Two cases of pulmonary aspergilloma confirmed histologically are reported fron Nigeria; they constitute the first cases reported from this country. We advise that a high index of suspicion is necessary to detect more cases of pulmonary aspergilloma in Nigeria.

A case of idiopathic granulomatous orchitis in a 37-year-old Nigerian male is discussed. He presented with a history of pain and swelling of left testis of three weeks duration. A clinical impression of tumor of the testis was made followed by orchidectomy. A systemic review failed to show evidence of any infective proccess. Histology showed non-caseating tuberculoid granulomas replacing the seminiferous tubules.

A 10-year old Nigerian boy was admitted to hospital with a history of swelling in the neck, change of voice, dysphagia and dyspnoea of 2 weeks duration. He died on the 2nd day of admission. Autopsy revealed a thymoma infiltrating the thyroid, trachea and neck muscles and transforming the tracheal lumen into a slit like space. The condition is considered worthy of record on account of its rarity and of the short clinical course terminating in death. The authors feel that the most important factor in determining the prognosis of thymoma is the presence or absence of gross invasion.

Two cases of African histoplasmosis culturally proven to be caused by Histoplasma duboisii are reported from Eastern Nigeria. One case had cutaneous papules and subcutaneous nodules all over his body while the other patient had cutaneous papules, an abscess over the right scapula, enlarged cervical, axillary and inguinal lymph nodes, and possible lung involvement. None of the 35 soil samples collected from the environment of the two patients were found positive for H. duboisii. In the first case, most of the lesions cleared with Septrin given orally. The 2nd case responded very well to treatment with intravenous Amphotericin B except for persistance of swellings in the inguinal lymph nodes. The effect of intralesional injection of Amphotericin B on this residual lymph node infection is now being assessed.

Three cases of chromomycosis from Eastern Nigeria are described. The aetiological agent was identified as Phialophora pedrosoi. Amphotericin B cleared most of the lesions in one case while 5-fluorocytosine gave a complete cure in another case.

Pathological features of twenty-one cases of malignant melanoma studied in the University of Nigeria Teaching Hospital, Enugu during the period January, 1974 to December, 1975 are presented. Malignant melanoma accounted for 2.4% of all tumours and 4.5% of all malignant tumours, greatest age incidence being in the fifth to seventh decades. The male to female sex ratio was 2:1. 73.2% of cases were of the nodular variety. 81% melanomas occurred on the sole of feet validating the hypothesis that the pigmented skin in Africans is resistant to malignant melanoma. Melanoma in Nigerians would appear essentially to be arising from epidermal melanocytes and not from preexisting naevus cells. Hence we do not feel prophylactic removal of plantar moles as suggested by Onuigbo (1975) is desirable. Histologically, there was no clear association between the cell types and the kind of melanoma or invasion of the tumour. The difference in behaviour and natural history of malignant melanoma would appear to have a bearing on the local tissue and also general immune mechanisms of the host.

A case of teratoma of the thyroid gland in a 3-month-old child is reported. Present since birth, it presented as an acute respiratory emergency. This was relieved by surgical removal. The tumor showed normal thyroid tissue flanking the mass and at surgery was seen to derive its blood supply from the main thyroid vessels.

A case of cutaneous cladosporiosis due to Cladosporium trichoides, manifesting as a chronic discharging ulcer near the wrist of the left arm in a farmer with ascites and hypoalbuminaemia is described. The isolate was neurotropic in experimentally infected mice. The present case constitutes one of the few reports of cutaneous cladosporiosis.
