Advanced lymphedema is associated with a number of adverse skin changes including color, thickening of the epidermis, dryness, and hyperkeratosis. These changes are related to prolonged lymph stasis and contribute to an increased risk of infection. Similarly, lipedema is associated with skin thickening and appearance of nodular adipose deposition. Skin care is essential in both conditions. We examined whether inclusion of targeted skin products for 2 weeks to an established pre-surgical conservative treatment program was associated with beneficial effects on the skin condition in 150 patients with lymphedema and lipedema. Patients were randomly assigned to control or one of two treatment groups. All three groups (and for both lymphedema and lipedema) demonstrated a significant reduction in softness. Dimpling/ redness was significantly reduced in the targeted skin product groups for both patients with lymphedema or lipedema. Only patients with lipedema demonstrated a significant reduction in dryness/ hyperkeratosis following targeted skin product treatment. This study demonstrates that short-term use of targeted skin products in both patients with lymphedema and lipedema can be of benefit and further studies are needed to replicate these results and explore possible mechanisms.

RESUMEN

Experimental measurements of the SOL power decay length (λ(q)) estimated from analysis of fully attached divertor heat load profiles from two tokamaks, JET and ASDEX Upgrade, are presented. Data was measured by means of infrared thermography. An empirical scaling reveals parametric dependency λ(q) in mm = 0.73B(T)(-0.78)q(cyl)(1.2)P(SOL)(0.1)R(geo)(0), where B(T)(T) describes the toroidal magnetic field, q(cyl) the cylindrical safety factor, P(SOL)(MW) the power crossing the separatrix and R(geo)(m) the major radius of the device. A comparison of these measurements to a heuristic particle drift-based model shows satisfactory agreement in both absolute magnitude and scaling. Extrapolation to ITER gives λ(q) ≃ 1 mm.

A multiwire proportional x-ray (MPX) detector is used on the TCV tokamak (Tokamak à configuration variable) as a high spatial and temporal resolution soft x-ray emissivity imaging diagnostic. The MPX system consists of 64 vertically viewing channels and has been designed to complement the existing TCV soft x-ray tomography system by enhancing the spatial resolution. The MPX detector is suitable for the measurement of fast and localized phenomena and can be used, for instance, for the observation of magnetohydrodynamic activity, for the characterization of transport barriers or for an improved determination of the electron cyclotron heating power deposition profile. The MPX detector operates in continuous-current mode and measures the plasma soft x-ray emission in the 3-30 keV range with a radial resolution of about 5 mm-1% of plasma diameter--and a frequency bandwidth of 50 kHz. A detailed description of the MPX detector construction and the principle of its operation are given. The properties of the detector in photon-counting and continuous-current operation modes are studied. The implementation of the system on TCV and experimental results illustrating the potential of the diagnostic are also presented.

RESUMEN

Bulk plasma toroidal rotation is observed to invert spontaneously from counter to cocurrent direction in TCV (Tokamak à Configuration Variable) Ohmically heated discharges, in low confinement mode, without momentum input. The inversion occurs in high current discharges, when the plasma electron density exceeds a well-defined threshold. The transition between the two rotational regimes has been studied by means of density ramps. The results provide evidence of a change of the balance of nondiffusive momentum fluxes in the core of a plasma without an external drive.
