RESUMEN
Diagnostic electrocardiostimulation (ECS) of the right atrium with evaluation of the functional state of the heart conduction system and coronary reserve was carried out in 177 patients. The level of blood catecholamines, activity of cholinesterase and content of destroyed acetylcholine, cortisol, free fatty acids, glucose, insulin, thyroxine, triiodothyronine, renin, testosterone, calcium was determined before and at the height of diagnostic ECS. In cases with a tendency to bradycardia one could note compensatory tension of the sympathetic-adrenal system and mechanisms of general adaptation. In organic weakness of the sinus node with stable bradysystole there were signs of exhaustion of the adrenal reserves of catecholamines and cortisol manifested in paradoxic reduction of them in the blood in response to frequent ECS and corresponding changes of energy provision.

Analysis of tolerance to physical load and blood lipid composition in 28 patients with the first clinical symptoms of coronary insufficiency and in 93 patients with cardialgia showed that diminished load tolerance with impaired induces of lipid metabolism is encountered more frequently (77.5%) than isolated hyperlipoproteinemia (15.2%) and low load tolerance without hyperlipoproteinemia (7.3%). In patients with low tolerance to load and hyperlipoproteinemia, the decrease of the threshold power and volume of work corresponds to the degree of disturbances in the blood lipid composition. The combination of cardialgia, hyperlipoproteinemia, and low load tolerance, manifested by tachycardia, may indicate that this is a case with ischemic heart disease.

Most of the 104 patients with ischaemic heart disease had distinct functional and structural microcirculation disorders. The slugging syndrome was revealed in 98% of the cases. In 56% of the patients microthrombus formation was noted in the microcirculatory bed, as well as a reduction of the perfusion capillary blood flow and an increase of the shunting flow, the half-resorption of sodium-131I from the skin and muscle depot being decreased. A clear relationship is noticed between the intravascular microcirculatory disorders, the functional state of the erythrocytes and platelets, the increased coagulation potential of the blood, the appearance of components that indicate the possibility of a diffuse intravascular blood coagulation (fibrinogen B, products of fibrinogen and fibrin decomposition), the changes in lipid metabolism. Stasis of the blood flow with distinct signs of the slugging syndrome, microthrombosis, activation of the kallikrein-kinin system and increasing shunting flow were revealed in patient with a severe course of the disease. Criteria for a quantitative assessment of the noted changes were developed.
