RESUMEN

BACKGROUND:Resistance exercise increases muscle mass and improves muscle strength,but there is a lack of conclusive evidence on its effects on body composition.Controversial results from randomized controlled trials and multiple Meta-analyses pose a problem for clinical decisions on exercise interventions. OBJECTIVE:Based on the methodological paradigm of umbrella review,to explore the efficacy and safety of resistance exercise in improving the body composition of overweight and obese people. METHODS:We searched PubMed,Cochrane Library,Web of Science,and CNKI for systematic reviews on the effect of resistance exercise in overweight and obese people published from database inception to August 31,2022.Chinese search terms included"anti-resistance,resistance,self-weight,Meta,Meta-analysis,body composition,body fat percentage,fat mass,lean body mass,body mass."English search terms included"resistance training,strength training,weight-lifting strengthening program,body composition,body fat distribution,Meta-analysis,data pooling,overviews,clinical trial,clinical trial overviews."PRISMA was used to evaluate the methodological quality of the included studies,and a GRADE evaluation system was used to grade the quality of evidence. RESULTS AND CONCLUSION:A total of 14 Meta-analyses were included,and GRADE evidence quality evaluation showed that 2 outcome indicators were intermediate,12 were low,and 14 were extremely low.Compared with the control group,23 of the 32 outcome measures showed significant improvement.The intervention effect of resistance exercise on body fat percentage has reached a consensus,but the intervention effects on lean body mass,body mass index and visceral fat still need further confirmation.Resistance exercise is an effective and safe method to improve the body composition of overweight and obese people.The overall quality of the research and the evidence quality of the outcome indicators included in the systematic evaluation are generally low.In the future,the initial state,exercise goal and willingness of the subjects should be fully combined on the basis of clarifying the sample characteristics and refining the scheme design.Through multiple,scientific and personalized exercise prescription designs of water,air and elastic resistance,the safety and compliance of resistance exercise can be improved,providing clear evidence-based support and decision-making basis for resistance exercise intervention.

OBJECTIVE:Masks are one of the most important defenses against the virus.However,the impact of wearing masks during daily activities or sports on respiratory and circulatory function remains controversial.A comprehensive quantitative evaluation of the effects of mask-wearing on human heart rate,oxygen saturation and end-expiratory carbon dioxide by Meta-analysis was conducted.The effects of wearing different types of masks at different exercise intensities and time of exercise on the human respiratory and circulatory system were explored. METHODS:By February 2023,with"mask,face mask,N95,training,sports,running,walking,cycling"as the Chinese search terms and"masks,respiratory protective devices,N95 respirators,surgical face masks,exercise,resistance training,explosive training,muscle exercises"as English search terms,the experimental studies addressing the influence of exercise with a mask on hemodynamic indexes were retrieved from CNKI,Web of Science,PubMed,Cochrane Library and WanFang databases.The outcome indicators included three continuous variables-exercise center rate,blood oxygen saturation and end-expiratory carbon dioxide.Stata16.0 software was used to analyze the outcome indicators of the included literature.The PEDro scale was used as a quality assessment tool,and the funnel plot was used to analyze the impact of publication bias. RESULTS:Totally 25 articles involving 857 healthy children and adults were included in this Meta-analysis.The overall methodological quality was high,with 22 studies scoring 6 points on the PEDro scale,2 studies scoring 7 points and 1 study scoring 8 points.The meta-analysis results showed that compared with the control group,exercise with masks had no significant effect on heart rate(SMD=0.02,95%CI:-0.11 to 0.15,P=0.81),but increased end-expiratory carbon dioxide(SMD=0.60,95%CI:0.37 to 0.83,P=0.00),decreased oxygen saturation(SMD=-0.28,95%CI:-0.47 to-0.09,P=0.03).Intensity and duration were the factors that affected the heterogeneity between studies.Wearing a mask during high-intensity exercise significantly increased heart rate(SMD=-0.20,95%CI:-0.36 to-0.04,P=0.02).The effect of high-intensity and short-time exercise on blood oxygen saturation was significantly higher than that of other exercises(SMD=-0.40,95%CI:-0.70 to-0.10;SMD=-0.25,95%CI:-0.45 to-0.04).For end-expiratory carbon dioxide,maintaining a certain intensity and increasing the exercise time or increasing the intensity further increased the index significantly,reaching a moderate effect size(SMD=0.61,95%CI:0.06 to 1.15;SMD=0.58,95%CI:0.04 to 1.13). CONCLUSION:Existing evidence suggests that exercise with masks may have the adverse effect of increasing end-expiratory carbon dioxide and decreasing blood oxygen saturation.The influence of different exercise test time and intensities on the three outcome indexes was different.Wearing a mask during high-intensity exercise can significantly increase heart rate and decrease blood oxygen saturation.Maintaining moderate intensity for a long period or further increasing the intensity of exercise will lead to increased end-expiratory carbon dioxide levels.

