RESUMEN

Replacing the conventional endodontic irrigants with herbal agents could avoid complications associated with using sodium hypochlorite (NaOCl). Endodontic irrigants alter the surface roughness of the dentinal wall surface, which affects sealer mechanical retention. This study aimed to assess the effect of experimental herbal Moringa oleifera and orange peel extract irrigant on intraradicular dentin (IRD) surface roughness using quantitative 3D surface analysis by scanning electron microscopy (SEM) regarding the smear layer assessment. Sixty human root sections were divided into four groups (n = 15): NaOCl combined with 17% ethylenediaminetetraacetic acid (EDTA); negative control (saline); moringa extract (MO); and orange oil (OO). SEM images were assessed quantitatively for surface roughness (Ra) in the coronal, middle, and apical IRD. The data were analysed by Kruskal-Wallis, Friedman, and Dunn's tests. All groups showed statistically significant differences (P = 0.007). MO exhibited significantly greater Ra values at the coronal, middle, and apical root levels than OO (P = 0.007, 0.009, and 0.046, respectively). There was no significant change in Ra values at various root levels within each group at P = 0.091, 0.819, 0.819, and 0.549 for the EDTA, saline, MO, and OO groups. Considerable (IRD) surface roughness analysis makes Moringa extract a promising herbal endodontic irrigant alternative to the NaOCl plus EDTA regimen.

RESUMEN

OBJECTIVE: This scoping review aimed to map the literature on the utilization of intraoral scanners (IOS) in fabricating post and core restorations. DATA: The study protocol is available at https://osf.io/ja9xh/. This scoping review followed Joanna Briggs Institute guidelines, and reporting was based on the checklist proposed by PRISMA-ScR. SOURCES: Searches were conducted in PubMed/MEDLINE, Web of Science, Scopus, and grey literature up to August 2023. RESULTS: A total of 455 studies were identified. After screening, 16 studies were deemed suitable for scoping review. Of these, 13 were in vitro studies, two were case reports, and one was a clinical study. Different IOS systems for scanning post spaces were evaluated, with Trios being most prevalent, followed by Prismescan. While most studies did not use any intraoral scanning devices, five studies reported the use of scan posts. Results regarding retention favored fully digital workflows. However, results about the adaptation and accuracy varied between included studies, highlighting the importance of standardized approaches. Future clinical studies should encompass variations in IOS systems, post space depths, scanner depth of field, light conditions, scan post utilization, operator experience, scanning time and strategy, and direct comparison with conventional techniques. CONCLUSION: IOS is increasingly important in capturing digital impressions for post spaces and provides valuable insights for clinicians and researchers. Although included studies, primarily in vitro, demonstrated promising results for the utilization of IOS systems in fully digital workflows for post and core restorations, further well-conducted studies, particularly clinical trials, are recommended due to data heterogeneity and limitations. CLINICAL SIGNIFICANCE: This mapping highlights the potential of modern IOS in fully digital workflows for post and core restorations. Despite promising results, challenges related to different variables associated with IOS require careful consideration. Standardization and additional clinical validation are essential for optimize IOS benefits in post and core restorations.

OBJECTIVE: This systematic review aimed to analyze the clinical (survival rate, failure risk, or fracture) and laboratory performance (fracture mode or failure) of rehabilitations of endodontically treated teeth, with and without posts. MATERIALS AND METHODS: A systematic search was conducted in the Pubmed, Scopus, Web of Science, Embase, Cochrane Library, and OpenGrey databases up to March 2023, according to PRISMA guidelines. In vitro and clinical studies that compared the clinical and laboratory performance of endodontically treated teeth with and without intraradicular posts were included. Studies selection, data extraction, and risk of bias analysis were performed. RESULTS: Thirty-one in vitro and 7 clinical studies were included. For in vitro studies, fiberglass post (n = 24) was the most mentioned. The follow-up time of the clinical studies ranged from 1 to 17 years, with the fiber-reinforced composite post (n = 3) being the most evaluated, and only failure risk proved to be more favorable for using intraradicular posts. CONCLUSION: Rehabilitations of endodontically treated teeth with and without intraradicular retainers showed no difference in fracture resistance and failure mode, evaluated by in vitro studies. Clinical studies showed no difference in survival rate, but failure risk proved to be more favorable for the use of posts. CLINICAL SIGNIFICANCE: This analysis revealed significant variability between results, however, most laboratory and clinical studies revealed no difference with using the post. Furthermore, it is important to emphasize the need to evaluate the coronary remnant and the general characteristics of the tooth in each situation.

