RESUMEN

This study aimed to identify YouTube videos focused on choosing a nursing home. The contents of the videos were analyzed using 19 items from the guidelines proposed by the Donabedian model. An analysis of 57 videos showed that informal presentations for format (54.39%), laypersons for speaker (36.84%), and personal channel for uploader (61.40%) exhibited the highest frequency. According to the analysis of video content, most videos included the environment (49.12%), programs (38.63%), and cost (35.09%). However, fewer videos focused on care protocols (5.26%), participation (5.26%) and health outcomes (5.26%). More informal presentations, laypersons, and personal channels than health providers, professional groups, and mass media were included in videos focused on choosing nursing homes. It is necessary to provide information regarding the philosophy, care, and health outcomes of residents in addition to the information regarding environment and cost provided by reliable suppliers, such as health professionals, the government, and mass media.

BACKGROUND: Witnessing the death and dying of patients is a strong, often overwhelming experience during clinical practice. It is necessary to explore how nursing students experience care for dying patients and to understand their perceptions of death and dying. OBJECTIVE: This study aimed to explore the experiences of nursing students who witness dying patients and terminal care in their clinical practice. DESIGN: A qualitative study using focus group discussion methodology. SETTING(S): A university in Seoul, South Korea. PARTICIPANTS: A total of 16 nursing students with more than 1 year of clinical practice experience participated. Their median age was 21.9 years (range 21-24), and all were women. METHODS: A qualitative study using three focus groups (10 juniors in two focus groups and 6 seniors in one focus group) was conducted from December 2018 to January 2019. Data were analyzed using the Colaizzi analysis method. RESULTS: The analysis resulted in 26 themes, 9 theme clusters, and 3 categories. Three categories emerged: 1) Experience the distance death up close; 2) Dilemma at the end of life; and 3) Strategy on a better end to life. CONCLUSIONS: For nursing students, it is necessary to develop and apply internal and external reinforcement programs on providing person-centered care so that they can accept the death of others with equanimity. Nursing students, who are future nurses, should receive education covering various aspects of clinical scenarios involving dying patients and their family members.

INTRODUCTION: To analyze the content of Korean YouTube videos related to febrile seizures and examine the general characteristics, reliability, and quality of the videos. METHOD: A search of YouTube was performed using three Korean keywords meaning "febrile seizure", and a total of 1,641 videos were identified. Among them, 73 eligible videos were analyzed for their characteristics, quality, and reliability. The quality and reliability were rated using global quality (GQS) on a scale of 1-5 and the DISCERN instrument. RESULTS: The mean reliability and quality scores were 2.37±1.16 and 3.11±1.17 out of 5, respectively. Fifty-one of the 73 (69.8%) videos are related to febrile seizure management. Longer videos (13.94±20.06 vs 6.68±7.34) and videos with physicians (82.61% vs 32.00%) as the main speaker were higher quality. DISCUSSION: Both the quality and reliability of YouTube videos on febrile seizures were relatively low, and approximately only 30% of all videos were classified as high quality. Healthcare professionals should be aware that there is misinformation and low-quality information on social media and warn parents of this issue.

PURPOSE: This study investigated the practices of preventing infectious diseases by daycare center teachers. The focus was on the mediating effects of self-efficacy in the relationship between communication skills and practices of preventing infectious diseases. METHODS: This study used a descriptive correlational design. The participants were 164 daycare center teachers. Hierarchical regression was used to test the mediating effect of self-efficacy in the relationship between communication skills and practices of preventing infectious diseases. Data were analyzed using the independent t-test, one-way analysis of variance, Pearson correlation coefficients, and hierarchical regression. The mediation effect was analyzed using an SPSS Macro program, PROCESS. RESULTS: Significant relationships were found between communication skills and practices of preventing infectious diseases (ß=.19, p<.001), self-efficacy and practices of preventing infectious diseases (ß=.59, p<.001), and communication skills and self-efficacy (ß=.27, p<.001). Self-efficacy had a significant mediating effect (ß=.17, 95% confidence interval=0.07-0.29) on practices of preventing infectious diseases. CONCLUSION: In order to improve practices of preventing infectious diseases among daycare teachers, it is necessary to develop and apply interventions and methods that can increase effective communication skills and self-efficacy.

