RESUMEN

Oral lichen planus (OLP) is a chronic mucocutaneous inflammatory condition of stratified squamous epithelia. OLP is a potentially malignant condition in oral mucosa. Patients with OLP have an increased risk of developing squamous cell carcinoma. Therefore an early and accurate diagnosis is necessary to avoid further damage to the oral mucosa. Biopsy followed by histopathological examination is the gold standard for the diagnosis of oral mucosal lesions including OLP. But this invasive procedure is traumatic and time consuming with limited statistical confidence level. Autofluorescence spectroscopy (AFS) has recently emerged as a potential tool to evaluate the biochemical changes associated with oral cavity disorders. In this study, we used AFS to differentiate the oral cavity tissue of 20 OLP patients from that of 16 normal volunteers. Spectra from oral mucosa were acquired at 280, 320 and 410 nm excitation wavelengths which correspond to the excitation energy of major endogenous fluorophores. Normalized spectral data at 320 nm excitation showed significant increase in the intensity of collagen peak for OLP. Optical redox ratio and total hemoglobin concentration estimated from the spectral data revealed significant increase and decrease respectively in OLP and normal patients. Principal component analysis followed by linear discriminant analysis (PCA-LDA) provided sensitivity and specificity of 71 and 80%, 80 and 90%, and 72 and 75% respectively for 280, 320 and 410 nm excited spectral datasets. Meanwhile, partial least square discriminant analysis (PLS-DA) provided sensitivity and specificity of 69 and 77%, 78 and 91% and 73 and 78.13% respectively for 280, 320 and 410 nm excited spectral datasets. From the results, it is concluded that AFS is an efficient tool for the non invasive diagnosis of OLP, with 320 nm light identified as the best wavelength for excitation.

The aim was to compare age estimation using Cameriere's and Demirjian's methods to chronologic age in children with mixed dentition in a rural population of Kerala. The present study comprised of 10 subjects of age range 7-12 years. Dental age was assessed using Cameriere's and Demirjian's methods and was compared to the chronologic age. Panoramic radiographs were used for assessment of dental age. Data were analysed using paired t-test. The mean of difference obtained was 0.92 and the pvalue was 0.172 which showed insignificant difference between the two methods. Cameriere's method showed a positive linear correlation (0.6393) with chronologic age and was statistically significant (P=0.0171), whereas Demirjian's method showed a negative correlation (-0.7598) and was statistically insignificant (P=0.9967). The present study indicated that Cameriere's method is reliable for age estimation in our population and is more accurate than Demirjian's method.

Radiography plays an important role in detection of interproximal caries. The aim of study is to compare diagnostic ability of photo stimulable phosphor (PSP) with direct measurement using stereomicroscope in detecting proximal caries. Hundred proximal surfaces of 50 extracted human posterior teeth were radiographed with dental X-ray unit. The image receptors used was storage phosphor plate Vista scan (size 2), (time of exposure 0.4 s). Radiographs were interpreted and caries lesions were classified on a 4-point scale suggested by Abesi et al. The teeth were sectioned with diamond disc and were examined under a stereomicroscope with 20x magnification. Diagnostic accuracy of digital image is similar to that observed with stereomicroscope. The PSP plate digital X ray system can effectively be employed for detecting proximal caries as compared to direct observation by stereomicro-scope. Further study with more number of observer/evaluator and large sample size is recommended.

BACKGROUND: The role of retinoblastoma (Rb) protein in cell cycle regulation prompted us to take up this study with the aim of assessing its role in the progression of oral cancer and to correlate with various clinicopathological parameters, including habits such as smoking, Paan chewing, and alcoholism. MATERIALS AND METHODS: This observational study included surgical specimens from 10 apparently normal oral mucosa, 14 oral reactive lesions (ORL), 29 precancerous lesions and 43 oral cancers. The expression of Rb protein in tissue samples were evaluated by immunohistochemistry and correlated with clinicopathological data. The percentage and mean expression of Rb protein were statistically analyzed using Student's t-test and P < 0.05 was considered as statistically significant difference. RESULTS: The expression of Rb protein was found to increase from normal, ORL, precancerous lesions to cancers. A consistently high expression of Rb protein was seen in oral cancers, with an increase in well-differentiated and moderately differentiated tumors. Patients with combined habits of Paan chewing, smoking, and alcohol consumption had a higher expression compared with those without habits. CONCLUSION: Within the limitations of this study, it seems that overexpression of Rb protein noted in oral cancer, with an increase in well and moderately differentiated tumors suggest a possible role of Rb in differentiation. The high expression of Rb in patients with combined habits of Paan chewing, smoking and alcohol consumption indicates that Rb pathway may be altered in habit-related oral malignancies.

RESUMEN

The occurrence impacted teeth, single or multiple is very common. But, phenomenon of kissing molars is an extremely rare phenomenon. Mandibular third molars are the most common impacted teeth. Mandibular first or second molars does not share the same frequency of occurrence. But, there are rare cases in which the occlusal surfaces of impacted molars are united by the same follicular space and the roots point in the opposite direction, and are termed as kissing molars. Sometimes, these teeth will be associated with pathologies. This article reports a rare case of mandibular bilateral kissing molars.
