RESUMEN

Objetivo: Determinar a incidência de flebite, fatores de risco associados e custos diretos de tratamento. Métodos: Estudo descritivo, correlacional, com análise de custo direto. Utilizaram-se dados da documentação dos enfermeiros entre janeiro 2019 e agosto 2021. Resultados: Incluíram-se 2.374 pessoas com cateter venoso periférico, com internamento na cardiologia. A incidência de flebite foi de 12,38%, das quais 78,23% eram de grau 1 de severidade. Verificou-se associação estatística entre o desenvolvimento de flebite e a administração de amiodarona endovenosa, dias de hospitalização e serviço de internamento. Estimaram-se 1662€ de custos adicionais ao tratamento da flebite, em material clínico e horas de cuidados de enfermagem. Conclusão: Os cuidados de enfermagem são eficazes na identificação precoce e tratamento da flebite, promovendo redução de custos adicionais e garantindo melhores cuidados e ganhos em saúde.


Objective: This study aimed to identify the incidence rate of phlebitis, associated risk factors and treatment direct costs. Methods: Descriptive, correlational study with direct cost analisys. Data from the nurses' clinical records between January 2019 and August 2021 were used. Results: Included 2,374 files of people with peripheral venous catheter who were admitted to Cardiology. The phlebitis incidence rate was 12.38% and regarding severity 78.23% were grade 1. The phlebitits was significantly related with intravenous amiodarone administration, length of stay and physical department. This represents €1662 of additional treatment costs, in clinical supplies and nursing time. Conclusion: Nursing care is effective in the early identification and treatment of phlebitis, reducing costs and improve clinical and economic outcomes.

Peritoneal dialysis (PD) modalities affect solute removal differently. However, the impacts of switching PD modalities on serum levels of biomarkers of different sizes are not known. Our objective was to analyze whether a change in the PD modality associates with the levels of two routine biochemical laboratories. In this multicentric prospective cohort study. we selected all patients who remained on a PD modality for at least 6 months and switched PD modality. Patients were also required to be treated with the same PD modality for at least 3 months before and after the modality change. The primary outcome was change in potassium and phosphate serum levels. We identified 737 eligible patients who switched their PD modality during the study. We found mean serum phosphate levels increased during the 3 months after switching from CAPD to APD and conversely decreased after switching to from APD to CAPD. In contrast, for potassium the difference in the mean serum levels was comparable between groups switching from CAPD to APD, and vice versa. In conclusion, CAPD seems to be as efficient as APD for the control of potassium serum levels, but more effective for the control of phosphate serum levels. The effect of a higher removal of middle size molecules as result of PD modalities in terms of clinical and patient-reported outcomes should be further explored.

RESUMEN

Penile necrosis is an uncommon condition associated, in most cases, with calciphylaxis, also known as calcific uremic arteriolopathy. It is a progressive disease found in approximately 1%-4% of patients with end stage renal disease. Patients with penile calciphylaxis present a mortality rate of up to 70%, with life expectancy of two-and a half months following the diagnosis. We report a severe calciphylaxis case that had to be submitted to a penectomy but survived the penile event.