PURPOSE: Preterm birth and admission to a neonatal intensive care unit (NICU) can disrupt the parent-infant bonding relationship. Although neonatal nurses are in the best position to support maternal postpartum bonding in the NICU, few qualitative studies have described their challenges, strategies, and lived experiences. METHODS: This study aimed to explore and understand the experiences and perspectives of nurses supporting infants hospitalized in the NICU and their families in relation to the bonding process. We conducted a qualitative study using interpretive phenomenological analysis with 12 in-depth, semi-structured interviews recorded and transcribed verbatim between April and November 2018. We thematically analysed the data using NVivoTM software. RESULTS: Two themes emerged: (1) Being a bridge between separated mothers and infants (five subthemes); (2) Challenges in providing supportive care for maternal postpartum bonding in the NICU (three subthemes). CONCLUSIONS: Nurses have a variety of experiences regarding maternal postpartum bonding; however, the clinical reality of NICUs limits support for bonding formation. Although nurses face challenges (e.g., institutional policies, insufficient resources, training) when supporting maternal postpartum bonding, they act as the bridge between mothers and infants, becoming advocates for NICU families and taking care of their growth and developmental needs as caregivers.

PURPOSE: The aim of this study was to evaluate the validity and reliability of the Korean version of the Person-centered Care Assessment Tool (P-CAT). METHODS: The English P-CAT was translated into Korean with forward and backward translation. Survey data were collected from 458 staff in 17 long-term care facilities in Korea. Construct validity and criterion related validity were evaluated. Cronbach's alpha was used to assess reliability. RESULTS: The Korean version of P-CAT was shown to be valid homogeneously by factor, item and content analysis. Internal consistency reliability was satisfactory in which the values of factor 1, factor 2 and the total scale were .84, .77 and .86 respectively. Exploratory factor analysis supported the construct validity with a two-factor solution. Factor loadings of the 13 items ranged in .34~.80. Criterion validity to the Person-centered Climate Questionnaire-staff (PCQ-S) was .74 (p<.001). CONCLUSION: The Korean version of the P-CAT was found to be an applicable instrument with satisfactory reliability and validity for further use in measuring successful person-centered care in long-term care facilities for older persons.

PURPOSE: Mycoplasma pneumoniae (M. pneumoniae) is one of the most common causes of community-acquired pneumonia in children. The clinical course is typically self-limited and benign; however, rare cases of severe pneumonia can develop despite appropriate antibiotic therapy. We studied the effects of methylprednisolone pulse therapy on severe refractory M. pneumoniae pneumonia in children. METHODS: The clinical effects of methylprednisolone therapy were evaluated retrospectively in 12 children with severe refractory M. pneumoniae pneumonia, which was diagnosed serologically. All patients developed respiratory distress, high fever, and initial lobar pneumonic consolidation based on radiological findings. All clinical symptoms deteriorated despite appropriate antibiotic therapy. Thus, children were treated with intravenous methylprednisolone pulse therapy in addition to antibiotics. RESULTS: The average febrile period before admission was 4.9±1.7 days, and fever persisted in all children until steroid administration. Methylprednisolone pulse therapy (30 mg/kg) was given 5.4±2.5 days after admission. After methylprednisolone pulse therapy, clinical symptoms improved in all patients without adverse events. The fever subsided 0-2 h after initiation of corticosteroid therapy. The abnormal radiological findings resolved within 2.6±1.3 days, and the high C-reactive protein levels (6.7±5.9 mg/dL) on admission decreased to 1.3±1.7 mg/dL within 3.0±1.1 days after starting corticosteroid therapy. CONCLUSIONS: Three-day methylprednisolone pulse therapy could be applied to treatment of refractory M. pneumoniae pneumonia despite appropriate antibiotic therapy and appeared to be efficacious and well-tolerated.
