RESUMEN

The article considers the comparison of effectiveness of cultivation of 290 surgery samples from focus of destruction in patients with tubercular spondylitis in liquid medium Middlebrook 7119 with fluorescent detection of growth and two dense egg medium -Levenschtein-Yensen and Finn-II. The sensitivity of methods of inoculation in liquid medium Middlebrook 7H9 and in dense egg mediums has no difference (38.2% and 43.1%, p = 0.271). The duration of growth consisted 2 +/- 2.3 days for liquid mediums and 40.6 +/- 3.2 days for dense mediums. The method of pre-inoculation processing of surgery material. In case of inoculation of 440 samples in liquid mediums contamination consisted 3.9%. In case of inoculation of bone fragments of focus of destruction in liquid medium Middlebrook 7119 the culture was isolated in 43.5% of samples of patients with tubercular spondylitis.

RESUMEN

Drug resistance of Mycobacterium tuberculosis (MBT) to essential and reserve antituberculous drugs (ATD) was monitored in patients with tuberculosis of respiratory organs (ROT) and at extrapulmonary sites (EPT) in 2005-2007. The standard indirect absolute concentration technique was used to study the drug resistance of 578 MBT strains from pretreated patients with ROT and that of 159 MT strains from those with EPT. With all sites of tuberculosis, a poor trend continues to show an increase in the total frequency of MBT resistance (from 82.3 to 85.2% in ROT and from 43.7 to 59.3% in EPT) and its severer pattern due to MRD (from 69.5 to 78.0% in patients with ROT and from 20.8 to 35.5% in those with EPT). The rate of MTB resistance to 4 ATDs or more is 73.7 and 34.6% in patients with ROT and in those with ERT, respectively. The total resistance of MBT to 7-8 ATDs was revealed in every three patients with ROT (33.0%) and in 12.6% of those with EPT.

RESUMEN

The detection rate and clinical and diagnostic values of L-forms of pathogens were determined in patients with pulmonary and extrapulmonary tuberculosis. Simultaneous culturing the specimens for typical and L forms of Mycobacterium tuberculosis (MBT) increased the number of positive results by isolating only L-forms by 10.3% in patients with pulmonary tuberculosis and by 27.7% in those with extrapulmonary tuberculosis. No bacterial isolation in tests only for typical forms of MBT is shown not to be true and a purposeful search for L-forms of MBT enhances the efficiency of a bacteriological test. This is of great significance in confirming the specific nature of the disease, its progression, in choosing a treatment policy, in evaluating is efficiency, in defining prognosis, and in correcting preventive measures in the focus of tuberculous infection. With extrapulmonary tuberculosis, the tuberculous nature of isolated L-forms has been evidenced by the polymerase chain reaction.

Spoligotyping was used for genotyping of 238 M. tuberculosis cultures isolated from 302 patients with pulmonary tuberculosis living in the north-west of Russia, including those in Saint Petersburg, in 1998-2001, and the M. tuberculosis strains H37Rv, M. bovis, and M. bovic BCG and M. tuberculosis in the specimens painted for microscopy. The quality of spoligotyping was high and similar to that when DNA from the cultured mycobacteria and slide scrapes. The findings were compared with the data available in the International database. There was heterogenicity in the microbial population: the clinical strains showed 58 (RO-R57) types of profiles, of them 56% were revealed in single cases. The spoligotype RO(1; S1) was found in 153 (51%) patients. This spoligotype is predominant in the world and characteristic of polyresistant high-transmissive strains of the genetic family Beijing. The spoligotypes R24 (251), R14 (252), and R8 (253) first described by the authors are now encountered only in Russia. To extend the Russian database that contains M. tuberculosis spologotyping profiles from different regions will promote the improvement of an epidemiological surveillance system and tuberculosis control programmes not only within administrative territorial entities, but also within the whole country.

RESUMEN

In patients with early respiratory tuberculosis serum levels of specific antibodies were measured before and after Koch's test using an enzyme immunoassay (EIA). The latter was most informative in evaluating the baseline level of the antibodies to make a differential diagnosis of tuberculosis. When followed by EIA, the tuberculin provocative test enhances EIA sensitivity thus contributing to more effective detection of specific process of minor activity.
