RESUMEN

In this study, the Sobolev-type equation is considered analytically to investigate the solitary wave solutions. The Sobolev-type equations are found in a broad range of fields, such as ecology, fluid dynamics, soil mechanics, and thermodynamics. There are two novel techniques used to explore the solitary wave structures namely as; generalized Riccati equation mapping and modified auxiliary equation (MAE) methods. The different types of abundant families of solutions in the form of dark soliton, bright soliton, solitary wave solutions, mixed singular soliton, mixed dark-bright soliton, periodic wave, and mixed periodic solutions. The linearized stability of the model has been investigated. Solitons behave differently in different circumstances, and their behaviour can be better understood by building unique physical problems with particular boundary conditions (BCs) and starting conditions (ICs) based on accurate soliton solutions. So, the choice of unique physical problems from various solutions is also carried out. The 3D, line graphs and corresponding contours are drawn with the help of the Mathematica software that explains the physical behavior of the state variable. This information can help the researchers in their understanding of the physical conditions.

Background@#Surgical repair of congenital inguinal hernia results in significant postoperative discomfort and pain. The aim of the current study was to evaluate the pre-emptive analgesic efficacy of a transversalis fascia plane (TFP) block after pediatric inguinal herniorrhaphy. @*Methods@#Forty-four patients aged 12 to 60 months who underwent unilateral inguinal herniorrhaphy were enrolled. Four patients were excluded, and the remaining were allocated to the control group and the TFP block group. In the TFP block group, 0.4 mL/kg bupivacaine 0.25% was instilled in the plane between the transversus abdominis and transversalis fascia, while in the control group 0.9% saline was used instead of bupivacaine. The collected data were the total dose of paracetamol consumed during the first 12 h postoperatively, the postoperative Face, Leg, Activity, Cry, Consolability (FLACC) pain score, time to first use of rescue analgesia, number of patients required additional postoperative analgesics, and parents' satisfaction. @*Results@#The median paracetamol consumption was significantly lower in the TFP block group than in the control group, and FLACC pain scores were significantly lower for all study times in the TFP block group with higher parental satisfaction scores than those for the control group. The number of patients who required additional analgesics was significantly lower in the TFP block group than in the control group. @*Conclusions@#The use of a TFP block decreases postoperative analgesic consumption and postoperative pain intensity after pediatric inguinal herniorrhaphy. Future studies with larger sample size are required to evaluate the actual complications rate of TFP block.

Healthy knees require full range squatting movements. Vastus medialis (VM) muscle regulates and adjusts the extensor apparatus that inf luences the patellofemoral function. This work was designed to investigate the anatomy and morphometry of vastus medialis oblique (VMO) muscle by widely used imaging techniques and investigate how VMO muscle participates in anterior knee pain. Ten dissected cadaveric specimens were examined, focusing on fiber orientations, origin, insertions and nerve supply of VMO muscle. Magnetic resonance imaging and ultrasound of VMO muscle were recorded. Anatomical cross-sectional areas of VMO muscle were determined in painless and painful knees and statistically analyzed. In cadaveric specimens, there was distinct separation between VM longus and VMO (change in fiber angle or fibrofascial plane). VMO inserted directly into the medial proximal margin of the patella, capsule of the knee joint and continuous with the patellar tendon. Separate branch of femoral nerve run along the anteromedial border of the muscle. Anatomical cross-sectional area was significantly decreased in painful knee by –17.2%±11.0% at lower end of shaft of femur, –21.1%±6.0% at upper border of patella, –36.7%±11.0% at mid-patellar level. VMO is distinct muscle within quadriceps femoris group. VMO muscle would track the patella medially and participate in last phase of knee extension. Assessment of the VMO muscle anatomical cross-sectional area by ultrasonography may constitute promising and reliable tool to evaluate patellofemoral pain syndrome staging.

RESUMEN

Soft rough set model represents a different mathematical model to which many real-life data can be connected. In fact, this theory represents a link between soft set and rough set theories. The main goal of the present paper is to introduce a new approach to modify and generalize soft rough sets. We are discussing and exploring the basic properties for these approaches. In addition, we use the suggested approaches as a mathematical modeling for an uncertain data and deal with the ambiguity. Comparisons among the proposed methods and the previous one are obtained. Finally, a medical application of the suggested approximations in decision making of diagnosis of COVID-19 is illustrated. Moreover, we develop an algorithm following these concepts and apply it to a decision making problem to demonstrate the applicability of the proposed methods.
