RESUMEN

BACKGROUND: According to previous research, 2.8% of lesions clinically identified as endodontic pathosis were ultimately diagnosed as non-endodontic periapical lesions via histopathology, and 3.7% of these non-endodontic periapical lesions were malignant neoplasms. Rhabdomyosarcoma, a malignant tumor most commonly observed in children, is uncommon in the oral cavity. CASE PRESENTATION: This is a report of a rare case of embryonal rhabdomyosarcoma in a 41-year-old female, in which the lesion was in the maxillary gingiva. The biopsy reports confirmed the diagnosis of embryonal rhabdomyosarcoma. The wide excision of the tumor, free flap reconstruction, chemotherapy, and radiotherapy were performed. Clinical, radiological, and histopathological and management aspects of the neoplasm were also discussed. CONCLUSIONS: This case report aimed to create awareness that rhabdomyosarcoma is one of the differential diagnoses of periapical lesions.

RESUMEN

(1) Background: The quantitative evaluation and comprehensive measurement of resources and environmental carrying capacity (RECC) are key links in the study of RECC from classification to synthesis. Laos, as the only landlocked country of Mainland Southeast Asia (MSEA), is an important economic corridor (i.e., China-MSEA Economic Corridor) of the Belt and Road Initiative (BRI). (2) Methods: Based on the human settlements index (HSI), resource carrying index (RCI), and socio-economic development index (SDI), here, a three-dimensional tetrahedron model for the comprehensive assessment of RECC with equilibrium significance was constructed, including HSI-based suitability classification, RCI-based restrictive classification, and SDI-based adaptability classification. Taking provinces as the basic unit, we quantitatively assessed and comprehensively evaluated RECC in Laos using a three-dimensional tetrahedron model. (3) Results: The human settlement environment in Laos is mainly characterized by the moderate suitable category (85%), while the high suitability area (merely 5%) supports more than 30% of the total population. Laos had over 90% of its area in good condition in resources and environmental carrying status (surplus or balanced state), translating into more than 95% of the population. The social and economic development level is mainly characterized by low-level development (43%), with nearly 30% of the population living in these low-level areas. The comprehensive bearing state of resources and environment is characterized by surplus, and 85% of the population is distributed in the surplus area, which occupies 63% of the land. (4) Conclusions: It is possible to better explore the adaptation strategies and countermeasures for enhancing RECC in Laos and provide a scientific reference for regional sustainable development. We believe that the three-dimensional tetrahedron method can be applied to quantitatively evaluate and comprehensively measure RECC at larger scale, e.g., the BRI regions.

BACKGROUND: This study aimed to evaluate the impact of Infiltration between the Popliteal Artery and Capsule of the posterior Knee (IPACK) combined with an adductor canal block under the guidance of ultrasound on early motor function after Total Knee Arthroplasty (TKA). METHODS: A sample of 60 cases who were scheduled for elective unilateral TKA were divided into two groups using random number table method: a group with IPACK combined with an adductor canal block (I group, n = 30), and a group with femoral nerve block combined with superior popliteal sciatic nerve block (FS group, n = 30). Before anesthesia induction was completed, the patients in I group received an ultrasound-guided adductor canal block with 15 mL of 0.375% ropivacaine and an IPACK block with 25 mL of ropivacaine, and the patients in FS group received a femoral nerve block and a superior popliteal sciatic nerve block with 20 mL of 0.375% ropivacaine under ultrasound guidance. Post-operation, all the patients received patient-controlled intravenous analgesia combined with an oral celecoxib capsule to relieve pain and maintain a visual analogue scale score of ≤ 3. RESULTS: The quadriceps femoris muscle strength score was significantly higher in Ⅰ group than in FS group (p = 0.001), while the modified Bromage score were significantly lower and walking distance results were significantly higher in Ⅰ group than in FS group (both p = 0.000). CONCLUSION: Compared with femoral nerve block combined with superior popliteal sciatic nerve block, IPACK combined with adductor canal block had a mild impact on early motor functions after TKA.

