RESUMEN

Bilateral congenital cholesteatomas are rare. We present the 8th and 9th case of this entity in the literature. One of these cases is especially unusual. In that patient an ear that had been documented as normal, both radiographically and clinically, was found to have a large cholesteatoma 18 months after the contralateral ear had been treated for the same disease. The theories of the pathogenesis of this entity are also reviewed. Our cases support several of these theories. In fact, in one of the patients, it is possible that a different etiology was involved in each ear. Incidental note is made of the fact that three of the nine reported cases of bilateral congenital cholesteatoma have occurred in the same geographic locale.

RESUMEN

Dentigerous cysts can develop from unerupted primary, permanent, or rarely supernumerary teeth. When a supernumerary tooth is in the maxilla, near the midline, it is called a mesiodente. Such a mesiodente lies in the palate and a cyst developing about this tooth can extend directly up into the nasal fossae. More commonly a dentigerous cyst arising from unerupted maxillary teeth occurs about the molar, premolar, or canine teeth and the cyst extends up into the maxillary sinus. The CT and MR findings in a rare case of a dentigerous cyst of a mesiodente are presented.

RESUMEN

The reconstruction of circumferential and noncircumferential defects of the pharynx and cervical esophagus remains a challenging problem. A variety of different techniques are presently in use for this reconstruction. Free jejunal autografts were the first tissue to be transferred to the head and neck using microvascular techniques. A review of the literature on free jejunal autografts provides a collective experience that permits evaluation of the success rates and complications of the procedure. The results of this review reveal that this method of reconstruction is highly successful in restoring various ablative defects with an acceptable morbidity.

RESUMEN

Phonatory airflow was recorded in 150 patients with various laryngeal diseases and in 60 persons with normal voices. All subjects produced several sustained vowels at different loudness levels. Disturbances of voice due to laryngeal disease may be manifested as variations of mean flow (DC), alternating flow (AC), or as a variation in the amplitude of the frequency components of the airflow signal (frequency spectra). The quantification of airflow characteristics is important if the clinician is to have a better understanding of laryngeal disease. Furthermore, preoperative and postoperative analysis can serve to document therapeutic effectiveness.
