RESUMEN

Objective: To compare outcomes in patients with repeated implantation failure undergoing Intracytoplasmic Sperm Injection/In vitro fertilization (IVF/ICSI) plus immunosuppressants such as prednisolone, prednisone, or cyclosporine A versus the use of IVF/ICSI alone. Data source: Databases were systematically searched in PubMed, Cochrane, and Embase databases in September 2023. Study Selection: Randomized clinical trials and observational studies with the outcomes of interest were included. Data collect: We computed odds ratios (ORs) for binary endpoints, with 95% confidence intervals (CIs). Heterogeneity was assessed using I2 statistics. Data were analyzed using Review Manager 5.4.The main outcomes were live birth, miscarriage, implantation rate, clinical pregnancy, and biochemical pregnancy. Data synthesis: Seven studies with 2,829 patients were included. Immunosuppressive treatments were used in 1,312 (46.37%). Cyclosporine A improved implantation rate (OR 1.48; 95% CI 1.01-2.18) and clinical pregnancy (1.89, 95% CI 1.14-3.14). Compared to non-immunosuppressive treatment, prednisolone and prednisone did not improve live birth (OR 1.13, 95% CI 0.88-1.46) and miscarriage (OR 1.49, 95% CI 1.07-2.09). Prednisolone showed no significant effect in patients undergoing IVF/ICSI, clinical pregnancy (OR 1.34; 95% CI 0.76-2.36), or implantation rate (OR 1.36; 95% CI 0.76-2.42). Conclusion: Cyclosporine A may promote implantation and clinical pregnancy rates. However, given the limited sample size, it is important to approach these findings with caution. Our results indicate that prednisolone and prednisone do not have any beneficial effects on clinical outcomes of IVF/ICSI patients with repeated implantation failure. PROSPERO: CRD42023449655.

BACKGROUND: Surgical site infection (SSI) remains a challenge in healthcare, contributing to prolonged hospital stays, increased healthcare costs, and adverse patient outcomes, including mortality. Effective preoperative skin disinfection interventions, such as povidone-iodine (PVI) and chlorhexidine (CHG), are widely used but their efficacy remains debated. To address this gap, this meta-analysis aims to evaluate the efficacy of PVI and CHG. METHOD: We searched PubMed, Embase, and Cochrane databases up to June 2024 to identify studies comparing PVI versus CHG for preoperative skin antisepsis. We calculated odds ratios (ORs) for binary outcomes, with 95% confidence intervals (CIs). A random-effects model was used with statistical significance set at p < 0.05. Data were analysed using R software (version 4.4.0), and heterogeneity was assessed using I2 statistics. FINDINGS: Sixteen randomised controlled trials (RCTs) were included, involving a total of 13,721 patients, among whom 6,836 (49.8%) received PVI. Compared to CHG, PVI was associated with a non-significant reduction in deep SSI (OR 1.00; 95% CI 0.66 - 1.50; p = 0.994), but an increased risk of overall SSI (OR 1.25; 95% CI 1.06 - 1.48; p = 0.007) and superficial SSI (OR 1.67; 95% CI 1.25 - 2.24; p < 0.001). CONCLUSION: PVI as preoperative skin antisepsis demonstrated a non-significant reduction in deep SSI compared to CHG but was associated with an increased risk of overall and superficial SSI. Despite these findings, PVI remains an effective option, especially in resource-limited settings. Further research is needed to optimise its use and improve infection prevention strategies in clinical practice.

Objectives: Chagas disease (CD) is an infectious disease that predominantly affects poor and vulnerable populations. The last estimate conducted by the World Health Organization in Latin America regarding the prevalence of CD occurred more than 10 years ago. However, there is a scarcity of data assessing the magnitude of CD in populations residing in considered high-risk regions. Therefore, this study aimed to assess the seroprevalence of CD in an endemic region in Northern Minas Gerais through serologic screening. Methods: This is a prevalence study conducted in the municipalities of Catuti, Mato Verde, Mirabela, Montes Azul, and São Francisco, Minas Gerais, Brazil. Data collection occurred between December 2021 and December 2022, involving a questionnaire with closed-ended questions. The variables analyzed included serologic test results, stratified age groups, health indicators, and housing conditions. Results: Of the 2978 participants, 272 individuals (9.1%) tested positive for CD serology. In the age group of 4 to 14 years, 15 to 49 years, and 50 years or older, the prevalence of positive serology was 0.8% (95% confidence interval [CI] 0.16-1.43), 5.5% (95% CI 4.20-6.83), and 18.8% (95% CI 16.48-21.11), respectively. Among the participating municipalities, Mato Verde had the highest prevalence of positive serology for CD (17%). For participants aged 4 to 14 years with positive serology for CD, first-degree relatives were invited to undergo serologic testing. It was possible to collect samples from relatives of all participants in this age group. However, none of the relatives tested positive. Conclusion: This study identified a 9.1% prevalence of individuals affected by CD who were unaware of their condition. In addition, having infected children in the 4 to 14 age group with mothers with negative serology would rule out congenital transmission of the disease.

