RESUMEN

OBJECTIVE: An unusual case of Klinefelter's syndrome with ischemic priapism is described. METHODS: The clinical course of the present case is described and discussed. CONCLUSIONS: This unusual case cannot be completely ascribed to the hormonal changes in Klinefelter's syndrome, although hormone imbalance may have favoured the development of ischemic priapism.

RESUMEN

OBJECTIVES: We report our experience in the diagnosis and treatment of ureteral injury. METHODS: Review of the experience in our hospital. RESULTS: We describe two cases of complete rupture of the ureter from blunt and penetrating abdominal trauma that had been recognized late. A urinoma was the presenting feature in both cases. Treatment was by end-to-end anastomosis in the first case and by appendix interposition in the other case. CONCLUSIONS: Ureteral injuries owing to abdominal trauma are usually silent. Veriform appendix interposition of the ureter is, in selected cases, a therapeutic option.

RESUMEN

We report a case of complete separation of the epididymis and testis discovered during surgery for cryptorchidism. The epididymis was found in the scrotum and the testis in the inguinal canal. Although this malformation has been reported sporadically, the incidence in patients with cryptorchidism may be higher. These cases support the hypothesis that testicular descent is guided by the tail of the epididymis.

RESUMEN

Analysis of 10 fistulae in 9 patients, all male. The etiology was diverse although there was a predominance of colovesical diverticulitis. Mictional urinary symptomatology was most frequent. In one case symptoms had been first referred 10 months earlier, while the others ranged between 24 hours and 15 days. Cystography was the radiological procedure which diagnosed more cases whereas cystoscopy was the endoscopic procedure which provides more data. Seven fistulae were treated surgically, in 5 instances with curative and 2 with palliative purposes. Three cases were subsidiary to surgery. 44.4% of patients died within one month from diagnosis.

RESUMEN

Sonourethrography has recently become available for evaluating the urethra. To perform this procedure correctly, precise knowledge of the ultrasound anatomy of the penis and perineum is required. Sixty-one males were studied using the 7.5 Mhz ultrasound transducer. During sonographic evaluation the site where the suspensory ligament of the penis is attached must be identified routinely. The nothc of the compressor nudae muscle in the posterior bulbous urethra can be visualized occasionally. The origin of the membranous urethra and urogenital diaphragm can be identified upon visualizing the entire urethral bulb and the cavernous bulb muscle.

RESUMEN

Using ultrasound scan urethrography (U.S.U.) and radiological retrograde urethrography (R.U.), the anterior urethra has been studied in 61 males in an attempt to establish the best technique to perform U.S.U. and to compare it, from the technical point of view, with R.U. U.S.U. should be made retrogradely filling the urethra with a saline solution, under direct ultrasound guidance, which allows to see the anterior urethra, from the syringe come introduced into the glandular urethra up to the urogenital diaphragm and the beginning of the membranous urethra. Overall, U.S.U. is faster to perform than R.U., nine times cheaper, more comfortable for the patient, and there is no risk of exposure to irradiation or radiological markers.
