RESUMEN

A case of thyroid carcinoma metastatic to the mandible is presented. Particular attention has been called to its extremely vascular nature and a recommendation has been made that all lesions of such suspected vascularity should be preoperatively studied by angiography in an effort to provide insight into the nature of the lesion as well as the exact blood supply. With this information, appropriate devascularization either by surgical ligation or intravascular embolization can be performed before biopsy and hence sudden and potentially overwhelming hemorrhage during incisional or excisional surgery can be avoided.

RESUMEN

The malignant transformation of a plantar nevus is presented. Perilesional vitiligo developed concurrently with the onset of malignancy. The primary lesion was controlled surgically, but the patient succumbed to metastiatic melanoma within a year. The relation of perilesional vitiligo in malignant melanoma to the so-called halo nevus, and the immunologic significance of these discolorations is reviewed.

RESUMEN

A case of cystic mesenchymal hamartoma in a 2-year-old boy is presented. The overall angiographic appearance closely resembled that of the more common renal angiomyolipoma, with the addition of multiple focal avascular areas representing the cystic component of the lesion. The coexistence of these angiographic findings in a young patient with an otherwise asymptomatic upper abdominal mass should suggest cystic mesenchymal hamartoma.

RESUMEN

Two patients with primary and four with metastatic distal phalangeal lesions in whom a wrong diagnosis of benign disease was initially made are presented with a brief review of the literature and a discussion of the clinico-radiographic features. Primary phalangeal lesions may be either epidermoid carcinoma or malignant melanoma are are often confused with paronychia, while most metastatic lesions are due to pulmonary or breast cancer and simulate a felon clinically. Primary nail bed lesions are characterized radiographically by external cortical erosions; metastatic lesions tend to show a diffuse demineralization and extensive destruction of the whole digit.

RESUMEN

Within a month after Roentgen's discovery of the X-ray, the first cadaveric peripheral arteriogram was performed. It was not until 28 years later that the initial in vivo femoral arteriogram was reported. Arteriographic growth and development since that time have been punctuated with such names as Seldinger, Steinberg, Viamonte, Abrams, Sones, and many more. Keeping pace with their technical and diagnostic achievements has been the cause for a whole new industrial field - that of angiographic equipment and technology. New X-ray tubes, generators, film, etc. have been developed. Angiographic tables are available which have been designed to do highly subspecialized studies. The spectrum of catheter curves and materials available allows visualization of vessels down to the order of 0.1 mm when combined with magnification. Chemoangiography for treatment and subselective venous sampling for hormonal assays are commonplace. Although "the beginnings" for all angiography lie in the evaluation of peripheral vasculature, equipment sophistication for this basic study has often lagged behind that of the more glamorous organal examinations. The development of the biphasic ijection technique hopefully represents an anwer to both multiple injections and X-ray exposures. It allows single injection and single exposure visualization of the vascular tree from distal aorta to the pedal vessels.
