RESUMEN

There is no data on the level of unmet needs in the heterogenic group of chronically ill patients. The purpose of this study was to define the degree of unmet needs in patients with chronic respiratory diseases and to identify the factors that determine them. The study group consisted of 214 adult patients with the median age of 65 (min-max: 18-90 years). Variables affecting the level of satisfied needs were the following: gender, age, marital status, place of residence, number of chronic diseases, somatic symptoms, level of disease acceptance, level of quality of life (QoL), and health behaviors. The prevention program to increase the level of satisfied needs in patients with chronic respiratory diseases, apart from obviously being addressed to those with low levels of health satisfaction and quality of life indices, should be addressed to men, the elderly, those having no partner, living in the countryside, and having multiple somatic symptoms. Patients with a high level of disease acceptance, those maintaining physical quality of life and advantageous health behaviors should be considered as well.

RESUMEN

Staphylococcus aureus is a widespread human pathogen. One the most striking characteritics of this bacterium is resistance to methicillin and all beta-lactam antibiotics. The agar disk diffusion method is the most widely used in vitro susceptibility test, but recently molecular methods, e.g. Polymerase Chain Reaction, have been also introduced. We compared the detection of methicillin resistant coagulase positive Staphylococcus aureus isolated from clinical materials in Silesian microbiological laboratories by diffusion method and PCR through the detection of nuc and mec A genes. Our results show that PCR used for the detection of mec A gene increases the detection of methicillin-resistant Staphylococcus aureus strains by 10% as compared to the agar disk diffusion method. Among Staphylococcus aureus strains, detected as methicillin-resistant, 17% of organisms showed no presence of mec A gene.

42 patients (27 women and 15 men aged 14-65 years) suffering from atopic and non atopic rhinitis were submitted for 24 days to the subterraneo-therapy in salt chambers of Kinga Spa in Wieliczka during the exacerbation of their illness. 20 patients suffered from pollen or dust mites allergy and the next 22 patients from non atopic rhinitis with recurrent nasal polyposis. In the first week and at the end of the therapy rhinoscopy, peak nasal inspiratory flow and smell were evaluated as well as daily symptoms score was noted in each treated patient. The results of performed observations indicate that unrelatively to the type of the rhinitis: 1. the nasal symptoms (running nose, sneezing, block of the nose) resolved directly during the stay of the patient in the salt chamber but the more constant effect was visible with the time of the therapy. 2. an increase in the peak nasal expiratory flow was connected with the decrease of nasal mucosal edema, 3. the smell disturbances disappeared in the patients together with the decrease of mucosal edema. The observed changes seem to be connected with the influence of the salt chambers' microclimate on the cells osmolarity disturbances in the nasal mucosa provoked by the non infectious chronic inflammation.

An urine routine and bacteriological analysis was performed in 100 female patients in third "trimester" of pregnancy and in the second day after delivery. When bacteriuria was detected a next urine bacteriological examination was done in 8-10 and 30-50 days after the delivery. A rate of bacteriuria in female patients before the delivery was 33% and after 56%. The results of routine urine analysis were in normal limits in 31.1% of females with significant bacteriuria. A spontaneous disappearance of bacteriuria between 8 and 50 day after the delivery was observed in 46.6% of patients, but in 53.4% symptoms of urinary tract infection were noticed. Escherichia coli (serotype A and B) was defined in 79.3% of observed urinary tract infections.

RESUMEN

(14C)-cAMP was injected into the lateral ventricle of the rat brain and autoradiographic studies and assay of metabolism of this nucleotide were carried out. (14C)-cAMP rapidly penetrates into the cerebrospinal fluid in other brain ventricles and into the subarachnoidol cerebellar space. 14C was also present in the brain tissue adjacent to the lumen of the ventricles. Given intraventricularly (14C)-cAMP is rapidly metabolized and the presence of 14C in the brain tissue is probably the consequence of the incorporation of metabolites of (14C)-cAMP.
