RESUMEN

Two strains of Bacillus pumilus (M11 and M38) and one strain each of Bacillus cereus (M27), Bacillus subtilis (M67), and Enterobacter agglomerans (M14) were identified from the air of Lancashire cotton mills. These strains were tested for cytopathic effects in Vero cells; B pumilus and B cereus strains were also examined for haemolytic activity, lecithinase production, and proteolytic action on casein. Rounding and clumping of the Vero cells occurred after the addition of supernatants prepared from B pumilus and B cereus strains; finger-like projections developed in the cells treated with B pumilus supernatants. Minimal effects occurred with B subtilis and E agglomerans. After two hours of exposure B pumilus (M11) produced the greatest effect, but treatment with trypan blue showed that most cells exposed to the M11 strain were still alive after 96 hours of exposure; those exposed to the supernatant prepared from the M38 strain of B pumilus were dead. Sheep erythrocytes were lysed more rapidly by B cereus than by B pumilus, B cereus (strongly positive) had a greater effect on lecithin than either of the B pumilus strains (M38 weakly positive, M11 negative). All hydrolised casein but the effect was more rapid with one of the B pumilus (M11) strains. It is concluded that not only do the toxins of B pumilus differ from those of B cereus, but there are also differences between the toxins produced by the two strains of B pumilus (M11 and M38).

This tutorial paper reviews the action that should be taken at the planning stage of a bacteriological research project to ensure that the final results will have maximal value. Particular consideration is given to the problems of environmental sampling, patient involvement and testing for disinfection and chemotherapy.

Selected samples of healthy people (804 males, 796 non-pregnant females and 400 pregnant women) were questioned about present and previous urinary symptoms. Mid-stream specimens of urine were cultured quantitatively. Symptoms in the males occurred more frequently in the presence of ;significant bacteriuria', but the numbers were too small to allow statistical analysis. Among the non-pregnant females frequency or burning micturition was found more frequently in those who had significant bacteriuria than in those whose urinary bacterial counts were low; for nocturia this difference was statistically significant (p <0.001).Of the pregnant women, comparison of those who had significant bacteriuria with those whose urine was normal showed that diurnal and nocturnal frequency, and loin pain, occurred more frequently in those with significant bacteriuria (for each of these symptoms p <0.0.1).These results suggest that the recent onset of nocturia is the most reliable symptom of urinary tract infection. There remain, however, many people with urinary symptoms and with low urinary bacterial counts in whom other causes for the symptoms should be sought.

In 1967 we admitted 339 cases of infantile gastroenteritis; one-third of these were dehydrated, and in this group the commonest biochemical abnormality found was hypernatraemia, sometimes with metabolic acidosis. A higher incidence of dehydration was found in the patients who had received oral glucose fluids before admission. EnteropathicEscherichia coliwere isolated from the faeces of 16% of the cases. Associated infections, especially of the respiratory tract, were common. Treatment was aimed at the restoration of fluid and electrolyte balance. Usually this was achieved with oral fluids, though intravenous fluids were used in the most severely dehydrated cases. Recovery was complete in 320 cases and a further 14 cases were discharged as carriers of enteropathicE. coli. There were five deaths (1.5%) in the series; three occurred immediately after admission.

A SURVEY was undertaken to find the prevalence of asymptomatic bacteriuria in people in normal health who were patients of members of the North-west Faculty of the Royal College of General Practitioners. Midstream specimens were collected from 341 males and 456 females seen in visits to 19 practices in Lancashire and Cheshire. Three males and 45 females had > 10(5) bacteria/ml; three males and 26 females had > 50 pus cells/cu mm. Both significant bacteriuria and pyuria were found in three males and seven females. The percentage incidence varied in each practice from 0-24 per cent. About one third of the patients with significant bacteriuria or pyuria had urinary symptoms, but there were also 53 males and 104 females, who had urinary symptoms, whose urine was normal.The highest attendances were in those practices where the family doctor made the initial approach to the subjects.
