask R-CNN, GCNet, SOLO, Point_Rend), evaluated on Retinal Fundus Images for Glaucoma Analysis (REFUGE), and G1020 datasets. Reported metrics were Average Precision (AP), F1-score, IoU, and AUCPR. Several models achieved the highest AP with a perfect 1.000 when the threshold for IoU was set up at 0.50 on REFUGE, and the lowest was Cascade Mask R-CNN with an AP of 0.997. On the G1020 dataset, the best model was Point_Rend with an AP of 0.956, and the worst was SOLO with 0.906. It was concluded that the methods reviewed achieved excellent performance with high precision and recall values, showing efficiency and effectiveness. The problem of how many images are needed was addressed with an initial value of 100, with excellent results. Data augmentation, multi-scale handling, and anchor box size brought improvements. The capability to translate knowledge from one database to another shows promising results too.

Resumen: Reportamos el manejo exitoso de la vía aérea con intubación orotraqueal a través de una máscara laríngea AuraOnceTM en un paciente con lesión de la médula espinal cervical. Su uso fue como dispositivo de rescate de la vía aérea tras intubación orotraqueal fallida con laringoscopía directa y dificultad para la ventilación con mascarilla en una paciente traumatizada de 46 años. Se tomó la decisión de utilizar la mascarilla laríngea sin intubación como conducto para la colocación de un tubo orotraqueal, lo cual se realizó con éxito. Este uso alternativo de la mascarilla laríngea Ambu AuraOnce podría ser de gran valor para los trabajadores de la salud en países de escasos recursos donde los dispositivos avanzados para las vías respiratorias no están fácilmente disponibles.


Abstract: We reported a successful airway management in a patient with a diagnosed cervical spinal injury with a non-intubating laryngeal mask AuraOnceTM. Its use was as a rescue airway device after failed orotracheal intubation with direct laryngoscopy and difficulty with facemask ventilation in a 46-year-old trauma patient. The decision was made to use the non-intubating laryngeal mask as a conduit for the placement of an orotracheal tube, which was done successfully. This alternative use of the Ambu AuraOnce laryngeal mask could be of great value to healthcare workers in resource-poor countries where advanced airway devices are not readily available.

Abstract Objective: Continuous Positive Airway Pressure (CPAP) is the gold standard treatment for OSA. A wide variety of masks are used during CPAP therapy. The aim of the study is to compare polysomnography outcomes during titration for different types of masks (oronasal, nasal and pillow) and assess the impact on PAP titration. Methods: We retrospectively analyzed all CPAP titration polysomnography for one year. Anthropometric data and baseline apnea severity were documented. Results: A total of 497 patients were evaluated. Nasal masks were used in 82.3% (n = 409), pillow in 14.1% (n = 70) and oronasal in 3.6% (n = 18). There was no difference according to body mass index and baseline apnea-hypopnea index among the groups. The oronasal group was older and had higher titrated CPAP pressure, higher residual apnea-hypopnea index, lower sleep efficiency, more superficial N1 sleep and longer wake time after sleep onset. All other polysomnography outcomes were similar among the groups. The mean CPAP level was 11.6 ± 2.1 cm H2O for the oronasal mask, 10.1 ± 2.1 cm H2O for the nasal mask and 9.8 ± 2.2 cm H2O for the pillow. The residual apnea-hypopnea index was 10.4 ± 7.9 for the oronasal mask, 5.49 ± 5.34 events/h for the nasal mask and 4.98 ± 5.48 events/h for the pillow. The baseline apnea-hypopnea index was correlated with of a higher CPAP pressure for all the groups (p< 0.001 for the nasal group, p = 0.001 for the pillow group and p = 0.049 for the oronasal group). Body mass index and residual AHI were correlated with of a higher CPAP pressure for the nasal and pillow groups only (p < 0.001). Conclusion: The interface can have a significant impact on the effectiveness of PAP titration. Patients with oronasal masks have higher CPAP pressure, higher residual apnea-hypopnea index, lower sleep efficiency and higher wake time after sleep onset. Oronasal masks should not be recommended as the first choice for apnea patients. Level of evidence: Level 3.

