BACKGROUND: A single administration of mesenchymal stromal cells (MSCs) has been shown to reduce lung inflammation in experimental elastase-induced emphysema; however, effects were limited in terms of lung-tissue repair and cardiac function improvement. We hypothesized that two doses of MSCs could induce further lung and cardiovascular repair by mitigating inflammation and remodeling in a model of emphysema induced by multiple elastase instillations. We aimed to comparatively investigate the effects of one versus two doses of MSCs, administered 1 week apart, in a murine model of elastase-induced emphysema. METHODS: C57BL/6 mice were randomly divided into control (CTRL) and emphysema (E) groups. Mice in the E group received porcine pancreatic elastase (0.2 IU, 50 µL) intratracheally once weekly for four consecutive weeks; the CTRL animals received sterile saline (50 µL) using the same protocol. Three hours after the last instillation, the E group was further randomized to receive either saline (SAL) or murine MSCs (105 cells) intratracheally, in one or two doses (1 week apart). Fourteen days later, mice were euthanized, and all data analyzed. RESULTS: Both one and two doses of MSCs improved lung mechanics, reducing keratinocyte-derived chemokine and transforming growth factor-ß levels in lung homogenates, total cell and macrophage counts in bronchoalveolar lavage fluid (BALF), and collagen fiber content in airways and blood vessels, as well as increasing vascular endothelial growth factor in lung homogenates and elastic fiber content in lung parenchyma. However, only the two-dose group exhibited reductions in tumor necrosis factor-α in lung tissue, BALF neutrophil and lymphocyte count, thymus weight, and total cellularity, as well as CD8+ cell counts and cervical lymph node CD4+ and CD8+ T cell counts, as well as further increased elastic fiber content in the lung parenchyma and reduced severity of pulmonary arterial hypertension. CONCLUSIONS: Two doses of MSCs enhanced lung repair and improvement in cardiac function, while inducing T cell immunosuppression, mainly of CD8+ cells, in elastase-induced emphysema.

Many experimental models have been proposed to study the pathophysiological features of emphysema, as well as to search for new therapeutic approaches for acute or chronically injured lung parenchyma. We aimed to characterize an emphysema model induced by multiple instillations of elastase by tracking changes in inflammation, remodeling, and cardiac function after each instillation. Forty-eight C57BL/6 mice were randomly assigned across two groups. Emphysema (ELA) animals received 1, 2, 3, or 4 intratracheal instillations of pancreatic porcine elastase (PPE, 0.2 IU) with a 1-week interval between them. Controls (C) received saline following the same protocol. Before and after implementation of the protocol, animals underwent echocardiographic analysis. After the first instillation of PPE, the percentage of mononuclear cells in the lung parenchyma increased compared to C (p = 0.0001). The second instillation resulted in hyperinflated alveoli, increased mean linear intercept, and reduced elastic fiber content in lung parenchyma compared to C (p = 0.0197). Following the third instillation, neutrophils and collagen fiber content in alveolar septa and airways increased, whereas static lung elastance was reduced compared to C (p = 0.0094). After the fourth instillation, the percentage of M1 macrophages in lungs; levels of interleukin-1ß (IL-1ß), keratinocyte-derived chemokine, hepatocyte growth factor (HGF), and vascular endothelial growth factor (VEGF); and collagen fiber content in the pulmonary vessel wall were increased compared to C (p = 0.0096). At this time point, pulmonary arterial hypertension was apparent, with increased diastolic right ventricular wall thickness. In conclusion, the initial phase of emphysema was characterized by lung inflammation with predominance of mononuclear cells, whereas at the late stage, impairment of pulmonary and cardiovascular functions was observed. This model enables analysis of therapies at different time points during controlled progression of emphysema. Accordingly, early interventions could focus on the inflammatory process, while late interventions should focus on restoring cardiorespiratory function.

