BACKGROUND: HIV infection continues to be a global public health challenge, affecting approximately 1.7 million reproductive-aged women. Protease inhibitor-based highly active antiretroviral therapy (PI-HAART) has significantly reduced the risk of vertical transmission of HIV from mother to child. Nevertheless, concerns linger regarding the long-term effects, particularly on body composition, notably subcutaneous fat tissue (SFT). Although HIV-associated lipodystrophy syndrome (LS) has been well documented in adults and older children, its impact on fetuses exposed to PI-HAART remains underexplored. This study aims to evaluate SFT in the fetuses of HIV-pregnant women exposed to PI-HAART, assessing the potential clinical implications. METHODS: We conducted a comparative study between HIV-pregnant women receiving PI-HAART and an HIV-negative control group. Fetometry measurements were obtained via 3D ultrasound. SFT in the fetal arm and thigh segments was assessed. Data were analyzed using lineal multivariate regression and receiver-operating characteristics (ROC)-curve analysis. RESULTS: Fetuses exposed to PI-HAART exhibited a significant reduction in subcutaneous fat, particularly in the proximal third-middle union of the femur (coefficient: -2.588, p = 0.042). This reduction was correlated with lower newborn serum glucose levels (65.7 vs. 56.1, p = 0.007; coefficient: -1.277, p = 0.045). CONCLUSIONS: Our study sheds light on the connection between PI-HAART, fetal subcutaneous fat, and neonatal health. These findings might reveal the long-lasting effects of PI-HAART on newborns and children's well-being. Our results emphasize the need for a more balanced approach to managing pregnant women with HIV in developing countries and open new venues for research on the impact of intrauterine PI-HAART exposure on energy metabolism and fetal programming.

Cachexia may be caused by congestive heart failure, and it is then called cardiac cachexia, which leads to increased morbidity and mortality. Cardiac cachexia also worsens skeletal muscle degradation. Cardiac cachexia is the loss of edema-free muscle mass with or without affecting fat tissue. It is mainly caused by a loss of balance between protein synthesis and degradation, or it may result from intestinal malabsorption. The loss of balance in protein synthesis and degradation may be the consequence of altered endocrine mediators such as insulin, insulin-like growth factor 1, leptin, ghrelin, melanocortin, growth hormone and neuropeptide Y. In contrast to many other health problems, fat accumulation in the heart is protective in this condition. Fat in the heart can be divided into epicardial, myocardial and cardiac steatosis. In this review, we describe and discuss these topics, pointing out the interconnection between heart failure and cardiac cachexia and the protective role of cardiac obesity. We also set the basis for possible screening methods that may allow for a timely diagnosis of cardiac cachexia, since there is still no cure for this condition. Several therapeutic procedures are discussed including exercise, nutritional proposals, myostatin antibodies, ghrelin, anabolic steroids, anti-inflammatory substances, beta-adrenergic agonists, medroxyprogesterone acetate, megestrol acetate, cannabinoids, statins, thalidomide, proteasome inhibitors and pentoxifylline. However, to this date, there is no cure for cachexia.

RESUMEN

Cellulite affects almost all women, and it is characterized by surface relief alterations, mainly located on the thighs and buttocks, and other areas. Whereas depressed lesions occur due to the presence of thick subcutaneous fibrous septa that pull the skin surface down, raised areas result from the projection of underlying fat to the skin surface. We support that the absence of cellulite can be defined as the ideal balance between the mechanical forces that act between the subcutaneous structures, such as fat and fibrous septa and muscles, and the overlying skin.

