RESUMEN

Given the myriad revenue and cost pressures faced by today's organizational providers, it behooves healthcare financial managers to identify opportunities to improve the terms of their managed care agreements when renewal time is near. Selection of a contract renegotiation strategy should be preceded by a focused analysis of situational variables and followed by conscientious performance monitoring. Through careful consideration of the compatibility to their goals, core values, and operational capabilities, healthcare providers and managed care organizations can craft agreements with the potential for sustaining meaningful long-term partnerships.

RESUMEN

The authors investigate the influence of various psychographic characteristics in distinguishing between those elderly patients who complain about dissatisfying experiences with health care providers and those who do not. Discriminant analysis results suggest that patients who are low in trust in their physicians and who are younger in terms of cognitive age are more likely to complain than are patients who are high in trust and older in terms of cognitive age. In light of these findings, the authors propose a number of managerially-relevant courses of action for health care providers.

Understanding the evaluative criteria used to select a health plan is central to effective marketing of an HMO. The determinant criteria that guide the reenrollment decision are shown to differ from those that drive initial enrollment. The authors' findings suggest several operational and strategic policy implications for HMO management.

RESUMEN

It is generally assumed that conventional strategic planning techniques can be applied to all complex organizations. However, the approach used to organize for the strategic planning function may vary to accommodate institutional differences. While health administration literature addresses the issue of strategic planning, there is little research to validate prescriptive theory, particularly for smaller and rural hospitals. To shrink the "research gap," a survey questionnaire developed in cooperation with the Hospital Association of Pennsylvania was mailed to 101 hospitals registered by the American Hospital Association; of these, 66 usable responses were received. The scope of the study was limited to smaller (100 to 299 beds) not-for-profit community hospitals in Pennsylvania. The results discussed in this article focus on the creation of an appropriate organizational framework to support strategic planning efforts. The findings shed light on the proper role for board members, staff specialists, and outside consultants as participants in the planning process.

RESUMEN

The first reported cases of preoperatively embolized hemangiopericytomas are presented. Both lesions presented in the retroperitoneum where most lesions are now considered to be malignant. In the past, the highly vascular nature of these tumors has made resection in these areas difficult. Since the angiographic picture of hemangiopericytomas is now thought to be specific, it became feasible to add preoperative embolization to the overall management of these cases. In the first case, the diagnosis had been established 15 years previously. When first seen at Thomas Jefferson University Hospital, extensive bone destruction of the sacrum and lumbar vertebrae were present. Preoperative Gelfoam embolization aided in the palliative debulking of the tumor at operation. With this experience, preoperative embolization became part of the management in the second case and aided in the complete surgical removal of the tumor. Radiation therapy in the dosage of 5000 rad was given postoperatively in this case and should also be part of the treatment plan for these lesions.