Children and adolescents suffering from moderate-to-severe atopic dermatitis (AD) face a significant disease burden that greatly impacts their quality of life. Treatment options for AD are currently limited. To assess the safety and efficacy of biologic drugs, dupilumab, lebrikizumab, or tralokinumab, in improving outcomes in patients with moderate to severe inadequately controlled AD. We searched PubMed, Embase and Cochrane databases for randomized controlled trials (RCTs) comparing dupilumab, lebrikizumab or tralokinumab to placebo in patients with AD. We computed odds ratios (ORs) for binary endpoints, with 95% confidence intervals (CIs), random effects model was used and a p-value <0.05 was considered as statistically significant. We analysed data into Review Manager 5.4. A total of five RCTs and 973 patients were included, of whom 592 were prescribed a biologic drug. Compared with placebo, patients receiving a biologic drug had a greater improvement, achieved an Investigator Global Assessment (IGA) score of 0 or 1 (OR 5.05; 95% CI 3.08-8.29), Eczema Area and Severity Index (EASI) 75 (OR 6.87; 95% CI 4.71-10.02), EASI 50 (OR 8.89; 95% CI 6.18-12.78) and EASI 90 (8.30; 95% CI 4.81-14.31). The proportion of patients with 3 points or more (OR 6.56; 95% CI 4.34-9.90) or 4 points or more (OR 8.09; 95% CI 5.19-12.59) improvement from baseline in peak pruritus NRS was significantly higher with biologic drugs than placebo. There were no significant differences between groups regarding adverse events (OR 0.79; 95% CI 0.58-1.07), and conjunctivitis (OR 2.08; 95% CI 1.00-4.33). In this meta-analysis, dupilumab, lebrikizumab, and tralokinumab have shown significant improvements in signs, symptoms and quality of life in children or adolescents with moderate to severe AD. Larger studies may be needed to continue evaluating the safety and efficacy of these biologic drugs in this patient population.

BACKGROUND: After recently published randomized clinical trials, the choice of the best anesthetic procedure for mechanical thrombectomy (MT) in acute ischemic stroke (AIS) due to large vessel occlusion (LVO) is not definite. OBJECTIVE: To compare the efficacy and safety of general anesthesia (GA) versus conscious sedation (CS) in patients with AIS who underwent MT, explicitly focusing on procedural and clinical outcomes and the incidence of adverse events. METHODS: PubMed, Embase, and Cochrane were systematically searched for randomized controlled trials (RCTs) comparing GA versus CS in patients who underwent MT due to LVO-AIS. Odds ratios (ORs) were calculated for binary outcomes, with 95% confidence intervals (CIs). Random effects models were used for all outcomes. Heterogeneity was assessed with I2 statistics. RESULTS: Eight RCTs (1,300 patients) were included, of whom 650 (50%) underwent GA. Recanalization success was significantly higher in the GA group (OR 1.68; 95% CI 1.26-2.24; p < 0.04) than in CS. No significant difference between groups were found for good functional recovery (OR 1.13; IC 95% 0.76-1.67; p = 0.56), incidence of pneumonia (OR 1.23; IC 95% 0.56- 2,69; p = 0.61), three-month mortality (OR 0.99; IC 95% 0.73-1.34; p = 0.95), or cerebral hemorrhage (OR 0.97; IC 95% 0.68-1.38; p = 0.88). CONCLUSION: Despite the increase in recanalization success rates in the GA group, GA and CS show similar rates of good functional recovery, three-month mortality, incidence of pneumonia, and cerebral hemorrhage in patients undergoing MT.

