RESUMEN

INTRODUCTION AND AIM: Chronic nausea and vomiting syndrome is a disorder of gut-brain interaction that affects the productive-age population. Our aim was to determine the association of this disorder with quality of life, workplace performance, and socioeconomic impact related to gastrointestinal health. METHODS: A cross-sectional study on a Mexican population was conducted. The patients were classified as having chronic nausea and vomiting syndrome or other disorders of gut-brain interaction. A comparative analysis of quality of life, workplace productivity, annual medical consultations, and digestive health-related expenses was carried out, applying a logistic regression model. RESULTS: One thousand patients were included, 79.2% of whom met the criteria for a disorder of gut-brain interaction. Of the 792 patients, 10.3% presented with chronic nausea and vomiting syndrome. Said syndrome was associated with a negative impact on usual activities (OR 4.34, 95% CI 1.90-9.30, p ≤ 0.001), pain/discomfort (OR 2.09, 95% CI 1.31-3.33, p ≤ 0.001), anxiety/depression (OR 2.08, 95% CI 1.30-3.40, p ≤ 0.001), workplace presenteeism (OR 3.96, 95% CI 2.47-6.44, p ≤ 0.001), and workplace absenteeism (OR 2.54, 95% CI 1.52-4.16, p ≤ 0.001). There was also a higher number of annual medical consultations for digestive health (p = 0.013), without generating a greater annual expense due to digestive health (p = 0.08). CONCLUSIONS: Chronic nausea and vomiting syndrome produces a negative impact on quality of life, which could be secondary to its symptomatology or its association with anxiety and depression.

RESUMEN

OBJECTIVE: The peptide hormone ghrelin exerts potent effects in the brain, where its receptor is highly expressed. Here, we investigated the role of hypothalamic tanycytes in transporting ghrelin across the blood-cerebrospinal fluid (CSF) interface. METHODS: We investigated the internalization and transport of fluorescent ghrelin (Fr-ghrelin) in primary cultures of rat hypothalamic tanycytes, mouse hypothalamic explants, and mice. We also tested the impact of inhibiting clathrin-mediated endocytosis of ghrelin in the brain ventricular system on the orexigenic and locomotor effects of the hormone. RESULTS: In vitro, we found that Fr-ghrelin is selectively and rapidly internalized at the soma of tanycytes, via a GHSR-independent and clathrin-dependent mechanism, and then transported to the endfoot. In hypothalamic explants, we also found that Fr-ghrelin is internalized at the apical pole of tanycytes. In mice, Fr-ghrelin present in the CSF was rapidly internalized by hypothalamic ß-type tanycytes in a clathrin-dependent manner, and pharmacological inhibition of clathrin-mediated endocytosis in the brain ventricular system prolonged the ghrelin-induced locomotor effects. CONCLUSIONS: We propose that tanycyte-mediated transport of ghrelin is functionally relevant, as it may contribute to reduce the concentration of this peptide hormone in the CSF and consequently shortens the duration of its central effects.

RESUMEN

Abstract Introduction: This review article develops the basic principles for the use and action mechanisms of neuromodulators applied in clinical practice and their role in treating different disorders of gutbrain interaction (DGBI), particularly, esophageal disorders in part I. Materials and methods: The working group reviewed the most frequent pathologies and medications used according to the most recent literature and presented those with the best clinical evidence in each case. Results: Due to the diversity of disorders, types of studies, and therapeutic options, we decided to present the evidence with the best results for each case. We determined the doses used, their results, and the side effects of each one. Conclusions: The basic principles of the use and mechanisms of action of the main neuromodulators were reviewed, including their use in this section in the main esophageal gastrointestinal functional disorders. Given that the available evidence is not definitive, more controlled clinical trials are needed for each condition to confirm the effectiveness and safety of neuromodulators.

Abstract Introduction: Throughout history there have been systems of consciousness that allow us to more accurately interpret the environment, which in the case of our species allowed us to dominate matter and develop neurophysiological aspects so complex that they allow our species to wonder about its origin, physiology and existence. Materials and methods: Data were collected from various sources from various libraries that include neuroanatomy, psychology and ethology, and data obtained from geology, paleontology and archaeology, which guide us to reconstruct the phylogenetic history and ground it with current knowledge of neurology and psychiatry. Results: The appearance of complex systems such as the nervous system in species, expresses its greatest exponent the human nervous system, and based on stable evolutionary strategies, opened the way to develop more sophisticated systems of consciousness like ours. Discussion: The various hypotheses that attempt to explain the origin of our brain have revealed data that allow us to understand and interpret various forms of primitive and complex consciousness, which, even with great difficulty, allow us to reconstruct said origin, from various neurophysiological aspects and that lead us to trying to understand the very structure of the psyche, it seems to develop functionally and phylogenetically, from deep layers to superficial layers. Conclusions: We understand that the appearance of the nervous system and consciousness have a history, an important process to understand to interpret the origin of our brain and its relationship with consciousness, however, it still leaves many questions to be interpreted.

