RESUMEN

1,4-Butanediol ether (BDDE) is widely used as a cross-linker for hyaluronic acid in dermal fillers. The purpose of this scoping review was to determine the state of knowledge about the behaviour of cross-linked substances and safety of BDDE application. The rationale behind the review came from the clinical experience of one of the authors (KS), who noticed adverse reactions after BDDE-linked hyaluronan application. The scoping review was conducted according to PRISMA-ScR guidelines. Out of 399 articles, 52 met the inclusion criteria. Data on study design, sample/population, aims, methodology, outcomes and funding were extracted. Results were charted according to 6 subtopics: rheological properties, hydrogel stability, BDDE toxicity, immunogenicity, tissue interactions and clinical studies. In vitro, cross-linked hydrogels were characterized as effective fillers in terms of viscosity and elasticity; however, previously uncharacterized by-products of the cross-linking reaction were found. Most in vivo studies reported increased dermis regeneration, vascularization and anti-inflammatory cytokine release after implantation of BDDE-cross-linked substances. In clinical studies, BDDE was shown to sensitize subjects to 1,6-hexanediol ether and other substances found in epoxy resin systems. Occupational dermatitis and hypersensitivity reactions were documented. Our review shows that BDDE may have long-term adverse effects, which are overlooked in the safety assessment of fillers. Reviews on BDDE conducted so far have mostly been sponsored by the industry, potentially leading to incomplete reporting of adverse effects. A review of the occurrence of allergic reactions after commercial dermal filler use and analysis of possibly harmful by-products of BDDE hyaluronan degradation are needed.Level of Evidence III This journal requires that authors assign a level of evidence to each article. The objective of this study was to investigate if delivering multiple doses of N-acetylcysteine (NAC) post-surgery in addition to pre-incisional administration significantly impacts the wound healing process in a rat model. Full-thickness skin incisions were carried out on the dorsum of 24 Sprague-Dawley rats in six locations. Fifteen minutes prior to the incision, half of the sites were treated with a control solution, with the wounds on the contralateral side treated with solutions containing 0.015%, 0.03% and 0.045% of NAC. In the case of the NAC treated group, further injections were given every 8 h for three days. On days 3, 7, 14 and 60 post-op, rats were sacrificed to gather material for the histological analysis, which included histomorphometry, collagen fiber organization analysis, immunohistochemistry and Abramov scale scoring. It was determined that scars treated with 0.015% NAC had significantly lower reepithelization than the control at day 60 post-op (p = 0.0018). Scars treated with 0.045% NAC had a significantly lower collagen fiber variance compared to 0.015% NAC at day 14 post-op (p = 0.02 and p = 0.04) and a lower mean scar width than the control at day 60 post-op (p = 0.0354 and p = 0.0224). No significant differences in the recruitment of immune cells and histological parameters were found. The results point to a limited efficacy of multiple NAC injections post-surgery in wound healing.

BACKGROUND: The Puck commune is one of the largest agricultural regions in the Pomeranian Voivodship that due to the pollution of the coastal zone negatively affects the functioning of the Puck Bay, including health of its inhabitants, and causes decrease in tourism as well as in overall economic value of the region. The objective of the undertaken study was to assess the extent of risk to the environment posed by the pesticides used in agricultural production in the coastal area of the Puck commune. METHODS: The study focused on organochlorine insecticides (DDT and its metabolites: α, ß, ϒ, δ-HCH, aldrin, dieldrin, endrin, isodrine), glyphosate and its metabolite AMPA, and 309 active substances used as pesticides. Analyses were carried out using GC-MS, GC-MS/MS and LC-MS/MS techniques. The undertaken novel approach included "tracking" of a large number of substances in multiple environmental matrices (surface water, groundwater, seawater, soil, sediment and fish), along with examination of their transport routes from the pesticides application locality to the Puck Bay. RESULTS: Glyphosate and its metabolite AMPA, anthraquinone, boscalid, chlorpyrifos-ethyl, dimethachlor, diflufenican, difenoconazole, epoxiconazole, fluopicolide and metazachlor were found in soil samples and surface water samples collected from drainage ditches surrounding the studied agricultural plots. In the samples of seawater and fish taken from the Puck Bay no studied pesticides were found.
