RESUMEN

Ingested or aspirated dental prostheses may cause dysphagia and respiratory obstructions. We present the case of a patient with dementia who had a dental prosthesis lodged in his hypopharynx. This patient had routine radiologic studies and diagnostic scans to rule out primary and metastatic disease, was placed on total parenteral nutrition, and underwent several endoscopies before his dysphagia was related to aspiration of a dental prosthesis. The prosthesis was evident on radiologic examination, but was overlooked by the admitting service in the patient's differential diagnosis. After removal of the appliance, the patient had an uneventful recovery and was discharged after a 22-day hospital stay. Early diagnosis and definitive patient treatment depends on prompt recognition. The importance of recognizing the radiographic appearance of a dental prosthesis is emphasized.

RESUMEN

With the increased amount of orthognathic surgery being performed by orthodontists and oral-maxillofacial surgeons, more efficient treatment techniques are being perfected. Modular intermaxillary fixation has been developed to help the orthodontist prepare his patient for surgery and provide the surgeon with the stable fixation needed for proper healing. It is efficient, saves chair time, is comfortable and esthetic, and is well accepted by patients. Modular intermaxillary fixation has been evaluated for more than a year, with good results in correcting mandibular skeletal dysplasias in all three planes of space.

RESUMEN

A review of the literature disclosed many potential complications associated with endotracheal anesthesia. A retrospective review of the records of the general anesthetics administered to 5,223 outpatients receiving nasal endotracheal intubation at the Ohio State University was undertaken. This review showed minimal serious complications associated with nasal endotracheal anesthesia used for outpatients.

RESUMEN

The purpose of this project was to determine if senior dental students were capable of meeting the rigid didactic and clinical performance requirements of the American Heart Association's course in advanced life support. The course includes cardiopulmonary resuscitation, mechanical ventilation, endotracheal intubation, intravenous cannulation, interpretation of certain electro-cardiographic dysrhythmias, synchronized cardioversion, defibrillation and the use of intravenous vasopressors, cardiotonics, and antiarhythmics. Of 30 students who were randomly selected from over 120 applicants for this senior elective course, 27 met the minimum proficiency requirement of 85 percent on a standardized written examination, and no one scored less than 78 percent. Of the students who were offered clinical testing stations, 19 out of 20 met the clinicae support can be considered a feasible addition to other instruction in emergency care in the undergraduate dental curriculum.

A polylactic-polyglycolic acid synthetic absorbable suture material (Vicryl, Polyglactin 910) was compared with black silk and plain catgut in human oral tissues. Inflammation was graded clinically and histologically after seven days. Handling characteristics, including suture retention, were evaluated. Vicryl and black silk sutures produced comparable inflammation after seven days. Plain catgut sutures were usually absorbed before seven days, but when retained generally produced a more severe reaction. The polylactic-polyglycolic acid suture is braided and dyed, and has handling characteristics similar to those of black silk. Although they are absorbable, Vicryl sutures may be retained longer than desired and probably should be used like silk when exposed to the oral cavity.

Certain pain disorders may be difficult to diagnose because their presenting symptoms are confusing and difficult to classify, or because they occur so infrequently that they are not commonly considered. This article has presented a classification and discussion of selected pain states which the practitioner may find difficult to diagnose. When faced with a perplexing pain problem, the dentist is encouraged to perform a careful evaluation to determine whether the patient's pain is or is not of dental origin. Certain pain states require that the patient be referred for evaluation to other dental or medical specialists. The value of local anesthesia as a diagnostic aid in evaluating face and head pain has been presented.