No entanto, ainda não está claro qual é o procedimento anestésico mais benéfico para a TM. OBJETIVO: Nosso objetivo foi comparar a eficácia e a segurança da anestesia geral (AG) versus sedação consciente (SC) em pacientes com AVCI submetidos à TM, focando especificamente nos resultados procedimentais e clínicos, bem como na incidência de eventos adversos. MéTODOS: Foram realizadas buscas sistemáticas nas bases PubMed, Embase e Cochrane por ensaios clínicos randomizados (ECRs) comparando AG versus SC em pacientes submetidos à TM devido a AVCI por OGV. Razões de chances (ORs) foram calculadas para desfechos binários, com intervalos de confiança de 95% (ICs). Modelos de efeitos aleatórios foram usados para todos os resultados. A heterogeneidade foi avaliada com estatísticas I2. RESULTADOS: Oito ensaios clínicos randomizados (1.300 pacientes) foram incluídos, dos quais 650 (50%) foram submetidos à AG. O sucesso da recanalização foi significativamente maior no grupo AG (OR 1,68; IC 95% 1,26­2,24; p < 0,04) em comparação com SC. AIM: To evaluate the effectiveness of aerobic and/or resistance group exercise programs associated with pelvic floor muscle training (PFMT) during prenatal care for the prevention and treatment of urinary incontinence (UI) using the best level of evidence. METHODS: A search was carried out in the MEDLINE/PubMed, LILACS, PEDro, CENTRAL, and SCOPUS databases, without restrictions. The terms "urinary incontinence" and "pregnant woman" were used. Randomized and quasi-randomized clinical trials were included using aerobic and/or resistance exercise programs plus PFMT as an intervention compared to usual care. The Cochrane tool (RoB 2.0) and GRADE were used to assess risk of bias and certainty of evidence, respectively. Quantitative analysis was assessed by meta-analyses. RESULTS: Five publications were included. There was a reduction in the reports of UI postintervention at 16 weeks (RR: 0.83; 95% CI: 0.74-0.93, one study, 762 women, random effects: p = 0.002) and after 3 months (RR: 0.76; 95% CI: 0.60-0.95, one study, 722 women, random effects: p = 0.02), based on moderate certainty of evidence and improvement in UI-specific quality of life (MD: -2.42; 95% CI: -3.32 to -1.52, one study, 151 women, random effects: p < 0.00001), based on low quality of evidence. Other results showed no difference between the postintervention groups, with low and very low evidence. CONCLUSION: There is moderate evidence that the aerobic and/or resistance exercise program associated with PFMT compared to usual care can reduce postintervention UI, as well as 3 months postintervention, and that it can improve UI-specific quality of life, but with low-evidence certainty.

Objetivo: Descrever o manejo clínico da Doença de Chagas (DC) por profissionais médicos da Atenção Primária à Saúde (APS) de região endêmica. Metodologia: Estudo transversal entre médicos da APS de 23 municípios. A coleta contemplou questões Demográficas, Formação Acadêmica, Características da APS de Atuação, Práticas e Conhecimento Relativos à DC. Conduziram-se análises descritivas de todas as variáveis e bivariadas para os blocos de práticas e conhecimento relativos à DC considerando o tempo de conclusão da graduação. Resultados: Foram incluídos 136 médicos. A maioria mencionou conhecer o Protocolo Clínico e Diretriz Terapêutica da DC, mas não prescreveu o benzonidazol e se sente insegura no manejo do paciente. Na análise bivariada, a variável conhecimento sobre a DC adquirido na graduação mostrou-se associada estatisticamente ao tempo de graduação. Conclusão: Importantes obstáculos ainda persistem no manejo médico ao paciente com DC na APS. A partir desse achado espera-se incentivar a qualificação.


Objective: To describe the clinical management of Chagas Disease (CD) by Primary Health Care (PHC) physicians in an endemic region. Methodology: Cross-sectional study with PHC physicians from 23 municipalities. The collection included questions on Demographics, Academic Background, Characteristics of the APS in Action, Practices and Knowledge Relating to CD. Descriptive analyzes of all variables and bivariates were carried out for the blocks of practices and knowledge related to CD, considering the time of completion of graduation. Results: 136 physicians were included. Most mentioned knowing the CD Clinical Protocol and Therapeutic Guideline, but did not prescribe benznidazole and feel insecure in-patient management. In the bivariate analysis, the variable knowledge about CD acquired during graduation was statistically associated with graduation time. Conclusion: Important obstacles still persist in the medical management of patients with CD in PHC. Based on this finding, it is expected to encourage qualification.