Objetivo: Relatar cenário de supervisão clínica de estudante de enfermagem no primeiro ensino clínico, com base no ciclo de supervisão de Nicklin. Métodos: Relato de Experiência no ensino com análise crítica de um cenário de supervisão clínica de estudante de enfermagem, em contexto do primeiro ensino clínico, num serviço de medicina, com a duração de 10 semanas, com base no ciclo de supervisão de Nicklin. Resultados: Analisaram-se as práticas e foram identificados problemas de natureza cognitiva, técnica, comunicacional, comportamental, relacional, de gestão das emoções e de autocontrole. O plano de intervenção de supervisão baseou-se na mobilização estratégias supervisivas que permitissem concretizar objetivos no sentido da aquisição de competências nos domínios em que foram identificados problemas. A implementação do plano decorre a partir da avaliação intercalar, nas últimas cinco semanas de ensino clínico. Conclusão: Concluiu-se que o ciclo de supervisão clínica de Nicklin, cuja estrutura encontra paralelismos na metodologia científica do processo de enfermagem, possibilita a análise do processo de desenvolvimento de estudantes em ensino clínico, com base no método científico, o que permite uma supervisão estruturada e baseada na identificação e resolução dos problemas. Objective: To report a clinical supervision scenario of a nursing student in the first clinical training based on Nicklin's supervision cycle. Methods: Teaching Experience Report with a critical analysis of a nursing student clinical supervision scenario, in the context of the first clinical training, in a medical ward, lasting 10 weeks, based on Nicklin's supervision cycle. Results: Practices were analysed and problems of a cognitive, technical, communicational, behavioural, relational, emotional management and self-control nature were identified. The supervisory intervention plan was based on the mobilization of supervisory strategies that would make possible to achieve objectives in the sense of acquiring skills in the areas in which problems were identified. The implementation of the plan takes place from the mid-term evaluation, in the last five weeks of clinical training. Conclusion: We concluded that Nicklin's clinical supervision cycle, whose structure finds parallels in the scientific methodology of the nursing process, allows the analysis of the students' development process in clinical training, based on the scientific method, which allows a supervision structured and based on the identification and resolution of problems. Objetivo: Informar el escenario de supervisión clínica de un estudiante de enfermería en la primera educación clínica, basado en el ciclo de supervisión de Nicklin. Métodos: Informe de experiencia docente con un análisis crítico del escenario de supervisión clínica de un estudiante de enfermería, en el contexto de la primera enseñanza clínica, en un servicio médico, con una duración de 10 semanas, basado en el ciclo de supervisión de Nicklin. Resultados: Se analizaron las prácticas y se identificaron problemas de naturaleza cognitiva, técnica, comunicacional, conductual, relacional, emocional y de autocontrol. El plan de intervención de supervisión se basó en la movilización de estrategias de supervisión que permitirían alcanzar objetivos en el sentido de adquirir habilidades en las áreas en las que se identificaron problemas. La implementación del plan se lleva a cabo desde la evaluación intermedia, en las últimas cinco semanas de enseñanza clínica. Conclusión: Se concluyó que el ciclo de supervisión clínica de Nicklin, cuya estructura encuentra paralelos en la metodología científica del proceso de enfermería, permite el análisis del proceso de desarrollo del estudiante en la enseñanza clínica, basado en el método científico, que permite la supervisión estructurado y basado en la identificación y resolución de problemas. BACKGROUND: The chronic use of angiotensin-converting enzyme inhibitors or angiotensin II receptor blocker has been associated with hyperkalemia in patients with reduced renal function even after the initiation of hemodialysis. Whether such medications may cause a similar effect in peritoneal dialysis patients is not well established. So, the aim of our study was to analyze the impact of renin-angiotensin-aldosterone inhibitors on the serum levels of potassium in a national cohort of peritoneal dialysis patients. METHOD: A prospective, observational, nationwide cohort study was conducted. We identified all incident patients on peritoneal dialysis that had angiotensin converting enzyme inhibitors (ACEi) or angiotensin receptor blockers (ARB) prescribed for at least 3 months and a similar period of time without these medications. Patients were divided into 4 groups: Groups I and III correspond to patients using, respectively, an ACEi or ARB and then got the drug suspended; Groups II and IV started peritoneal dialysis without the use of any renin-angiotensin aldosterone system inhibitor and then got, respectively, an ACEi or ARB introduced. Changes in potassium serum levels were compared using 2 statistical approaches: (1) the non-parametric Wilcoxon test for repeated measures and (2) a crossover analysis. RESULTS: Mean potassium serum levels at the first phase of the study for Groups I, II, III, and IV were, respectively, 4.46 ± 0.79, 4.33 ± 0.78, 4.41 ± 0.63, and 4.44 ± 0.56. Changes in mean potassium serum levels for Groups I, II, III, and IV were -0.10 ± 0.60, 0.02 ± 0.56, -0.06 ± 0.46, and 0.03 ± 0.50, respectively. CONCLUSION: The use of ACEi and ARB was not associated with a greater risk for hyperkalemia in stable peritoneal dialysis patients independently of residual renal function.

The prevalence of patients on automated peritoneal dialysis (APD) is increasing worldwide and may be guided by clinical characteristics, financial issues and patient option. Whether socioeconomic factors at the patient level may influence the decision for the initial peritoneal dialysis (PD) modality is unknown. This is a prospective cohort study. The primary outcome of interest was the probability to start PD on APD. The inclusion criteria were adult patients incident in PD. Exclusion criteria were missing data for either race or initial PD modality. We used a mixed-model analysis clustering patients according to their PD center and region of the country. We included 3,901 patients of which 1,819 (46.6%) had APD as their first modality. We found a significant disparity for race and educational level with African American patients less likely to start on APD (Odds ratio 0.74 CI95% 0.58-0.94) compared to Whites whilst those with greater educational levels were more likely to start on APD (Odds ratio 3.70, CI95% 2.25-6.09) compared to illiterate patients. Limiting the use of APD in disadvantaged population may be unethical. Demographics and socioeconomic status should not be necessarily part of the decision-making process of PD modality choice.