RESUMEN

Abstract: Ischemic stroke is a major cause of mortality worldwide; however, few studies have been conducted to measure the impact of the distribution of healthcare services on ischemic stroke fatality. This study aimed to explore the relationship between three ischemic stroke outcomes (incidence, mortality, and fatality) and accessibility to hospitals in Spain, considering its economic development. A cross-sectional ecological study was performed using data on hospital admissions and mortality due to ischemic stroke during 2016-2018. Gross geographic product (GGP) per capita was estimated and a healthcare accessibility index was created. A Besag-York-Mollié autoregressive spatial model was used to estimate the magnitude of association between ischemic stroke outcomes and economic development and healthcare accessibility. GGP per capita showed a geographical gradient from southwest to northeast in Spain. Mortality and case-fatality rates due to ischemic stroke were higher in the south of the country in both women and men aged 60+ years. In women and men aged 20-59 years a EUR 1,000 increase in GGP per capita was associated with decreases in mortality of 5% and 4%, respectively. Fatality decreased 3-4% with each EUR 1,000 increase of GGP per capita in both sexes and in the 20-59 and 60+ age groups. Decreased healthcare accessibility was associated with higher fatality in the population aged 60+. Economic development in southwest Spain would not only improve employment opportunities but also reduce ischemic stroke mortality. New health related strategies to improve hospital accessibility should be considered in more sparsely populated regions or those with worse transport and/or healthcare infrastructure.

Depressive symptoms during pregnancy and the postpartum period (PP) are prevalent and can have profound implications for maternal and infant well-being. Emerging evidence suggests that the gut microbiota may play a role in mood regulation. This study explores the relationship between gut microbiota composition and depressive symptoms in pregnant and postpartum women with different symptom severities. A cohort of pregnant women were recruited from HCFMUSP and HU-USP. Participants completed standardized depression assessment tools and were allocate in groups of absent or mild depressive symptoms (AL) and moderate or severe depressive symptoms (MG) and provided stool samples in three different time periods. One at the third gestation trimester (G) and two at the postpartum period. The first sample from this period was collected during postpartum hospitalization(P1), and the second during the onemonth postpartum follow-up appointment (P2). Their gut microbiota composition was analyzed using high-throughput sequencing techniques and Gas chromatography mass spectrometry (GS-MS) for quantification of short-chain fatty acids (SCFAs). Bioinformatic and statistical analyses were performed using softwares QIIME 2 (2022.2) and R (4.3.1) to identify potential associations between gut microbiota composition and depressive symptom severity. Findings that the Enterobacteriaceae family appears more abundantly in women of the MG group, especially during period P1 (p<0.05), and that there is a significant decrease (p<0.05) in depressive symptoms among the participants of the MG group from their screening to the end of the study follow-up, suggesting that the therapeutic approach is effective. Although no statistical differences in alpha and beta diversity indices were established between groups and across periods, it is possible to observe a trend of microbiota change during the treatment, with an increase in the Bifidobacterium genus, a decrease in the Enterobacteriaceae family, and an apparent inverse correlation between the reduction in the intensity of depressive symptoms and the increased abundance of the Bifidobacterium, Clostridium, and Dorea genera, as well as an increase in the concentrations of SCFAs. In conclusion, composition of gut microbiota appears to be influenced by the severity of depressive symptoms in pregnant and postpartum women. Further research is warranted to explore links between gut microbiota and perinatal depression and to determine the clinical implications of these findings for maternal and infant health

Introduction: Neuroscience plays a crucial role in educational development, spanning from preschool to university, providing knowledge that enriches culture and human values. Objective: The GME-CZH group developed a social outreach project focused on childhood and adolescence, specifically in the teaching of neuroscience. The main objective was to stimulate the cognitive development of children and adolescents through innovative methods such as games, talks, Brain Rally, etc., aiming to reinvigorate their minds and educate them about the functioning of the brain. Materials and Methods: The project was divided into two phases. The first, "Brain Week 2018," focused on primary and pre-university children and adolescents, implementing activities to stimulate their interest in brain-related topics. The second phase involved the collaboration of 150 children and adolescents (7-17 years old) and neuroscience experts for the creation of the book "100 Questions Children Ask About the Brain." Results: The collaboration between children and experts culminated in the successful creation of the book, addressing the concerns and questions posed by children about the brain and its functioning. Conclusion: The project demonstrated that the integration of playful and educational methods can be effective in stimulating interest and cognitive development in children and adolescents, emphasizing the importance of teaching neuroscience in these educational stages. This innovative project not only contributes to educational advancement but also promotes understanding of the brain from an early age, fostering the comprehensive development of students.