As novas tecnologias utilizadas na Odontologia, como a digitalização de imagens, estão levando a mudanças significativas na obtenção de próteses e infraestruturas protéticas, de funcionamento de softwares. A grande área da engenharia desenvolve processos para fabricação de diversos produtos industrializados com auxílio da tecnologia CAD/CAM. O uso desta técnica vem sendo sugerido na clínica odontológica desde a década de setenta, com o objetivo de simplificar, automatizar e garantir níveis de qualidade com adaptações micrométricas das próteses dentárias. O processo pode envolver diferentes ambientes: industrial, laboratorial ou clínico. O objetivo do presente estudo é revisar de forma integrativa a utilização do escaneamento na Odontologia na área de Prótese Dentária, enfatizando os resultados do seu uso e as diferentes formas de escaneamento e comparando, quando possível, este sistema ao método convencional de moldagem. Notou-se que o escaneamento intraoral resulta em próteses fixas com funcionalidade clínica similar ou melhor do que a moldagem convencional, além de estar relacionada a um conforto aumentado por parte do paciente. Além disto, o escaneamento intraoral reduz o tempo de cadeira do paciente, além de estar associado ao fluxo digital laboratorial mais rápido, o que reduz também o tempo total de fabricação destas próteses, sendo ideal para uma melhor administração do tempo de consultório.

Diet composition has a great impact on body composition. Several studies have suggested a beneficial effect of adding olive oil to a calorie-restricted diet as a weight loss strategy. However, there is no clear direction regarding the effect of olive oil on body fat distribution. This systematic review with meta-analysis aims to investigate the effect of olive oil consumption (for cooking or as a supplement) on body fat distribution in adults. The present study was conducted following the recommendations of the Cochrane Handbook for Systematic Reviews of Interventions and was registered in the International Prospective Register of Systematic Reviews (PROSPERO CRD42021234652). All randomized clinical trials of parallel or crossover design found in PubMed (via MEDLINE), EMBASE, Web of Science and Scopus databases that compared the effects of olive oil with other oils on body fat distribution in adults were included. 52 articles were included. The results indicate that the consumption of olive oil does not seem to alter the distribution of body fat, despite a small indication that supplementation in capsules can increase adipose mass (Mean Difference = 0.28 kg, 95% CI [-0.27, 0.83]; between-groups difference p = 0.59) and waist circumference (mean difference = 1.74 kg, 95% CI [0.86, 1.62]; between-groups difference p < 0.01) and decrease its auxiliary culinary use (mean difference = -0.32 kg, 95% CI [-0.90, 0.26]). Lean mass responds negatively to OO the higher the dose (slope = -0.61, 95% CI [-1.01, -0.21], p = 0.003) and time offered (slope = -0.8822, 95% CI [-1.44, -0.33], p = 0.002). In conclusion, this systematic review showed that OO ingestion in different delivery vehicles, dosages, and durations can interfere body composition. It is important to emphasize that some other aspects of the population and the intervention, that were not possible to be explored in the analysis, could confound the real effects of OO on body composition.

Abstract Objectives: to describe behavioral changes related to mental health in children and adolescents with Attention Deficit Hyperactivity Disorder during social isolation due to the COVID-19 pandemic. Methods: this is a systematic review conducted under the PRISMA protocol (2020) in the PubMed, SciELO and VHL databases, with a period of 2019-2022. Results: 3,735 studies were screened and ten were selected, according to the eligibility criteria. The sample had 4,688 participants. There was evidence of a worsening of signs and symptoms regarding the mental health of this population, expressed mainly through changes in mood, with increased anxiety, sadness or depressed symptoms, in addition to increased hyperactivity. Behavioral changes also included reduced sleep quality and physical activity, and increased use of digital technologies and screen time. Conclusions: it was described that children and youth groups, especially those with attention deficit hyperactivity disorder, had increased emotional symptoms and conduct problems when compared to before the pandemic, making it possible to recognize the negative repercussions of the changes imposed by it. These factors are important for planning more effective care strategies.