BACKGROUND: Primary central nervous system lymphoma (PCNSL) is an aggressive extranodal lymphoma exclusively occurring within the central nervous system. Inflammatory brain lesions as "sentinel lesions" of PCNSL are very rare. We present a rare case of PCNSL with preceding inflammatory lesions in an immunocompetent patient who underwent two biopsies, one craniotomy and two genetic testing. CASE REPORT: A 66-year-old male patient presented with left limb weakness and ataxia. Brain magnetic resonance imaging showed a contrast-enhancing lesion with perifocal brain edema in the near midline of right frontal lobe. Histological examination of a brain biopsy specimen revealed inflammatory lesion characteristics with infiltration of T-cell dominant lymphocytes and few B-cell. Given that the patient developed cerebral hematoma after biopsy, lesion resection by craniotomy was performed. An excised sample demonstrated mixed T-cell and B-cell infiltrating inflammatory lesions. Four months after total resection of the right frontal lobe lesion, another lesion appeared in the left frontal parietal lobe, which was diagnosed as diffuse large B-cell lymphoma by biopsy. In addition, genetic testing of the lesions at two different locations was performed, and the results showed that the inflammatory lesions had the same three gene (RELN, PCLO, and CREBBP) mutations as PCNSL. Interestingly, the three mutated genes are associated with tumor. CONCLUSION: Our present case is the first to demonstrate inflammatory brain lesions heralding PCNSL from genetic and pathological perspectives. This may help clinicians to select new auxiliary diagnostic methods for timely diagnosis of patients with suspected PCNSL.

RESUMEN

The classic formula Guizhi Shaoyao Zhimutang, listed as the 15th formula in the Catalog of Ancient Classic Formulas (First Batch) published in 2018, originated from Synopsis of the Golden Chamber (《金匮要略》) written by ZHANG Zhongjing in the Eastern Han Dynasty. It consists of Cinnamomi Ramulus, Ephedrae Herba, Aconiti Lateralis Radix Praeparata, Anemarrhenae Rhizoma, Paeoniae Radix Alba, Atractylodis Macrocephalae Rhizoma, Saposhnikoviae Radix, Zingiberis Rhizoma Recens, and Glycyrrhizae Radix et Rhizoma, and is effective in dispelling wind, eliminating dampness, dispersing cold, relieving impediment, nourishing Yin, and clearing heat. It is mainly used to treat diseases characterized by wind, cold, and dampness invading the body, combined with heat damaging Yin, such as joint disorders, rheumatism, gout, and knee osteoarthritis. Based on the call for "inheritance of essence and application of ancient knowledge for modern use", this study conducted a comprehensive analysis of historical evolution, composition, formulation principles, processing, dosage, decocting methods, and indications of Guizhi Shaoyao Zhimutang using textual research on ancient and modern literature. When analyzing modern literature, it has been found that this formula is widely used in the treatment of various diseases. It is mainly applied to rheumatic diseases such as rheumatism, rheumatoid arthritis, gout, gouty arthritis, and psoriatic arthritis, as well as orthopedic diseases like knee osteoarthritis, ankylosing spondylitis, sciatica, and knee joint effusion. It can also be used for diseases in other systems, including the endocrine system, gynecology, respiratory system, and circulatory system. The pathological mechanisms involve the invasion of wind, cold, and dampness accompanied by heat pathogens, reflecting the concept of treating different diseases with the same principles in traditional Chinese medicine (TCM). Through the analysis of ancient and modern literature on Guizhi Shaoyao Zhimutang using a literature statistical method, the historical evolution and key formula and syndrome information were clarified to provide a theoretical basis for the development and further research of Guizhi Shaoyao Zhimutang in terms of its formulation and subsequent in-depth studies.