Accelerated urbanization and population growth have resulted in the loss of ecological land and biodiversity, accompanied by the degradation of ecosystem services. Identifying and improving existing ecological security patterns are of great significance for maintaining the sustainable development of cities. In this study, Jinan, the capital of China's Shandong Province, was used as a case study area. Based on three ecosystem services, namely, soil conservation, water conservation and carbon fixation, ecological sources were determined. Furthermore, a resistance surface was constructed based on biodiversity. On these bases, the circuit theory concept of random walks was applied to simulate ecosystem processes in a heterogeneous landscape and identify ecological corridors, pinch points and barriers. A total of 25 ecological sources, 48 ecological corridors and 19 pinch points were identified, and restoration areas were delimited to three levels. These elements together constituted the ecological security patterns. Specifically, the ecological sources were mainly distributed in southern Jinan and were covered mostly with forest land. The ecological corridors were located mainly in the eastern and southwestern plains below the southern mountainous areas and were covered mostly with cropland. Furthermore, the eastern corridors were much longer than the southwestern corridors. Pinch points were distributed mostly along rivers or around large-scale construction land. Barriers were distributed mainly in Zhangqiu District and northern Licheng District. Based on these findings, hierarchical restoration areas were delimited. Differentiated development contradictions in restoration areas were discussed, and corresponding ecological protection measures were proposed. An ecological security optimization pattern of "one center, two wings, and two belts" was finally proposed to provide planning strategies for decision-makers.

Background: Veterinary management of mitral valve regurgitation due to mxyomatous valve disease in dogs is limited to medical treatments, which only postpones the onset of congestive heart failure or alleviates clinical symptoms. Most surgical procedures to manage this condition in humans require cardiopulmonary bypass and have a high risk of complications. Animals: Eight dogs with naturally occurring mitral valve regurgitation. Methods: Prospective observational study. All dogs were treated with a novel edge-to-edge transcatheter device named ValveClamp. The total surgical procedural time and total catheterization time were recorded. Echocardiographic variables measured pre- and post-procedure were compared using Wilcoxin-signed rank test with a P < 0.05 considered significant. Data were expressed as median and interquartile range and absolute numbers and percentages. Results: The procedural success rate was 100% and all the dogs survived without complications. The median (interquartile range) total surgical procedural time was 86.5 (76-96.2) minutes and catheterization time was 23.5 (22-33.8) minutes. Echocardiography revealed a significant reduction in mitral regurgitation severity in all dogs following the procedure based on both a reduced mitral regurgitant maximum jet area (P = 0.012) and a reduced mitral regurgitant maximum jet area to left atrial area (P = 0.018). Conclusion: The ValveClamp device is effective at reducing the severity of mitral regurgitation in dogs with naturally occurring myxomatous valve disease.

Overuse of land resources has increasingly contributed to environmental crises in China. To mitigate widespread land degradation, actions have been taken to maintain and restore the ecological environment through efforts such as ecological engineering. By analyzing trends in land use, the impact and effectiveness of ecological engineering can be determined. In this study, such changes in Huanjiang County in China were considered. In the early 1990s, an eco-immigration policy and "returning farmland to forest program" were implemented in the county, drastically impacting land use. Land use/land cover changes were detected and analyzed using remote sensing data recorded over 4 years (1995, 2000, 2005, and 2010). Land transfer flow and the rate of land use change elucidated the extent of changes, while nuclear density analysis indicated spatial agglomeration. The results indicate that, over a period of 15 years, farmland area increased, while forest area decreased initially before subsequently increasing. From 1995 to 2000, the highest transfer flow was observed in the grassland to farmland conversion (79.34%). From 2000 to 2005, the transfer flow of conversions was the highest for forest to farmland (56.79%). Land use changes were not prominent from 2005 to 2010. Direct drivers of land use change exert obvious impacts on land use, and indirect drivers impact direct drivers that are then channeled through direct anthropogenic drivers (e.g., land use policies). We found that ecological engineering has a very significant impact on land use change, and that impact varies from region to region.

In order to solve the problem of extensive land use in rural residential areas and promote the construction of a new socialist countryside and farmland protection, it is important to graspe the temporal and spatial evolution of rural settlements. In this study, with Beijing as an example, the efficiencies of CLUE-S and Markov-CA models in simulating spatial temporal evolution of rural residential areas are analyzed. With 14 driving factors chosen, the land uses in Beijing in 2000 and 2005 are simulated and predicted using the two models. Grid size of 220 m × 220 m is used. The accuracies and Kappa coefficients of the simulation and prediction results are analyzed and the following conclusions are made. The core part of CLUE-S model is logistic regression, which grants it obvious advantage in capturing the trends of land use changes with more discrete distributions. Meanwhile, Markov-CA model takes neighborhood into consideration, making it relatively more advantageous in simulating and predicting changes of land uses with the character of nearest-neighbor diffusion. However, among various land uses, only urban land use has this character, the correct prediction rates of the CLUE-S model in 2000 and 2005 were 60.99% and 81.35%, respectively, while the accuracy of the Markov-CA model prediction was 51.33% and 73.68%, and it is the main reason that CLUE-S model returns better simulation and prediction results of rural residential areas for both 2000 and 2005 than Markov-CA model.