Resumo Objetivos: descrever mudanças comportamentais, relacionadas à saúde mental, em crianças e adolescentes com Transtorno de Déficit de Atenção e Hiperatividade durante o isolamento social devido à pandemia da COVID-19. Métodos: trata-se de uma revisão sistemática conduzida sob protocolo PRISMA (2020) nas bases de dados PubMed, SciELO e BVS, com período de 2019-2022. Resultados: foram triados 3.735 estudos e selecionados dez, de acordo com os critérios de elegibilidade. A amostra contou com 4.688 participantes. Evidenciou-se um agravamento de sinais e sintomas quanto à saúde mental dessa população expresso principalmente através de mudanças no estado de humor, com aumento da ansiedade, tristeza ou sintoma deprimido, além do aumento da hiperatividade. As mudanças comportamentais incluíram ainda redução da qualidade do sono e da prática de atividades físicas e aumento do uso de tecnologias digitais e tempo de tela. Conclusões: foi descrito que os grupos infanto-juvenis, sobretudo com Transtorno de Déficit de Atenção e Hiperatividade, tinham sintomas emocionais e problemas de conduta aumentados quando comparados a antes da pandemia, sendo possível reconhecer as repercussões negativas das mudanças impostas por ela. Esses fatores são importantes para o planejamento de estratégias de cuidado mais efetivas.

Esse relato de experiência aborda uma vivência que ocorreu no mês de agosto de 2019 em um Centro de Saúde da Comunidade de Palmas (CSC) ­ TO. O objeto da experiência é a atuação do NASF na oferta de saúde na puericultura (Projeto Bebê Clean), a qual tinha como objetivo intensificar a relação mãe e filho a respeito da amamentação, introdução alimentar e o shantala no CSC a partir da vivência em aulas práticas do Módulo Integração-Ensino-Serviço-Comunidade/IESC I do curso de Medicina do ITPAC Palmas. A metodologia aplicada consistiu na participação dos acadêmicos em uma oficina pedagógica no CSC com a equipe do NASF e a preceptora do módulo, visando conhecer a multidisciplinaridade e compreender as atividades realizadas por este, considerando o processo colaborativo e o papel do ACS como elo entre a equipe e a comunidade. A partir dessa vivência acadêmica foi possível reconhecer que a formação médica com aulas práticas na atenção primária à saúde possibilita a reflexão sobre o verdadeiro papel do médico, o qual vai além do ato de curar doenças e fazer cirurgias.

Objetivou-se descrever o perfil dos sistemas de criação de caprinos e ovinos da Ilha de São Luís no estado do Maranhão. Para isso, realizou-se um estudo descritivo, exploratório e quali-quantitativo com 80 produtores rurais selecionados pela técnica Snowball, no período de fevereiro a abril de 2021. Em relação ao perfil do produtor rural, verificou-se predominância do gênero masculino (93,8%), apresentando faixa etária entre 25 a 60 anos (86,2%). A maioria dos produtores apresentavam ensino médio completo (23,8%) ou ensino superior completo (23,8%) e 88,8% dos produtores não estavam vinculados a organizações coletivas, associações, cooperativas ou sindicatos. Nas propriedades rurais o sistema de criação predominante era o semi-extensivo (73,8%) com presença de apriscos (76,2%) e as criações eram consorciadas com caprinos e ovinos (58,8%). A principal causa de morte nos animais verificada foi a verminose (58,8%). Conclui-se que se faz necessário o estabelecimento de medidas de estímulo às criações na Ilha de São Luís por meio de políticas públicas voltadas aos produtores locais, requerendo, sobretudo, assistência técnica, orientações zootécnicas, educação sanitária e acesso ao crédito, para, assim, deixar de ser apenas uma atividade de subsistência e passar a gerar maiores rendas para as famílias produtoras.


The objective of this research was to describe the profile of the goat and sheep rearing systems on the Island of São Luís in the state of Maranhão. For this, a descriptive, exploratory and qualitative-quantitative study was carried out with 80 rural producers selected by the Snowball technique, from February to April 2021. Regarding the profile of the rural producer, there was a predominance of males (93.8%), aging between 25 and 60 years (86.2%). Most producers had completed high school (23.8%) or higher education (23.8%) and 88.8% were not members of collective organizations, associations, cooperatives or unions. In rural properties, the predominant rearing system was semi-extensive (73.8%) with the presence of sheepfolds (76.2%) and mutual goats and sheep farming (58.8%). The main cause of death of animals was warm infections (58.8%). In conclusion, it is necessary to establish measures to support sheep and goat farming on the Island of São Luís through public policies focused on local producers, which require, above all, technical assistance, zootechnical guidelines, health education and access to credit in order to the farms stop being just a subsistence activity and start generating higher incomes for producing families.

