RESUMEN

Psoriasis is a common dermatosis which affects the patient's skin and general well-being because of its link to diseases such as depression, kidney disease and metabolic syndrome. Pathogenesis remains unknown; however, genetic, environmental and immunological factors seem to play a role in the development of the disease. Due to a lack of complete understanding of the psoriasis pathology, effective treatment is yet to be developed. The kynurenine pathway is one of the ways amino acid tryptophan is metabolised. In comorbidities typical for psoriasis such as chronic kidney disease, depression and atherosclerotic alterations in the activation of the kynurenine pathway were observed, which were mainly characterised by higher activity compared to that in healthy individuals. However, the kynurenine pathway has not been thoroughly studied among patients with psoriasis even though increased levels of L-kynurenine, one of the enzymes in the kynurenine pathway, were found in psoriatic skin lesions. Given the unknown pathogenesis of the disease, this finding seems to be a potential new field of study and shows a possible link between psoriasis and its comorbidities that could also lead to novel effective treatment for this chronic condition.

This paper is dedicated to the study of clinical-psychopathological manifestations of co-dependence in the families of HIV-infected injecting drug users and to finding out the structure of these manifestations and how often they happen, as well as the evaluation of the role of hereditary factors in the formation of co-dependence among those who are married to HIV-infected injecting drug users. It has been established that the specificity of genealogies of patients with co-dependence is confirmed by the following factors: high frequency of mental and behavioral disorders caused by alcohol abuse, as well as mental and behavioral disorders caused by opiate abuse, assortative mating of co-dependent persons, accumulation of chemical dependencies and other dependencies like pathological gambling, personality disorders and affective disorders, comorbidity of pathologies in families of co-dependent persons and their spouses. It has also been established that in accordance with ICD-10 criteria the following clinical signs of co-dependence were manifested: mental and behavioral disorders caused by alcohol abuse (9.7%), mental and behavioral disorders caused by opiate abuse (15.0%), depressive episodes (14.1%), recurrent depressive disorders (34.5%), somatoform disorders (13.3%), hysterical personality disorders (32.7%). It was proved that the comorbidity of pathology is the key feature of clinical manifestations of co-dependence of persons who are married to HIV-infected injecting drug users. And it was determined that hereditary factors, the phenomenon of assortative mating and HIV-infection of a spouse as a main chronic stressful factor are involved in the formation of the clinical manifestations of co-dependency.

RESUMEN

The article deals with staging surgical service for patients with ulceronecrotic damages of the distal parts of lower extremities associated with diabetic foot. The authors grounded the deadlines of sanitive operations, performing after reconstructive vascular operations, on the basis of assessment of outflow tract according to Rutherford and index of TcPO2 increase. High efficiency of revascularization in order to maintenance of lower-extremity function in patients with complicated forms of diabetic foot is proved.

RESUMEN

A study is presented of 150 in-patients with paroxysmal tachycardias and tachyarrhythmias. Urgent management was carried out using a diagnostic algorithm designed by the authors that helped to establish early diagnosis using clinical adequate rational methods. Identification of clinico-pathogenetic form was realized in 95% of patients. Invasive examination was carried out in dubious cases. Transesophageal examination of the heart is of high value in emergency cardiology.

RESUMEN

The authors propose a new stepwise method of intensive therapy of atrial flutter (AF) based on atrial transesophageal pacing (ATEP) the efficacy of which was raised by previous digitalization. Patients of group 1 (n = 25) were scheduled to have ATEP, in its inefficacy ATEP was to be followed by antiarrhythmic drugs and on-demand cardioversion. In patients of group 2 (n = 27) ATEP was preceded by digitalization. ATEP and antiarrhythmic chemotherapy resulted in restoration of the sinus rhythm in 48% of the cases, cardioversion was performed in 11 patients of group 1. In group 2 normal rhythm was reestablished in 88% of the patients, 2 patients experienced cardioversion. The proposed variant of treatment is advocated as an alternative to cardioversion.

RESUMEN

Forty patients with paroxysmal tachycardias were studied. After intravenous bolus administration of ATP, 10-20 mg sinus rhythm restored in 24 patients with nodal reciprocal tachycardia or Wolff-Parkinson-White syndrome. In patients with ventricular tachycardia, antidromal paroxysmal tachycardia in the presence of the Wolff-Parkinson-White syndrome and atrial flutter, bolus administration of ATP made the diagnosis more precise. Adverse reactions were seen in 75% of patients. There was a case of cardiac asystole, 50 sec long, with symptoms of clinical death. The sick sinus syndrome and atrioventricular block are regarded as a contraindication for bolus administration of ATP due to binodal dysfunction induced by the agent.

RESUMEN

The study was undertaken to examine 105 patients with various circulatory diseases complicated by atrial fibrillation. Patients with refractory atrial fibrillation who had taken combined antiarrhythmic therapy were found to be more responsive to the combinations of cordarone + kinilentin (quinidine disulphate) and cordarone + ethacizin. The combinations of cordarone+digoxin and cordarone + finoptin were demonstrated to be less beneficial.

RESUMEN

A total of 70 patients aged 38-73 years who suffered from various circulatory disorders and refractory atrial fibrillation were examined. The majority of patients with refractory atrial fibrillation displayed normalized cardiac rhythm after the combined therapy with verapamil plus one of the fast sodium channel blockers (novocainamide, ethmozine, ethacizine, kinilentin). If the therapy failed, electric cardioversion was applied.

RESUMEN

Studied were 24 patients with the syndrome of premature excitation of the ventricles. In 18 of them transesophageal electrophysiological examination was carried out. Reciprocal paroxysmal tachycardia was revealed in 16 patients (orthodromic form--in 14, antidromic--in 2 patients). Cardiac fibrillation with a cardiac contraction rate of 320-340 per minute was noted in 2 patients. Difficulties are noted in the differential diagnosis of antidromic form with ventricular paroxysmal tachycardia and risk of development of ventricular fibrillation in auricular fibrillation. The authors propose a method of diagnosis of latent forms of the syndrome of premature excitation of the ventricles using short-term pharmacological block of atrioventricular conduction in intravenous administration of ATP.