Emphysema is characterized by loss of lung tissue elasticity and destruction of structures supporting alveoli and capillaries. The impact of mechanical ventilation strategies on ventilator-induced lung injury (VILI) in emphysema is poorly defined. New ventilator strategies should be developed to minimize VILI in emphysema. The present study was divided into two protocols: (1) characterization of an elastase-induced emphysema model in rats and identification of the time point of greatest cardiorespiratory impairment, defined as a high specific lung elastance associated with large right ventricular end-diastolic area; and (2) comparison between variable (VV) and conventional volume-controlled ventilation (VCV) on lung mechanics and morphometry, biological markers, and cardiac function at that time point. In the first protocol, Wistar rats (n = 62) received saline (SAL) or porcine pancreatic elastase (ELA) intratracheally once weekly for 4 weeks, respectively. Evaluations were performed 1, 3, 5, or 8 weeks after the last intratracheal instillation of saline or elastase. After identifying the time point of greatest cardiorespiratory impairment, an additional 32 Wistar rats were randomized into the SAL and ELA groups and then ventilated with VV or VCV (n = 8/group) [tidal volume (VT) = 6 mL/kg, positive end-expiratory pressure (PEEP) = 3 cmH2O, fraction of inspired oxygen (FiO2) = 0.4] for 2 h. VV was applied on a breath-to-breath basis as a sequence of randomly generated VT values (mean VT = 6 mL/kg), with a 30% coefficient of variation. Non-ventilated (NV) SAL and ELA animals were used for molecular biology analysis. The time point of greatest cardiorespiratory impairment, was observed 5 weeks after the last elastase instillation. At this time point, interleukin (IL)-6, cytokine-induced neutrophil chemoattractant (CINC)-1, amphiregulin, angiopoietin (Ang)-2, and vascular endothelial growth factor (VEGF) mRNA levels were higher in ELA compared to SAL. In ELA animals, VV reduced respiratory system elastance, alveolar collapse, and hyperinflation compared to VCV, without significant differences in gas exchange, but increased right ventricular diastolic area. Interleukin-6 mRNA expression was higher in VCV and VV than NV, while surfactant protein-D was increased in VV compared to NV. In conclusion, VV improved lung function and morphology and reduced VILI, but impaired right cardiac function in this model of elastase induced-emphysema.

BACKGROUND: Non-ablative fractional lasers have been effectively used in skin rejuvenation. OBJECTIVES: This study evaluates the efficacy of 1550-nm Erbium glass laser for facial rejuvenation through the correlation of clinical evaluation and histopathology, immunohistochemistry, and electron microscopy analysis. METHODS: Fifteen subjects (average age: 56.4 years, skin types: I-III) with mild-to-moderate photodamage were submitted to biopsies and 3 facial treatments. Data from the photo assessments and the clinical improvement were analyzed 4 months after the treatments. The biopsy skins were fixed in neutral buffered formalin before being embedded in paraffin, and stained with hematoxylin and eosin. The histomorphometric quantification of collagen and elastic fibers; intercellular adhesion molecule 1 expression by immunohistochemistry; and analysis of cell cycle phases, the electrical potential of the mitochondrial, and interleukin (IL)-1, CD34, transforming growth factor (TGF)-ß, and caspase-3 expression by flow cytometry were analyzed. RESULTS: After 4 months of treatment, collagen fibers had increased by 6.68%, and intercellular adhesion molecule 1 (ICAM-1) had increased by 4.47% in vessel area. Significantly enhanced IL-1 and TGF-ß receptor expressions were identified after treatment. Proliferative responses and non-apoptosis-dependent caspase-3 activity were both observed in the cell after dermal treatment. CONCLUSION: The histopathology, immunohistochemistry, and electron microscopy showed an improvement compatible to the clinical effectiveness after 4 months.

Emphysema is an intractable pulmonary disease characterized by an inflammatory process of the airways and lung parenchyma and ongoing remodeling process in an attempt to restore lung structure. There is no effective drug therapy that regenerates lung tissue or prevents the progression of emphysema; current treatment is aimed at symptomatic relief. We hypothesized that LASSBio-596, a molecule with potent anti-inflammatory and immunomodulatory effects, might reduce pulmonary inflammation and remodeling and thus improve lung function in experimental emphysema. Emphysema was induced in BALB/c mice by intratracheal administration of porcine pancreatic elastase (0.1 IU) once weekly during 4 weeks. A control group received saline using the same protocol. After the last instillation of saline or elastase, dimethyl sulfoxide, or LASSBio-596 were administered intraperitoneally, once daily for 8 days. After 24 h, in elastase-induced emphysema animals, LASSBio-596 yielded: (1) decreased mean linear intercept, hyperinflation and collagen fiber content, (2) increased elastic fiber content, (3) reduced number of M1 macrophages, (4) decreased tumor necrosis factor-α, interleukin-1ß, interleukin-6, and transforming growth factor-ß protein levels in lung tissue, and increased vascular endothelial growth factor. These changes resulted in increased static lung elastance. In conclusion, LASSBio-596 therapy reduced lung inflammation, airspace enlargement, and small airway wall remodeling, thus improving lung function, in this animal model of elastase-induced emphysema.

