RESUMEN

Introduction: Authors report a rare case report about split hand and foot malformation (SHFM) also sometimes referred to as ectrodactyly. Case Report: The patient with hand and foot malformations presented to casualty. A 60-year-old male was brought with alleged history of road traffic accident with tenderness and deformity in left thigh. On further physical examination, a malformation was present in bilateral feet and right hand. Plain radiographs were taken after emergency primary management which revealed a fracture of shaft of femur of the left side and absence of 2nd and 3rd phalanges in bilateral feet and lobster claw like malformation in the right hand. The patient was further investigated and operated with femur interlocking nail and later discharged under stable condition. Screening for other congenital defects was done. Conclusion: Patients with SHFM should undergo screening for other congenital anomalies. Electrocardiogram, 2D ECHO, chest radiograph, and ultrasonography abdomen should be done. Genetic analysis ideally should be done to identify mutations involved. Surgical intervention is only required when patient demands improved function of limb.

RESUMEN

The availability of freshwater is limited for agriculture systems across the globe. A fast-growing population demands need to enhance the food grain production from a limited natural resources. Therefore, researchers and policymakers have been emphasized on the production potential of agricultural crops in a sustainable manner. On the challenging side, freshwater bodies are shrinking with the pace of time further limiting crop production. Poor-quality water may be a good alternative for fresh water in water scarce areas. It should not contain toxic pollutants beyond certain critical levels. Unfortunately, such critical limits for different pollutants as well as permissible quality parameters for different wastewater types are lacking or poorly addressed. Marginal quality water and industrial effluent used in crop production should be treated prior to application in crop field. Hence, safe reuse of wastewater for cultivation of food material is necessary to fulfil the demands of growing population across the globe in the changing scenario of climate.

The low T3 syndrome, the most common type of Sick Euthyroid syndrome, once believed to be a beneficial adaptive mechanism under conditions of stress, has emerged as a strong prognostic determinant in chronic systolic heart failure. Sick Euthyroid Syndrome is frequently observed in Chronic Heart Failure, Acute Myocardial Infarction. Acute myocardial infarction (AMI) may be associated with a number of endocrine alterations, including those of the SES which reflect the acute hormone response to stress and trauma. It is known from several studies that several cytokines can be found elevated in patients with cardiac ischemia or AMI. From in vitro studies it is of particular interest that ischemic myocytes produce cytokines such as interleukin-6 (IL-6) and its synthesis is accelerated by reperfusion. Interleukin-6 seemed to be an important cytokine produced by the injured myocytes in patients with AMI, and strong negative correlation between serum IL-6 concentration and left ventricular ejection fraction (LVEF) has been demonstrated. Similar observations have been made by studying tumor necrosis factor-a (TNF-a), IL-1a and soluble IL-2 receptor (sIL-2-R) which were found to be significantly elevated in AMI, with the highest levels noted in the most severe and complicated cases of myocardial infarction. MATERIAL: Study design : hospital based analytical cross-sectional study. MATERIALS: serial ECGs, Thyroid profile (FT3, FT4, TSH, rT3) and echocardiography. The study group included 100 patients who were admitted in ward/ICU with the diagnosis of myocardial infarction. INCLUSION CRITERIA: All patient with age 18yrs or above. History of chest pain with ECG changes and cardiac biomarkers of myocardial infarction. EXCLUSION CRITERIA: Patient below 18 yrs. Known case of hypothyroidism/hyperthyroidism Known case of malignancy Patients who have reached iodinated contrast in past one week. OBSERVATION: Out of 100 patients included with myocardial infarction, 27 patients had sick euthyroid state. Seven patients died, five with SES and two with normal thyroid profile. Mean value of fT3 was 2.37pg/ml for the patients who improved and 1.61pg/ml for the patient who expired, indicating statistical significance. Mean fT4 and TSH was not significant across those who improved or expired. However high rT3 value was associated with the worst outcome. CONCLUSION: Prevalence of SES is commom in patient with ACS. SES is a strong prognostic indicationin ACS. It is frequently observed in chronic heart failure, acte MI and is related to increased mortality.

The aquatic plant, Canna indica L. (Indian shoot) of Cannaceae family was investigated to assess cadmium scavenging potential at 5, 10, 25, 50, 100 and 150 mg Cd L-1 exposers. The results showed that Canna has considerable potential of cadmium accumulation, which was up to 58.69 and 10.13 mg Cd kg-1 dry weight in root and shoot of Canna, respectively. The effects of different cadmium levels on biomass production of plant tissues were significantly (p = 0.05) showed negative relation due to cadmium toxicity. The root concentration factor was higher than the bioconcentration factor which indicated the lower translocation factor of Canna. Considering the high root concentration factor, average bioconcentration factor, rapid growth and optimum adaptive properties up to 100 mg Cd L-1 level, the Canna could be employed as an eco-friendly and efficient aquatic plant for cadmium scavenging. This study plays a potential role in remediation of cadmium contaminated wastewater.