This review article describes the methods and clinical recommendations for reinforcing traumatized anterior immature teeth with pulp necrosis treated with mineral trioxide aggregate (MTA) apexification. Traumatic injury can cause pulp necrosis and incomplete root formation in immature teeth. MTA apexification is the treatment of choice for necrotic immature teeth, particularly during the middle or late stages of root development. MTA apexification has a high success rate; however, failures due to cervical or root fractures occasionally occur. The risk of fracture is higher in immature teeth with thin root dentin, particularly those with external root resorption. Furthermore, the loading force from any parafunctional habit also increases fracture risk. Therefore, intra-radicular reinforcement may be necessary after MTA apexification. In vitro, intraradicular restoration with a resin composite/core build-up material or a prefabricated fiber post demonstrated better root reinforcement than root canal obturation materials (i.e., gutta-percha and sealer). However, the root-reinforcement effect of MTA orthograde filling in the entire root canal remains unclear. In vivo, the survival of fractured teeth with intraradicular restorations (resin composite/core build-up material or prefabricated fiber posts) is extremely high. Moreover, the survival of teeth with gutta-percha/sealer obturation or MTA orthograde filling and restoration with resin composite extending into the cervical third of the root canal approximately 1-2 mm below the cemento-enamel junction is acceptably high. Based on this evidence, the remaining tooth/root structure and loading force should be carefully examined when considering intra-radicular reinforcement of immature anterior teeth treated with MTA apexification.

The current study evaluated the stress distribution in a maxillary central incisor with mid-root fracture after splinting with different intra-radicular posts using 3D-finite element analysis (FEA). Five 3D-FEA models were constructed. Model 1 was an intact tooth with no fracture, Model 2: A tooth with a horizontal mid-root fracture, with no treatment. Model 3: Same as model 2, and intraradicular splinting using fiber post. Model 4: Same as model 2 and intra-radicular splinting using Protaper Gold file F3. Model 5: Same as model 2, and with intraradicular splinting with Ribbond. The FEA of all models was done to obtain the maximum Von-Mises stress in the root canal space, the dentin, the periodontal ligament, and the bone. The highest Von Mises stresses for the root canal space and the dentin were found in Model 3, followed by models 4, 5, and 2, and least in Model 1. The Von Mises stress of the periodontal ligament was the least in model 1. The Von Mises stress of bone was higher in all experimental models than in the baseline model. The results suggest that in cases where intra-radicular splinting is indicated, fiber posts and Ribbond are better alternatives to endodontic files due to the lower stresses exerted.

INTRODUCTION: Smear layer removal from root canals aid in the penetration of both irrigants and endodontic sealer into the dentinal tubules, thereby improving the efficacy of endodontic treatment. The aim of this in vitro study was to compare the smear layer removal ability of a conventional endodontic irrigation regimen, MTAD (mixture of tetracycline, acid, and detergent), and QMix™ (Dentsply Sirona, Charlotte, North Carolina, United States) with that of a mixture of herbal irrigants, namely, aqueous extracts of Azadirachta indica (neem) and Citrus limon (lemon), evaluated using scanning electron microscopy (SEM). MATERIALS AND METHODS: We selected 40 extracted human premolar teeth for the study, which we randomly divided into five groups (eight samples each) according to irrigation solution: (i) Group A (normal saline); (ii) Group B (conventional endodontic regimen, 3% sodium hypochlorite (NaOCl) + 17% ethylenediamine tetraacetic acid (EDTA) + 2% chlorhexidine (CHX)); (iii) Group C (MTAD); (iv) Group D (QMix 2-in-1); and (v) Group E (aqueous extracts of Azadirachta indica and Citrus limon). After we prepared the canals with ProTaper Universal nickel-titanium (Ni-Ti) rotary files (Dentsply Sirona) and the respective irrigants, we split the teeth longitudinally to evaluate the amount of remnant smear layer in the coronal, middle, and apical thirds using SEM photomicrographs. We performed statistical analyses of the data using the Kruskal-Wallis and Mann-Whitney U tests, where the level of significance was set at 0.05. RESULTS: The SEM analysis of the coronal third showed mean values of 3.83 in Group A, 3.67 in Group B, 2.79 in Group C, 3.63 in Group D, and 4.00 in Group E. The SEM analysis of the middle third showed mean values of 4.00 in Group A, 3.88 in Group B, 3.75 in Group C, 3.50 in Group D, and 3.50 in Group E. The SEM analysis of the apical third showed mean values of 3.92 in Group A, 3.63 in Group B, 3.71 in Group C, 3.88 in Group D, and 3.17 in Group E. Therefore, we found that there were significant statistical differences between the groups when an overall comparison was done for the coronal, middle, and apical third, with a p-value of 0.001. On multiple comparisons across the different tooth-section thirds. Groups A and B showed statistically significant differences in the apical third (p-value=0.017). Groups A and C showed statistically significant differences in the coronal third and middle third (p-values=0.001 and 0.010, respectively). Groups A and D showed statistically significant differences in the middle third (p-value=0.001). Groups A and E showed statistically significant differences in all thirds (p-values=0.039, 0.001, and 0.001, respectively) Conclusion: The conventional needle irrigation with MTAD showed the highest level of smear layer removal ability on the root canal surface, followed by QMix 2-in-1, the Azadirachta indica leaf and Citrus limon extract mixture, and the conventional endodontic regimen. Normal saline showed the lowest smear layer removal effect.

