RESUMEN

OBJECTIVE: To study the influence of COVID-19, as well as various factors on the rate of deaths caused by suicide. MATERIAL AND METHODS: The statistical analysis of mortality according to the data of «Bureau of FME¼ in the period from 2019 to 2020 yrs. and the search of scientific publications over the past 10 years in PubMed database have been conducted. Articles describing the factors influencing suicidal deaths in different population groups were selected from the publications. RESULTS: Statistically significant features that determine the influence of quarantine measures in the period COVID-19 on the mental component of citizens and suicidal dynamics were considered and established.

We consider a two-dimensional system of elongated particles driven over a landscape containing randomly placed pinning sites. For varied pinning site density, external drive magnitude, and particle elongation, we find a wide variety of dynamic phases, including random structures, stripe or combed phases with nematic order, and clogged states. The different regimes can be identified by examining nematic ordering, cluster size, number of pinned particles, and transverse diffusion. In some regimes we find that the pinning can enhance the particle alignment, producing a nonmonotonic signature in the nematic ordering with a maximum at a particular combination of pinning density and drive. The optimal nematic occurs when a sufficient number of particles can be pinned, generating a local shear and leading to what we call a combing effect. At high drives, the combing effect is reduced when the number of pinned particles decreases. For stronger pinning, the particles form a heterogeneous clustered or clogged state that depins into a fluctuating state with high diffusion.

The purpose of the work is to study the current state of the performance of situational examinations in district and interdistrict departments of forensic medical examination (FME of the 1st level) with the formation of a unified methodological basis and the introduction of the developed integrated methodology for conducting forensic medical situational examinations in practice work of FME of the 1st level. In this study, the assessment of the current state of performance of situational examinations in FME of the 1st level was carried out on the example of the Tula Regional Bureau of Forensic Medical Examination. At the initial stage, the actual (both absolute and relative) volumes of the performance of situational examinations in the FME of the 1st level were established, with the identification of features of their annual dynamics. The necessity of analyzing certain methodological and epidemiological aspects of the performance of situational examinations is determined.

BACKGROUND: Numerous studies have been conducted by using sophisticated methodologies for the deduction of range in cases of firearm injuries. However, the stark dearth of literature for similar purpose by just simple analysis of blood traces on items of clothing is astonishing. PURPOSE: This study was done to determine the relationship between range of a rifled firearm and blood traces on the clothing of victims in firearm injuries. METHODOLOGY: Simulation experiments were performed by firing test shots in closed shooting range by using a Glock 17 pistol (9 mm luger) into pieces of clothing soaked with sheep blood. A total of 132 shots were fired. The morphologies and amount of blood spatter formed over a range varying from 5 cm to 100 cm (shooting angle-90°) were studied. RESULTS: An inverse relationship was found to exist between morphology of blood traces produced on the target and the distance of the shot fired. The misting and micro-blood spatters were recorded at a range of 5-60 cm. The distance from which shots were fired carried significant impact on the appearance and density of the spray and the blood spatter. CONCLUSIONS: The results provide a scientifically determined, statistically valid and simple equation formula in the most cost effective and easy manner for the deduction of range by mere examination of clothing in cases of firearm injuries. The requirements of the sacrifice of animals, and/or extrapolation of the results to humans by conducting experiments using hypothetical targets were also obviated in this study.

