BACKGROUND: The problem of transfusion-transmitted infections still remains serious and actual for health care despite the detailed testing of donors. Human immunodeficiency virus, hepatitis B and C viruses and human cytomegalovirus are among the most dangerous pathogens that can be transmitted with blood. Previously, a composition consisting of fullerene layer applied on silica gel particles was shown to inactivate influenza virus up to complete loss of infectivity. METHODS: In the present study the unit has been developed with source of irradiation whose spectrum is appropriate for solid-phase fullerene. The ability of the unit to inactivate the enveloped influenza virus in protein fraction of donor blood has been studied. RESULTS: It was shown that at optimized conditions complete inactivation of enveloped virus of extremely high initial titer (7.0-9.5 log 10 EID 50/0.2 mL) in the solution of albumin was achieved after as short time as 30 min of irradiation. This process did not affect the oxidative metabolism of neutrophils and membranes of erythrocytes evaluated by NBT reduction test and morphological analysis of erythrocytes, respectively. CONCLUSION: The data obtained suggests that the method described can be recommended for further development and optimization of the procedure of inactivation of viruses in the preparations of the plasma of donor blood.

Immunocorrection by means of transfusions of donor leukocytes was performed in 64 patients. Transfusions of the leukocytic suspensions in short courses (2-3 transfusions to one patient or 300-500 ml per a course) during the first three days after operation allowed the immune status of organism to be improved and increased the effectiveness of the antibacterial and infusion-detoxication therapy. The greatest medical effect was obtained when using the leukosuspension from donors with natural high (from 1:160 and higher) titers of antibodies to E. coli.

Leukotransfusion from ordinary donors and those immunized by staphylococcal anatoxin were used in complex treatment of 55 patients in a catabolic phase of different forms of sepsis. It was found that transfusion of leukosuspensions, immune ones in particular, had pronounced immunostimulating, detoxicating, antibacterial and anti-inflammatory effects. A short course of transfusions of allogenic leukocytes can be fulfilled without a selection by the HLA system.

The authors studied the effect of transfusions of a leukocyte suspension on the main clinico-immunological values of homeostasis in 56 patients with acute pyodestructive diseases of the lungs. They found that inclusion of leukocyte transfusions in the complex of therapeutic measures produced a marked clinical effect, reduced the term of treatment, and allowed successful control of endotoxicosis and immunodeficiency in patients with pulmonary suppuration. Immune leukocytes obtained according to a specially developed method had a more marked immunocorrective effect.

The application of leukocyte mass against the background of altered reactivity of the organism was followed by positive changes of clinical and immunological data by as early as the 2-3d days of the postoperative period. The level of antistaphylococcal immunity was preserved after completion of the course of leukotransfusions. It results in complete arrest of the inflammation in the mammary gland, excludes the development of recurrences and various pyo-septic complications. Leukotransfusion is not only a method of substitution therapy but also an effective immune-correcting factor.

In patients with a severe course of suppurative lactation mastitis there develops a relative deficiency in the cellular and humoral links of immunity. The compensatory activation of factors of nonspecific antiinflammatory resistance of the organism fails to exert a substantial positive effect on the development of the destructive process in these patients. On the basis of the data obtained, in order to improve results of the surgical treatment of patients with suppurative lactation mastitis means of the cellular anti-infection therapy, transfusions of donor leukocyte masses in particular, must be included in the complex of curative measures in the postoperative period.

The observation over 20 cases of hemophilia with severe renal bleedings proved the possibility of rendering an effective surgical aid to these patients. Besides, on the grounds of the observations the authors concluded that in all the cases of hematuria and microhematuria constitute indications for x-ray examination of the kidneys and the urinary tract.
