RESUMEN

A total of 73 species of Argia Rambur, 1842 are reported as present from Mexico, Central America and the Lesser Antilles, of which five are new to science and are described here: Argia annae n. sp. (holotype ♂: MEXICO, Veracruz, Puente Texolo, 19.4028°N, 96.9867°W, 1065 m, 26 August 1988, Rosser W. Garrison leg., in INECOL), A. paludicola n. sp. (holotype ♂: U. S. A., Arizona, Apache County, Bog Tank, north of Highway 260, 34.0467°N, 109.683°W, 1400 m, 1 July 2016, Pierre Deviche leg., in CSCA) and A. paulsoni n. sp. Redescriptions of male and female are provided for the following species: Argia calida (Hagen, 1861), A. chelata Calvert, 1902, A. deami Calvert, 1902, A. fulgida Navás, 1934, A. herberti Calvert, 1902, A. johannella Calvert, 1907, A. mayi González-Soriano, 2012, A. percellulata Calvert, 1902, A. pocomana Calvert, 1907, A. rogersi Calvert, 1902, A. talamanca Calvert, 1907, A. terira Calvert, 1907, A. underwoodi Calvert, 1907, and A. variabilis Selys, 1865. All 73 species are measured, illustrated, and distinguished from their congeners. The following synonymies are suggested based on examination of type material: A. wilsoni Calvert, 1902, a junior synonym of A. calida (Hagen, 1861), A. plana Calvert, 1902, a junior synonym of A. funebris (Hagen, 1861), and A. trifoliata Fraser, 1942 (Fig. 736), a junior synonym of A. variegata Förster, 1914. Lectotypes are designated for Agrion calidum Hagen, 1861, Agrion funebris Hagen, 1861, and Argia variabilis Selys, 1865. Keys to the 73 known Argia species from Mexico, Middle America and the Lesser Antilles are provided as well as distribution maps based on an examination of over 16,000 specimens.

RESUMEN

The Synlestidae (Odonata: Zygoptera) of southern Africa comprise some highly localized species. All but one species are endemic to South Africa, and many to the Cape Floristic Region. Here we present the first phylogenetic reconstruction of the southern African Synlestidae using nuclear and mitochondrial molecular data. The genera Ecchlorolestes and Chlorolestes are monophyletic, and we propose that the Neotropical family Perilestidae consisting of two genera, Perilestes and Perissolestes, be sunk within Synlestidae. We discuss the intra-familial relationships for the southern African Synlestidae.

The status of the Tramea species present in the Galapagos Islands (Odonata, Libellulidae) has been the subject of a long-standing debate among odonatologists. Here, we use molecular and morphological data to analyze a series of specimens from this genus collected in 2018 from the Islands of San Cristobal, Isabela, and Santa Cruz, with the aim of determining their relationship with Tramea calverti Muttkowski and with their currently considered senior synonym T. cophysa Hagen. We combined sequencing of mitochondrial and nuclear DNA with morphological examination of several specimens of Tramea, including representatives of continental T. cophysa and T. calverti. Our molecular analyses place the Tramea from Galapagos in the same clade as T. calverti, with T. cophysa as a closely related species. The morphological analyses found only one consistent difference between T. cophysa and T. calverti: the presence of an accessory lobe in the male vesica spermalis of T. cophysa that is absent in T. calverti and in the Tramea from Galapagos. In agreement with our genetic results, the overall morphological differences documented by us indicate that the Galapagos material examined is conspecific with T. calverti. In light of this, and following the principle of priority in taxonomic nomenclature, Tramea calverti Muttkowski, 1910 should hereafter be considered a junior synonym of Tramea darwini Kirby, 1889.

Based on comparison of specimens and descriptions, Mitragomphus ganzanus Needham, 1944, described from Braganza [Bragança], Pará State, Brazil, and known only from the holotype, is found to be a junior synonym of Gomphidia kirschii Selys, 1878, a species from the Philippines. The monotypic genus Mitragomphus Needham, 1944, is synonymized with the genus Gomphidia Selys, 1854.