The aim of this study was to assess adaptation and bonding to root canal dentin of discontinuous (short) glass fiber-reinforced composite to intra-radicular dentin (DSGFRC). METHODS: Seventy virgin human teeth were extracted and then endodontically treated; then samples were randomly divided into 7 groups (n = 10), based on the materials' combinations as follows: Group 1, a two-bottle universal adhesive + DSGFRC; Group 2, a single-component universal adhesive + DSGFRC; Groups 3 and 4, the same materials of Goups 1 and 2 were used but after cleaning of the canal walls with 17% EDTA and final irrigation with 5.25% NaOCl Ultrasound Activated (UA); Group 5, traditional prefabricated fiber posts were luted after being silanized with G-Multi Primer; Groups 6 and 7, like Group 5 but after ultrasonic irrigation (UA). All sample roots were cut 1 mm thick (n = 10) to be evaluated regarding root canal adaptation using a light microscope and scanning electron microscope (SEM) and push-out bond strength. These results were statistically analyzed by Kruskal-Wallis analysis of variance by ranks. The level of significance was set at p < 0.05. RESULTS: Bond strength forces varied between 6.66 and 8.37 MPa and no statistically significant differences were recorded among the groups. By microscopic examination, it was noted that ultrasonic irrigation increased the adaptation of the materials to the dentin surface. CONCLUSIONS: Within the limitations of this in vitro study, it may be concluded that when DSGFRC was used for intracanal anchorage in the post-endodontic reconstruction, similar push-out retentive force and strength to those of traditional fiber posts cemented with particulate filler resin composite cements were achieved.

INTRODUCTION: This scoping review aimed to map the evidence about the microbiota found in persistent endodontic infections. METHODS: The study protocol was prospectively registered and is available at https://osf.io/3g2cp. The electronic search was performed in MEDLINE via PubMed, Lilacs, BBO, Scopus, Web of Science, Cochrane Library, and Embase. The eligibility criteria were based on the PCC acronym, where P (Population) represents patients with teeth presenting persistent endodontic infection, C (Concept) represents microbial profile, and C (Context) represents undergoing endodontic retreatment. Clinical studies that evaluated the microbial profile of samples collected from root canals of teeth undergoing retreatment, using classical or molecular methods, were included. Studies that did not show a minimum period of 1 year between primary endodontic treatment and retreatment or did not radiographically evaluate the quality of primary root canal filling were excluded. Two reviewers independently selected the articles and collected data. RESULTS: From a total of 957 articles, 161 were read in full, and 32 studies were included. The most prevalent species were Enterococcus faecalis, Parvimonas micra, Porphyromonas endodontalis, Porphyromonas gingivalis, Prevotella intermedia, Dialister invisus, Propionibacterium acnes, Tannerella forsythia, and Treponema denticola. Cases with symptomatology or inadequate root canal filling presented an increase in specific bacterial species compared to those with no symptomatology or adequate filling. A greater number of microorganisms was observed in teeth with inadequate coronal restoration compared to those with adequate restoration. CONCLUSIONS: Persistent endodontic infections have a polymicrobial profile identified by the commonly used methods for bacterial detection/identification and are subject to the limitations present in each of those methods.

Abstract Considering the side effects in the oral cavity and dental structures of radiotherapy (RDT) for head and neck cancer, this study aimed to evaluate the effects of RDT on the root dentin concerning the obliteration of dentinal tubules, the inorganic composition of intra-radicular dentin, and the integrity of collagen fibers. Thirty human canines were selected from a biobank and randomly divided into two groups (n=15). The samples were sectioned buccolingually, and a hemisection was used for structural analysis by scanning electron microscopy (SEM) and energy-dispersive X-ray spectrometer (EDS). Low-vacuum SEM images were obtained at 2000-x magnification to observe the obliteration of the dentinal tubules. Moreover, compositional evaluation was performed using EDS. After RDT, the SEM and EDS analyses were repeated using the same methodology. RDT was applied fractionally at 2 Gy per day, 5 days per week, for 7 weeks, resulting in a total dose of 70 Gy. The collagen integrity of the irradiated and non-irradiated samples was analyzed using Masson's trichrome and picrosirius red staining polarization microscopy. Samples subjected to RDT exhibited dentinal tubule obliteration (p < 0.001); low integrity of type I and III collagen fibers (p < 0.05); compositional reduction of calcium (p = 0.012), phosphorus (p = 0.001), and magnesium (p < 0.001); an increased Ca/P ratio (p < 0.001). RDT affects the structure of dentinal tubules, the inorganic composition of intra-radicular dentin, and the collagen fiber integrity in the root dentin, which may interfere with the effectiveness and durability of dental procedures.