Intermittent food restriction (IFR) is used mainly for weight loss; however, its effects on adipose tissue are not known when alternating with an obesogenic diet. To demonstrate its effects on morphological dynamics of fat deposits, female Wistar rats were distributed into groups: standard control (ST-C), with commercial diet; DIO control (DIO-C), with a diet that induces obesity (DIO) during the first and last 15 d, replaced by a standard diet for thirty intermediate days; standard restricted (ST-R), with standard diet during the first and last 15 d, with six cycles of IFR at 50 % of ST-C; and DIO restricted (DIO-R), in DIO during the first and last 15 d, with six cycles of IFR at 50 % of DIO-C. At 105 d of life, white adipose tissue (WAT) and brown adipose tissue (BAT) deposits were collected, weighed and histology performed. The DIO-R group showed higher total food intake (DIO-R 10 768·0 (SEM 357·52) kJ/g v. DIO-C 8868·6 (SEM 249·25) kJ/g, P < 0·0001), energy efficiency during RAI (DIO-R 2·26 (SEM 0·05) g/kJ v. DIO-C 0·70 (SEM 0·03) g/kJ, P < 0·0001) and WAT (DIO-R 5·65 (SEM 0·30) g/100 g v. DIO-C 4·56 (SEM 0·30) g/100 g) than their respective control. Furthermore, IFR groups presented hypertrophy of WAT and BAT, as well as fibrosis in BAT. Thus, IFR can establish prospective resistance to weight loss by favouring changes in adipose tissue morphology, increased energy intake and efficiency. Finally, the DIO diet before and after IFR aggravates the damages caused by the restriction.

Objetivo. Analizar la asociación entre la fuerza de prensión manual relativa (FPMR) y la masa grasa (MG), controlando el posible efecto del estado de madurez.Metodología. Se estudiaron niños y adolescentes de ambos sexos con un rango de edad de ≥ 7,5 a ≤ 15,49 años. Se evaluó la FPMR mediante un dinamómetro hidráulico manual. El porcentaje de MG se evaluó mediante la absorciometría de rayos X de doble energía. Para analizar las diferencias por sexo y grupo etario y entre los niños normopesos y obesos en los niveles de FPMR, se utilizó el análisis de covarianza. Se consideraron como covariable los años de pico de velocidad de crecimiento (estado de madurez). Para analizar la asociación entre los niveles de FPMR y MG, se utilizó la correlación parcial, controlando el año de pico de velocidad de crecimiento.Resultados. Participaron 1685 escolares (731 mujeres y 954 varones). Fueron conformados 4 grupos de edad (7,5-9,4 años; 9,5-11,4 años; 11,5-13,4 años y 13,5-15,4 años). La FPMR aumentó con la edad en ambos sexos. Los valores de MG fueron elevados en todos los grupos etarios. No hubo diferencias en los grupos 3 y 4 en las niñas y en el grupo 4 de los niños. Los niños clasificados como peso normal presentaron significativamente mayor FPMR que los obesos. Se observó una asociación negativa entre FPMR y MG.Conclusión. Se demostró que la FPMR estaba negativamente asociada con la MG, tras controlar el efecto del estado de madurez

The objective was to evaluate whether the juice from the leaves of cacti of three species of the genus Pereskia promotes changes in the physiological parameters of Wistar rats. The study was divided into stage 1 (obesity induction with a hypercaloric diet, monosodium glutamate, and sucrose solution), and stage 2 (use of cactus juice). The data of body weight, Body Mass and Lee Indexes, feed intake, adipose tissue mass, and Visceral and Epididymal Fat Indexes were compared by the Tukey test at 5%. Monosodium glutamate and sucrose in association with the hypercaloric diet did not increase adipose tissues. No statistical difference was found between the means of body weight, Body Mass Index and Lee Index, liver weight, and Hepatosomatic Index. Pereskia grandifolia juice promoted a lower total weight gain due to the low feed intake. Pereskia aculeata juice increased the visceral adipose tissue mass. Thus, the Pereskia grandifolia juice presented a better effect on weight gain. These cacti are rich in nutrients and bioactive compounds that can improve food quality, and prevent chronic non-communicable diseases.(AU)


O objetivo foi avaliar se o suco das folhas de cactáceas de três espécies do gênero Pereskia promove alterações dos parâmetros fisiológicos de ratos Wistar. O estudo foi dividido na etapa 1 (indução da obesidade com dieta hipercalórica, glutamato monossódico e solução de sacarose) e etapa 2 (utilização do suco das cactáceas). Os dados do peso corporal, Índices de Massa Corporal e de Lee, consumo alimentar, massa dos tecidos adiposos e seus Índices de Gordura Visceral e Epididimal foram comparados pelo Teste de Tukey a 5%. O glutamato monossódico e a sacarose em associação com a dieta hipercalórica não aumentaram os tecidos adiposos. Não houve diferença estatística entre as médias do peso corporal, Índice de Massa Corporal e Índice de Lee, peso hepático e o Índice Hepato-Somático. O suco da Pereskia grandifolia promoveu um menor ganho de peso total como resultado do baixo consumo alimentar. O suco da Pereskia aculeata aumentou a massa do tecido adiposo visceral. Concluiu-se que o suco da Pereskia grandifolia apresentou melhor efeito sobre o ganho de peso. Tais cactáceas são ricas em nutrientes e compostos bioativos que poderão melhorar a qualidade alimentar e prevenir doenças crônicas não transmissíveis.(AU)