Objetivo: identificar os fatores associados ao estigma da hanseníase, manifestado na orientação quanto à separação de objetos no convívio familiar pelos agentes comunitários de saúde de Palmas, Tocantins, Brasil. Métodos: estudo transversal do tipo quantitativo realizado nas Unidades Básicas de Saúde de Palmas em uma população de 301 agentes comunitários de saúde, por meio de um questionário autoaplicável. Resultados: dos 301 agentes comunitários de saúde, 22,92% orientaram as pessoas no domicílio quanto à separação de objetos de uso pessoal, tais como talheres, copos, pratos, toalhas, roupa de cama, vestimentas e isolamento de dormitório. A orientação para separar objetos de uso pessoal foi significativamente maior para os agentes comunitários de saúde do sexo masculino (RP: 1,89; IC: 1,25-2,87), mais jovens (RP: 1,90; IC: 1,14-3,17), com menor tempo de moradia em Palmas (RP: 2,06; IC: 1,253,40), com localização da Unidade Básica de Saúde fora do plano diretor (RP: 1,75; IC: 1,11-2,76), que não fizeram curso em hanseníase (RP: 3,03; IC: 2,01-4,58), que nunca fizeram acompanhamento de casos (RP: 3,82; IC: 2,02-7,22) e que não teve um familiar acometido por hanseníase (RP: 1,96; IC: 1,10-3,47). Conclusão: a elevada prevalência de agentes comunitários de saúde que apresentam estigma da hanseníase representa uma barreira nas ações de controle da doença. Observou-se que as chances de ocorrência de estigma foram maiores nos agentes comunitários de saúde jovens, do sexo masculino, sem capacitação sobre hanseníase e com local de trabalho nas regiões periféricas do município. Fatores a serem considerados pelo programa de controle local com a adoção de Educação Permanente em Saúde. Investimentos em cursos direcionados à hanseníase podem contribuir para a redução do desconhecimento acerca da doença, pois os aspectos culturais e de crenças geracionais influenciam na manutenção de conceitos e do estigma social.


Objective: to identify the factors associated with the stigma of leprosy, manifested in guidance regarding the separation of objects in family life, by community health workers in Palmas, Tocantins, Brazil. Methods: quantitative cross-sectional study carried out in Basic Health Units in Palmas in a population of 301 community health workers, using a self-administered questionnaire. Results: of the 301 CHWs, 22.92% guided people at home regarding the separation of personal objects, such as cutlery, glasses, plates, towels, bed linen, clothing and bedroom isolation. Guidance to separate objects for personal use was significantly higher for male community health workers (PR: 1.89; CI:1.25-2.87), younger (PR:1.90; CI:1.14- 3.17), with less time living in Palmas (RP: 2.06; CI: 1.253.40), with the location of the Basic Health Units outside the master plan (PR: 1.75; CI: 1.11-2.76 ), who had not taken a leprosy course (PR:3.03; CI: 2.01-4.58), who had never followed up cases (PR:3.82; CI: 2.02-7.22) and who did not have a family member affected by leprosy (PR:1.96; CI: 1.10-3.47). Conclusion: the high prevalence of community health workers who have leprosy stigma represents a barrier in disease control actions. It was observed that the chances of stigma occurrence were higher in young, male community health agents, without training on leprosy and with a workplace in the peripheral regions of the city. Factors to be considered by the local control program with the adoption of Permanent Health Education. Investments in courses aimed at leprosy can contribute to reducing ignorance about the disease, as cultural aspects and generational beliefs influence the maintenance of concepts and social stigma.

Methods: a cross-sectional and analytical study, carried out from analysis of notifications of incidents related to bronchopulmonary aspiration of the Health Surveillance Notification System, from January 2014 to December 2018. Results: of the 264,590 notifications, 553 referred to aspiration, whose prevalence rate was 0.21%. There was an association between the event and age, ethnicity, main medical diagnosis, country region, service type, health unit and consequences for patients. Furthermore, four independent predictor variables for death were found: living in the North or South regions, being elderly and receiving healthcare at night. Conclusions: the prevalence rate of bronchopulmonary aspiration was small, but with a negative impact on patients.

