RESUMEN

Introduction: Freezing of gait (FOG) is a highly disabling symptom in Parkinson's Disease (PD) with varying degree of benefits from oral dopaminergic medications and several subtypes that present with different medication states (e.g., off FOG, on FOG, pseudo-on FOG, supra-on FOG). Levodopa-Carbidopa Intestinal Gel (LCIG) greately reduces the variability of cerebral dopamine replacement inherent to oral therapies by continuous levodopa intestinal infusion. While LCIG may be superior to oral therapy in its ability to treat motor fluctuations and minimize off-time, there is no consensus regarding the overall effectiveness of LCIG specifically for the treatment of FOG in PD patients. Methods: A systematic literature review was conducted to understand the efficacy of LCIG to treat FOG in PD patients. A PubMed search was conducted using the search query "Intestinal AND (Levodopa OR L-dopa) AND Freezing of Gait AND Parkinson." Additional eligibility criteria included articles written in English and currently published journal articles. Articles were excluded if they did not have a clinical design or if they did not yield reportable data on FOG. Results: The literature search yielded 16 articles, of which 10 articles were included. Of the 10 studies included, there were 3 retrospective studies, 6 case reports or case series, and 1 open-label study. (n = 449 patients total and 318 FOG patients). Nine of the 10 studies concluded that LCIG has a favorable effect on FOG, though the metrics to evaluate benefits of LCIG on FOG varied among the articles. Conclusion: LCIG may be an effective treatment for PD patients suffering from FOG including those with poor response to oral medication, likely because of its ability to maintain steadier dopamine levels. Further research is necessary on LCIG as a therapy for refractory FOG, with particular attention to the different subtypes of FOG.

OBJECTIVE: To investigate the effect of action observation therapy (AOT) in the rehabilitation of neurologic and musculoskeletal conditions. DATA SOURCES: Searches were completed until July 2020 from the electronic databases Allied and Complementary Medicine Database (via OVID SP), Cumulative Index to Nursing and Allied Health Literature, Cochrane Library, EMBASE, MEDLINE, and the Physiotherapy Evidence Database. STUDY SELECTION: Randomized controlled trials comparing AOT with standard care were assessed. Musculoskeletal (amputee, orthopedic) and neurologic (dementia, cerebral palsy, multiple sclerosis, Parkinson disease, stroke) conditions were included. There were no age limitations. Articles had to be available in English. DATA EXTRACTION: Two reviewers independently screened titles, abstracts and full extracts of studies for eligibility and assessed the risk of bias of each study using the Cochrane Risk of Bias Tool. Data extraction included participant characteristics and intervention duration, frequency, and type. RESULTS: The effect of AOT in different outcome measures (OMs) was referenced in terms of body structures and functions, activities and participation, and environmental factors as outlined by the International Classification of Functioning, Disability, and Health (ICF). Of the 3448 articles identified, 36 articles with 1405 patients met the inclusion criteria. Seven of the 11 meta-analyses revealed a significant effect of intervention, with results presented using the mean difference and 95% CI. A best evidence synthesis was used across all OMs. Strong evidence supports the use of AOT in the rehabilitation of individuals with stroke and Parkinson disease; moderate evidence supports AOT in the rehabilitation of populations with orthopedic and multiple sclerosis diagnoses. However, moderate evidence is provided for and against the effect of AOT in persons with Parkinson disease and cerebral palsy. CONCLUSIONS: This review suggests that AOT is advantageous in the rehabilitation of certain conditions in improving ICF domains. No conclusions can be drawn regarding treatment parameters because of the heterogeneity of the intervention. AOT has been considerably less explored in musculoskeletal conditions.

INTRODUCTION: Parkinson's disease patients are usually characterized by body motor dysfunction due to dopaminergic reduction in the central nervous system. Freezing of gait is a motor disorder that affects certain Parkinson's disease patients. However, it is hypothesized that non-motor functions mediated by the cholinergic system are also involved in developing freezing of gait. Visual information processing speed, or inspection time is independent of the motor response, and can be used a reliable measure of the cholinergic system integrity. OBJECTIVE: Inspection time can be used to investigate whether Parkinson's disease patients with freezing of gait symptoms have a larger impairment in cholinergic mediated functions than those patients who have no freezing of gait symptoms and healthy controls. METHODS: The inspection time was determined by a simple length discrimination task. Twenty-two Parkinson's disease patients with freezing of gait, 25 Parkinson's disease patients without freezing of gait, and 25 aged matched healthy controls participated in the study. RESULTS: Based on the log values of IT score, Parkinson's disease patients with freezing of gait symptoms had statistically significant slower inspection times (mean of 1.793 ms) than Parkinson's disease patients without freezing of gait (mean of 1.655 ms) and healthy controls (mean of 1.523 ms). Inspection times for the Parkinson's disease patients without FOG symptoms were also significantly slower than healthy controls. CONCLUSION: The results of this study support the hypothesis that the cholinergic system integrity is affected more in Parkinson's disease patients with freezing of gait symptoms.