Resumo Considerando os efeitos colaterais na cavidade oral e estruturas dentárias da radioterapia (RDT) para o câncer de cabeça e pescoço, este estudo teve como objetivo avaliar os efeitos da RDT sobre a dentina radicular quanto à obliteração dos túbulos dentinários, a composição inorgânica da dentina intra-radicular e a integridade das fibras colágenas. Trinta caninos humanos foram selecionados de um biobanco e divididos aleatoriamente em dois grupos (n=15). As amostras foram seccionadas no sentido vestíbulo-lingual, e uma hemissecção foi utilizada para análise estrutural por microscopia eletrônica de varredura (MEV) e espectrômetro de raios X de dispersão de energia (EDS). Imagens de MEV de baixo vácuo foram obtidas com aumento de 2500x para observar a obliteração dos túbulos dentinários. Além disso, a avaliação composicional foi realizada usando EDS. Após a RDT, as análises em MEV e EDS foram repetidas usando a mesma metodologia. A RDT foi aplicada fracionada a 2 Gy por dia, 5 dias por semana, durante 7 semanas, resultando em uma dose total de 70 Gy. A integridade do colágeno das amostras irradiadas e não irradiadas foi analisada usando microscopia de polarização de coloração com tricrômio de Masson e picrosirius red. As amostras submetidas a RDT apresentaram obliteração dos túbulos dentinários (p < 0,001); baixa integridade das fibras colágenas tipo I e III (p < 0,05); redução composicional de cálcio (p = 0,012), fósforo (p = 0,001) e magnésio (p < 0,001); e uma relação Ca/P aumentada (p < 0,001). A RDT afeta a estrutura dos túbulos dentinários, a composição inorgânica da dentina intra-radicular e a integridade das fibras colágenas na dentina radicular, o que pode interferir na eficácia e durabilidade dos procedimentos odontológicos.

Background: The aim of this study is to investigate the impact of ceramic material and preparation design on the marginal fit of endocrown restorations. Methods: Forty endocrown restorations were CAD/CAM-fabricated for forty extracted maxillary first premolar teeth. Samples were divided into two groups (n = 20) according to the ceramic materials used: Celtra Duo and Vita Enamic. Each group was divided into two subgroups (n = 10) according to the preparation design: with no intraradicular extension and with 3 mm intraradicular extension. The marginal gap was examined using a digital microscope. Results: Celtra Duo without intraradicular extension recorded the least mean marginal gap (7.74 ± 1.55 µm), while Group Celtra Duo with 3 mm intraradicular extension recorded the highest mean marginal gap (29.54 ± 6.32 µm). Group Vita Enamic recorded a lesser marginal gap (18.03 ± 12.11 µm) than group CD (Celtra Duo) (18.64 ± 12.05 µm). There is a statistically non-significant difference between the two groups of materials (p = 0.873). There is a statistically significant difference between the two tested preparation designs (p < 0.001). Conclusion: All groups recorded a marginal gap within clinically accepted values. Material selection may influence the fitting of restorations. Intraradicular extension for endocrown restorations adversely affects the marginal fit, however, the marginal gap is still within the clinically accepted range.

This study assessed the bond strength of prefabricated post systems at different root levels of endodontically treated teeth. One-rooted human premolars (N = 70; n = 10) were cut to 2 mm above the cement-enamel junction. Root canals were treated and randomly assigned to one of the seven post systems: T: Titanium (Mooser), ZrO: Zirconia (Cosmopost), G: Fiber (FRC Postec Plus), E1: Fiber (Direct) (Everstick post), E2: Fiber (Indirect) (Everstick post), PP: Fiber (PinPost), and LP: Injectable Resin/Fiber composite (EverX Posterior). All posts were luted using a resin cement (Variolink II), and the roots were sectioned at the coronal, middle, and apical root levels. Push-out tests were performed in the Universal Testing Machine (0.5 mm/min). Data (MPa) were analyzed using two-way ANOVA and Tukey's tests (α = 0.05). The results showed that the bond strength (mean ± SD) of E2 posts were highest (5.3 ± 2.7) followed by PP (4.1 ± 2.0); G (4.0 ± 1.6); LP (2.6 ± 1.9): T (2.2 ± 1.5) and ZrO (1.9 ± 1.0) posts systems. No significant differences were found in bond strength of all post systems. The bond strength in the coronal root level was the highest with 3.6 ± 2.2 MPa. The bond strength of FRC post systems was significantly higher than those of rigid posts of titanium or ZrO2. Bond strength results were the highest in the coronal root level for all tested post systems but did not differ significantly from the other two root levels.