BackgroundBy early April 2020, the COVID-19 pandemic had infected nearly one million people and had spread to nearly all countries worldwide. It is essential to understand where and how SARS-CoV-2 is transmitted. MethodsCase reports were extracted from the local Municipal Health Commissions of 320 prefectural cities (municipalities) in China, not including Hubei province, between 4 January and 11 February 2020. We identified all outbreaks involving three or more cases and reviewed the major characteristics of the enclosed spaces in which the outbreaks were reported and associated indoor environmental issues. ResultsThree hundred and eighteen outbreaks with three or more cases were identified, involving 1245 confirmed cases in 120 prefectural cities. We divided the venues in which the outbreaks occurred into six categories: homes, transport, food, entertainment, shopping, and miscellaneous. Among the identified outbreaks, 53{middle dot}8% involved three cases, 26{middle dot}4% involved four cases, and only 1{middle dot}6% involved ten or more cases. Home outbreaks were the dominant category (254 of 318 outbreaks; 79{middle dot}9%), followed by transport (108; 34{middle dot}0%; note that many outbreaks involved more than one venue category). Most home outbreaks involved three to five cases. We identified only a single outbreak in an outdoor environment, which involved two cases. ConclusionsAll identified outbreaks of three or more cases occurred in an indoor environment, which confirms that sharing indoor space is a major SARS-CoV-2 infection risk. OBJECTIVE: To explore the component, target and pathway of Panax notoginseng for coronary heart disease (CHD) and its potential molecular mechanism. METHODS: Based on network pharmacology, active components of P. notoginseng were retrieved with TCMSP platform. The targets of P. notoginseng for CHD were screened by using DRAR-CPI server, GeneCards and DisGeNET databases. Cytoscape 3.6.0 software was used to form the effective components-CHD targets network of P. notoginseng. String database was used to draw target interaction network. Network Analyzer tool was used to calculate target connectivity, and potential core targets were screened. Molecular docking between the core targets and the effective components of P. notoginseng was performed by Systems Dock Web Site server. KEGG pathway enrichment analysis and gene ontology (GO) enrichment analysis were also carried out to explore the important signal pathway and molecular function of P. notoginseng for CHD. “Effective component-target-signal pathway”network of important signal pathway were constructed. RESULTS: Five effective components (stigmasterol, β-sitosterol, ginsenoside rh2, quercetin, notoginsenoside r1) were screened from P. notoginseng for CHD, which acted on 96 targets and had 134 functional relationships. Five core targets were protein kinase B (AKT), interleukin 6 (IL-6), vascular endothelial growth factor A (VEGFA), c-JUN protein (c-JUN) and heparin binding epidermal growth factor (HB-EGF), which played an important role in the treatment of CHD by altering protein binding and regulating signaling pathways as phosphatidylinositol-3 kinase-protein/kinase B (PI3K/AKT), hypoxia-inducible factor-1 (HIF-1) and mitogen-activated protein kinase (MAPK). CONCLUSIONS: P. notoginseng in the treatment of CHD is not only play a variety of effects through the role of multiple targets, but also produce complex network regulation effect through the interaction between targets.

