RESUMEN

A clinical analysis of a patient with a rare form of secondary facial pain (Herzenberg's disease) is presented. The conducted research and pharmacological tests made it possible to exclude such diseases as trigeminal neuralgia, stylohyoid syndrome, TMJ pain dysfunction, dental plexalgia, myofascial facial pain syndrome. The patient is consulted by a dental surgeon to exclude pathology of the parotid salivary gland and sialolithiasis. The patient was treated with broad-spectrum antibiotics for 8 days. Also, supportive therapy was prescribed: desensitizing, vitamins, as well as an anxiolytic and an antidepressant in minimum therapeutic doses for a course of 14 days (with subsequent correction of the dosage and duration of administration) to correct the emotional state of the patient. We recommend a gentle diet (alkaline warm drink, soft food), and a diet that does not provoke salivation. Locally it was recommended to rinse with antimicrobial drugs, phonophoresis, electrophoresis, magnetotherapy. On the 6th day of therapy, the therapeutic effect was obtained. By the 14th day of therapy, the pain syndrome was leveled.

Due to the low efficiency of immunotherapy for colorectal cancer (CRC), it is extremely promising and relevant to study the mechanisms of immunosuppression. In this work, a comprehensive study of the expression of soluble and tissue forms of PD-1 and PD-L1 in blood serum and tumors of patients with CRC, as well as IDO1 in tumors was performed for the first time. The diagnostic and prognostic significance of the studied parameters was determined. A statistically significant decrease in the number of soluble forms of PD-1 and PD-L1 in the blood serum and the association of the number of PD-L1+ cells in the stroma of tumors with the CRC stage were established. The absence of correlations between soluble and tissue forms of the studied proteins was shown, indicating the presence of independent mechanisms of immunosuppression in CRC, which may explain the ineffectiveness of immunotherapy for this type of tumor.

Transcription factor PU.1 is involved in hematopoiesis and is expressed in various cells of the inflammatory infiltrate of the tumor stroma, mainly in macrophages. The expression of PU.1 in 100 samples of non-small cell lung cancer was analyzed by the immunohistochemical method. The number of PU.1+ cells did not correlate with clinical and morphological parameters of the tumors. However, increased number of PU.1+ cells significantly correlated with favorable prognosis in adenocarcinoma and poor prognosis in squamous cell carcinoma. Thus, transcription factor PU.1 can serve as a stromal prognostic marker of non-small cell lung cancer depending on the histological type of the tumor.

We carried out a comparative analysis of circadian rhythms of melatonin secretion in Caucasian menopausal women with and without insomnia depending on the 3111T/C polymorphism of the Clock gene. Melatonin levels was measured in the saliva 4 times a day (06.00-07.00, 12.00-13.00, 18.00-19.00, and 23.00-00.00 h). Carriers of the TT genotype with insomnia demonstrated significantly higher level of melatonin in the early morning hours compared to the carriers of the minor allele C (12.60±7.58 and 8.98±8.62 pg/ml, respectively, p=0.023). In the control group, no statistically significant differences were revealed. The carriers of the TT genotype with sleep disorders have higher morning melatonin level compared to control group women (12.60±7.58 and 5.48±4.74 pg/ml, respectively, p=0.005) and low nocturnal melatonin level (6.42±4.97 and 12.52±10.40 pg/ml, respectively, p=0.039).

AIM: To evaluate of parameters of lipid peroxidation - antioxidant defense changes in women of fertile age with chronic viral hepatitis (CVH) and with HIV (human immunodeficiency virus) infection, depending on the degree of inflammatory process activity in the liver. MATERIALS AND METHODS: 99 women of reproductive age were examined, of them 44 patients with chronic parenteral viral hepatitis, 27 patients with HIV co-infection (HIV + hepatitis B and/or C) and 28 practically healthy women (control group). RESULTS: Patients with CVH in combination with HIV-infection with the presence of minimal and low degree of inflammatory activity in comparison with patients with CVH-monoinfection had a higher content of lipid peroxidation products (an increase in the content of diene conjugates and ketodienes and conjugated trienes), reduced values of total antioxidant activity, superoxide dismutase activity, as well as the concentration of fat-soluble vitamins α-tocopherol and retinol. In the group with CVH and HIV with moderate and high degree of activity, similar, but even more pronounced changes in the system of lipid peroxidation were observed. CONCLUSION: The presence of co-infection - CVH in combination with HIV infection with minimal and low, as well as moderate and high inflammatory activity, is characterized by more intensive lipid peroxidation processes and a pronounced lack of antioxidant factors than with CVH-monoinfection.

AIM: To reveal the specific features of changes in the parameters of lipid peroxidation - antioxidant defense and the status of the reproductive system in women of fertile age with monoinfection (HIV) or coinfection (HIV and hepatitis B and/or C). SUBJECTS AND METHODS: 81 reproductive-aged women were examined and divided into 3 groups: 1) 28 apparently healthy women (controls); 2) 26 patients with HIV monoinfection; and 3) 27 patients with HIV coinfection (HIV + hepatitis B and/or C) who had not previously received antiretroviral therapy. RESULTS: The HIV coinfection group showed the higher values of primary (conjugated dienes) and secondary (ketodienes and conjugated trienes) lipid peroxidation products in the presence of lower superoxide dismutase activity and obvious α-tocopherol deficiency than the control and HIV monoinfection groups. HIV coinfection in reproductive-aged women was accompanied by the high frequency of luteal phase deficiency. CONCLUSION: HIV coinfection is characterized by a more pronounced oxidative stress, antioxidant factor deficiency, and the high frequency of luteal phase deficiency in the patients.

RESUMEN

In our work it was studied the metabolic reactions in adolescent girls representatives the two small nationalities (Tofalars and Evenki) and alien (Caucasians) of the population living in different areas of the Irkutsk region. It is found the hypothalamo-thyroid adaptive response development (increase in free T3 concentration, and decrease of TSH in Tofalars; and higher levels of total T4 and T3 in Evenki) in comparison with Caucasians. The study of antioxidant defense system demonstrated the increased content of a-tocopherol and retinol in Tofalars and only retinol in Evenki. The analysis of these two systems relationships revealed the correlation between the thyroid status and antioxidants (T3-α-tocopherol axis) in Evenki and Tofalars. This data proposed that indicated metabolic changes revealed in Tofalars and Evenki may be the result of genetically-determined long-term adaptation to the region environment conditions.

A comparative analysis of efficacy and safety of the neuroleptanalgesia (NLA) and balanced general anesthesia during operations on the biliary tract and liver. Analysis of the results of comprehensive studies of hemodynamic, humoral homeostasis, liver function and clinical anesthesia showed that the optimized on the basis of ketamine modified NLA provides effective neurovegetative protection during operations on the organs of the hepatobiliary system, accompanied by obscure variations in the studied parameters. Traditional methods of the modified NLA is characterized by insufficient anesthesia, accompanied by pronounced physical inactivity circulation and a large number of "critical incidents".