Introdução: Durante os últimos anos, os princípios cirúrgicos da abdominoplastia permaneceram inalterados. Portanto, muitos resultados observados apresentam desalinhamento, cicatrizes transversais altas e retas do abdome, levando ao posicionamento final da cicatriz umbilical a ser muito próximo da cicatriz transversal, o que dá a impressão de abdome curto. Propomos que a abdominoplastia modifique a concepção básica de sua marcação, pois acreditamos que é importante posicionar a cicatriz transversal mais baixa na região medial e púbica, e mais alta nas extremidades laterais, permitindo, no nível dos flancos, a rotação dos retalhos lombares no sentido anterior em direção inferomedial. Métodos: Foram analisados de forma retrospectiva 146 pacientes portadores de deformidades abdominais e os submetemos a lipomidiabdominoplastia, marcando com forte concavidade superior e orientando os lados da cicatriz em direção à linha transversa inferior do abdômen, 4cm equidistantes da raiz da coxa. Também associamos a lipoaspiração como um tratamento complementar ao contorno corporal. Resultados: Consideramos que os parâmetros da midiabdominoplastia são aplicáveis na maioria dos casos, obtendo resultados igualmente satisfatórios, tanto nos pacientes com flacidez e lipodistrofia abdominal supraumbilical, quanto nos pacientes com abdome em avental com importante flacidez e diástase dos retos abdominais. Conclusão: É importante determinar a área da deformidade abdominal e sua classificação, para estabelecer as estratégias do tratamento e associação de procedimentos complementares. Uma marcação mais baixa, respeitando as áreas de tratamento, permitirá uma melhor cicatriz estética e um contorno corporal harmônico, além de uma adequada colocação dos elementos: cicatriz umbilical, púbis e extremidades laterais da cicatriz abdominal transversa.


Introduction: During the last few years, the surgical principles of abdominoplasty have remained unchanged. Therefore, many patients undergoing this technique have misalignment and high and straight transverse scars of the abdomen, with the final position of the umbilical scar being very close to the transverse scar, making the abdomen seem short. We propose modifying the basic concept of marking in abdominoplasty, because we believe it is important to position the transverse scar lower in the medial and pubic region and higher at the lateral ends, allowing anterior lumbar flap rotation in an inferomedial direction. Methods: We retrospectively analyzed 146 patients with abdominal defects and subjected them to lipo-mid-abdominoplasty, marking with strong upper concavity and guiding the sides of the scar towards the lower transverse line of the abdomen, 4 cm equidistant from the root of the thigh. We also define liposuction as a complementary treatment to body contouring. Results: We consider that midabdominoplasty parameters are applicable in most cases, obtaining satisfactory results both in patients with flatness and supraumbilical abdominal lipodystrophy and patients with an "apron" abdomen with considerable flaccidity and diastasis of the abdominal rectus. Conclusion: It is important to determine the area of the abdominal defect and its classification to establish treatment strategies and association with complementary procedures. A lower marking with respect to the treatment areas will allow a more aesthetic scar and a harmonic body contour as well as an adequate placement of the umbilical scar, pubis, and lateral ends of the transverse abdominal scar.

