RESUMEN

The purpose of this pilot study was to determine the efficacy of using a less costly, low-bandwidth telemedicine system to evaluate occupational therapy clients in rural areas. Four residents (aged 63 +/- 10.2 years) from rural Tillery, North Carolina, were evaluated via either The Kohlman Evaluation of Living Skills or The Canadian Occupational Performance Measure. An occupational therapist in Tillery administered one of these tools, while a second occupational therapist at East Carolina University simultaneously scored the same tool by telemedicine link. Comparison of their responses revealed scoring differences in only one of four administered evaluations. Video images were insufficient for visualization of finer movements, but audio quality was excellent. The authors conclude that select occupational therapy evaluation data can be accurately transmitted and properly scored using low-bandwidth telemedicine systems.

OBJECTIVE: To determine the effectiveness of a home exercise program using transtelephonic exercise monitoring (TEM). DESIGN: Prospective, two-group experimental, random assignment. SETTING: Urban centered hospital and surrounding community. SUBJECTS: Twenty cardiac rehabilitation patients entering a Phase II cardiac rehabilitation program. OUTCOME MEASURES: Maximal oxygen consumption, blood pressure, pressure-rate product, workload. INTERVENTION: Twenty male cardiac patients were randomly enrolled in either a 12-week home- or hospital-based exercise program. Maximal exercise tolerance tests were conducted, before and after exercise intervention, on a computer-driven bicycle ergometer. Subjects trained 3 days per week for 12 weeks on a bicycle ergometer for a maximum of 35 minutes at 75% of maximum heart rate reserve or functional capacity. RESULTS: Posttraining results showed significant improvement in cardiac function for both groups. Two patients in the TEM group developed new arrhythmias while exercising that required medication changes; however, no medical emergencies arose in either exercise group. Independent Student t test showed no significant difference between groups before or after training. CONCLUSIONS: We conclude that TEM is an effective alternative for the rehabilitation of patients who are unable to return to a hospital-based program.

The present study was undertaken to determine the effect of Phase I exercise therapy on lean body mass loss in recovering coronary artery bypass graft (CABG) surgery patients. Thirty male CABG surgery patients volunteered for the study. Subjects were randomly divided into two groups with one group receiving Phase I exercise therapy. The other group was allowed to participate in ward activities and ad libitum walking. Select measures of lean body mass change, including urinary urea nitrogen, blood urea nitrogen, urinary creatinine, serum creatinine and nitrogen balance, were obtained during six days of hospitalization. Results reveal no statistical difference (p greater than 0.05) between those receiving and those not receiving Phase I exercise therapy relative to these measures. We conclude that the effects of exercise which ameliorate lean body mass loss may be realized by insuring even modest ambulation as an adjunct to frequent assumption of the upright position.

RESUMEN

The rationale for inpatient (phase 1) exercise therapy after coronary artery bypass graft (CABG) surgery has been generalized from studies after myocardial infarction (MI) and bed rest. However, patients after CABG surgery are unlike patients after MI with regard to surgical trauma restoration of coronary circulation, and length of hospital stay. Most phase 1 exercise routines include a variety of range of motion (ROM) exercises, many of which focus on shoulder joint mobility. The purpose of our study was to examine the effect of shoulder ROM exercise therapy on inpatients after CABG surgery, because data in this area are limited. The results indicate that ROM exercises do not significantly (p greater than 0.05) ameliorate the early ROM loss associated with surgery. We conclude that the loss of shoulder ROM observed after surgery is a function of the surgical procedure and not lack of ROM challenge.

In brief: Little is known about the nature, location, and mechanism of injuries among basketball officials. The purpose of the present study was to develop an injury profile for this group. A total of 145 high school basketball officials were surveyed by means of a questionnaire. Sixty-one officials (42.1%) reported 71 injuries, 59 of which (83.1%) occurred in the lower extremity. Most injuries were related to change of direction or speed while running, a contrast to football, in which more injuries to officials are from collisions with players. Overall, basketball officials seem to have a lower injury rate than football officials.

To study the efficacy of colon cleansing regimens, 500 hospital outpatients were randomly assigned to one of four regimens. Two featured castor oil followed by phosphate enema and two used magnesium citrate and bisacodyl. For the 349 patients who completed all components of the regimens, quality of the subsequent barium enema examination was scored by three radiologists in a blind manner. Regimens featuring magnesium citrate and bisacodyl were found to be significantly superior to those featuring castor oil. Calcium bisdiocytl sulfosuccinate did not improve the quality of colon cleansing but appeared to decrease the incidence of abdominal cramping in patients receiving magnesium citrate and bisacodyl. It is hoped that the investigative approach used in this study may serve as a model for future testing of colon cleansing regimens.