AIMS: Changes resulting from the gestational period may lead to changes in the biomechanics of women, which can alter the performance of functional activities such as sit-to-stand. Thus, the objective of this study was to investigate the influence of a virtual reality-based exercise protocol on the kinematic variables of the sit-to-stand movement in women in their second and third gestational trimesters. METHODS: The sample consisted of 44 women selected according to the eligibility criteria, allocated into 4 groups: control group, 2nd trimester (CG2T); experimental group, 2nd trimester (EG2T); control group, 3rd trimester (CG3T); and experimental group, 3rd trimester (EG3T). All the volunteers answered the identification and evaluation form and were sent to the kinematic evaluation through the Qualisys Motion Capture System®. An intervention with game therapy was performed in 12 sessions of 30 minutes each, three times a week. RESULTS: No statistically significant differences were found intra- (P> 0.54) and inter-groups (P> 0.059) for kinematic variables. However, there was a tendency for improvement in the analyzed variables after the proposed protocol. CONCLUSIONS: The data obtained suggest that the use of the Nintendo Wii Fit Plus® was not able to influence sit-to-stand kinematic variables in the analyzed women.(AU)

Abstract Introduction: During pregnancy women undergo several transformations, which promote changes in their gravity center (GC) and can result in postural and balance changes. Objective: To evaluate the influence of Nintendo WiiFit Plus® on postural balance and quality of life of pregnant women in the third trimester. Methods: 250 pregnant women enrolled through a non-probabilistic sampling process. The sample consisted of 32 women selected according to the eligibility criteria (nulliparity; low-risk pregnancy, absence of musculoskeletal disorders or surgical procedures in the spine, pelvis, hip or knee), musculoskeletal disorders, allocated as follows: 17 pregnant women in the control group (CG) and 15 pregnant women in the experimental group (EG). The intervention was performed in 12 sessions of 30 minutes each, three times a week. Sociodemographic and obstetric data were presented as median and interquartile range (25% - 75%). A comparison of the relative values of variables before and after the exercise program was performed using the 2x2 ANOVA test (5% significance level). Results: There were no significant statistical differences in variables related to postural balance (P > 0.06) and quality of life (P > 0.13). Conclusion: The use of Nintendo WiiFit Plus games was not able to promote improvement in postural balance and quality of life of the pregnant women studied.


Resumo Introdução: Durante a gravidez as mulheres passam por várias transformações, que promovem mudanças no seu centro de gravidade (CG) e pode resultar em alterações posturais e de equilíbrio. Objetivo: Avaliar a influência da Nintendo Wii Fit Plus® no equilíbrio postural e qualidade de vida das mulheres grávidas no terceiro trimestre. Métodos: 250 gestantes cadastradas através de um processo de amostragem do tipo não-probabilística. A amostra foi composta por 32 mulheres selecionadas de acordo com os critérios de elegibilidade (nuliparas, gestação de baixo risco, ausência de disfunções musculoesqueléticas ou cirurgia na coluna vertebral, pelve, quadril ou joelho) alocadas da seguinte maneira: 17 gestantes no grupo controle (GC) e 15 gestantes no grupo experimental (GE). A intervenção foi realizada em 12 sessões de 30 minutos cada, três vezes por semana. Os dados sociodemográficos e obstétricos foram apresentados como mediana e interquartil (25% - 75%). Uma comparação entre os valores relativos das variáveis antes e depois do programa de exercícios foi realizada utilizando o teste ANOVA 2x2 (nível de significância de 5%). Resultados: A mediana gestacional e idade cronológica foram, respectivamente, 30,50 (27,2 - 32,0) anos e 28,5 (27,7 - 30,0) semanas. Não houve diferenças estatisticamente significativas nas variáveis relacionadas ao equilíbrio postural (P > 0,06) e qualidade de vida (P > 0,13). Conclusões: O uso de jogos Nintendo Wii Fit Plus não foi capaz de promover a melhora no equilíbrio e qualidade de vida das gestantes estudadas postural.