Objective This study was to study the effect of 0.1% and 0.3% sodium hyaluronic ophthalmic solution on tear film stability and visual quality of dry eye patients after femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK).Methods A prospective randomly controlled study was designed.Pre-surgery normal patients were evaluated again at 1 week after FS-LASIK,60 mild and severe dry eye patients were enrolled from April 2016 to April 2017 in Tianjin Medical University Eye Hospital.Then 0.1% and 0.3% sodium hyaluronate were randomly given in each dry eye group.Ocular surface disease index (OSDI),optical quality analysis system Ⅱ(OQAS Ⅱ),Keratography 5M and corneal staining were performed before operation,1 week (before instillation and 15,30,60,120 and 180 minutes after instillation),1 month and 3 months after operation.This study was approved by the Ethic Committee of Tianjin Medical University Eye Hospital (2015KY-04).Informed consent was obtained from all subjects after explanation of the nature and possible consequence of the study.Results The 0.3% sodium hyaluronate group was better than 0.1% sodium hyaluronate group in alleviating OSDI score and cornea staining at 3 months in the mild dry eye group (Fgroup =10.913,P =0.003;P =0.027).Compared with the before operation,the objective scatter index(OSI) was increased at 1 week before instillation and many different time points after instillation in both 0.1% sodium hyaluronate group and 0.3% sodium hyaluronate group,with significant differences between them (all at P<0.05),there was no statistically significant difference in OSI at 1 month and 3 months after surgery compared with before surgery in both 0.1% and 0.3% sodium hyaluronatc group.In 0.1% sodium hyaluronic group and 0.3% sodium hyaluronic group,the mean value of total OSI at 1 week after surgery were siginifcantly higher than that before surgery (P=0.046,0.060).In 0.1% sodium hyaluronate group,the mean values of total OSI at various time points after instillation were not statistically different from that before surgery (all at P>0.05),total OSI at 1 month and 3 months after operation were higher than that before surgery (P =0.047,0.017);In 0.3% sodium hyaluronic group,the mean value of total OSI at 15,30,60 and 120 minutes after instillation were significantly higher than that before surgery (all at P<0.05),and compared with the total OSI before surgery,no statistical differences were found at 1 month and 3 months after operation (P =0.046,0.667).Significant differences were found in NIBUT avg among different time points in the mild dry eye patients (Ftime =2.290,P =0.022).In 0.1% sodium hyaluronic group and 0.3% sodium hyaluronic group,the NIBUT avg at 1 week after surgery were significantly lower than that before operation (both at P<0.05).Compared with the before operation,no significant differences were found at the following 3 hours after instillation in 0.1% sodium hyaluronic group (all at P>0.05).In 0.3% sodium hyaluronic group,the NIBUT avg at 60,120 and 180 minutes after instillation were significantly lower than that before surgery (P=0.040,0.047,0.041),and NIBUT avg at 3 months after operation was significantly lower than that before surgery (P =0.044).Conclusions The 0.3% sodium hyaluronic is better than 0.1% sodium hyaluronic in alleviating the OSDI score and cornea staining on the mild dry eye post FS-LASIK,and the tear film stability and related visual quality fluctuated over time after the application of 0.3% sodium hyaluronic.

Objective To observe the effect of low temperature compound propofol on the changes of apoptosis protein Caspase-3,autophagy-related proteins Beclin-1 and LC3-Ⅱ and thier changes of hippocampal neurons.Methods The rats were randomly divided into blank control group (Group A),propofol group at low temperature (Group B) and chloral hydrate group at low temperature (Group C),Group B and C were treated with low temperature for 30 min.Then,each group was subjected to cardiac perfusion and decapitated brain to prepare rat hippocampal neuronal tissue samples.The expression of Caspase-3 was detected by immunohistochemistry,Beclin-1 and LC3-Ⅱ protein was detected by immunoblotting.The ultrastructural changes of neurons were observed by transmission electron microscopy.Results The expression of Caspase-3,Beclin-1 and LC3-Ⅱ protein in each group were statistically significant differences(P<0.05).The results of transmission electron microscopy showed that the neurons in group B and C were changed in different degrees,especially in group C neuronal apoptosis is obvious.Conclusion Autophagy and apoptosis in existence still exist in low temperature condition,while propofol can reduce this damage and have better protective effect on neurons.