Background: Diode-assisted endodontics is nowadays utilized for pulp space disinfection, but little is known on the bonding potential of this lased root dentin when the tooth is restored with an intracanal polymer post. Objectives: to investigate the influence of diode laser irradiation settings, in laser-assisted endodontics, on the intraradicular bonding of composite materials. Methods: Sixteen two-rooted, maxillary first premolars were collected, prepared up to F4 (Protaper Universal. Dentsply-Maillefer, Ballaigues, Switzerland), and randomly assigned in two groups: group A (chopped mode or short pulse), diode irradiated according to protocol, pulse 25 ms, power 2.5 W, and group B (microchopped mode or ultrashort pulse), pulse 25 µs, peak power 12 W (both groups GentleRay. KaVo Dental, Biberach an der Riss, Germany). Buccal canals were irradiated, palatal ones served as controls. Canals were then obturated, post space was created in all canals, and quartz-fiber posts (ICE light Danville. Danville Materials, San Ramon, CA, USA) were cemented by self-etch self-curing cement (Max Cem Elite. Kerr, West Collins Orange, CA, USA) (Max Cem Elite. Kerr, Brea, CA, USA). A week later, teeth were sectioned horizontally in 1 mm increments. Push-out test was conducted in a Zwick testing machine (Zwick Roell, Ulm, Germany) at 1 mm/min speed, and the force required to dislodge the post from each specimen (F-max) was recorded. Weibull regression models were applied for statistical analyses. Results: Differences in F-max by group (control vs. chopped mode vs. microchopped mode) and height (meaning the apical-to-coronal position of each specimen along the root) were statistically significant (p < 0.05 in all cases). Conclusions: Short pulses (or chopped mode) had a profound positive effect on the quality of intraradicular bonding, while Ultrashort pulses (or microchopped mode) affected it negatively. In addition, apically positioned bonding proved weaker compared with more coronally located specimens.

This study explored the location of MMP-2, -3, -8 in human root dentin and the inhibition of EGCG/EGCG-3Me on dentin-originated collagen proteases activities. Also, the study evaluated EGCG/EGCG-3Me modified etch-and-rinse adhesives (Single Bond 2, SB 2) for their bonding stabilities to intraradicular dentin. Immunostaining and liquid chip analysis demonstrated that MMP-2 and MMP-8 are widely distributed in root dentin while MMP-3 shows a higher fluorescence intensity in the middle and apical third of the root. The contents of MMP-2, -3 and -8 varies in different locations of human tooth root and MMP-2 has the highest content than MMP-3 and MMP-8 at each third of teeth root. Both EGCG and EGCG-3Me showed an inhibitory effect on the root dentin-derived MMPs in a concentration dependent manner (P < 0.05) and the inhibitory activity of EGCG-3ME was stronger than that of EGCG at the same concentration (P < 0.05). EGCG and EGCG-3Me were incorporated separately into the adhesive SB 2 at concentrations of 200, and 400 µg/mL respectively. The immediate push-out strength of SB 2 was not compromised by EGCG/EGCG-3Me modification. EGCG/EGCG-3Me modified adhesive had higher push-out strength than SB 2 after thermocycling, showing no correlation with concentration.

BACKGROUND: Hydraulic materials are used in Endodontics due to their hydration characteristics namely the formation of calcium hydroxide when mixing with water and also because of their hydraulic properties. These materials are presented in various consistencies and delivery methods. They are composed primarily of tricalcium and dicalcium silicate, and also include a radiopacifier, additives and an aqueous or a non-aqueous vehicle. Only materials whose primary reaction is with water can be classified as hydraulic. OBJECTIVES: Review of the classification of hydraulic materials by Camilleri and the literature pertaining to specific uses of hydraulic cements in endodontics namely intra-coronal, intra-radicular and extra-radicular. Review of the literature on the material properties linked to specific uses providing the current status of these materials after which future trends and gaps in knowledge could be identified. METHODS: The literature was reviewed using PUBMED, and for each clinical use, the in vitro properties such as physical, chemical, biological and antimicrobial characteristics and clinical data were extracted and evaluated. RESULTS: A large number of publications were retrieved for each clinical use and these were grouped depending on the property type being investigated. CONCLUSIONS: The hydraulic cements have made a difference in clinical outcomes. The main shortcoming is the poor testing methodologies employed which provide very limited information and also inhibits adequate clinical translation. Furthermore, the clinical protocols need to be updated to enable the materials to be employed effectively.

Traumatic injuries resulting in damage to the teeth and associated structures have a grave psychological impact on the patient and hence, these require timely intervention. The fracture extent determines the appropriate treatment plan for the patient. Crown fractures can be treated aesthetically without the invasion of the biological width with meticulous interdisciplinary management. The objective of this article is to report a case of a 27-year-old Indian male patient who presented to our hospital the following day, post-trauma to the anterior teeth. On clinical examination, two teeth were found to have undergone fracture that involved the pulp chamber. Considering the overall oral health, pulpal and periodontal health, availability of the fragments, and invasion of the biological width, a tailor-made treatment plan was devised. The fractured crown fragments were reattached surgically, using a glass fiber post that resulted in an aesthetic biological restoration. This treatment has been successfully managed by an interdisciplinary approach. At the 18th-month follow-up, clinical and radiological examinations suggest a successful outcome.