ABSTRACT: Early blight is one of the most important fungal diseases of potato. The objective of this study was to evaluate the in vitro reaction of potato genotypes to the severity of early blight and compare the species Alternaria solani and Alternaria grandis , etiologic agents of disease, through the evaluation of physiological characteristics. Twenty-two genotypes of the Potato Breeding Program at UFLA, two cultivars, Aracy and Bintje, patterns of resistance and susceptibility, respectively were evaluated. For inoculation, three isolates of A. grandis and two isolates of A. solani were used. Plantlets obtained in vitro were inoculated with a 5mm diameter mycelial disk of isolates Alternaria spp. of. These were incubated in severity and classified according to the rating scale. Isolates of Alternaria spp. were evaluated for mycelial growth index (MGI) and for formation of mycelial compatibility groups (MCG). The methodology for evaluating the in vitro reaction of genotypes to severity of early blight was efficient for both isolates. The isolates of A. solani and A. grandis were similar in their aggressiveness. The isolates of A. grandis MGI's had higher, and therefore mycelium grew faster when compared to those isolated from A. solani . The occurrence of mycelial compatibility among isolates of A. solani and A. grandis indicated the possible occurrence of parasexual cycle.


RESUMO: A pinta preta é uma das doenças fúngicas mais importantes da cultura da batata. O objetivo deste trabalho foi avaliar, in vitro, a reação de genótipos de batata à pinta-preta e comparar as espécies Alternaria grandis e Alternaria solani , por meio da avaliação de características fisiológicas. Foram avaliados 22 genótipos do Programa de Melhoramento Genético de Batata da UFLA, sendo duas cultivares, padrões de resistência e suscetibilidade. Foram inoculados três isolados de A. grandis e dois de A. solani . As plântulas, obtidas in vitro, foram inoculadas com os isolados de Alternaria spp., incubadas e classificadas quanto à severidade. Os isolados de Alternaria spp. foram avaliados quanto ao índice de velocidade de crescimento micelial (IVCM) e a formação de grupos de compatibilidade micelial (GCM). A metodologia de avaliação precoce in vitro da reação dos genótipos à severidade da pinta-preta foi eficiente para os isolados de ambas as espécies de Alternaria . Os isolados de A. grandis e A. solani apresentaram comportamento semelhante quanto à agressividade. Os isolados de A. grandis apresentaram IVCMs superiores e, portanto, crescimento micelial mais rápido. A ocorrência de compatibilidade micelial entre isolados de A. grandis e A. solani indica a possibilidade de ocorrência do ciclo parassexual.

BACKGROUND AND OBJECTIVES: Hypokalemia has been consistently associated with high mortality rate in peritoneal dialysis. However, studies investigating if hypokalemia is acting as a surrogate marker of comorbidities or has a direct effect in the risk for mortality have not been studied. Thus, the aim of this study was to analyze the effect of hypokalemia on overall and cause-specific mortality. DESIGN, SETTING, PARTICIPANTS AND MEASUREMENTS: This is an analysis of BRAZPD II, a nationwide prospective cohort study. All patients on PD for longer than 90 days with measured serum potassium levels were used to verify the association of hypokalemia with overall and cause-specific mortality using a propensity match score to reduce selection bias. In addition, competing risks were also taken into account for the analysis of cause-specific mortality. RESULTS: There was a U-shaped relationship between time-averaged serum potassium and all-cause mortality of PD patients. Cardiovascular disease was the main cause of death in the normokalemic group with 133 events (41.8%) followed by PD-non related infections, n=105 (33.0%). Hypokalemia was associated with a 49% increased risk for CV mortality after adjustments for covariates and the presence of competing risks (SHR 1.49; CI95% 1.01-2.21). In contrast, in the group of patients with K <3.5 mEq/L, PD-non related infections were the main cause of death with 43 events (44.3%) followed by cardiovascular disease (n=36; 37.1%). For PD-non related infections the SHR was 2.19 (CI95% 1.52-3.14) while for peritonitis was SHR 1.09 (CI95% 0.47-2.49). CONCLUSIONS: Hypokalemia had a significant impact on overall, cardiovascular and infectious mortality even after adjustments for competing risks. The causative nature of this association suggested by our study raises the need for intervention studies looking at the effect of potassium supplementation on clinical outcomes of PD patients.

Introduction Pregnancy is characterized by several changes in her body. These changes contribute to the emergence of low back pain, which may influence the quality of sleep during pregnancy. Objective To compare the quality of sleep among pregnant women with and without low back pain during pregnancy, examining the relationship between two variables. Materials methods Thirty volunteers aged between 19 and 36 years, divided into control group (CG – n = 16) and Study Group (SG – n = 14), residents in the cities of Natal, were evaluated in the second trimester of pregnancy. To sleep evaluation were used to index the Pittsburgh Sleep Quality and the Epworth Sleepiness Scale. Low back pain was evaluated using the pressure algometer, Visual Analogue Scale (VAS) and Oswestry Disability Index. Statistical analysis used the Shapiro Wilk test, Student’s T test for independent samples and Pearson correlation test. Results The mean gestational and chronological ages were 28.2 ± 3.4 years and 19.9 ± 3.7 weeks, respectively. Sleep quality was lower in SG (8.21 ± 4.8) when compared to CG (5.94 ± 1.7) and was statistically significant (P = 0.021). Analyzing the relationship between sleep quality and pain intensity, it was observed that the variables have a positive correlation between them (r = 0.372, P = 0.043). Conclusion Our findings indicate that sleep quality is decreased in women with low back pain compared to those without pain. .


