RESUMEN

The radial forearm flap is one of the most commonly raised free flaps. In most cases an Allen's test is sufficient for preoperative assessment of the palmar arch, but an intact palmar arch does not imply that the radial artery is suitable for microvascular anastomosis. For a patient operated for oral squamous cell carcinoma, reconstruction was planned with a radial forearm flap. The flap had to be discarded because of advanced atherosclerosis in the presence of a negative Allen's test. The correlation of an ex-vivo angiography and the histopathologic findings was assessed. In cases of expected vascular disease, further examinations like color flow Doppler ultrasound are recommended to ensure sufficient perfusion of the radial artery.

RESUMEN

The complete traumatic dislocation of the globe into the maxillary sinus is a very rare event. Due to the intense force affecting the orbit, the impact on the globe and its accompanying structures is normally very severe. A case is reported of complete dislocation of the globe into the maxillary sinus with nearly complete recovery of vision and motility.

This double-blind, randomised, parallel-group trial compared the analgesic efficacy of single 50 mg doses of diclofenac potassium sachets and tablets with placebo in 184 patients with moderate/severe pain after third molar extraction. The primary efficacy variable was the average pain reduction from baseline during the first 2-h postdose, using a visual analogue scale (VAS). During the first 2-h postdose, sachets and tablets significantly reduced pain (p < 0.05) vs. placebo with an incremental benefit seen for sachets over tablets (p < 0.05). Onset of analgesic effect (VAS) was at 30 min for sachets and 45 min for tablets. Pain reduction vs. placebo (VAS) was maintained for 8 h for sachets and tablets (p < 0.05). VAS-findings were confirmed by pain relief and intensity verbal scale assessments. Fewer patients re-medicated vs. placebo. No safety issues were identified. This study demonstrates that both diclofenac potassium sachets and tablets offer patients suffering from acute pain conditions an effective treatment with incremental analgesic benefits seen for sachets.

BACKGROUND: In spite of a monocortical design, miniplate osteosynthesis can injure dental roots directly as well as damage dental substance indirectly by interrupting the apical blood stream. PURPOSE: The present retrospective study classifies different types of dental root trauma caused by monocortical screws, suggests therapeutic options based on diagnosis, and documents survival probability and prognosis after tooth trauma. PATIENTS AND METHODS: During a period of 11 years, 380 patients with permanent dentition underwent miniplate osteosynthesis for the treatment of mandibular fractures, 29 of whom sustained dental root trauma caused by drilling failure. These patients were clinically and radiographically examined for a follow-up time of not less than 38 months. RESULTS: The 29 patients could be classified into four different types of dental root trauma: 13 pulp injuries above the apical third of the root (type Ia), 6 pulp injuries in the apical third of the root or extradental lesions interrupting the apical blood stream (type Ib), 4 lesions to the central radicular dentin without pulp injury (type II), and 6 lesions to the peripheral radicular dentin and root cementum (type III). Of 13 type Ia injuries, 5 developed apical periodontitis and dilatation of the periodontal space. Therefore one root canal treatment and three apicoectomies were performed. One tooth had to be extracted. Three further type Ia injuries and two type Ib injuries showed root resorptions inducing two root canal treatments. One of six type Ib injuries required root canal treatment because of apical periodontitis. One of four type II injuries caused root resorption not requiring therapy. No relevant, pathological finding could be identified after type III injury. CONCLUSIONS: The type of dental root trauma caused by miniplate osteosynthesis determines therapy, complication rate, and survival of the injured tooth.

Cleidocranial dysplasia (CCD) and the Rubinstein-Taybi syndrome (RTS) are two rare congenital syndromes that have many clinical signs in common. We present an 18-year-old-patient with untypical CCD expression who was misdiagnosed with RTS at the age of 2 years. An extensive craniofacial examination was carried out with respect to morphological and dental aspects. The molecular-genetic analysis of two underlying genes (CBFA1 and CBP) for CCD and RTS was performed using SSCP, direct sequencing and FISH. While the clinical examination showed uncharacteristic CCD symptoms with some findings common for RTS, the molecular-genetic analysis revealed a missense mutation in the CBFA1 gene, which is considered to be the etiological factor for CCD. Our findings with this patient presented clear evidence for the wide morphologic variety that can be related to a certain gene such as CBFA1. The diagnosis of rare diseases is currently based on the clinical phenomenology of small groups or single cases. The use of molecular-genetic biology extends the horizon of diagnostic and scientific possibilities. In this patient, it allowed us to compare the clinically diagnosis to molecular-genetic data. We conclude that molecular-genetic analysis may be a helpful tool in the differential diagnosis of many congenital diseases such as CCD and RTS.

The aim of this study was to analyze the CBFA1 gene in a phenotypically variable family with autosomal dominant cleidocranial dysplasia (CCD). Five members of a family with CCD were characterized clinically. X-rays and photographs of the two clinically affected family members were taken. The genotype of all five affected family members was determined with the use of single strand conformation polymorphism (SSCP) and direct sequencing. A point-mutation in exon 2 (R148G) was detected in a patient with the full-blown clinical phenotype. His son, demonstrating the same mutation, showed only the dental CCD characteristics. No mutation could be found in the three clinically healthy family members. To conclude, a missense mutation in the CBFA1 gene was detected in a family with variably expressed CCD syndrome. A detailed clinical examination is necessary to detect minimally affected gene mutation carriers.

RESUMEN

In this investigation we compared the precision of different computer-aided systems for positioning osteotomized segments to reconstruct craniofacial asymmetries. The accuracy of a mechanical navigation system, a laser-guided navigation system and the precision of templates moulded on stereolithographic models are compared, using measurements on geometrical objects, anatomical specimens and clinical application. The precision of a mechanical navigation system is 0.5 mm, and a laser-guided system 0.3 mm. The accuracy of a computer and template guided application is 1 mm.