O objetivo deste estudo foi avaliar in vitro a influência de um novo dispositivo fotopolimerização e pino de fibra de vidro nas propriedades mecânicas, físicoquímicas e adesão dos cimentos resinosos. Foram utilizados oitenta dentes bovinos, submetidos ao tratamento endodôntico, distribuídos em 5 grupos (n=16): CD (Pino fibra vidro e cimento dual); PF (Pino perfurado e cimento fotoativado); PD (Pino perfurado e cimento dual); POF (Pino perfurado iluminador de fibra óptica e cimento fotoativado); POD (Pino perfurado iluminador de fibra óptica e cimento dual). Os dentes foram preparados para colocação de um protótipo de pino em fibra de vidro que possui um canal interno de diâmetro regular e com conicidade progressiva. A perfuração permite a inserção da fibra óptica ao longo da extensão do pino a fim de possibilitar a ação da luz ao longo de todo o comprimento do conduto radicular. Após a cimentação, os espécimes foram seccionados perpendicularmente, e obtida 1 fatia de 2 mm de espessura do terço apical, médio e cervical. A avaliação mecânica foi realizada através do ensaio de push-out para determinação da resistência adesiva nos terços cervical, médio e apical seguido pela análise da fratura em estereomicroscópio. Os retentores intrarradiculares foram também submetidos ao teste de flexão de 3 pontos para análise do material preenchedor do pino de fibra (n=10). As análises físico - químicas foram realizadas através da determinação do grau de conversão dos cimentos (RAMAN) e análise em espectroscopia de energia dispersiva (EDS) dos monômeros presentes. A adesão foi analisada pela interface de cimentação pelo MEV e reconstrução 3D do novo sistema através do Micro-CT. Os dados obtidos foram analisados estatisticamente pelo ANOVA (um e dois fatores) e comparação múltipla de Tukey, (p<0,05). Os resultados de resistência adesiva evidenciaram que o terço apical obteve o maior valor de resistência adesiva em comparação ao terço médio (p<0,001), o grupo POD do terço apical foi estatisticamente significante em relação ao grupo CD do terço médio (p<0,001). Na analise individual de cada terço, não houve diferença entre os grupos experimentais e o controle (p>0,05). A falha adesiva entre cimento e dentina foi a mais predominante entre todos os grupos e terços. O pino de fibra de vidro (controle) obteve o maior valor de resistência à flexão (p<0,001), seguido do pino de fibra de vidro perfurado preenchido com cimento resinoso (p<0,001). Os maiores valores de GC foram alcançados pelo grupo POD com 82,3% (cervical) 69,9 % (médio) e 76,21% (apical) e o EDS comprovou a presença de componentes químicos adequados. A análise da adesão do novo pino de fibra de vidro comprova uma excelente adaptação no interior do canal radicular nas regiões cervical, médio e apical. Portanto o novo dispositivo com fibra óptica e pino de fibra de vidro experimental aumentaram as propriedades mecânicas, físico-químicas e adesão do cimento resinoso (AU)


The aim of this study was to evaluate in vitro the influence of a new light curing device with optical fiber and experimental glass fiber post on the physicochemical, mechanical, and adhesion properties of resin cements. Eighty bovine teeth were used, submitted to an endodontic treatment, distributed in 5 groups (n=16): CD (Glass fiber post and dual cement); PF (Perforated post and light-cured cement); PD (Perforated post and dual cement); POF (Fiber optic illuminating with a perforated post and lightcured cement); POD (Fiber optic illuminating with a perforated post and dual cement). The teeth were prepared for placement of a glass fiber post prototype, which has an internal canal of regular diameter and progressive taper. The internal perforation extension allows the insertion of the optical fiber along the entire length of the post in order to allow the action of light along the entire length of the root canal.The specimens were sectioned perpendicularly for the tests, and 1 slice approximately 2 mm thick was obtained from the apical, middle and cervical thirds. The mechanical evaluation was carried out through the push-out test to determine the adhesive bond strength, in the cervical, middle and apical thirds, followed by the fracture analysis under a stereomicroscope, the intraradicular post were also submitted to the 3-point bending test for material analys of the fiber post filler (n=10). The physicochemical analyzes were performed by determining the degree of conversion of the cements (RAMAN) of each sample and the analysis of energy dispersive spectroscopy (EDS) of the monomers present. Adhesion was analyzed by the cementation interface and 3D reconstruction of the new system through micro-CT, and finally, SEM analysis of the adhesive interface. The data obtained were analyzed for normality and statistically by ANOVA (one and two ways) and Tukey's multiple comparison (p<0.05). The adhesive bond strength results showed that the apical third had the highest value of adhesive strength compared to the middle third (p<0.001), and the POD group of the apical third was statistically significant in relation to the CD group of the middle third (p<0.001). In the individual analysis of each third, there was no difference between the experimental and control groups (p>0.05). Adhesive failure between cement and dentin was the most prevalent among all groups and thirds. The conventional post (control) had the highest flexural strength value (p<0.001), followed by the perforated fiberglass post filled with resin cement (p<0.001). The POD group achieved the highest GC values with 82.3% (cervical), 69.9% (medium), and 76.21% (apical) and EDS confirmed the presence of adequate chemical components. The analysis of the adhesion of the new fiberglass post proves an excellent adaptation inside the root canal in the cervical, middle and apical regions. Therefore, the new light curing device with optical fiber and experimental glass fiber post improved the resin cement's mechanical, phycochemical, and adhesion. (AU)