Objective@#To investigate the usefulness and effectiveness of multi-slice spiral computerized tomographic fistulography (MSCTF) in the diagnosis and treatment of congenital fistula of neck.@*Methods@#Thirty-four patients with thyroglossal fistulasor branchial cleft fistulas who were initial treated from July 2008 to August 2015 in Fujian Provincial Hospital were retrospectively analyses. Thirteen males and 21 females patients aging from 3 to 46 years old with a median age of 37 were included. There were thyroglossal fistula in 6 cases, the first branchialcleft fistula in 9 cases, the second branchialcleft fistula in 3 cases, the third branchialcleft fistula in 9 cases, and the fourth branchialcleft fistula in 7 cases. All the patients underwent preopeative MSCTF and the diagnoses were finally confirmed with surgery and histopathology. Multiplanar reconstruction(MPR), maximumintensity projection(MIP)and volume rendering(VR) were completed with AW Volume Share 4.2 image processing software after initial CT scanning.The internal openings, distribution, and neighboring relationship of the fistulas showed by MSCTF were analyzed and the surgical strategies were subsequently made.@*Results@#Except 2 cases, 32 patients had obtained successfully MSCTF image. The presence and location of the fistulas could be showed clearly on MSCTF. Based on the results of MSCTF examination, the surgical planes to treat the fistulas were made. The fistulas in all cases were successfully found and excised. Three cases underwent selective neck dissection. Postoperative infection occurred in 1 case. Unilateral vocal fold paralysis due to surgery recovered 3 months after surgery with follow-up. One case lost follow-up, the remaining 33 cases were followed up for 13-97 months with no the fistula recurrence.@*Conclusions@#MSCTF could provide valuable information and benefit surgical planning by demonstrating the coursesof congenital fistulas of neck in detail.

The aim of this study is to develop creatine kinase isoenzyme MB (CK-MB) specific monoclonal antibodies (mAb), and characterize the monoclonal antibody and further development of quantitative detection assay for CK-MB. The BALB/c mice were immunized with purchased CK-MB antigen, then monoclonal antibodies were prepared according to conventional hybridoma technique and screened by indirect and capture ELISA method. To identify the epitopes and evaluate the classification, purchased creatine kinase isoenzyme MB (CK-MM/BB/MB) antigen was used to identify the epitopes, with immunoblotting and synthetic CK-MM and CK-BB in different linear epitope. A double antibody sandwich ELISA was applied to screen the mAb pairs for CK-MB detection, and the quantitative detection assay for CK-MB was developed. We used 74 cases of clinical specimens for comparison of our assay with Roche's CK-MB assay. We successfully developed 22 strains of hybridoms against CK-MB, these mAbs can be divided into linear, partial conformational CK-MB, CK-MM or CK-BB cross monoclonal antibody and CK-MB specific reaction with partial conformational monoclonal antibody, and CK-MB quantitative detection assay was developed by using partial conformational monoclonal antibody. The correlation coefficient factor r of our reagent and Roche's was 0.930 9. This study established a screening method for CK-MB partial conformational specific monoclonal antibody, and these monoclonal antibodies were analyzed and an established quantitative detection assay was developed. The new assay had a high concordance with Roche's.

