RESUMEN

This study was performed to address the outcomes of patients treated with onlay grafts from the iliac crest to augment the deficient jaw. The results of 173 consecutive patients who underwent bone grafting prior to implant surgery are presented. The grafts were taken from the anterior iliac crest to repair alveolar bone deficiencies that were too large to be corrected using intraoral bone grafts. Three months postoperatively, 869 implants were placed into 190 onlay grafts (167 grafts in the maxilla, 23 in the mandible). The follow-up ranged from 3 months to 23 years post implantation. All patients received a fixed prosthesis. Parameters examined included healing of the donor site and bone grafts, implant survival, peri-implant condition, and donor site morbidity. The overall survival rate for all implants was calculated to be 95%±2.7% according to Kaplan-Meier analysis. The implant survival rate compares favourably with those reported in studies using intraoral and extraoral bone.

A total of 2,261 2-stage implants was placed in 467 patients in combination with angled abutments ranging from 0 to 45 degrees. These were observed over a period of up to 96 months, with a mean observation time of 28.8 months. Single and multiple teeth were replaced and restored using angled abutments. For patients who contributed multiple survival data, the data were considered dependent. Therefore, a mean survival estimation was performed. With a certainty of 95%, an estimated mean survival rate better than 98.6% after a 5-year observation period was calculated. The statistical comparison of 2 independent, randomized implant groups (with abutments angled between 0 and 15 degrees and between 20 and 45 degrees) by means of a log-rank test showed a probability of 0.84 (P value) that the survival functions are the same for both groups. Good esthetic and functional outcomes were observed.

RESUMEN

This clinical report presents a means of retaining implant supported superstructures using lateral fixation screws (Novadent). 244 lateral fixation screws have been used for the retention of a variety of restorations including single teeth, short span and full arch bridgework as well as overdenture bars. Over a period of observation of 4 years, the authors have found the restorations to have effective retention, ease of retrievability, good aesthetics and occlusal contours.

RESUMEN

It is the purpose of this paper to present the preliminary results of 370 consecutively placed Osieo-Ti implants. Two stage screw form endosseous implants ranging in length from 7mm to 20mm in one mm increments were used. Four diameters of implant 2.75mm, 3.00mm, 3.75mm and 4.50mm were used to engage the maximum available width of bone. The larger, 3.75mm and 4.50mm diameter, implants were made from commercially pure titanium and the two smaller diameter, 2.75mm and 3.00mm, implants were manufactured from titanium alloy. The overall success rate for 370 exposed functioning Osteo-Ti implants was 98.6%.

RESUMEN

Although the permucosal site of implants can be made to coincide with tooth position, the problem of aligning multiple abutments remains. To use the maximum amount of available bone, an implant must often be placed at an angle to the long axis of the prosthetic replacement tooth, and it also can deviate mesiodistally. This paper discusses a technique using trial abutments to create parallelism while ensuring that each abutment is ideally positioned with respect to the final restoration. The procedure also encourages patient acceptance by transferring the appearance created by a preliminary diagnostic waxup through to the finished prosthesis.

RESUMEN

Restoring the maxillary arch by using a suprastructure retained by a Konus crown means that the patient can remove the crown for oral hygiene. This construction enables large embrasure spaces to be avoided without compromising phonetics, that is, without adversely affecting the patient's speech. It also enables the dental surgeon to monitor the tooth and implant abutments and the soft tissues around them. In this article we demonstrate successful restoration of the maxillary arch by the use of Konus crowns on a combination of implants and natural teeth.
