RESUMEN

Embolization of the renal arterial bed with embolization agent Vilan 500 was performed in 13 dogs. Embolized kidneys shrink after the procedure and the functional parenchyma is extinguished. Peripheral PRA values were followed to find out whether renin production by the embolized kidney is extinguished (the non-embolized kidney was surgically removed). In nine of the thirteen dogs peripheral PRA decreased below 0.5 ng/ml/h; this renin activity can be considered extrarenal. Embolization of the kidneys with Vilan 500 can thus substitute surgical nephrectomy in patients with secondary renal hypertension.

RESUMEN

Using in-situ "autotransplantation model" and dogs with allotransplant of pancreatic segment with the open duct, we confirmed the capacity of the canine peritoneum to tolerate the presence of inactive pancreatic juice and to resorb it. However, non-living tissue surrounding the graft may induce activation of pancreatic juice and lead to acute pancreatitis. Diffuse interstitial fibrosis, unaffecting endocrine parenchyma, develops in all autologous and allogenic pancreatic grafts. Reduction of acinose parenchyma probably accounts for the gradual normalization of serum amylase levels. We assume that graft fibrosis is caused by a gradual closure of the pancreatic duct. Diffuse interstitial fibrosis cannot be classified as a rejection change. It antiplatelet drugs are not administered, graft necrosis is caused by splenic artery thrombosis at a low flow rate. In diabetic recipients with antiplatelet therapy both the animal and the graft survivals averaged 22.5 days. All dogs died of complications at a normal glucose metabolism. We hold that at present research should concentrate on vascular complications of pancreatic segment transplantation.

RESUMEN

A new occlusive agent, Vilan 500, was used to embolize the arterial bed of the kidney in 12 dogs. This polyvinyl acetate, alcohol-soluble agent becomes gelantinous on contact with an aqueous environment and its low viscosity makes this material well suited for application through small-caliber catheters. Complete arterial occlusion of the embolized kidneys was observed in all animals. On histologic examination of tissue 4-6 weeks after the procedure, changes were seen such as shrinkage through fibrosis, diffuse necrosis and atrophic parenchyma. No signs of recanalization of the embolized artery or functional collateral circulation were noted in any dog.

RESUMEN

The development of pancreatic changes was studied after obliteration of pancreatic ducts by means af 2-hydroxyethylmethacrylate. The drug was injected into the main pancreatic duct of 15 dogs that were put to death successively and examined by histology. Necrosis of pancreatic duct epithelium was found in 4 days after the polymer administration in addition to focal periductal necrosis of acinar tissue and interstitial oedema with fibroblast activation. Later, a rapid development of fibrosis occurred which ensue in an extinction of exocrine tissue after 2 months. Pancreas consisted then but of connective tissue and cumulated Langerhans' islets with positive beta-granulations. Pancreatic ducts obliteration is a reliable method for selective inhibition of exocrine activity.

The autotransplantation of a pancreas segment with ligated duct to cervical vessels, autotransplantation and allotransplantation of a pancreas segment with unligated duct to iliac vessels were carried out in three groups of dogs. The occurrence was followed of the hemorrhagic necrosis and of their complications. When ligating the duct, the mortality and the rate of occurrence of hemorrhagic necrosis were of 54 and 77% respectively. In grafts with an open duct into the abdominal cavity, for the autotransplantation the mortality and occurrence of the hemorrhagic necroses were of 33%, for the allotransplantation the mortality and autodigestion were of 11 and 45%, respectively. The cause of the hemorrhagic necrosis in animals with unligated duct of the graft was most probably represented by a hematoma. The graft rejection was characterized by an asymptotic course without damaging the recipient. The transplantation method with open duct into the abdominal cavity is promising for the clinical application.

RESUMEN

The effect of Venalot, a combination of two benzo-pyrones, on canine experimental acute pancreatitis was examined. Activities of lymph and blood plasma enzymes, thoracic duct lymph flow, and morphological changes of the pancreas were compared with those of a control group of dogs. The drug was found to enhance the removal of amylase and trypsin via lymph from the gland and to decrease the elevation of plasma amylase and lactate dehydrogenase. When administered simultaneously with the induction of pancreatitis, benzo-pyrones were effective in the reduction of pancreatic edema and necroses.

Two atherosclerotic lesions (fatty streak and fibrous plaque) were seen in coronary arteries as early as in the 10-14 age group. Then their frequency increased with age, more rapidly in the left anterior descending coronary artery than in the left circumflex artery. After the age of 55 years fatty streak was still the sole type of lesion in 0.3% of men and an even higher proportion of women. In contrast to the findings in the aorta, the mean extent of fatty streak in the average coronary artery did not exceed 3-4% in all age groups. Fibrous plaque was found in over 90% of men over 40 and women over 50 years of age. The extent of fibrous plaque was greatest by about the age of 65 years in both sexes and showed little change thereafter. The first complicated and calcified lesions appeared in a small proportion of subjects after the age of 20. The frequency of complicated lesions did not exceed 50%, being much lower than that of calcified lesions. Complicated lesions occupied a very small area (mean extent 0.6% in men and 0.3% in women); the extent of calcified lesions in older age groups reached 9-10% of the intimal surface in both sexes. Coronary stenosis appeared after age 20 years in men and 10 years later in women, and its frequency increased with age. There were considerable inter-town differences in the prevalence and extent of atherosclerotic lesions.

Atherosclerosis of the aorta and coronary arteries and myocardial infarction were studied post mortem in 390 males and 190 female cirrhotic subjects in the 5 towns. Comparison with the reference groups revealed that calcification of the aorta and coronary arteries was more frequent (in the case of males) and more extensive (in the case of males and females) in cirrhotics than in noncirrhotics. Raised and complicated lesions were not increased. Coronary stenosis (in females), fresh myocardial infarction (in both sexes), and large myocardial scar (in both sexes) were less frequent in cirrhotics. "Obesity" was similar in cirrhotics and non-cirrhotics.

RESUMEN

A case of reticulum cell sarcoma with kidney involvement is described. The diagnosis was established by open renal biopsy only when the disease was far advanced. Treatment with combination chemotherapy and radiotherapy brought about very short remission and the patient died in uremia. Autopsy showed isolated kidney involvement with only metastasis in thyroid gland.
