RESUMEN

An analysis of 293 victims with cardiac and pericardium injuries was made. Cardiac complications could appear suddenly and be registered on any stage of treatment process. They differentiated by character and duration of wound process. Pericarditis took place in 288 patients. The syndrome of system inflammation reaction was noted in 47.9% of patients, a sepsis (sometimes severe sepsis) was in 14.3%. Complications in postoperative period were determined by posthypoxic and hemic hypoxia, coagulopathy. They were manifested by myocardial ischemia and thromboembolic complications. An acute myocardial infarction took place in 29 (9.8%) cases and rhythm and conductivity abnormalities were in 23 (7.8%) patients. The circulatory failure with clinical picture of pulmonary edema was developed in 12 (4.1%) cases. The lethality consisted of 44 (15%) patients.

RESUMEN

The authors made an analysis of variants of treatment strategy and its results in patients with different forms of erysipelas. The use of complex conservative and operative treatment allowed lethality to be reduced from 4 to 1.9%, in patients with severe course of the disease the frequency of complications being reduced to 10%.

RESUMEN

The authors analyze an experience with treatment of 2750 patients with different forms of nonlactational mastitis. Special attention is given to peculiarities of the clinical course of the disease, importance of early diagnosis of the inflammation in the mammary gland. Pathogenetical treatment of different forms of nonlactational mastitis is substantiated. Dissection of all pathogenetically altered tissues is thought to be necessary in operative treatment of patients with purulent forms with the following histological investigation.

RESUMEN

Milk ejection in lactating women was performed by breast pump with vacuum and tactile (touch-compression) components. It was found that milk flow rate changed in a pulsatile manner at about 1-2 min interval and peaks milk flow coincided with peaks intramammary. Switch off the tactile component increased the latency of milk ejection 1.5-2 fold and decreased total flow rate 2 fold. It was concluded that touch-compressions stimuli necessary for generation milk ejection reflex and milk ejection from the women mammary gland.

RESUMEN

An express evaluation of the functional state of neutrophil granulocytes by the indications of the lysosomal-cationic test are thought to be a necessary addition to routine clinico-laboratory methods of assessment of the wound process in patient with the suppurating epithelial coccygeal canal and acute paraproctitis. Use of cytochemical indices allow the antibacterial therapy to be optimized in patients with suppurating epithelial coccygeal canal and thus substantially decrease the amount of complications in the postoperative period.

RESUMEN

The application of leukocyte mass against the background of altered reactivity of the organism was followed by positive changes of clinical and immunological data by as early as the 2-3d days of the postoperative period. The level of antistaphylococcal immunity was preserved after completion of the course of leukotransfusions. It results in complete arrest of the inflammation in the mammary gland, excludes the development of recurrences and various pyo-septic complications. Leukotransfusion is not only a method of substitution therapy but also an effective immune-correcting factor.

RESUMEN

The authors consider that quantitative and qualitative characteristics of phagocytosis can be given with the help of lysosomal-cationic test (LCT) at different periods of acute purulent lactation mastitis. LCT gives prognosis of the course of the disease and a reliable estimation of the treatment efficiency.

RESUMEN

In patients with a severe course of suppurative lactation mastitis there develops a relative deficiency in the cellular and humoral links of immunity. The compensatory activation of factors of nonspecific antiinflammatory resistance of the organism fails to exert a substantial positive effect on the development of the destructive process in these patients. On the basis of the data obtained, in order to improve results of the surgical treatment of patients with suppurative lactation mastitis means of the cellular anti-infection therapy, transfusions of donor leukocyte masses in particular, must be included in the complex of curative measures in the postoperative period.
