RESUMEN

BACKGROUND: In Switzerland, non-medical right-to-die organisations such as Exit Deutsche Schweiz and Dignitas offer suicide assistance to members suffering from incurable diseases. OBJECTIVES: First, to determine whether differences exist between the members who received assistance in suicide from Exit Deutsche Schweiz and Dignitas. Second, to investigate whether the practices of Exit Deutsche Schweiz have changed since the 1990s. METHODS: This study analysed all cases of assisted suicide facilitated by Exit Deutsche Schweiz (E) and Dignitas (D) between 2001 and 2004 and investigated by the University of Zurich's Institute of Legal Medicine (E: n = 147; D: n = 274, total: 421). Furthermore, data from the Exit Deutsche Schweiz study which investigated all cases of assisted suicide during the period 1990-2000 (n = 149) were compared with the data of the present study. RESULTS: More women than men were assisted in both organisations (D: 64%; E: 65%). Dignitas provided more assistance to non-residents (D: 91%; E: 3%; p = 0.000), younger persons (mean age in years (SD): D: 64.5 (14.1); E: 76.6 (13.3); p = 0.001), and people suffering from fatal diseases such as multiple sclerosis and amyotrophic lateral sclerosis (D: 79%; E: 67%; p = 0.013). Lethal medications were more often taken orally in cases assisted by Dignitas (D: 91%; E: 76%; p = 0.000). The number of women and the proportion of older people suffering from non-fatal diseases among suicides assisted by Exit Deutsche Schweiz has increased since the 1990s (women: 52% to 65%, p = 0.031; mean age in years (SD): 69.3 (17.0) to 76.9 (13.3), p = 0.000), non-fatal diseases: 22% to 34%, p = 0.026). CONCLUSIONS: Weariness of life rather than a fatal or hopeless medical condition may be a more common reason for older members of Exit Deutsche Schweiz to commit suicide. The strong over-representation of women in both Exit Deutsche Schweiz and Dignitas suicides is an important phenomenon so far largely overlooked and in need of further study.

Methods developed for the separation of S-alk(en)yl- L-cysteines and their corresponding (+/-)-sulfoxide isomers by reversed-phase HPLC were applied to the analysis of various garlic samples including fresh garlic, dried extracts, and garlic preparations. Extracts were chromatographed following extraction with 50:50 methanol/water, sample clean-up using Bond Elut C18 or SCX cartridges, and pre-column derivatization with O-phthaldialdehyde/ TERT.-butylthiol.(+)- S-Methyl- L-cysteine sulfoxide and (+)- S-allyl- L-cysteine sulfoxide (alliin) were the only compounds which were identified with certainty. Other sulfur amino acids reported to occur in garlic were absent or were below detection limits under standard chromatographic conditions. Assays for alliin, which is an antibiotic precursor and may be used for standardization of garlic preparations, resulted in great variation between samples: alliin contents were found to range from < 0.1% to 1.15% fresh weight. The accuracy and precision of the assay method, including external calibration of alliin, were evaluated.

RESUMEN

An improved procedure for the direct determination of l-dehydroascorbic acid (DHAA), in addition to l-ascorbic acid (AA), has been developed. The two biologically active forms of vitamin C were separated using reversed-phase high-performance liquid chromatography. DHAA was reduced to AA with dithiothreitol (DTT) in a post-column reaction system. Complete conversion was achieved at 50 degrees C using a 1-ml reaction coil. Recoveries were in the range of 95-99% and both forms could be detected spectrophotometrically at 267 nm with high sensitivity. Reproducible results [relative standard deviations 2.4% (DHAA) and 1.0% (AA), n = 5] were obtained when the method was applied to the analysis of rose hip samples.

RESUMEN

A simple, fast, and selective method for the assay of L-ascorbic acid in rose hips was developed using RP-HPLC. Ascorbic acid was extracted with 2% metaphosphoric acid and sample clean-up was optimized with C18 disposal extraction cartridges. Use of 0.5% metaphosphoric acid as the mobile phase completely suppressed the dissociation of ascorbic acid. Sufficient retention was achieved so that the addition of ion-pairing agents was not necessary. The column effluent was monitored by spectrophotometric (242 nm) and electrochemical detectors (550 mV vs Ag/AgCl); the electrochemical detector was found to be both more selective and more sensitive. By coup-ling the two detectors in series, a true two channel detection with on-line validation was achieved. Reproducible results (r.s.d. 1-4%) were obtained from assays of several commercially available products, as well as from fresh rose hips, and revealed considerable differences in ascorbic acid content, ranging from 0.03 to 1.3%.
