RESUMEN

Peculiarities of malignant cardiac tumors surgical treatment were studied, basing on analysis of 33 observations. Rhabdomyosarcoma was diagnosed in 9 patients, angiosarcoma--in 11, leyomyosarcoma--in 6, malignant mixoma--in 4, fibrosarcoma, chondrosarcoma and liposarcoma--each as a single observation. Special attention was paid to guarantee the maximal radicalism in the tumor excision, what made it necessary in 8 observations to perform additive surgical procedures, such as interatrial septum and atrial walls plasty, plastic operations on cardiac valves and the valves prostheses. The operation volume extension did not influence significantly its immediate result.

RESUMEN

Peculiarities of surgical treatment of the heart malignant tumors were studied, up basing on analysis of 29 observations. Rhabdomyosarcoma was revealed in 9 patients, angiosarcoma--in 8, leyomyosarcoma--in 5, malignant myxoma--in 4, fibrosarcoma, chondrosarcoma and liposarcoma--in one for each of them. The peculiarities of excision of different tumors were depicted. Special attention was paid to the necessity of maximally radical excision of the tumor, demanding the additional surgical manipulations conducting in 8 patients. Mortality had constituted 35.7%. The cause of the death practically in all the patients was the pathological process spreading.

RESUMEN

Of the multiple myxomas of the heart (MH) the bilateral myxomas were revealed in 57.1%. Of 4 patients before the operation the correct diagnosis was established only in one. Meticulous data on clinical signs, diagnosis and surgical treatment peculiarities of patients with bilateral MH were adduced. Morphological peculiarities of the tumor were analyzed. Special attention was paid to the necessity of clinical and surgical guard toward revelation of undiagnosed tumors, the choice of optimal approaches to the heart cavities while performing radical operation. Basing on results of histological studying of operational specimen it was established that MH is originated from transformed vessels of interatrial septum.

RESUMEN

Experience of surgical treatment of 362 patients with the heart myxoma (HM) was summarized. The expediency of application of various approaches in the left auricle (LA) myxoma was analyzed. Significant advantages of application of the approach through the right auricle to reduce the danger of the tumor fragmentation while its excision were noted. There was elaborated and introduced transbiauricle approach to the LA tumor, application of which secures minimal risk of the tumor fragmentation as well as the possibility of adequate heart revision conduction and the processing of full value of tumor bed for any localization. In terms of observation up to 30 years in 5 (17%) of patients the HM recurrency had occurred. Application of the method, which envisages excision of part of interatrial septum, where HM is fixed, in its all thickness with subsequent suturing and plasty of defect using the patch, making additional processing of the tumor bed with iodine solution and diathermocoagulation, had permitted to reduce the recurrency of the disease from 4.3 to 0.5%.

RESUMEN

Experience of surgical treatment of 45 patients with corrected transposition of the main vessels (CTMV) is summarized. Indications and tactic of surgical intervention for CTMV with the pulmonary trunk (PT) stenosis using conduit as a parallel exit from venous ventricle are substantiated. Original construction of autopericardial conduit is adduced. Basing on analysis of results of surgical treatment an optimal type of conduit for treatment of CTMV with the PT stenosis is proposed.

RESUMEN

Experience of surgical correction of failures, concomitant with corrected transposition of main vessels (CTMV) in 29 patients was summarized. For pulmonary artery (PA) stenosis demolition in 10 patients the conduits of various design were applied, for moderate left atrioventricular valve (LAVV) insufficiency--its prosthetics. Surgical treatment mortality was 20.7%. Late follow-up results of the operation was studied up in 16 (66.7%) patients. Mean period of observation constituted 3.3 years. Good result was noted in 7 patients, fair--in 9. Lowering of contractile ability of "arterial" ventriculus and the LAVV insufficiency progress were the causes of follow-up results changing for worse. Application of conventional methods of treatment do not put away the basic disorder--CTMV.

RESUMEN

On the basis of studying the immediate and long-term results of treatment of 329 patients with calcinated mitral stenosis, the progressive aggravation of the immediate outcome of the operation in II and III-IV degree of mitral valve calcinosis is demonstrated. The data have been confirmed by actuarial analysis of the patients' survival and stability of a good result. The authors consider it expedient to perform mitral commissurotomy only in patients without valvular calcinosis and I degree calcinosis. In more severe affection, mitral valve replacement is indicated.