Pregnancies complicated by the placenta praevia are associated with an increased risk of massive obstetric bleeding and high rates of hysterectomy which are often caused by the placenta accreta. The aim of our study was to identify the risk factors for placenta praevia associated with PAS disorders and the efficacy of distal haemostasis during Cesarean delivery. METHODS: This was a cohort study carried out between 2014 and 2020 in 532 women with abnormal placental localization and attachment. The placental attachment spectrum (PAS) disorder diagnosis was confirmed during the surgery and by the histology results in 164/532 participants. Depending on the surgical approach during the Cesarean delivery, patients were divided into three groups. In Group 1 (n = 52), patients underwent bilateral uterine artery ligation. In Group 2 (n = 33), we used the combined compression haemostasis approach including the placement of tourniquets and insertion of an intrauterine balloon for controlled tamponade. In Group 3 (n = 79), we used the combination of surgical haemostasis with the controlled intrauterine tamponade using the vaginal and intrauterine Zhukovsky balloon. RESULTS: PAS was observed in 30.8% of the placenta praevia cases, and in 93.3% was associated with the presence of a uterine scar. Women with the placenta praevia and PAS had a significantly higher number of past deliveries (р = .001). According to the FIGO classification, 53.8% of women with placenta praevia observed during the Cesarean had РА1 and 46.2% PA2. With regards to the PAS disorders observed in 30.8% of patients, 38.4% had PAS3, 34.7% PAS4, 18.3% PAS5 and 8.5% PAS6. The histology analysis showed normal placental attachment in 42.9% of the total number of study participants, placenta accreta in 28.2%, placenta increta in 16.7%, and placenta percreta in 12.2%. In Group 1, we performed the resection of uterine wall with the attached portion of the placenta in 13.5% of women, in Group 2 in 30.3% women, and in Group 3 in 50.6% women. There was a significant 4.8-fold reduction in the number of hysterectomies in Group 3 versus Group 2 (р = .043) and a 4.4-fold reduction in Group 2 versus Group 1 (р = .003). In Group 2, the volume of blood loss was 1.3-fold lower and in Group 3 1.5-fold lower than in Group 1. Conclusion: The techniques of compression distal haemostasis evaluated in this study in women with PAS are efficacious in the reduction of adverse maternal outcomes and should be used more widely in clinical practice.

AIM: To identify risk factors and predictors of pregnancy loss and to compare the efficacy of Arabin's pessary with cervical cerclage in women at a high risk of pregnancy loss. MATERIALS AND METHODS: This was a two-center retrospective case-control study that included 240 women at a high risk of preterm delivery. Group I (n = 161) included women who underwent insertion of the Arabin's pessary between 14 and 24 weeks of pregnancy. Group II (n = 79) included women who had undergone circular cervical cerclage during the current pregnancy. All women included in the study received micronized vaginal progesterone at the dose of 200 mg/day until and including 34 weeks of gestation. RESULTS: Threatened pregnancy loss defined as spotting or vaginal bleeding in the first trimester was diagnosed in 29.8% (48/161) of patients in Group I versus 37.9% in Group II (p = .448). Postpartum bleeding occurred in 8.1% (13/161) in women in Group I versus 22.8% in Group II (χ2 = 6.500; p = .011). Our study showed that cervical cerclage was most suitable for patients with history of obstetric complications, cervical length <15 mm, and large isthmic uterine fibroids. The use of the Arabin's pessary reduced the rate of preterm births by 1.7-fold. A cluster analysis demonstrated that predictors of preterm birth in women with a high risk of pregnancy loss included: threatened pregnancy loss associated with chorionic/placental abruption, cervical incompetence, uterine fibroid growth to a large size, history of multiple spontaneous pregnancy losses, cervical tears during past labor, and gestational diabetes diagnosed for the first time during the current pregnancy. CONCLUSIONS: Women with a high risk of pregnancy loss treated with Arabin's pessary or cerclage plus vaginal progesterone had a term delivery rate of 70.4% (169/240). The combined strategy of pregnancy management allowed to markedly reduce the number of preterm births.

RESUMEN

An adaptive optical fiber sensor/demodulator of an optical phase modulation with a Sagnac interferometer configuration is reported. The dynamic population grating recorded in ytterbium-doped fiber (YDF) at a wavelength of 1064 nm enables adaptive properties of this configuration with a high-pass transfer function and with the cut-off frequency of about 260 Hz at ∼10mW cw recording power. A linear response with nearly 100% modulation depth is ensured by effective formation of the nonshifted phase dynamic grating with the amplitude one order of magnitude greater than can be expected from the saturation of the YDF absorption at the recording wavelength. This is associated with the photoinduced changes in the UV optical absorption of the YDF and enables minimal detected amplitude of the phase modulation ≈0.7∗10-7Hz in our experimental configuration. We believe that, in general, this mechanism of the phase grating formation can ensure the sensor resolution limited by the photonic noise of the utilized light power only.