Introduction: Endodontically treated teeth are usually affected by extensive structure loss requiring the use of intraradicular posts to provide retention and restoration. Objective: An in vitro assessment was performed on the bonding of glass fiber posts to the root dentin. Material and method: Ninety (n = 10) single bovine roots were used in a 3 x 3 x 3 factorial study with subdivided plots: post customization varying the presence and type of resin (without customization, conventional resin, and Bulk Fill resin), light-curing device (Valo, Radii-Cal, Rainbow), and root third (cervical, middle, and apical). Result: For the customization factor, Tukey's test (5%) showed the superiority of the Bulk Fill (8.16 MPa) and Z350 (7.40 MPa) groups compared to the control group (4.92 MPa), without differing from each other. All light-curing devices differed, showing the superiority of Valo (9.36 MPa), Radii (6.96 MPa) as an intermediate, and the inferiority of Rainbow (4.17 MPa). The cervical root third (7.81 MPa) was superior, the apical third was inferior (5.80 MPa), and the middle third (6.88 MPa) was an intermediate without differing from the others. Conclusion: The customization of glass fiber posts increases the bond strength to the root dentin, regardless of the resin used. There was a compromise in the apical third and when using light-curing devices with lower light intensity.


Introdução: Dentes com extensa perda de estrutura podem comprometer a retenção das restaurações ao remanescente dental, onde pinos intraradiculares são indicados. Objetivo: Avaliou-se in vitro a união de pinos de fibra de vidro à dentina radicular em função de diferentes modos de reanatomização, fotopolimerizadores e regionalização radicular. Material e método: Noventa (n=10) raízes bovinas uniradiculares foram usadas num estudo fatorial 3 x 3 x 3 com parcelas subdivididas: Reanatomização do pino, variando a presença e tipo de resina (Sem reanatomização, Resina Convencional e Resina Bulkfill); Fotopolimerizador (Valo, Radii-cal, Rainbow); e Terço radicular (cervical, médio e apical). Resultado: O teste de Tukey (5%) evidenciou para o Fator Reanatomização superioridade dos grupos BulkFill (8.16MPa) e Z350 (7.40MPa) ao grupo Controle (4.92MPa), sem diferirem entre si. Todos os fotopolimerizadores diferiram entre si, com superioridade de Valo (9.36MPa), Radii (6.96MPa) intermediário, e inferioridade de Raiwbow (4.17MPa). O terço radicular cervical (7.81MPa) foi superior e o apical inferior (5.80MPa), com o terço médio (6.88MPa) intermediário e sem diferir dos demais. Conclusão: Conclui-se que a reanatomização de pinos de fibra de vidro aumenta a resistência de união à dentina radicular, independentemente da resina utilizada, havendo prejuízo no terço apical e quando são empregados fotopolimerizadores com menor intensidade luminosa.

Objetivo: realizar uma revisão de literatura narrativa sobre a importância e a influência do efeito férula na resistência à fratura de restaurações em dentes tratados endodonticamente com utilização ou não de retentores intrarradiculares. Metodologia: Foram utilizados os seguintes descritores indexados no DeCS (Descritores em Ciências da Saúde): "Férula", Prótese Dentária", "Falha da Restauração Dentária", "Técnica de Retentor Intrarradicular" e "Pinos de Retenção Dentária" para busca nas bases de dados PubMed, Embase, Lilacs, Web of Science, Scopus e Cochrane Library. Resultados: Os dados da maioria dos estudos in vitro ou in vivo incluídos nesta revisão sugerem o uso da férula para aumento da resistência à fratura e longevidade dos dentes tratados endodonticamente. Tem sido recomendada a confecção de uma férula com altura uniforme de 2 mm e espessura superior a 1,5 mm. Conclusão: O uso de retentores radiculares foi indicado pela maioria dos trabalhos, sendo geralmente recomendados os pinos de fibra de vidro ao invés de núcleos metálicos fundidos para minimizar os riscos de fratura radicular em razão do módulo de elasticidade semelhante à dentina, além de possibilidade de retratamentos em casos de falhas adesivas. Apesar disso, ainda são necessários estudos clínicos com acompanhamento a longo prazo para avaliar a longevidade das restaurações de dentes tratados endodonticamente utilizando pinos de fibra de vidro sem ou com a presença de férula em diferentes espessuras e alturas.