Background: Appetite disorders are frequent and scantly studied in peritoneal dialysis (PD) patients and are associated with malnutrition and cardiovascular complications. Objective: We investigated the relationship between uremic insulin resistance, pro-inflammatory cytokines, and appetite-related peptides release (ARPr) with eating-behavior disorders in PD patients. Methods: We included 42 PD patients (12 suffering anorexia, 12 obese with high food-intake, and 18 asymptomatic) and 10 controls. We measured blood levels of ARPr including orexigens [neuropeptide-Y (NPY), ghrelin, and nitric-oxide], anorexigens [cholecystokinin, insulin, corticotropin-releasing factor, leptin, and adiponectin (Ad)], and cytokines (TNF-α, sTNFα-R2, and IL-6) both at baseline and after administering a standard-food stimulus (SFS). We also measured the expression of TNF-α, leptin and Ad-encoding mRNAs in abdominal adipose tissue. We compared these markers with eating motivation measured by a Visual Analog Scale (VAS). Results: Anorexics showed both little appetite, measured by a VAS, and low levels of orexigens that remained constant after SFS, coupled with high levels of anorexigens at baseline and after SFS. Obeses showed higher appetite, increased baseline levels of orexigens, lower baseline levels of anorexigens and cytokines and two peaks of NPY after SFS. The different patterns of ARPr and cytokines pointed to a close relationship with uremic insulin resistance. In fact, the euglycemic-hyperglycemic clamp reproduced these disorders. In anorexics, TNF-α fat expression was increased. In obese patients, leptin expression in fat tissue was down-regulated and showed correlation with the appetite. Conclusion: In PD, appetite is governed by substances that are altered at baseline and abnormally released. Such modulators are controlled by insulin metabolism and cytokines and, while anorexics display inflammatory predominance, obese patients predominantly display insulin resistance.

BACKGROUND/AIM: Periprostatic adipose tissue (PPAT) directs tumour behaviour. Microenvironment secretome provides information related to its biology. This study was performed to identify secreted proteins by PPAT, from both prostate cancer and benign prostate hyperplasia (BPH) patients. PATIENTS AND METHODS: Liquid chromatography-mass spectrometry-based proteomic analysis was performed in PPAT-conditioned media (CM) from patients with prostate cancer (CMs-T) (stage T3: CM-T3, stage T2: CM-T2) or benign disease (CM-BPH). RESULTS: The highest number and diversity of proteins was identified in CM-T3. Locomotion was the biological process mainly associated to CMs-T and reproduction to CM-T3. Immune responses were enriched in CMs-T. Extracellular matrix and structural proteins were associated to CMs-T. CM-T3 was enriched in proteins with catalytic activity and CM-T2 in proteins with defense/immunity activity. Metabolism and energy pathways were enriched in CM-T3 and those with immune system functions in CMs-T. Transport proteins were enriched in CM-T2 and CM-BPH. CONCLUSION: Proteins and pathways reported in this study could be useful to distinguish stages of disease and may become targets for novel therapies.

BACKGROUND: Cellulite is a common aesthetic condition that affects almost every woman. OBJECTIVE: To evaluate the efficacy of acoustic wave therapy (AWT) for cellulite and body shaping. METHODS: In this open-label, single-centre trial, 30 women presenting moderate or severe cellulite underwent 12 sessions of AWT on the gluteus and back of the thighs, over six weeks. The following assessments were performed at baseline, and up to 12 weeks after treatment: Cellulite Severity Scale (CSS), body circumference measurements, subcutaneous fat thickness by magnetic resonance imaging (MRI), quality of life related by Celluqol® and a satisfaction questionnaire. RESULTS: The treatment reduced cellulite severity from baseline up to 12 weeks after the last treatment session (subjects presenting severe cellulite: 60% to 38%). The mean CSS shifted from 11.1 to 9.5 (p < 0.001). Hips circumferences reduced from 99.7 and 103.2 cm to 96.2 and 100.3 cm (respectively, p < 0.001). The average thickness of the subcutaneous fat tissue also decreased (28.3 ± 6.5 mm to 26.7 ± 6.1mm; p < 0.001). The treatment also improved quality of life. More than 90% of the subjects would undergo the treatment again and 89% were satisfied with the results. No serious adverse events were reported. CONCLUSIONS: AWT is a safe treatment to improve cellulite appearance and reduce body circumferences.