The interaction of positronium (Ps) with molecular oxygen dissolved in liquids is experimentally investigated. Computer software has been developed for fitting the positron annihilation lifetime spectra in liquids using parameters with clear physical meaning: rate constants of the Ps chemical reactions, annihilation rate constants of the different positron states, probability of Ps formation in a quasi-free state, typical formation time of a Ps nanobubble. Such processing of the spectra allowed identification of the dominant interaction of the Ps atom with dissolved oxygen. It turns out to be mainly ortho-para-conversion (Ps → 1/4 p-Ps + 3/4 o-Ps), but not oxidation (Ps + O2→ e+ + O2-). Values of the reaction rate constants are obtained.

Application of the phase memory of acetylene (C2H2) vibrational-rotational transitions in the 1520-1540 nm wavelength range for a self-referencing homodyne detection of a sub-nanosecond optical phase modulation is reported. In the proposed configuration the collinearly propagating coherent dipole radiation of the excited molecule acts like a phase-locked reference wave (local oscillator) that enables transformation of the initial phase modulation into the intensity one. This technique does not need high light intensity and can operate in a linear range of the medium optical absorption. The linear optical phase demodulation (i.e., transformation of the phase-to-amplitude light modulation) is interpreted as an introduction of an additional phase shift to the carrier frequency component of the modulated signal in the maximum of the dispersion curve of a narrow optical absorption peak. It has been experimentally demonstrated with the bulk 10 cm long cuvette filled with low pressure (∼2 Torr) acetylene at room temperature. Effective demodulation of the milliwatt-scale incident laser wave of a single- and multi-mode structure is shown. As expected, the response to the fast (<1 ns) phase modulation was quadratic when the acetylene inhomogeneous Doppler-broadened (∼500 MHz) absorption line is excited in its center and was linearized by tuning at one of the absorption line sides. It is of a differentiating (high-pass) type with the cutoff frequency determined by the total spectral width of the utilized absorption line. Expected detection resolution is determined by the photon noise of the incident light intensity.

The aim of the present work was to investigate the effect of the ultrasonic treatment on the optical and cathodoluminescent properties of translucent ZrO2-8 mol.% Y2O3 (YSZ) ceramics obtained by conventional sintering of the pressed compacts. Treatment by intense ultrasound during dry pressing of the YSZ nanopowder leads to an increase in the relative density, a decrease in the pore size and an increase in the grain size of the sintered ceramics. It was shown that when the ultrasonic treatment is applied, the optical absorption cutoff wavelength of the sintered material is shifted to longer wavelengths, while the optical density of the material increases over the whole measurement spectrum. Samples subjected to ultrasonic treatment during pressing show higher intensity of luminescence than those obtained without the use of ultrasound, the shape of the luminescence spectra remaining unchanged. A correlation was obtained between the integral intensity of cathodoluminescence and the vacancy concentration in the sintered YSZ.

The aim of the present study was to compare the effect of sulfated fucopolysaccharides isolated from Fucus vesiculosus on cell lines HeLa G-63, Hep G2 and Chang liver. Native fucoidan F3 and two fractions (F3-0.5 and F3-1), obtained by anion-exchange chromatography, were analyzed using chemical methods and IR-spectroscopy. Their action on cells demonstrates that F3 and F3-1, characterized by higher content of sulfates, preferable location of sulfo-groups at C4 atom of fucose residue and low content of uronic acids, inhibit cell proliferation more efficiently. Human liver tumor cells Hep G2 appeared to be the most sensitive to fucoidan treatment whereas nonmalignant human cells Chang liver were the least sensitive.

X-ray beam stability is crucial for acquiring high-quality data at synchrotron beamline facilities. When the X-ray beam and defining apertures are of similar dimensions, small misalignments driven by position instabilities give rise to large intensity fluctuations. This problem is solved using extremum seeking feedback control (ESFC) for in situ vertical beam position stabilization. In this setup, the intensity spatial gradient required for ESFC is determined by phase comparison of intensity oscillations downstream from the sample with pre-existing vertical beam oscillations. This approach compensates for vertical position drift from all sources with position recovery times <6 s and intensity stability through a 5 µm aperture measured at 1.5% FWHM over a period of 8 hours.