The Cochrane Collaboration helps people make well-informed decisions about health care by preparing, maintaining and promoting the accessibility of systematic reviews of the effects of healthcare interventions. In all, 51 Cochrane Review Groups are responsible for preparing and maintaining the reviews. Most of these Review Groups are problem-based. However, there are a number of areas or dimensions of health care, such as the setting of care (eg primary care) or the type of patient/consumer (eg older persons), that cannot be usefully conceptualised as 'health problems'. In order to reflect the interests of these dimensions, or 'fields', of health care more effectively, the Cochrane Collaboration created another type of entity: Cochrane Fields or Networks. The core functions of these Fields are described. It is proposed that a new Cochrane Subfield for Diet and Nutrition should be set up to attract a cadre of new reviewers in order to ensure the proper representation of diet and nutrition expertise in the relevant Collaborative Review Groups. The methodological pitfalls of the use of nonrandomised studies will be a key issue for the new Subfield, since the focus of the Cochrane Collaboration's programme of searching databases, journals and conference proceedings has been on the identification of reports of randomised controlled trials (RCTs). Some of these sources will be need to be searched again for non-RCT studies. The financial considerations of establishing of a Cochrane Subfield for Diet and Nutrition are discussed.

OBJECTIVE: To investigate, using the Health Belief Model as a theoretical framework, the incentives and barriers to breast cancer screening in a recent immigrant group, older Tamil women from Sri Lanka. METHOD: Tamil women who had had a mammogram and Tamil women who had never had a mammogram were compared on the following variables: socio-demographics, personal risk estimates for breast cancer, risk-reduction expectancies, beliefs and knowledge about breast cancer and screening recommendations, and acculturation. RESULTS: Groups differed significantly in terms of education, years living in North America, acculturation, and beliefs/knowledge about breast cancer. When education and acculturation were controlled, perceived barriers to mammography were most predictive of mammography utilization. DISCUSSION: Results are discussed with a view to developing culture-appropriate educational campaigns.

RESUMEN

With more than 1000 new guidelines produced annually over the past decade, it is impossible for the practicing family physician to determine which ones should be adapted into their clinical practice. The Ontario Ministry of Health and Long-Term Care and the Ontario Medical Association formed the Guideline Advisory Committee (GAC) in 1997 to assess and disseminate guidelines that would improve the quality and utilization of health care services in the province. Over the past 3 years the GAC has developed a strategy to identify important topics, to rank order guidelines published on these topics based on the quality of their development, and to reformat guidelines as necessary to make them user-friendly for implementation in clinical practice. The GAC is currently assessing a number of strategies to enhance the dissemination of selected guidelines to improve the quality of care delivered in the province.

INTRODUCTION: The literature shows that physicians do not change their patterns of practice quickly. With thousands of conflicting guidelines available, many disseminated by print, there has been little measurable impact of new guidelines on physician prescribing patterns. METHODS: A series of interventions designed to improve physician uptake of guidelines is described. Interventions evaluated in a series of trials include mail dissemination, small group continuing medical education sessions, academic detailing by both academic detailers and pharmaceutical detailers, and a whole community program known as the Program for Appropriate Anti-Infective Community Therapy. RESULTS: Only the whole community intervention had significant impact on physician prescribing patterns. The changes involved a significant reduction in anti-infective use and a significant change from the use of second- and third-line drugs to first-line drugs, as specified in the Ontario Anti-Infective Guidelines. CONCLUSIONS: Acceptable guidelines disseminated by a multidisciplinary community program can have a positive impact on physician prescribing patterns. To increase the acceptability of guidelines for whole community dissemination, a new innovation (Guideline Advisory Committee) that scores the quality of guidelines and selects the most objectively produced guideline is described.

PROBLEM BEING ADDRESSED: Research is not new to family medicine, yet it is pursued less than in other clinical disciplines. We need to establish a critical mass of family medicine researchers. OBJECTIVE OF PROGRAM: To establish a departmental research organization using a strategy implemented in 1995 by the Department of Family and Community Medicine at the University of Toronto. MAIN COMPONENTS OF PROGRAM: We set out to establish a critical mass of researchers. Applicants were required to complete credible and feasible 3- to 5-year research plans and to have formal support from their clinical chiefs. Once selected, researchers were supported for 40% of their time. Support was provided for 3 years and was renewable according to progress on their research plans. Researchers were expected to publish on average two papers yearly and be involved as principal investigator or co-principal investigator on at least one successful grant after the first 3 years. Since implementation in 1996, funded researchers have become principal investigators in 80% of the grants in which they are involved compared with 20% before the support program. Nine of 15 Medical Research Council grants held by family physicians in Canada have department members as principal investigators. Faculty-supported researchers contributed more than 200 peer-reviewed publications to the literature between 1996 and 2000. CONCLUSION: Four years of experience allows for early assessment of the first step taken to build a thriving family medicine research organization using limited departmental resources.
