Possibilities of the procedures conduction of the left atrium (LA) plasty in a mitral valve prosthesis (MVP) were studied. There were examined 553 patients, in whom surgical treatment in the clinic was conducted. In all the patients MVP was conducted for isolated mitral valve failure and dilatation of LA. In 371 patients (the main group) MVP and LA plasty were conducted, in 182 (control group)--LA was not corrected. Morphometric indices of left cardiac cameras, survival, stability of the operation good results in late terms have witnessed high efficiency of MVP with LA reduction in comparison with such in a control group. Late results of MVP in conjunction with LA reduction exceed such in a control group, witnessing expediency of the proposed methods of surgical treatment application.

Peculiarities of deformity of the left ventricle (LV) walls were studied in 93 patients in various variants of hypertrophic cardiomyopathy (HCMP). In accordance to complex echocardiography data, including vector-echocardiography, in HCMP the deformity grade and quantity of the affected segments of the LV myocardium have had depended directly on the stenosis degree of the LV tractus deferens (TD). Initially, a capacity for deformity lowers in basal segments of interventricular septum and in the LV anterior wall. While progressing of the LV TD obstruction, other myocardium segments become involved in the process, what is accompanied by deepening of the cardiac insufficiency (CI) severity. In maximal affection of myocardium and mostly expressed signs of CI gradient of systolic pressure in LV TD have had eliminated, in spite of presence of the LV expressed hypertrophy.

The method of prediction the enability of closure atrial septal defect before operation or in pre-bypass period during the one and one-half ventricle repair of CHD with hypoplastic right ventricle was presented. The method is based on comparison of the initial blood flow on superior vena cava (SVC) and right-to-left shunt at atrial level. Defect can be closed if flow on SVC and shunt flow are approximately equal. According to prognosis in 16 of 37 patients defect was closed. After operation there were not signes congestive heart insufficiensy and cyanosis. This approach allowes to avoid reoperations at least in half of patients. If shunt flow exceeds flow on SVC, defect must be left open.

Radical anatomic correction--operation of arterial switch--is performed in the main vessels transposition in majority of the world big cardiosurgical clinics, but its late results are studied insufficiently yet. Echocardiographic cardiac parameters preoperatively and in immediate postoperative period after anatomic correction in the main vessels transposition were presented. Comparative analysis of indexes, including estimation of anatomy and function of cardiac ventricles, depending on initial data and terms of the operation performance was conducted.