<p><b>OBJECTIVE</b>To assess the value of combined BACs-on-Beads(BoBs) and chromosomal karyotyping for the diagnosis of women with high-risk pregnancy.</p><p><b>METHODS</b>For 1371 women with singleton pregnancy and various indications for prenatal diagnosis, karyotyping and BoBs were simultaneously applied on their amnionic fluid samples.</p><p><b>RESULTS</b>In total 23 cases of trisomy 21, 11 cases of trisomy 18, 5 cases of sex chromosome aneuploidies, 6 cases of microdeletions/microduplications, 2 cases of chimeric chromosomes and 1 case of structural chromosomal abnormality were detected by the BoBs assay, among which the 6 microdeletions/microduplications were not detected by karyotyping. Karyotyping analysis has identified an extra yield of 19 chromosomal abnormalities and 34 chromosomal polymorphisms.</p><p><b>CONCLUSION</b>Combined use of BoBs and chromosomal karyotyping can improve the detection rate of fetal chromosomal abnormalities including microdeletions/microduplications, which should find a wider use in the clinics.</p>

Objective To obtain two types of optical isomers from 1-(Benzamidomethyl)-1,2,3,4-tetrahydro-isoquinoline (BTIQ). Methods ( ±) BTIQ as raw materials , optically pure camphor sulfonic acid as resolution agent was used and repeated resolution in acetone , And the resolution product was detected by specific optical rotation .The product of BTIQ was hydrolyzed , and the ratio of ATIQ was compared with that of the literature .ResuIts After two repeated chemical resolutions , the specific rotations of both enantiomers are no longer changed .It was showed that the products of higher optical purity.The specific rotation of both isomers are -35.65°(CH2Cl2, C=0.5)with the yield of 27.52%,and +35.17°(CH2Cl2, C =0.5) with the yield of 31.55% respectively.The specific rotation value of chiral 1,2,3,4-tetrahydroisoquino line ( ATIQ) which was the hydrolysis product of BTIQ consistent with values reported in the literature .ConcIusion The (-) BTIQ and ( +) BTIQ enantiomers were successfully obtained by the method of resolution , and the yield and optical purity of the obtained products are higher , laying the foundation for the further development of high efficiency , low toxicity of chiral schistosomicide ( praziquantel ) and other containing tetrahydroisoquinoline structure of chiral drugs .

A sensitive method based on high-performance liquid chromatography-tandem mass spectrometry (LC-MS/MS) has been developed for the simultaneous determination of folic acid (FA) and its active metabolite, 5-methyltetrahydrofolic acid (5-M-THF), in human plasma. The analytes were extracted from plasma with methanol solution containing 10 mg/mL of 2-mercaptoethanol and 0.025% (v/v) ammonium hydroxide. FA and 5-M-THF were more stable after the addition of 2-mercaptoethanol and ammonium hydroxide in the sample preparation procedures of this study than they were in the previously published methods. Chromatographic separation was performed on a Hedera ODS-2 column using a gradient elution system of acetonitrile and 1 mM ammonium acetate buffer solution containing 0.6% formic acid as mobile phase. LC-MS/MS was carried out with an ESI ion-source and operated in the multiple reaction monitoring (MRM) mode. The assay was linear over the concentration ranges of 0.249-19.9 ng/mL for FA, and 5.05-50.5 ng/mL for 5-M-THF. The developed LC-MS/MS method offers increased sensitivity for quantification of FA and 5-M-THF in human plasma and was applicable to a pharmacokinetic study of FA and 5-M-THF.