The main objective of this study was to obtain information on indicators of obesity among the unemployed male subjects from the Tuzla Canton. Also, we wanted to determine whether there are significant differences in the observed parameters between subjects in relation to age. The main criterion for the selection of test subjects is that they are unemployed. This is because we felt that those persons have more free time for practicing sports and recreational activities, and there is a very small number of studies in the area of Tuzla Canton with the characteristics of the sample. For the purpose of this research we applied analysis of percentage/ratio, t-test for independent samples and the Mann-Whitney U Test. Applied analysis showed that the increased value of Body Mass Index (BMI) and to the account of increased amounts of body fat has 31.5% of the respondents. It also found that there are significant differences in the variables Body Mass Index (BMI), body fat percentage (% FAT) and the total weight of fat mass (in kg) in the body (FATMAS-kg) in favor of the subjects belonging to the older age group. The results indicate the existence of the problem of obesity in males from Tuzla Canton, especially in the older age group. It is therefore necessary in adolescence or even earlier, to start with education and systematic practice of sports and recreational activities with respect to dietary habits.


El objetivo de este estudio fue obtener información sobre los indicadores de obesidad entre los varones desempleados en Tuzla Canton. Además, determinar si existen diferencias significativas en los parámetros observados entre los sujetos en relación con la edad. El criterio principal para la selección de los sujetos fue que se encontraran desempleados. Esto debido a que estas personas tienen más tiempo libre para la práctica de actividades deportivas y recreativas, y existe un número muy pequeño de estudios en el área de Tuzla Canton con las características de esta muestra. A los efectos de esta investigación se aplicó el análisis de porcentaje/proporción, prueba t, muestras independientes y la prueba U de Mann-Whitney. El análisis aplicado mostró que el aumento del valor de índice de masa corporal (IMC) y una mayor cantidad de grasa corporal se observó en un 31,5% de los encuestados. También se encontró que existen diferencias significativas en las variables del IMC, porcentaje de grasa corporal y en el peso total de la masa de grasa (en kg) en el cuerpo en los sujetos pertenecientes al grupo de mayor edad. Los resultados indican la existencia del problema de la obesidad en los varones de Tuzla Canton, especialmente en el grupo de mayor edad. Por ello es necesario en la adolescencia, e incluso antes, comenzar con la educación y la práctica sistemática de actividades deportivas y recreativas con respecto a los hábitos alimenticios.

BACKGROUND: Epicardial fat thickness (EFT) has been evaluated as a marker of cardiovascular disease, with good correlation with classical cardiovascular risk factors in the general population. The aim of this study was to evaluate the EFT in subclinical hypothyroidism (SCH), in comparison to a group without thyroid dysfunction. METHODS: A cross-sectional study was performed with 100 participants, including 52 SCH patients and 48 individuals without any thyroid dysfunction (euthyroid group-EU). Transthoracic echocardiography (TE), thyroid hormone levels, lipid profile, and assessment of body composition by bioelectrical impedance (BIA) and anthropometry were measured in all subjects. RESULTS: The SCH and EU groups were comparable with respect to age, gender, and Framingham risk scores. Serum thyroid-stimulating hormone (TSH) was 6.7 ± 1.4 mIU/L in the SCH group and 2.0 ± 0.84 mIU/L in the control group. EFT was similar in both groups (SCH 3.5 ± 1.3 mm, EU 3.5 ± 1.1 mm, p = 0.43). EFT showed a slight trend for a positive correlation with serum TSH in the SCH group (r s = 0.263, p = 0.05). EFT correlated with the body fat percentage in the SCH group (r s = 0.350, p = 0.03) and EU group (r s = 0.033, p = 0.04). EFT in this cohort was not independently correlated to changes in TSH and Framingham risk score. CONCLUSIONS: EFT determination by TE does not seem to be a good marker of cardiovascular risk in SCH patients with serum TSH <10.0 mIU/L and no pre-existing cardiovascular morbidity.

Background: Body contouring has recently assumed an important role in body aesthetics. Liposuction in combination with free transplantation of adipose tissue is critical in such procedures. This study assessed the results of fat grafts implanted in unusual sites. Method: Over the past 21 years, the authors have performed 4,405 adipose tissue grafting procedures; 1,407 were classified as transfers to unusual sites. The technique used is based on histological studies and is thoroughly described, including obtaining, preparation, implantation, and postoperative care. The viability of the transplanted tissue was assessed by photographic documentation, a patient questionnaire, and clinical assessment. Results: Part of the transplanted tissue was viable in 100% of cases. The percent volume that remained in the implanted site was approximately 40% of the grafted tissue. The possibility of a late increase in transplanted adipose tissue volume was considered; such changes may be associated with a lack of genetic control in the mesenchymal stem cells present in the adipose tissue. Conclusions: Free transplants of adipose tissue in this series exhibited clinical and percent-wise progress in terms of viability, similar to grafting performed at other sites. Aviability of approximately 40% of grafted adipose tissue was possible because of the care taken throughout the procedure.