RESUMEN

SUMMARY: Although COVID-19 is primarily considered a respiratory pathology, it has been observed to impact other bodily systems, including the nervous system. While several studies have investigated anatomical changes in brain structures, such as volume or thickness post-COVID-19, there are no comprehensive reviews of these changes using imaging techniques for a holistic understanding. The aim of this study was to systematically analyze the literature on brain changes observed through neuroimaging after COVID-19. We conducted a systematic review according to PRISMA guidelines using Web of Science, Scopus, Medline, Pubmed, Sciencedirect, and LitCOVID. We selected studies that included adult patients during or after COVID-19 development, a control group or pre-infection images, and morphometric measurements using neuroimaging. We used the MSQ scale to extract information on sample characteristics, measured anatomical structures, imaging technique, main results, and methodological quality for each study. Out of 1126 identified articles, we included 19 in the review, encompassing 1155 cases and 1284 controls. The results of these studies indicated a lower volume of the olfactory bulb and variable increases or decreases in cortical and limbic structures' volumes and thicknesses. Studies suggest that brain changes occur post-COVID-19, primarily characterized by a smaller olfactory bulb. Additionally, there may be variations in cortical and limbic volumes and thicknesses due to inflammation or neuroplasticity, but these findings are not definitive. These differences may be attributed to methodological, geographical, and temporal variations between studies. Thus, additional studies are required to provide a more comprehensive and quantitative view of the evidence.

Case presentation. This case is about a 44 years old woman with a history of occipital headache, incoherent speech and confused thinking. She initially presented ten points on the Glasgow scale and left hemiparesis. Cranial CT scan reported cerebral edema with right thalamic hypodense lesion and progressive neurological deterioration. The electroencephalogram showed unilateral right hemispheric deceleration. The cerebrospinal fluid study showed hyperproteinuria and a predominantly lymphocyte count of 450 cells with preserved glycorrhachia, without the presence of bacteria. Treatment.was managed with invasive ventilatory support and antibiotic and antiviral treatment at meningeal doses, in addition to anticonvulsants. Control tomographic findings showed hydrocephalus; a Becker type ventricular shunt was placed. IgM serology was positive for Epstein Barr virus and the viral genome was identified in the cerebrospinal fluid by polymerase chain reaction test. The control brain tomography showed persistent ventriculomegaly and cerebral edema, which led to the diagnosis of encephalitis of viral etiology complicated by epilepsy secondary to a demyelinating structural lesion of the right cerebral hemisphere. Outcome. Therapeutic intervention with intravenous immunoglobulin was performed with improvement of the general condition, it was possible to remove the ventricular shunt and pulmonary ventilation ten and 19 days after admission, respectively. The patient is currently in physical therapy with persistence of left hemiparesis, gait disturbances, dysarthria, and controlled convulsive episodes during the last six months.

RESUMEN

Hippocrates was one of the greatest physicians ever, and merited unquestionably, the title of 'Father of Medicine'. He, together with other authors, formed an assemble of texts known as 'Hippocratic Collection' (or 'Hippocratic Corpus'). There, the ample and brilliant medical experience remained documented, and left as a legacy. Even after almost two and a half millennia, these medical writings continue to be admired, and regarded as a lasting fount of inspiration. However, regarding anatomy, including that of the nervous system, and related structures, the texts are poor. About the nervous system, he mentioned the brain and its two hemispheres, the spinal cord, and the sensory organs. The related structures are also cited, comprising the meninges, the cerebrospinal fluid, and the blood vessels, as well as the skull, and the vertebral column. The functions of the brain were partly explained, and he ascribed to the brain the main ruling function of the body, a mark of encephalocentric position. Thus, counterposed to his ample medical accomplishments was his poor anatomical contribution. However, it must be recognized that the incipient contribution on anatomy that was legated, represented an important starting point for upcoming scholars and further studies on the subject.

The SARS-CoV-2 virus continues to replicate. Its new variants would not be causing the deaths and serious hospitalizations of the years 2020-2022, so the World Health Organization (WHO) has declared the end of the state of health emergency. Until May 24, 2023, the WHO coronavirus dashboard shows more than 766 million confirmed cases, and about 7 million deaths in the world, with greater concentration in Europe, Western Pacific, and America (especially the U.S.A. and Brazil). Europe and China prepare for new virus breakthroughs. Therefore, health precaution is recommended, as well as compliance with vaccination and boosters, and the development of new vaccines. We comment on the fragility of randomized trials published in the first two years of the pandemic, as well as recent findings of which patients with COVID could develop long COVID. It is important to know the new scopes on the affectation of the coronavirus on the pregnant woman, the fetus and the neonate and the initial follow-up of the latter.

RESUMEN

The human sensory receptors are morphologically specialized to transduce specific stimuli into the brain. However, when an injury occurs, mainly in the spinal cord, which can be of traumatic or non-traumatic origin, it provokes various degrees of sensory deficits, autonomic, motor and sphincter dysfunction below the level of the injury. Based on this, a new therapeutic modality is being proposed by neuroscientist Miguel Nicolelis, which is based on the brain-machine interface, that is, using other pathways so that the information can reach the cerebral cortex and thus be consciously processed (AU).