Introdução A gravidez caracteriza-se por diversas mudanças no corpo da mulher. Essas alterações contribuem para o surgimento da dor lombar, que pode influenciar a qualidade do sono durante a gestação. Objetivo Comparar a qualidade do sono entre mulheres grávidas com e sem dor lombar gestacional, analisando a relação entre as duas variáveis. Materiais e métodos Trinta voluntárias com faixa etária entre 19 e 36 anos, divididas em grupo controle (GC, sem lombalgia – n = 16) e grupo de estudo (GE, lombalgia – n = 14), residentes nos municípios da Grande Natal, foram avaliadas no segundo trimestre de gestação. Para a avaliação do sono foram utilizados o Índice de Qualidade do Sono de Pittsburgh e a Escala de Sonolência de Epworth. A dor lombar foi avaliada utilizando-se o algômetro de pressão, a Escala Visual Analógica (EVA) e o Oswestry Disability Index. Na análise estatística, utilizou-se o teste de Shapiro Wilk, Teste T de Student para amostras independentes e o teste de correlação de Pearson. Resultados A média das idades cronológica e gestacional foram 28,2 ± 3,4 e 19,9 ± 3,7 semanas, respectivamente. A qualidade do sono foi menor no GE (8,21 ± 4,8) quando comparado ao GC (5,94 ± 1,7), sendo estatisticamente significativa (P = 0,021). Analisando-se a relação entre qualidade do sono e intensidade dolorosa, observou-se que as variáveis apresentam uma correlação positiva entre si (r = 0,372; P = 0,043). Conclusão Os achados deste estudo indicam que a qualidade do sono encontra-se diminuída em gestantes com lombalgia quando comparadas àquelas sem dor lombar. .

RESUMEN

An isolate of the Andean strain of potato virus S (PVS), named BB-AND, was detected for the first time in a Brazilian potato crop, fully sequenced and analyzed. A comparison of BB-AND with other PVS isolates (Andean and Ordinary) showed that BB-AND is quite distinct. The lowest amino acid sequence identity to the only other fully sequenced Andean isolate was found in ORF 1 (82%) and ORF 6 (87%). Recombination analysis showed that the isolate Vltava (AJ863510), from Germany, is a recombinant between PVS(O) and PVS(A) isolates, with the recombination event located between nucleotides 6125 and 8324.

RESUMEN

Objetivo: Avaliar a influência da fisioterapia pré-operatória na amplitude de movimento do ombro e na medida de independência funcional em mulheres submetidas à mastectomia radical modificada com linfadenectomia axilar. Métodos: Foi realizado estudo piloto de um ensaio clínico randomizado com 14 mulheres que realizaram cirurgia de mastectomia radical com linfadenectomia axilar, divididas em grupo-controle (n=7) e grupo intervenção (n=7). As avaliações ocorreram no momento pré-operatório, no pós-operatório imediato (1° dia pós-operatório) e no pós-operatório tardio (15º dia pós-operatório), através de fichas de avaliação fisioterapêutica e aplicação do medida de independência funcional. Para o grupo intervenção, foi realizado atendimento fisioterapêutico pré-operatório, através de cinesioterapia do ombro. Para o grupo-controle, foram dadas somente informações através de cartilha como intervenção no pré-operatório. Resultados: A idade média das mulheres do grupo-controle foi de 61,1 (±14) e no grupo intervenção foi de 64,3 (±11,1) anos (p=0,65). Observou-se que o movimento de abdução do ombro apresentou maior déficit de amplitude de movimento para o grupo-controle. Com relação à medida de independência funcional, demonstrou-se uma diferença significativa (p<0,05) do escore no pré-operatório e no 15º dia de pós-operatório. Conclusão: A fisioterapia pré-operatória exerce papel fundamental na recuperação dos movimentos do ombro e maior independência funcional nas atividades de vida diárias do 1° dia pós-operatório ao 15° dia pós-operatório