OBJECTIVE: the aim of study was testing and introduction in clinical practice combination of EUS with sonoelastography, fine-needle biopsy and needle-based confocal laser endomicroscopy in advanced diagnostic of pancreatic disease. MATERIALS AND METHODS: in the period from February 2014 to December 2015, we accumulated experience of EUS-FNA in 72 patients mostly with tumor pathology of the pancreas. In 16 cases we used 19G, in 49-22G, and in 7 patients - 25G needles. In 5 patients we used COOK Echotip Procore needle. In 8 cases we performed confocal laser endomicroscopy through the 19G needle (nCLE) in 3 patients with cystic and in 5 - with solid tumors. In 20 patients, there we used.HITA-CHI-PENTAX sonoelastography was performed. RESULTS: There were no complications in EUS-FNA. In all cases we received pathology verification of disease. nCLE results always confirmed by morphological examination of material. In elastography examination normal pancreatic tissue had a equable green-yellow color, in chronic pancreatitis on the same background there were areas of blue seal parenchyma, in the cases pancreatic adenocarcinoma it was marked predominance of blue color scale, in neuroendocrine tumors - diffuse distribution of green and blue areas. Using nCLE in patients with adenocarcinoma revealed the destruction of certain glands, polymorphic nuclei of epithelial cells, with their enlargement, deformation, loss of polarity. Appliance of Procore needle has its technical features, but allowed us to obtain more material with less bloody - due to fewer number of passes. CONCLUSION: Thus, our study confirms the effectiveness of EUS-FNA with sonoelastography and nCLE. It defines objectives for improvement and expansion of the range for their clinical use.

This work aimed to reveal the statistically reliable clinical and laboratory markers, which are associated with fatal outcomes in patients with hemorrhagic shock of the III degree on the stage of hospital treatment. It was stated that decrease of body temperature (till 35,9 °C), pH of venous blood (till 7,19) and content of ionized calcium (till 0,32 mmole/l) in venous blood and increase of lactate content (up to 4,1 mmole/l) in venous blood and rise of activated partial thromboplastin time (up to 59 sec) are connected with fatal outcomes.

The aim of the research was to investigate cytotoxicity of fucoidans on mammals cells. Three different samples of fucoidans were isolated from mechanically grounded brown algae Laminaria digitata and Fucus ve- siculosus. The sample F2 that differed from the others by higher sulfatation level and suppression of HeLa G-63 line culture growth was taken for further study in cell lines HeLa G-63, ECV 304 and PC 12. We have shown that fucoidan preparation F2 inhibits proliferation and induces cell death in a dose- and time-dependent manner for all investigated cell lines. Neuroendocrine tumor rat cell line PC 12 appeared to be the most sensitive to fucoidan treatment whereas endothelial human cells ECV 304 were the least sensitive.

Carriage of polymorphic alleles of genes of cytokines-interleukines IL-1ß, IL-1RN, TNFα, IL-4 can be a specific feature of chronic periodontitis patients. Genetic tests can be used to predict the course of the disease at its early manifestations. Objective: To establish the relationship of clinical manifestations of periodontal disease, inflammatory cytokines gene polymorphism and systemic levels of cytokine production. Periodontal tissue assessment and cone-beam computed tomography (CBCT) were performed in 150 periodontitis patients. A molecular--genetic testing for the presence of polymorphic alleles of genes IL-1ß -511 C>T and +3953 C>T, IL-1RN (VNTR intron 2), IL-4 (VNTR intron 3), TNFα-308 G>A; content determined IL-1ß, TNFα, IL-4 in peripheral blood was carried out in 150 patients with periodontitis and 150 healthy donors. Based on the analysis of the speed and nature of the supporting bone resorption and clinical manifestations patients are divided in "aggressive", "moderately progressive" and "slowly progressive" periodontits course groups. Disease severity was associated with distribution of genotypes and alleles of polymorphic genes cytokine IL-1RN (VNTR intron 2), TNFα-308 G>A and IL-4 (VNTR intron 3); haplotype IL-1ß-511 TIL-1ß +3953 T/IL-1RN 2R. There was no statistically significant difference in systemic level of IL-1ß, TNFα and IL-4 between periodontitis groups but the donor level of cytokines was 2-4 times less.

