RESUMEN

Clostridioides (Clostridium) difficile infection (CDI) is the leading cause of infectious diarrhoea in hospitalised patients, representing a substantial economic burden driven mainly by increased length of hospital stay (LoS). Currently in Japan, limited evidence on CDI-associated excess LoS is available. We conducted a retrospective, matched-cohort study using a large, Japanese, hospital-based administrative database. CDI was defined as CDI treatment plus either CDI diagnosis or positive enzyme immunoassay result. Propensity score matching at the time of CDI or recurrent CDI (rCDI) onset was applied to adjust baseline confounding and immortal time bias. The analysis included 5 994 054 hospitalisation records during 2008-2017, of which 11 823 were identified as CDI and 1359 as rCDI. The median excess LoS attributable to CDI and rCDI was 3 days and 6.5 days, respectively. The excess mortality attributable to CDI was 6.9%; there was no excess mortality attributable to rCDI (-1.9%). The median difference in costs attributable to CDI and rCDI during the residual stay was JPY 130 296 (USD 1185) and JPY 81 054 (USD 737) per hospitalisation, respectively. By adjusting the biases, the burden of CDI in Japan was evaluated. The findings could support decision making and resource allocation for CDI management in Japanese hospitals.

RESUMEN

Measurements of dielectric constants and conductivities of invasive ductal carcinoma (IDC) tissues such as scirrhous carcinoma, solidtubular carcinoma and papillotubular carcinoma were made by use of microwave dielectric spectroscopy. Dielectric constants of inhomogeneous breast tumor tissues were analyzed by use of Bruggeman's effective medium approximation theory. It is found that the dielectric constants of scirrhous carcinoma, solidtubular carcinoma and papillotubular carcinoma of IDC tissues have correlation to the volume fractions of the cancer cells in the stroma tissues.

Sperm morphological examination, computer-assisted sperm analysis (CASA) and histopathologic examination of the testis and epididymis were performed for male rats treated orally with boric acid for 3 weeks at dosage levels of 50, 150 and 500 mg/kg/day, and treated males were mated with untreated females. None of the males treated with 500 mg/kg/day could impregnate untreated females. The fertility index showed a tendency to decrease in males treated with 150 mg/kg/day. At necropsy, the pre-implantation loss rate in females mated with males treated with 150 mg/kg/day was higher than the control values. Upon epididymal sperm analysis using the CASA system, all parameters including the number of sperm and sperm motions were found to be affected in males treated with 500 mg/kg/day, and the number of sperm, percent motile, velocities and amplitude of lateral head displacement (ALH) were affected in males treated with 150 mg/kg/day. Upon sperm morphological examination, head and tail abnormalities were observed in males treated with 150 and 500 mg/kg/day. In the histopathological examination, atrophy of the seminiferous tubules and multinucleated giant cells in the testes were observed in males treated with 500 mg/kg/day.

The aim of this study was to clarify whether histological parameters reflected tumor aggressiveness in patients with hepatocellular carcinoma (HCC). The tumor volume doubling times (TVDTs) of 21 HCCs, less than 3 cm in diameter at the start of the observation period, were calculated in 21 patients in whom the natural progression of the lesion was observed by ultrasonography. Paraffin-embedded sections were prepared from samples obtained by ultrasound-guided fine-needle liver biopsy at the end of the observation period. The histological parameters examined were the MIB-1 labeling index (LI), for which we performed immunohistochemical staining with the MIB-1 monoclonal antibody, using an antigen retrieval method; the nucleo-cytoplasmic (N/C ratio), cellularity, and the nuclear form factor (NFF), were calculated with an imaging analyzer. We performed multiple regression analysis for estimating the growth of small HCCs. With the N/C ratio (0.154 +/- 0.068; mean +/- SD), cellularity (453 +/- 21.8 cells/10(4) microm2), NFF (1.150 +/- 0.096), and degree of HCC differentiation as independent variables, only the MIB-1 LI (11.8 +/- 6.1%) showed a significant correlation with TVDT (207.5 +/- 162.6 days) (r = -0.658; P < 0.05). Compared to the conventional indices of histological atypism tested, i.e., N/C ratio, cellularity NFF, and degree of HCC differentiation, only MIB-1 LI was significantly correlated with small HCC growth rate. The MIB-1 LI may therefore be a simple and useful index of tumor aggressiveness.

