RESUMEN

Our department is committed to a process of continuous quality improvement focusing on delivering the best possible pharmaceutical care services. Three committees, each with representation from pharmacists and pharmacy technicians from all areas of the department, were convened in 1991 to 1992 to further identify areas for service enhancement and to plan for the future. Based on the recommendations of these committees, further expansion in ambulatory services is a priority. Other changes will include further automation of the drug-distribution system, examination of the role of the pharmacy technician, development of an automated patient-care system with direct physician order entry, computerized documentation of clinical interventions, and reaffirmation of the role of the pharmacist as the professional responsible for proper use of medications at UMHC.

RESUMEN

Thirty-seven of 137 patients had a "second-look" laparotomy in the course of their management of carcinoma of the ovary. Patients were stratified according to three indications: (1) evaluation of disease with intent of stopping therapy, (2) assessment of signs of recurrent or persistent disease with a view to debulking tumor mass and changing chemotherapy, and (3) further tumor resection following cis-platinum combination therapy and determination of further chemotherapeutic agents. "Second-look" laparotomy may be performed after a shorter time interval when combination therapy is given because of the dose-limiting side effects of some of these agents and a more aggressive surgical approach in debulking tumors. At the time of laparotomy, cytologic testing is performed on the peritoneal fluid, and only areas suspicious for malignancy are biopsied. Thirteen percent of patients with no evidence of disease at "second-look" laparotomy developed recurrent disease. Twenty-nine percent of patients classified as clinically free of disease had malignancy present at the time of operation. Continued routine use of "second-look" laparotomy after appropriate chemotherapy is recommended.

RESUMEN

The design and operating characteristics of a heart rate transmitter designed for a wild grizzly bear (Ursus arctos) study is described. The rigid capsule is durable and is easily implanted in the field. The transmitter can be regularly monitored at 4-5 km and has a life expectancy of about 1 year in bears which experience winter torpor, and thus reduced heart rate, for about 7 months in northern Alaska.

RESUMEN

A 54-year-old diabetic patient had unexplained fever and embolic occlusion of the splenic, right renal, right hypogastric, right superficial femoral, and left popliteal arteries. Aspergillus fumigatus was recovered from a femoral clot. An aortogram revealed a mycotic aneurysm of the thoracic aorta to be the source of the infected emboli. Surgical excision of the aneurysm and therapy with amphotericin B were unsuccessful.

RESUMEN

A buffer system composed of perchloric acid, citric acid and triethanolamine has been used for the successive determination of chloride, fluoride and sodium in a single sample of an orthophosphate mineral. The conventional sodium-sensitive glass electrode compares favourably with the more expensive Orion sodium-sensitive electrode.

RESUMEN

The chloride content of some inorganic orthophosphates and phosphate rock samples has been determined in a medium buffered to pH 2.5 with a mixture of perchloric acid, citric acid and trisodium citrate. The method has been compared with an earlier method which employed a mixture of perchloric acid, citric acid and triethanolamine. The present method is of similar reproducibility and does not suffer from decomposition of the reagents. The interference patterns of several metals are discussed.

RESUMEN

A method is described for the determination of fluoride in inorganic orthophosphates. Interference by complex-forming metal ions is avoided by the use of a citric acid-triethanolamine buffer, pH < 7.5.