Métodos: estudio transversal y analítico, realizado a partir del análisis de notificaciones de incidentes relacionados con la aspiración broncopulmonar del Sistema de Notificación de Vigilancia Sanitaria, de enero de 2014 a diciembre de 2018. Resultados: de las 264.590 notificaciones, 553 se refirieron a aspiración, cuya tasa de prevalencia fue del 0,21%. Hubo asociación entre el evento y la edad, etnia, diagnóstico médico principal, región del país, tipo de servicio, unidad de salud y consecuencias para el paciente. Además, se encontraron cuatro variables predictoras independientes de muerte: vivir en la región Norte o Sur, ser anciano y recibir atención médica por la noche. Conclusiones: la tasa de prevalencia de aspiración broncopulmonar fue pequeña, pero con impacto negativo en los pacientes.

Vaccines are a recommended strategy for controlling influenza A infections in humans and animals. Here, we describe the effects of hydrostatic pressure on the structure, morphology and functional characteristics of avian influenza A H3N8 virus. The effect of hydrostatic pressure for 3 h on H3N8 virus revealed that the particles were resistant to this condition, and the virus displayed only a discrete conformational change. We found that pressure of 3 kbar applied for 6 h was able to inhibit haemagglutination and infectivity while virus replication was no longer observed, suggesting that full virus inactivation occurred at this point. However, the neuraminidase activity was not affected at this approach suggesting the maintenance of neutralizing antibody epitopes in this key antigen. Our data bring important information for the area of structural virology of enveloped particles and support the idea of applying pressure-induced inactivation as a tool for vaccine production.

OBJECTIVE: This article aims to identify and analyze the legal and regulatory frameworks with an interface with patient safety, considering the historical path of the patient safety policy in Brazil. METHODS: This is a historical review based on the relevant literature to the topic such as papers, legislation, and official documents with an interface with public health policies from 1988 to 2019. We also performed a documentary search to include data from the Brazilian Health Regulatory Agency (ANVISA) such as normative and nonnormative regulatory instruments. After organizing the data, the process of content analysis was performed. RESULTS: We debated initially the historical aspects of sanitary surveillance of health services in addition to main actions taken by the Brazilian Health Regulatory System, which includes sanitary regulation and patient safety challenges. We identified a diversity of regulations published by ANVISA in the past decade related to patient safety, in addiction to sanitary actions. These initiatives culminated in the establishment of the National Patient Safety Program in 2013, followed by other health improvements, such as surveillance, incidents monitoring, and safe practices self-assessment. CONCLUSIONS: The regulation and sanitary actions directed to patient safety in Brazil have increased after the creation of ANVISA. In the face of this activities, the social role played by the Brazilian Health Regulatory System toward the advancement in the field of risk minimization in health services can be highlighted as a protagonist in the process of promoting patient safety.

Objetivou-se avaliar a carga parasitária da cama de frango de uma granja avícola localizada no município de Paço do Lumiar - MA. Foram coletadas 50 amostras de cama de frango de 10 galpões. Essa cama já havia sido utilizada seis vezes nos lotes anteriores. Para as análises parasitológicas foram utilizados os métodos de Sedimentação Espontânea, Método de Sheather, Esporulação para diagnóstico de oocistos de coccídios. Foram identificados ovos de nematódeos dos gêneros Strongyloides (72%), Heterakis (18%) e Ascaridia (10%). As larvas as do gênero Strongyloides destacaram-se como as mais prevalentes, com frequência de 97% de casos, seguido geohelmintos (3%). Encontrou-se 4900 oocistos de Eimeria, dos quais, 1200 estavam em estágio de esporulação. Foram encontrados também ácaros e cascudinhos (Alphitobius diaperinus). Conclui-se que a alta frequência de parasitos presentes na cama de frango pode estar relacionada à falha de manejo.(AU)


The objective of this study was to evaluate the parasitic load of chicken litter from a poultry farm located in the municipality of Paço do Lumiar - MA. Fifty samples of chicken bed of 10 sheds were collected. This bed had already been used six times in the previous batches. For the parasitological analyzes the methods of Spontaneous Sedimentation, Sheather Method, Sporulation for the diagnosis of coccidia oocysts were used. Eggs of nematodes of the genera Strongyloides (72%), Heterakis (18%) and Ascaridia (10%) were identified. The larvae of the genus Strongyloides were the most prevalent, with a frequency of 97% of cases, followed by geohelminths (3%). There were 1200 Eimeria oocysts, in sporulation stage. Mites and kites were also found (Alphitobius diaperinus). It is concluded that the high frequency of parasites present in chicken litter may be related to management failure.(AU)