RESUMEN

Objective To analyze the tend of hospital infection ,to understand the hospital infection control situation and pro‐vide a scientific basis for hospital infection prevention and control .Methods Investigation by the bed in combination with case in‐vestigation form 2012 to 2014 ,and the data of three consecutive years were analyzed by statistical analysis .Results In 2012 ,2013 and 2014 ,the hospital infection rates were 4 .15% ,3 .38% and 2 .76% respectively ,there was no statistically significant difference (χ2 =3 .908 ,P=0 .142) .Hospital infection site mainly respiratory tract(61 .33% ) ,followed by the digestive tract(16 .67% ) and u‐rinary tract(6 .67% ) ,different parts of the infection rate after statistical tests ,the difference had no statistically significant (χ2 =12 .167 ,P=0 .239);Different departments category incidence of hospital infection ,infection rate of internal system presented the declining trend ,in 2012-2014 by the statistical tests ,the difference had statistically significant(χ2 =8 .347 ,P=0 .015) .Conclusion Continuous observation of hospital infection rates helps to understanding the trend of hospital infection ,finding focus of infection , improving the method of hospital infection management .

RESUMEN

The main purpose of this study was to evaluate the pharmacokinetics of levosulpiride in humans after single and multiple intramuscular injections. Six males and six females received single dose of either 25 mg or 50 mg levosulpiride, or multiple doses of 25 mg every 12 h for 5 consecutive days. In the single 25 mg study, the mean peak plasma concentration (C max) was 441 ng/mL, the mean area under the concentration-time curve from 0 to 36 h (AUC0-36) was 1724 ng h/mL, and the mean elimination half-life (t 1/2) was 7.0 h. In the single 50 mg study, the mean C max was 823 ng/mL, the mean AUC0-36 was 3748 ng·h/mL, and the mean t 1/2 was 6.8 h. After multiple doses of 25 mg levosulpiride, the average plasma concentration (C av) was 136 ng/mL, the fluctuation index (DF) was 3.60, and the accumulation ratio (R) was 1.2. Levosulpiride injections appeared to be well tolerated by the subjects, and can be used for successive administration.

Objective To compare the efficacy and safety of liquid nitrogen cryotherapy combined with acyclovir and mecobalamin in the treatment of herpes zoster in elderly patients.Methods 520 elderly patients with herpes zoster were randomly divided into two groups,experimental group and control group (n=260,each).Patients in experimental group were treated with liquid nitrogen cryotherapy,using liquid nitrogen cotton swab to graze the skin lesions 2-3 times per day for successive 5 days,combined with acyclovir and mecobalamin.Patients in control group were treated with acyclovir and mecobalamin.New blisters and dry skin and crust were observed and compared between the two groups on the 5th,14th,30th day after treatment.The skin lesion situation and reaction times for pain complete remission,vesicular scab,postherpetic neuralgia (PHN) were recorded and analyzed.Results The effective rate was 93.9% (244/260) in the experimental group versus 84.2% (219/260) in the control group (x2 =12.32,P< 0.05).The reaction times for vesicular scab,pain complete remission and subside were shorter in experimental group than in control group [(2.63 ±0.78)d vs.(4.75±1.39)d,(8.17±1.68)d vs.(11.25±2.23)d,(9.24±2.53)d vs.(12.78±3.13)d,all P<0.01].There was no significant difference in new blister time between the experimental group and the control group [(1.85±0.62) d vs.(1.79±0.83)d,P>0.05].Postherpetic neuralgia incidence was lower in experimental group than in control group [6.9% (18/260) vs.18.1% (47/260),x2 =14.79,P<0.01].Conclusions Treatment with liquid nitrogen cryotherapy combined with acyclovir and mecobalamin can shorten the reaction times of vesicular scab,and pain complete remission and subside,and reduce postherpetic neuralgia as compared to acyclovir and mecobalamin treatment in elderly patients with herpes zoster.