Introdução: Recentemente a definição do contorno corporal assumiu importante papel na estética do corpo. A lipoaspiração associada aos transplantes livres de tecido adiposo é fundamental nesses procedimentos. O objetivo deste estudo foi avaliar os resultados dos enxertos gordurosos implantados em locais pouco habituais. Método: Nos últimos 21 anos, os autores realizaram 4.405 enxertos de tecido adiposo, dos quais 1.407 foram classificados como transferidos para locais pouco habituais. A técnica utilizada é fundamentada em estudos histológicos e descrita em todas as suas etapas, isto é, na obtenção, no preparo, na implantação e nos cuidados pós-operatórios. A viabilidade do tecido transplantado foi avaliada através de documentação fotográfica, de questionário respondido pelos pacientes e de avaliação clínica. Resultados: Foi possível comprovar a viabilidade de parte do tecido transplantado em 100% dos casos. O porcentual do volume que permaneceu na local implantado foi de aproximadamente 40% do tecido enxertado. Os autores chamam a atenção para a possibilidade tardia de aumento do volume do tecido adiposo transplantado, e relacionam essa alteração como um provável descontrole gênico das células-tranco mesenquimais presentes no tecido adiposo. Conclusões: Os transplantes livres de tecido adiposo avaliados nesta série apresentaram evolução clínica e porcentual de viabilidade semelhantes aos de enxertos realizados em outros locais. A viabilidade em torno de 40% do tecido adiposo enxertado foi possível graças aos cuidados tomados em todas as etapas do procedimento, conforme a técnica descrita.

The objective was evaluated the effects of the use of additives antioxidants associated or not with ractopamine in feed for finishing pigs. Were used 51 crossbred pigs, 27 barrows and 24 females with initial weight of 83.38 kg. Three experimental treatments were established based on the inclusion of two complexes to feed: complex 1 (4g de vitamin E + 1g de vitamin C/kg of product) and complex 2 (0.2g de ractopamine + 4g de vitamin E + 2g de vitamin C/kg of product), defining the ration control (free of complexes), ration control + complex 1 (0.1%) and ration control + complex 2 (0.05%). The experimental design was blocked randomically, presenting a factorial model 3x2 (3 treatments and 2 sexes). Were observed difference (P 0.05) for daily feed intake, feed, hot and cold carcass weight to the treatment with ractopamine. The values of vitamin E in meat were higher (P 0.05) in muscle of animals in groups receiving the vitamin. There was a difference (P 0.05) in the values of oxidation and marbling in the treatments with antioxidant. The inclusion of ractopamine, with the antioxidants, resulted in improvement in performance and carcass characteristics, and supplementation with the antioxidant, alone, showed improvement in parameters related to meat quality.


O objetivo do trabalho foi avaliar a utilização de aditivos antioxidantes associados ou não à ractopamina em rações de suínos em terminação. Utilizou-se 51 suínos, 27 machos castrados e 24 fêmeas, com peso médio inicial de 83,38 kg. Foram estabelecidos três tratamentos experimentais, baseados na inclusão de dois complexos às rações: Complexo 1 (4g de vitamina E + 1g de vitamina C/kg do produto) e Complexo 2 (0,2g de ractopamina + 4g de vitamina E + 2g de vitamina C/kg de produto), definindo: ração controle (isenta dos Complexos); ração controle + Complexo 1 (0,1%); e ração controle + Complexo 2 (0,05%). O delineamento experimental foi em blocos casualizados, num arranjo fatorial 3 x 2 (3 tratamentos e 2 sexos). This neoplasia has an excellent prognosis but its growth rate can be rapid and reach a large size. In veterinary literature lipoblastoma has been reported only in a new born (2 day old) male calf. This case report describes the clinical, histochemical and immunohistochemical features of a lipoblastoma in a dog. The dog was followed up for 20 months after surgery and found to be healthy with no signs of recurrence.(AU)
