RESUMEN

OBJECTIVE: Endodontic treatment generally results in a wide range of postoperative pain intensity. The aim of this study was to investigate the effect of preoperative acupuncture on postoperative pain in molar teeth with symptomatic apical periodontitis. METHOD AND MATERIALS: Thirty patients having symptomatic apical periodontitis with a preoperative and percussion pain of visual analog scale (VAS) more than 60 were included in this study. The patients were randomly distributed into two groups; G1, real acupuncture; and G2, placebo (mock acupuncture). After 15 minutes of application, root canal treatment was performed. A logistic regression analysis was used to determine the variable(s) (group, age, gender, tooth number, preoperative pain, preoperative percussion pain, and radiographic status) that controls the postoperative pain significantly. Chi-square, Mann Whitney U, and independent t tests were performed to analyze the data, and the level of significance was set at .05 (P = .05). RESULTS: A regression analysis demonstrated that the group variable had the most significant effect on postoperative pain at day 1 (P = .003). Results showed that acupuncture reduced the preoperative and percussion pain levels significantly more than placebo group at all day intervals (P < .05). For postoperative pain at the 7-day follow-up, the pain ranged from "mild" to "no pain" in G1, compared with "moderate" to "minimal" in G2. Only one patient required postoperative analgesics in the acupuncture group compared to eight patients for placebo. CONCLUSIONS: Preoperative acupuncture can be beneficial in reducing postoperative pain in teeth with symptomatic apical periodontitis.

OBJECTIVE: The objective of this study was to evaluate the effect of various occlusal reduction levels on the postoperative pain in teeth diagnosed as symptomatic apical periodontitis by using a computerized analysis system. MATERIALS AND METHODS: Thirty-nine molar teeth with symptomatic apical periodontitis were included. After the root canal preparation and filling was performed, the subjects were divided into three groups according to the level of occlusal reduction determined by using T-Scan (T-Scan III for Windows, Tekscan Inc., South Boston, MA) as follows: (1) no reduction (control), (2) semi-reduction, and (3) full-reduction groups. Preoperative and postoperative pain levels using visual analogue scale, percussion pain, and analgesic intake were recorded for each subject. The data were evaluated statistically using two-way analysis of variance and Holm-Sidak post hoc tests at a 95 % confidence level (P = 0.05). RESULTS: There was no significant difference between the groups in terms of occlusal reduction (P > 0.005). CONCLUSION: Within the limitation of the present study, there were no significant differences among the no reduction, semi-reduction, and full-reduction groups at 1, 3, 5, and 7 days in terms of postoperative pain levels. CLINICAL RELEVANCE: Occlusal reduction, determined using a computerized analysis system, did not influence the postoperative pain in teeth diagnosed as symptomatic apical periodontitis.

RESUMEN

The aim of this study was to make a comparison between the cyclic fatigue (CF) resistance of F360, twisted files (TF), FlexMaster (FM) and RaCE instruments with 4% taper. A total of 40 instruments were evaluated 8 mm from the tip. A stainless steel block with a simulated canal of 1.5 mm diameter, and a 60° angle of curvature was tested using CF testing. One-way ANOVA and posthoc Tukey's test (P < 0.05) were used. The F 360 files showed the highest CF resistance while the TF files had greater CF resistance than the FM and RaCE (P < 0.05). There was no significant difference between the FM and RaCE (P > 0.05). F360 instruments with a double S cross-section had the highest CF resistance among the group. The TF led the NiTi rotary files to be more resistant to fatigue than the FM and RaCE instruments.