RESUMEN

For some 10 years it has been usual practice to determine tissue drug concentrations following the application of topical antirheumatic agents. However, testing the measured drug concentrations for their therapeutic relevance continues to be difficult. An appropriate reference parameter is, for example, the IC50 of the inhibition of the prostaglandin synthesis. Moreover, tissue concentrations should be compared with those achieved following oral administration of clinically effective doses. Based upon practical experience with ibuprofen (CAS 15687-27-1) following both topical and oral administrations, it is illustrated how such studies and the tissue concentrations found could be evaluated and interpreted taking into particular consideration the sample size calculation and the therapeutic relevance of the results.

Based upon the results of a bioequivalence study in 18 male and female healthy volunteers receiving each ibuprofen 400 mg at a time interval of 3 days the issue of the intrasubject variation is discussed. It was shown that there was bioequivalence as to the parameters Cmax, AUC0-tlast und AUC0-infinity in the volunteers who repeatedly received the same preparation. The same is true for the secondary target parameters tmax und t1/2. In the present study the intrasubject variability was determined to be 10%. Since ibuprofen does not belong to the drugs with problematic bioavailability it can be expected that in case of repeated application at an interval limited in time equal results are achieved with considerable intra-and intersubject variations.

RESUMEN

80 patients with 83 fractures of the mandibular angle were treated by lag screw fixation. The screws were placed through a transoral incision with transbuccal trochar instrumentation. No maxillomandibular fixation was used. Complications occurred in 12 fractures (14%), of which 5 (6%) required secondary surgical intervention. In all cases bony union was achieved 6 months postoperatively. Indications and advantages of the technique are discussed.

In a one-armed prospective study including 60 patients (27 male, 33 female; age 17-75 years), 109 teeth were treated by means of a screw system made of Syntacoben and Niob according to Wirz. 103 teeth were transfixed successfully: 48% of them were provided with a partial removable denture; 34% were used as a bridge pier; 11% underwent conservative treatment; in 7% prosthetic treatment is planned. The use of the Wirz-System for primary transfixation combines in an ideal way the advantages of endodontic conservation and the ready-made post with the improved stability of the prosthetic pier.

In a within-subject comparative trial in 16 healthy volunteers, the bioequivalence of two sulpiride 200 mg preparations was tested using a model-free method of calculation as well as assuming 2- and 3-compartment models. As to AUC act., AUC inf. and Cmax, the test preparation was shown to be significantly superior. Some other differences were found depending on which calculation method was used. The Cmax of the test preparation showed therapeutically relevant plasma concentrations of greater than 400 ng/ml, compared with about 300 ng/ml for the reference preparation. Based upon the raw data obtained, it was the aim of this study to gain further knowledge about other kinetic parameters on which only few data is available. T1/2 beta, clearance, volume of distribution and MRT were calculated using 2- and 3-compartment models. Comparisons with literature data were made and it resulted that a better description of the sulpiride kinetics was obtained when a 3-compartment model was used. Finally, the course of the plasma level was calculated by computer extrapolation assuming a thrice daily administration of sulpiride 200 mg. As a result, steady-state is reached after 3-4 days showing plasma concentrations around 650 ng (reference preparation) and 850 ng (test preparation).

RESUMEN

As in the case of systemically administered drugs, there is also a requirement for reliable kinetic evidence for the clinical effectiveness of local therapeutic NSAIDs. The conditions required for this are discussed, and compared with provisions already existing in EC countries. On the basis of a comprehensive review of the literature, it is shown that among all the locally employed NSAIDs, kinetically reliable and plausible evidence of therapeutic effectiveness is, at present, available only for indomethacin, diclofenac, salicylic acid salts and ibuprofen.

Concentrations of Ibuprofen and Protein Concentration and pH-Value in Synovial Fluid and Plasma Following Oral Administration of Ibuprofen in Patients Suffering from Arthritis. In 16 patients suffering from arthritis of the knee the concentration of ibuprofen in the synovial fluid was examined in correlation with the synovial fluid volume, cell count, pH value and protein concentration. The mean ibuprofen concentration in plasma 4 h after oral administration of 400 mg ibuprofen amounted to 15.45 micrograms/ml, the concentration in the synovial fluid was 9.4 micrograms/ml. Due to the inflammatory nature of the effusions the protein concentration in the synovial fluid was evidently increased to an average of 4.46 g/dl and thus was about 65% of the mean plasma protein concentration of 6.88 g/dl. The ibuprofen in the synovial fluid showed a correlation to the protein concentration. On the other hand, there were no significant differences between the pH value in the plasma and in the synovial fluid. There was no tendency to an acid pH. Furthermore, there was no correlation between ibuprofen concentration and cell count. The tests showed that the "accumulation" of the nonsteroidal antiinflammatory drug in the synovial fluid is positively correlated with the high protein concentration but not with the pH value.

The percutaneous penetration of an ibuprofen cream at 5% (Dolgit Creme) was tested in guinea pigs. Already 20 min after application of the cream on the shaved skin high ibuprofen concentrations of about 10 micrograms/g were found in the subcutaneous and in the muscular tissue. The plasma concentrations reached only one tenth of the values ascertained in the tissues. 2 h after application the concentrations in the tissue increased by 50%, whereas the increase of the plasma concentrations was more significant. The results confirm in agreement with kinetic studies in man that ibuprofen penetrates from the tested formulation directly and very quickly in deeper tissues. The deviation of the active ingredient over the systemic circulation is improbable.