Objective: This study aimed to conduct a narrative literature review on the importance and influence of the ferrule effect on the fracture resistance of restorations in endodon-tically treated teeth with or without the use of intraradicular posts. Methodology: The following descriptors indexed in the DeCS (Health Sciences Descriptors) were used: "Ferrule", Dental Prosthesis", "Dental Restoration Failure", "Post and Core Technique", and "Dental Posts", to search on the databases of PubMed, Embase, Lilacs, Web of Science, Scopus, and Cochrane Library data. Results: Data from most of the in vitro or in vivo studies included in this review suggest the use of the ferrule to increase fracture resistance and longevity of endodontically treated teeth. It has been recommended to make a ferrule with a uniform height of 2 mm and a thickness greater than 1.5 mm. Conclusion: The use of intraradicular posts was indicated by most studies, and fiberglass posts are generally recommended instead of cast metal cores to minimize the risk of root fracture due to the modulus of elasticity similar to dentin, in addition to the possibility of retreatments in ad-hesive failure cases. Despite this, clinical studies with a long-term follow-up are essential to evaluating the longevity of restorations of endodontically treated teeth using fiberglass posts without or with the presence of ferrule in different thicknesses and heights.

The gold standard for restoring Endodontically Treated Teeth (ETT) with successful clinical longevity requires having minimal invasive preparations and maximal tissue conservation. Many dentists still consider hybrid post/core/crown to be the first choice for restoring ETT. Endocrown is a viable alternative treatment modality to hybrid post/core/crown. This study aims to assess the proper judgment of dentists working in Riyadh, Saudi Arabia on the use of monolithic endocrown versus hybrid post/core/crown for restoring ETT. The IRB of Princess Nourah Bint Abdulrahman University (PNU) Institutional Review Board reviewed this study. The questionnaire was validated and electronically distributed. The participants were pre-informed that their responses are completely anonymous and used for professional purposes only. The questionnaire surveyed dentists working in Riyadh, Saudi Arabia, about their preference for different ETT restorative modalities at various clinical scenarios. Data were analyzed using One-way ANOVA and t-test. All P-values of < 0.05 were considered statistically significant. A total of 275 responses were collected; 61.45% were females and 38.55% males. 56% of them were general practitioners, while 16% were consultants. Prefabricated post/core was the most preferred technique among the participants (18.55%), followed by endocrown (12.36%), and lastly, cast post/core (8.73%). The amount of remaining tooth structure was the most influential in the treatment selection (30.18%), followed by the presence or absence of 1-2 mm ferrule (17.82%). Interocclusal space (12.36%) was the least influential factor. Endocrown recorded 63.27% as the most preferred line of treatment in case of insufficient inter-occlusal space. 40.36% preferred endocrown for patients with occlusal risk factors. The amount of the remaining tooth structure and the tooth position significantly affect the treatment options of the participants. Endocrown was the most preferred treatment modality for restoring ETT for patients with occlusal consideration.

OBJECTIVE: The aim of this study was to perform an integrative review on the layer thickness and microstructure of resin-matrix cements around custom-made or standard teeth root intracanal posts. MATERIALS AND METHODS: An electronic search was conducted on the PubMed using a combination of the following scientific terms: intraradicular post, root intracanal post, resin cement, thickness, adaptation, endodontic post, layer thickness, fit, shape, and endodontic core. The literature selection criteria accepted articles published in the English language, up to May 2021, involving in vitro analyses, meta-analyses, randomized controlled trials, and prospective cohort studies. RESULTS: The search identified 154 studies, of which 24 were considered relevant to this study. The selected studies provided important data considering cement layer thickness, tooth preparation, endodontic post, and type of resin-matrix cement. The anatomical variability of root canal systems, such as the oval- or C-shaped, represents a challenge in dental restoration with tooth root intracanal posts. The fitting of intracanal posts to different root regions is variable resulting in thick and irregular layers of resin-matrix cement. Defects like pores, micro-cracks, and micro-gaps were detected in the resin-matrix cement microstructure and represent spots of stress concentration and fracture. Custom-made tooth root intracanal posts provide a proper fitting and decrease the layer thickness of resin-matrix cement. CONCLUSIONS: In fact, the layer thickness of resin-matrix cements depends on the fitting of endodontic posts to tooth root canals. An increase of resin cement thickness causes the appearance of defects like pores, micro-cracks, and micro-gaps that can induce stress concentration and fractures at interfaces. CLINICAL RELEVANCE: The fitting of the endodontic post into the teeth root canal determine the layer thickness of the resin-matrix cement to establish an adequate retention. However, the increase in the thickness of the resin-matrix cement layer can lead to a high number of defects like pores or cracks and therefore decrease the strength of the interface.
