BACKGROUND: Structured settings, such as school, childcare, afterschool programs, summer camps, and physical activity/sport programs are crucial to promoting physical activity (PA) opportunities and reducing sedentary (ST) for children and adolescents. However, little is known about how much PA and ST children and adolescents accumulate in structured settings. The aim of this study is to conduct a systematic review and meta-analysis of the absolute amount of time youth spend physically active and sedentary in different structured settings (Prospero number: 42018111804). METHODS: Observational and experimental/quasi-experimental studies (baseline data only) with full-text available, written in English and published in a peer-reviewed journal, reporting the total amount of objectively measured PA (light, moderate, vigorous, and/or total physical activity) and/or time spent ST during structured settings among youth (3 to 18 years) were eligible. Adjusted meta-analysis was conducted to estimate the pooled mean of time spent in PA and ST, by settings and sex. RESULTS: A total of 187 studies (childcare n=60; school n=91; afterschool programs n=14; summer camp n=4; and Physical activity/ sport programs n=18) from 30 countries (47.9% United States), representing 74,870 youth (mean age 8.6 years old) were included. Overall, there was a high variation between studies in outcomes and settings. The meta-analyses revealed, on average, youth spend 221.8 minutes (36.7 min/hour) in ST and 32.1 minutes (5.1 min/hour) in MVPA during childcare hours, and 223.9 minutes (36.7min/hour) in ST and 27.8 min (4.4 min/hour) in MVPA at school. Relatively, youth are engaged in more MVPA in afterschool programs (11.7 min/hour), PA/ sport programs (20.9 min/hour), and summer camps (6.4 min/hour), when compared to childcare and school. CONCLUSION: Total PA accumulated during childcare and MVPA accumulated during schools hours were close to recommendations, despite high proportion of ST. Afterschool programs, summer camp and PA/ sport programs are important settings that can contribute to daily PA and reduced ST. Ensuring all youth have access to these structured settings may be an important step forward for public health.

The purpose of this study was to investigate the relationship between organized sport (OS) participation and health-related fitness (HRF) in adolescents. A total of 320 adolescents (176 boys) aged between 10 and 16 years reported their engagement in OS and were assessed on 5 components of HRF (cardiovascular endurance, push-ups, curl-ups, flexibility, and body mass index). Data on OS participation frequency and duration were collected through a self-reported questionnaire. Adolescents were stratified by OS participation (engaged, n = 113; nonengaged, n = 220; 55% boys). Nonparametric quantile regression models were used to estimate the differences in HRF by participation group. Less than 30% of the participants reported they were regularly engaged in OS. Frequency of participation ranged from 2 to 5 days per week (median = 2; SD = 3), and duration of participation ranged from 45 to 180 minutes per week (median = 81.7; SD = 32.4). Adolescents who participated in OS displayed better cardiovascular endurance (+4.1 laps completed), with no statistically significant difference detected on any other HRF component. Our results highlight engagement in OS as a promising strategy for achieving cardiovascular endurance. Engagement in OS alone does not seem to be sufficient to enhance fitness components other than cardiovascular endurance.

The elastodynamic Green function can be retrieved from the cross correlations of the motions of a diffuse field. To extract the exact Green function, perfect diffuseness of the illuminating field is required. However, the diffuseness of a field relies on the equipartition of energy, which is usually described in terms of the distribution of wave intensity in direction and polarization. In a full three dimensional (3D) elastic space, the transverse and longitudinal waves have energy densities in fixed proportions. On the other hand, there is an alternative point of view that associates equal energies with the independent modes of vibration. These two approaches are equivalent and describe at least two ways in which equipartition occurs. The authors gather theoretical results for diffuse elastic fields in a 3D full-space and extend them to the half-space problem. In that case, the energies undergo conspicuous fluctuations as a function of depth within about one Rayleigh wavelength. The authors derive diffuse energy densities from both approaches and find they are equal. The results derived here are benchmarks, where perfect diffuseness of the illuminating field was assumed. Some practical implications for the normalization of correlations for Green function retrieval arise and they have some bearing for medium imaging.

In Los Angeles County, California, 142 cases of human listeriosis were reported from January 1 through August 15, 1985. Ninety-three cases (65.5 percent) occurred in pregnant women or their offspring, and 49 (34.5 percent) in nonpregnant adults. There were 48 deaths: 20 fetuses, 10 neonates, and 18 nonpregnant adults. Of the nonpregnant adults, 98 percent (48 of 49) had a known predisposing condition. Eighty-seven percent (81 of 93) of the maternal/neonatal cases were Hispanic. Of the Listeria monocytogenes isolates available for study, 82 percent (86 of 105) were serotype 4b, of which 63 of 86 (73 percent) were the same phage type. A case-control study implicated Mexican-style soft cheese (odds ratio, 5.5; 95 percent confidence interval, 1.2 to 24.8) as the vehicle of infection; a second case-control study showed an association with one brand (Brand A) of Mexican-style soft cheese (odds ratio, 8.5; 95 percent confidence interval, 2.4 to 26.2). Laboratory study confirmed the presence of L. monocytogenes serogroup 4b of the epidemic phage type in Brand A Mexican-style cheese. In mid-June, all Brand A cheese was recalled and the factory was closed. An investigation of the cheese plant suggested that the cheese was commonly contaminated with unpasteurized milk. We conclude that the epidemic of listeriosis was caused by ingestion of Brand A cheese contaminated by one phage type of L. monocytogenes serotype 4b.

Two siblings, one male and one female, were noted to have a distinct skeletal dysplasia. The clinical and radiographic features resemble those observed in Kniest dysplasia and Rolland-Desbuquois syndrome, but important differences were noted. Specifically, these two patients have microstomia, "pursed" lips, and ectopia lentis, and their radiographs reveal no coronal clefts. Chondro-osseous features also differ from those observed in either of the other disorders. Scattered dense patches consisting of collagen fibers 10 to 30 times broader than normal are seen scattered throughout the cartilage matrix; the "Swiss cheese" appearance characteristic of Kniest dysplasia is not observed. These patients appear to have a new skeletal dysplasia, most likely inherited in an autosomal recessive fashion.

Signs of myasthenia gravis developed by age 3 years in 11 children. Six of these patients had persistent neonatal myasthenia gravis, a familial abnormality of neuromuscular transmission that is not immunologically mediated. Five patients had juvenile onset myasthenia gravis, an autoimmune disorder similar to myasthenia gravis in adults. Autoimmune myasthenia has rarely been recognized by age 3 years, but the presence of five cases in our series suggests that the disorder may be more common in young children than once believed. The development of anti-acetylcholine receptor antibody assays makes it easier to distinguish autoimmune myasthenia gravis from the congenital forms. This distinction is important, because the prognosis, treatment, and risk of recurrence in family members is different for each type of myasthenia.