Objective To study the efficacy of pelvic reconstruction surgery in the treatment of pelvic organ prolapsey (POP).Methods 18 patients with POP underwent pelvic reconstruction surgery were selected.Clinical parameters during peri-operation were recorded.After 3 months and 6 months,pelvic organ prolapse quantitative examination(POP-Q) and anatomic improvement in these patients were analyzed.Comparisons of pelvic floor distress impact questionnaire short form 7 (PFIQ-7) and quality of sexual life of chinese women questionnaire in these patients before and after surgery were used to evaluate quality of life and quality of sexual life.Results Median operation time was (90.0 ± 13.2) minutes and median blood loss was (30.0 ± 1.2) ml,all the patients were followed.The objective cure rate was 100%.The mean score of post-operative PFIQ-7 were significantly lower than those of pre-operation (P < 0.01),and those of qualify of sexual life of chinese women questionnaire was no significantly difference (P >0.05).The rate of mesh crosion was 0,and there was no case of de novo urinary incontinence and de novo dyspareunia.Conclusion Pelvic reconstruction surgery is a safe and efficacious management.It can improve qualify of life remarkably and have no influence on sexual quality of life.

The protein C anticoagulant pathway plays a fundamental role in the control of coagulation system and inflammatory response. It has been well established that physiological levels of shear stress induce endothelial structural change and modulate gene and protein expression. However, the role of shear stress in protein C pathway remains unknown. In the present study, we evaluated the effect of shear stress on the activation of protein C as well as on the expression of endothelial protein C receptor (EPCR) and thrombomodulin (TM) in human umbilical vein endothelial cells (HUVECs) which were exposed to TNF-alpha alone, shear stress alone, and TNF-alpha under shear stress. We found: (1) Either TNF-alpha or shear stress alone significantly reduced EPCR expression and protein C activation in HUVECs; and simultaneous exposure of HUVECs to TNF-alpha and shear stress resulted in a further decrease of EPCR expression and protein C activation (P<0.05); (2) Simultaneous exposure of HUVECs to TNF-alpha and shear stress resulted in the increase of soluble EPCR level more significantly than did the exposure of HUVECs to either TNF-alpha or shear stress alone (P<0.05); (3) Shear stress significantly increased TM expression on HUVECs, whereas TNF-alpha inhibited TM expression; shear stress could strongly neutralize TNF-alpha's inhibitive effect on TM expression. We therefore conclude that shear stress may play an important role in protein C pathway, which may be fulfilled by regulating EPCR expression and TM expression in endothelial cells.

It has been demonstrated that the activated protein C (APC) plays an important role in the inhibition of inflammation. The activation of protein C can be significantly enhanced by the endothelial cell protein C receptor (EPCR). Previous studies proposed that the APC regulates the inflammatory response in endothelial cells by suppressing the expression of adhesion molecules and the secretion of chemokines and cytokines. However, the precise mechanism of the inhibitory effect of APC on inflammation is still poorly understood. In the present study, we evaluated the anti-inflammatory effect of recombinant human APC (rhAPC) and whether its inhibitory effect is conducted through the EPCR-dependent mechanism on human umbilical vein endothelial cells (HUVECs). By exposing HUVECs to: (1) TNF-alpha; (2) rhAPC plus TNF-alpha; (3) anti EPCR antibody that prevents rhAPC interaction with EPCR; (4) TNF-alpha plus anti EPCR antibody; (5) rhAPC plus TNF-alpha in the presence of anti EPCR antibody, we found that APC was able to significantly inhibit the TNF-alpha-induced secretion of cytokines such as IL-1beta and IL-8, as well as the expression of adhesion molecules such as ICAM-1, VCAM-1 and E-selction in HUVECs. These results reveal a novel pathway by which APC protects endothelial cells from inflammatory mediators through an EPCR-dependent mechanism.

Objective To explore the demands of nursing staff on nursing repository and provide reference for the develop-ment of nursing repository. Methods In-depth interview was conducted on 21 nursing staff by qualitative research. The themes were formed by category analysis. Results There were four themes about the demands of nursing staff on nursing repository:necessity to develop nursing repository ,contents of the repository ,forms of the repository and prospect of the reposi-tory. Conclusions Nursing staff need a nursing repository. They hope that the repository can provide comprehensive,concrete and practical knowledge,and provide a good interface with digitization. The design of repository should be consistent with in-ternational standards.