It is known that the addition of methanol to the culture medium stimulates the photosynthetic productivity of some species of microalgae, but its influence on the biochemical composition of the biomass has not been investigated. The aim of the present work is to determine the effect of methanol (50 mM) on the content of free amino acids, soluble proteins and reduced nicotinamide coenzyme NAD(P)H in C. reinhartdii cells. It is shown that in case of illumination of C. reinhardtii methanol raises intracellular content of NAD(P)H four times more efficiently than in the darkness. Total content of free amino acids is increased and their ratio is changed. The concentration of glutamic acid, glutamine, alanine, serine and tyrosine also increases. The concentration of valine and methionine is reduced. Growth of culture with methanol is followed by an increase in the content of intracellular protein by 30% after 20 hours of cultivation. The obtained data indicate that methanol stimulates growth of C. reinhardtii, not only as a result of additional carbon utilization, but also due to improved nitrogen assimilation and the impact on the energy metabolism of cells.

RESUMEN

On the cells of neuroendocrine tumor of rats (line PC12) in culture was estimating of the governing mechanisms of the cytotoxicity of the oxidative stress and the role of the amyloids in increasing this stress. Using flowcytometric assessment of the cytotoxicity H2O2 and fragment beta-amyloid (Abeta) peptide (25-35) has been shown the dose-dependent increasing of the quote of the cells with DNA content < 2c. Isoeffective consentrations were 1 MM H2O2 and 5 MkappaM Abeta. The cytotoxicity H2O2 and A were accompanig with the increasing of the intracellular level of O2-. The treatment of the cells GSNO (donor of NO) and o-phenantrolin (chelators of Fe ions) significantly decreased the intracellular level of O2- as well as the cytotoxicity H2O2 and Abeta. Thus, in direct experiments has been shown of the part amyloids in the increasing of the oxidative stress and participation of the reactive oxide radicals in the cytotoxic effect of the Abeta. The addition argument which confirmed contribution of the oxidative stress in the cytotoxic effect of the Abeta was the similarity of the cellular response on the action of the oxidative agent - H2O2 and Abeta.

RESUMEN

The frequency of use of assist blood device as the "bridge" to the heart transplantation increased in last years. An assessment of results of first 7 implantations of assist circulation device using biventricular type "Excor" was made. The implantations were performed in Federal Almazov centre of the heart, blood and endocrinology. An observation period after implantation was since 11 till 301 days. The heart transplantation of 4 patients was carried out in different terms after implantation. One of the patients passed away on the fifth day due to the pulmonary embolism. Another patient died on the eleventh day after the implantation because of multiple organ failure against the background of severe chronic cardiac failure. The waiting list of heart transplantation includes 2 patients.

OBJECTIVE: to prospectively study and compare the capacities of magnetic resonance imaging (MRI) and ultrasound study (USS) in the preoperative assessment of the extent of endometrial cancer. SUBJECTS AND METHODS: The study covered 50 patients with FIGO stages IA-IIIA endometrial cancer. A week before surgery, all the patients underwent small pelvic MRI and USS. The results of MRI and USS were compared with the data of a postoperative histological study. RESULTS: The diagnostic value of MRI in preoperatively assessing the local extent of endometrial cancer was 82%, including its sensitivity, specificity, and accuracy; the prognostic value of a positive result and a negative one was as much as 94 and 56%, respectively. The accuracy, sensitivity, and specificity of USS were 70, 72, and 64%, respectively. Its prognostic value of a positive result and a negative one was 84 and 47%, respectively. CONCLUSION: In this study, MRI versus USS showed a higher diagnostic efficiency in the preoperative staging in patients with endometrial cancer. The former promotes the optimization of assessment of the local extent of the tumor, including the depth of myometrial invasion and the spread into the cervix uteri, which affects the algorithm and policy of treatment for endometrial cancer.