Background: Face masks are widely used during the COVID-19 pandemic as one of the protective measures against the viral infection risk. Some evidence suggests that face mask prolonged use can be uncomfortable, and discomfort can be exacerbated during exercise. However, the acute responses of mask-wearing during exercise on affective/psychological and exercise performance responses is still a topic of debate. Purpose: To perform a systematic review with meta-analysis of the acute effects of mask-wearing during exercise on affective/psychological and exercise performance responses in healthy adults of different/diverse training status. Methods: This review (CRD42021249569) was performed according to Cochrane's recommendations, with searches performed in electronic (PubMed, Web of Science, Embase, SportDiscus, and PsychInfo) and pre-print databases (MedRxiv, SportRxiv, PsyArXiv, and Preprint.Org). Syntheses of included studies' data were performed, and the RoB-2 tool was used to assess the studies' methodological quality. Assessed outcomes were affective/psychological (discomfort, stress and affective responses, fatigue, anxiety, dyspnea, and perceived exertion) and exercise performance time-to-exhaustion (TTE), maximal power output (POMAX), and muscle force production] parameters. Available data were pooled through meta-analyses. Results: Initially 4,587 studies were identified, 36 clinical trials (all crossover designs) were included. A total of 749 (39% women) healthy adults were evaluated across all studies. The face mask types found were clothing (CM), surgical (SM), FFP2/N95, and exhalation valved FFP2/N95, while the most common exercises were treadmill and cycle ergometer incremental tests, beyond outdoor running, resistance exercises and functional tests. Mask-wearing during exercise lead to increased overall discomfort (SMD: 0.87; 95% CI 0.25-1.5; p = 0.01; I2 = 0%), dyspnea (SMD: 0.40; 95% CI 0.09-0.71; p = 0.01; I2 = 68%), and perceived exertion (SMD: 0.38; 95% CI 0.18-0.58; p < 0.001; I2 = 46%); decreases on the TTE (SMD: -0.29; 95% CI -0.10 to -0.48; p < 0.001; I2 = 0%); without effects on POMAX and walking/running distance traveled (p > 0.05). Conclusion: Face mask wearing during exercise increases discomfort (large effect), dyspnea (moderate effect), and perceived exertion (small effect), and reduces the TTE (small effect), without effects on cycle ergometer POMAX and distance traveled in walking and running functional tests. However, some aspects may be dependent on the face mask type, such as dyspnea and perceived exertion. INTRODUCTION: COVID-19 superspreader events have occurred when symptomatic individuals without wearing face masks boarded buses. OBJECTIVE: To report the risk of superspreader events when presymptomatic individuals boarded buses to-gether with unvaccinated passengers, but with non-pharmacological preventive interventions being maintained. METHODS: Prospec-tive study of health personnel transported in buses to a COVID-19 vaccination center for two weeks. Open windows, correct use of face masks and exclusion of symptomatic individuals were mandatory. Prospective surveillance identified workers with COVID-19 within 14 days after vaccination. Each asymptomatic passenger of buses where cases were identified was monitored for a similar time period. Voluntary screening results were available for workers who were tested in the month before or after vaccination. RESULTS: 1,879 workers boarded 65 buses. On-board time ranged from three to eight hours. Twenty-nine cases of COVID-19 and four asymptomatic cases were identified among 613 passengers of 21 buses. Median time between vaccina-tion and COVID-19 symptoms onset was six days. One case of suspected transmission on a bus was identi-fied. CONCLUSIONS: Strict nonpharmacological preventive interventions substantially reduced the risk of COVID-19 super-spreader events in buses boarded by presymptomatic individuals.


ANTECEDENTES: Ha ocurrido superpropagación de COVID-19 cuando individuos sintomáticos sin uso de cubrebocas abordaron autobuses. OBJETIVO: Reportar el riesgo de superpropagación cuando individuos presintomáticos abordaron autobuses junto con pasajeros no vacunados pero se mantuvieron intervenciones preventivas no farmacológicas. MATERIAL Y MÉTODOS: Estudio prospec­tivo de personal de salud transportado durante dos semanas en autobuses a un centro de vacunación contra COVID-19. Fue obligatorio llevar ventanas abiertas, uso correcto de cubrebocas y exclusión de personas con síntomas. La vigilancia prospectiva identificó a trabajadores con COVID-19 los 14 días siguientes a la vacunación. Cada pasajero asintomático de autobuses donde se detectaron casos fue vigilado durante un periodo de tiempo similar. Los resultados de tamizaje voluntario estuvieron disponibles para los trabajadores que se realizaron prueba el mes previo o el siguiente a la vacunación. RESULTADOS: 1879 trabajadores abordaron 65 autobuses. El tiempo a bordo varió de tres a ocho horas. Veintinueve casos de COVID-19 y 4 casos asintomáticos fueron identificados entre 613 pasajeros de 21 autobuses. La mediana de tiempo entre la vacunación y el inicio de síntomas en casos de COVID-19 fue de seis días. Fue identificado un caso de transmisión sospechada en autobús. CONCLUSIONES: Las intervenciones preventivas no farmacológicas estrictas redujeron sustancialmente el riesgo de superpropagación de COVID-19 en autobuses ocupados por individuos presintomáticos.

Abstract Cranial nerve injury by a laryngeal mask airway is rare but a serious complication. The nerve injuries must be prevented during the intubation using a laryngeal mask airway. We report a female patient who complained of tongue numbness, slurred speech, and slight difficulty in swallowing solid food after a hand surgery. She was then diagnosed with unilateral lingual nerve and hypoglossal nerve injuries. Extreme head rotation, relatively small oral cavity, and wide rigid composition at the lower part of the novel laryngeal mask probably resulted in cranial nerve injury.