To assess the optimal dosing period and parameters for measurement of effects on male fertility, Compound T was administered to male Jcl:Wistar rats at dosage levels of 0, 2, 10 and/or 50 mg/kg/day for 4 weeks (Experiment 1) or for 9 weeks (Experiment 2). Experiment 1: In the 50 mg/kg group, the ventral prostate weight was low when compared to the control value, and desquamation of round spermatids was observed in the testes. Experiment 2: When treated males were mated with untreated females after dosing for 9 weeks (the first mating), the fertility index was slightly lowered and preimplantation loss was significantly elevated in the 50 mg/kg group as compared to the control values. In this treatment group, serum testosterone level at 2.5 hours after dosing was significantly decreased after dosing for 12 weeks, and degeneration of spermatids/spermatocytes in the testis and epididymis was observed after dosing for 13 weeks. After a recovery period of for 6 weeks, remating resulted in copulatory and fertility indices and cesarean section data which were comparable in all groups. In conclusion, there were no differences in toxicity relevant to male fertility between 4 and 9 weeks of dosing, and it is considered that the observed changes resulted from decreased function of Sertoli cells due to depressed production/secretion of testosterone.

alpha-Glucosyltransferase was purified from Pseudomonas mesoacidophila MX-45. The molecular weight was estimated to be 63,000 by SDS-PAGE, and the isoelectric point was pI 5.4. For enzyme activity based on sucrose decomposition, the optimum pH and the optimum temperature were pH 5.8 and 40 degrees C, respectively. The ranges of stable pH and temperature were pH 5.1-6.7 and below 40 degrees C, respectively. The purified enzyme of MX-45 converted sucrose into trehalulose (1-O-alpha-D-glucopyranosyl- D-fructose) and isomaltulose (palatinose, 6-O-alpha-D-glucopyranosyl-D-fructose) simultaneously, and the ratio of trehalulose to isomaltulose increased at lower reaction temperatures. Therefore, optimum conditions for trehalulose production were pH 5.5-6.5 at 20 degrees C. The yield of trehalulose from sucrose (20-40% solution) was 91%. The Km for sucrose was 19.2 +/- 3.3 mM estimated by the Hanes-Woolf plot. Product inhibition was observed, and the product inhibition constant was 0.17 M. Hg2+, Fe3+, Cu2+, Mg2+, Ag+, Pb2+, glucono-1,5-lactone, and Tris(hydroxymethyl)aminomethane inhibited the reaction.

RESUMEN

We report a case with an adenocarcinoma mucocellulare and muconodulare of the esophagus which is limited to the lamina propria mucosa and the submucosal layer. The patient, a 74-year-old man had the chief complaints of hematemesis and melena. A superficial and protruding (0-I) type of lesion was found at the lower thoracic portion of the esophagus by X-ray and endoscopic examination. The surgically resected esophageal specimen showed 0-I and predominantly subepithelial (Sep) type. The tumor is presumed to have arisen from the cardiac glands in the lamina propria mucosa of the lowermost region of the esophagus.

The host response to natural infection with Edesonfilaria malayensis in six female adult cynomolgus monkeys (Macaca fascicularis) imported from Indonesia was examined retrospectively by histopathology and clinical investigations. Reduced values of hemoglobin and hematocrit, an increased number of eosinophils, an elevated level of total protein and a decrease in A/G ratio were noted in hematological and blood chemical examinations of the infected animals. The filarial worms inhabited subserosal connective tissues of the abdominal and the thoracic cavities but not the lumina of the body cavities. There was thickening of the connective tissues, hemorrhage and adhesion of the serosa in the site occupied by the worms and the cysts. Histopathologically, proliferation of the connective tissues and/or granulation tissues, infiltration of eosinophils, lymphocytes and other cells associated with inflammation and hemorrhage were observed. The worms occasionally caused mechanical damage in nearby tissues such as the pancreas and the iliopsoas muscle. In addition, splenic nodules were found in five of the six infected monkeys as a secondary lesion specifically related to the infection with Edesonfilaria malayensis.