Introdução: Os lasers fracionados não ablativos são efetivamente utilizados no rejuvenescimento da pele. As novas tecnologias a laser permitem uma remodelação dérmica seletiva, sem ablação da epiderme. Objetivo: Avaliar a eficácia do laser Erbium Glass (Sellas Evo) fracionado não ablativo 1550nm no rejuvenescimento facial através do estudo da quantificação histomorfométrica de fibras colágenas e elásticas, a expressão da molécula de adesão intercelular 1 (ICAM-1) por imuno-histoquímica , a análise das fases do ciclo celular, o potencial elétrico da membrana mitocondrial, a expressão de interleucina-1 (IL-1), o marcador celular endotelial CD34, o receptor do fator transformador de crescimento beta (TGF-beta), atividade da caspase-3 por citometria de fluxo e as alterações ultraestruturais na pele por microscopia eletrônica de varredura, 4 meses após o tratamento com laser. Materiais e Métodos: Quinze indivíduos (média de 56,4 anos, fototipo II-IV), com fotoenvelhecimento cutâneo na face fizeram 3 tratamentos com laser Erbium Glass fracionado não ablativo 1550nm usando uma fluência de 70 mJ e uma densidade de 100 cm2. Foram avaliados biópsias na região pré-auricular em 15 pacientes no início e 4 semanas após o tratamento final. Os níveis de expressão dos receptores e a atividade do potencial elétrico mitocondrial foram analisados na suspensão de células da derme obtida a partir da digestão por colagenase. A avaliação clínica e fotográfica foi analisada quatro semanas após o final do tratamento. Resultados: Após 4 meses do início do tratamento foi observada melhora clínica moderada, com uma média de satisfação de 8,8, as fibras de colágeno aumentaram em 6,68%, as fibras elásticas mostraram uma diminuição de 12,85%, o ICAM-1 aumentou 4,47% significativamente na área dos vasos. Houve aumento significativo dos níveis de expressão do receptor de IL-1 e TGF-beta após o tratamento com laser Erbium Glass...

Objectives: to evaluate the efficacy of the 1550nm Erbium Glass Laser (Sellas Evo) for facial rejuvenation through study of the histomorphometric quantification of colagen and elastic fibers; the expression of intercellular adhesion molecule 1 (ICAM-1) by immnohistochemistry; and analysis of the cell cycle phases, the electrical potential of the mitochondrial membrane and the expression of interleukin 1 (IL-1), the endothelial cell marker CD34 and transforming growth factor beta (TGF-beta) receptors, caspase-3 activity by flow cytometry and the ultrastructural changes in the skin by scanning electron microscopy 4 months after the laser treatment. Materials and methods: fifteen subjects (mean age 56,4 skin typoes II-IV) with photodamage on the face and wrinkles had 3 treatments with the 1550 nm Erbium Glass Laser using a fluence of 70 mJ and a density of 100 cm2. Pre-auricular biopsies from 15 subjects were evaluated at baseline and 4 weeks after the final treatment. Receptor expression levels and the presence of potentially functional mitochondria were analyzed in a cell suspension obtained from collagenase digestion of the skin. Data from the photo assessments and the subjects' self-assessed improvements were analyzed 4 weeks after the final treatment.Results: Four months after the last treatmenbt application, clinical improvement was observed, with an average satgisfation score of 8.8, corresponding to moderate improvement. After 4 months of treatment, collagen fibers had increased up to 6.68%, while the average proportion of collagen fibers in the dermis, the elastic fibers, showed a 12.85% decrease of ICAM-1 and a mean increase in vessel area of 4.47%...

To evaluate the elastic system fibers in the vesicourethral junction of wistar rats at different ages, histological and histomorphometric studies were conducted. The histology study of the elastic system fibers for selective staining methods showed the presence of three types of elastic fibers. In all age groups studied, histology study satisfactorily showed the structurals differences between the mature elastic fibers, elaunin and oxytalan fibers, which were located among the intercellular spaces of the muscular layer as well as between collagen fibers. Histomorphometric studies have revealed that with the advanced age, there is a decrease in the linear density of the mature elastic and oxytalan fibers, while the linear density of elaunin fibers has increased. Thus, it could be inferred that in the vesicourethral junction of old animals, there is a fall in the elasticity, elastic recoil and anchorage properties due a loss of elastin and microfibrils, but they are compensated by the elaunin fibers, since they show intermediate characteristics between mature elastic fibers and oxytalan fibers. It could be concluded that the aging process of the elastic fibers in the vesicourethral junction does not contribute alone and directly in the urinary incontinence state, but it compensates and gives muscular support, mainly due to the increase of the elaunin fibers, what makes the elastic system in the vesicourethral junction responsible for the maintenance of the urinary continence.
