RESUMEN

BACKGROUND: Dermatofibrosarcoma protuberans (DFSP) is an uncommon malignant tumor of the skin, the diagnosis of which can be difficult not only clinically but also histologically. The tumor has a low metastatic potential, but tends to recur locally if removal is incomplete. METHODS: We reviewed the clinical and histologic features of 19 cases of DFSP, seen at the Department of Dermatology (14 cases) and Department of Surgery (5 cases) at the Hull Royal Infirmary between 1975 and 1992. RESULTS: There were only 19 cases of DFSP diagnosed from a population of 500,000 over a period of 17 years. None of the cases were diagnosed clinically prior to histologic diagnosis. The highest incidence was in the 4th decade of age. The most frequent site was on the head and neck. None of the lesions metastasized but there were a total of eight incidents of local recurrence in five patients. Histologically all the cases showed presence of storiform-pattern spindle-shaped cells and new collagen formation. CONCLUSIONS: Dermatofibrosarcoma protuberans is a soft tissue tumor that is often not recognized clinically due to its rarity and variable appearance. Local recurrence is frequent. A wide surgical excision is the treatment of choice.

BACKGROUND: The Spitz nevus is a rare form of benign nevus, which may be confused with malignant melanoma. METHODS: We reviewed the clinical and histologic features of 29 cases of Spitz nevus, of which 20 were dealt with by our department of dermatology at the Hull Royal Infirmary between 1969 and 1990. The other nine cases were from the records of our pathology department dated between 1979 and 1991, of which seven of the lesions were excised by the surgeons and two by the general practitioners. RESULTS: In all the cases, the final diagnosis was established histologically. In three cases, there were difficulties histologically in excluding the diagnosis of malignant melanoma. All the lesions were solitary, and none of the lesions recurred following excision despite the possibility of incomplete excision in some cases. CONCLUSIONS: We found the size of the lesion and the age of the patient important factors to consider in the diagnosis of Spitz nevus. The small number of cases we encountered over a period of 22 years in a department that serves a population of around 500,000 emphasises the rarity of the condition.

RESUMEN

Fourteen cases of pyoderma gangrenosum were seen over a period of 24 years at the Hull Royal Infirmary Dermatology Department. Several associated conditions were found. Seven cases were associated with rheumatoid arthritis of which five were sero-positive, including one with Felty's syndrome. One case was associated with both ulcerative colitis and psoriasis; one with polycythemia rubra vera; two patients had diverticular disease including one who also had rheumatoid arthritis; one had positive syphilis serology. In three cases there was no significant associated disease identified. Ten out of the fourteen cases were women, indicating a female preponderance by a ratio of about 2F:1M; a figure similar to that stated by Seitzinger. The age of presentation ranged from 30 to 80 years.

Although rosacea was formerly believed to be associated with gastrointestinal upsets, no one any longer finds a significant association between rosacea and the intestinal tract. We describe four patients with a combination of ulcerative colitis and rosacea. In all four, ulcerative colitis preceded the onset of severe papulopustular rosacea, and we therefore feel that the severity of rosacea could have been due to the associated bowel disorder. In one, the severity and poor initial response of rosacea to treatment was clearly related to the activity of the ulcerative colitis, and the rosacea improved only after proctocolectomy. While it is possible that this purported association is fortuitous, we report these cases in the hope that others may have seen this combination of diseases, to our knowledge previously unreported.

RESUMEN

Three elderly, female patients presented with scalp ulceration. One developed sudden blindness before the ulceration of the scalp. Biopsy from the ulcerated area in this patient did not show evidence of giant cell arteritis, but the two other patients had histological evidence of giant cell arteritis and one of these also became blind.

RESUMEN

Seventy-eight cases of pilomatricoma are reported. The unexpected findings are new lesions occurring in middle and old age; the lack of association with other diseases; and the large number of lesions located deep in the skin. As contemporary knowledge of the nature of this disorder is advancing, it is suggested that the eponymous title be abandoned.

RESUMEN

Sebum excretion and acne grades were measured in 20 pairs each of identical and non-identical like-sex twins. The identical twins had virtually identical rates of sebum excretion (P greater than 0.05), but they had a significantly different degree of acne severity (P less than 0.01). The non-identical twins had significantly different sebum excretion rates (P less than 0.01) and acne grades (P less than 0.01). These findings suggest that sebum excretion is under genetic control and the development of clinical lesions is modified by environmental factors.

RESUMEN

Four hundred and sixty-two patients suffering from primary irritant hand dermatitis were sent postal questionnaires after 1-16 years regarding the outcome of their dermatitis. Two hundred and thirteen patients replied (a compliance rate of 46%). Of the 213 replies 188 were accepted for analysis, of whom only 58 (30.8%) had complete remission of their dermatitis, whilst 130 patients (69.2%) had continuous, or intermittent disease. Of sixty-four patients who changed their occupation only twenty-one (32.8%) had complete remission of their dermatitis as opposed to thirty-seven (29.8%) of 124 who continued their occupation after the diagnosis of primary irritant hand dermatitis was made. It would appear that the outcome of primary irritant hand dermatitis is not favourable, even after 10 years, and change of occupation has no significant effect.

RESUMEN

Surface features of rat hairs from different parts of the body have been studied by scanning electron microscopy. Ear hairs and vibrissae are similar in having much smaller cuticular scales than hairs from other parts of the body. The scales on the head hairs of females are smaller than those of males.

The cuticular scale pattern of adult human terminal hair sampled from 10 different body sites was examined by scanning electron microscopy and showed: (1) differences according to site, the size of the cuticular scales being significantly greater in scalp hair than in hair taken from the nine other sites; (2) differences according to sex, the size of cuticular scales of facial, axillary and abdominal hairs being significantly larger in females; and (3) differences according to age, the size of cuticular scales from scalp hairs being significantly smaller in older than younger people.
